Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

Glucocorticoids play an important role in the treatment of acute lymphoblastic leukemia (ALL). However, supraphysiological doses may cause suppression of the adrenal. Adrenal suppression resulting in reduced cortisol response may cause an inadequate host defence against infections, which remains a cause of morbidity and mortality in children with ALL. The occurrence of adrenal suppression before and after glucocorticoid therapy for childhood ALL is unclear. The aim of this study is to analysis the effect of glucocorticoid on cortisol levels during induction phase chemotherapy in children with acute lymphoblastic leukemia. A cross-sectional, observational prospective study was conducted to determine the effect of glucocorticoid on cortisol levels in children with acute lymphoblastic leukemia. Patients who met inclusion criteria were given dexamethasone or prednisone therapy for 49 days according to the 2013 Indonesian Chemotherapy ALL Protocol. Cortisol levels were measured on days 0, 14, 28, 42 and 56 of induction phase chemotherapy. There were 24 children, among 31 children recruited, who suffered from acute lymphoblastic leukemia. Before treatment, the means of cortisol levels were 228.95 ng/ml in standard risk group (prednisone) and 199.67 ng/ml in high risk group (dexamethasone). In standard risk group, the adrenal suppression occurs at about day 56. There was a significant decrement of cortisol levels in high risk group in days 14, 28, 42 against days 0 of induction phase (p=0.001). Both groups displayed different peak cortisol levels after 6 week of induction phase (p=0.028). Dexamethasone resulted in lower cortisol levels than prednisone during induction phase chemotherapy in children with acute lymphoblastic leukemia.

Abstract

Singawalang leaf is used as a traditional medicine by the communities suffering from pneumonia due to TB disease. However, experiments to explain it's toxicity are few. Therefore, this study was conducted to disclose it's toxicity. This was an experimental laboratory study using post–test only control group design. The sampling method was simple random sampling. The animals were randomly divided into 4 (four) groups, each group consisted of 8 (eight) mice, the control group (P0) received distilled water as a solvent material. Treatment group 1 (P1) obtained ethanol extract of singawalang (Petiveria alliaceae) in low doses of 90 mg/kgbw. Treatment group 2 (P2) obtained ethanol extract of singawalang (Petiveria alliaceae) in medium doses of 180 mg/kgbw. Treatment group 3 (P3) obtained ethanol extract of singawalang (Petiveria alliaceae) in high doses of 360 mg/kgbw. Th extracts were given with sonde once a day for 30 days. Results showed that the leaf ethanol extract singawalang (Petiveria alliaceae) does not induce sub-acute necrosis effects on kidney of male white mice (Mus musculus) in doses 90 mg/kgbw, 180 mg/kgbw, and 360 mg/kgbw.

Abstract

b-thalassemia major is a genetic disease and a decline in production due to imperfect of hemoglobin. Clinical manifestations are anemia, treated with regular blood transfusions. Blood transfusions lead to an increase in iron in the body that can lead to organ complications. Iron chelation drug delivery is expected to reduce complications due to an increase in the amount of iron in the body by measuring serum ferritin. The study aims to analyze the use of deferasirox and deferipron in patients with b-thalassemia major children at Dr. Kariadi Hospital of January 1, 2012 until December 31, 2013 as well as the need for blood transfusions. Patients who met the inclusion criteria, ie patients with b-thalassemia major children, retrospectively conducted observations of medical records include basic data and laboratory data. After descriptive analysis was performed to determine the use of two types of iron chelating drugs. In this study, 9 patients included in the inclusion criteria. A total of 5 people using deferasirox and 4 using deferipron at baseline. There are 3 people who turned deferipron be deferasirox. Serum ferritin values at study entry was> 1200 mcg/L, and at the end of the study serum ferritin is> 1200 mcg/L. The mean dose of deferasirox study was 19 ± 4.3 mg, and the dose deferipron mean was 80.8 ± 7.7 mg. Mean hemoglobin levels before transfusion was 6.60 ± 0.89 mg/dL. The average number of blood transfusions given was 336.52 ± 73.85 ml. Type of blood transfusion is used Washed erythrocyte. Splenomegaly occurred in 2 patients. There is no change in renal function, and hepatic meaning. The final conclusion until the reduction target serum ferritin <1000 mg/L has not been reached. Deferasirox dose can be increased to achieve the expected serum ferritin.

Abstract

Stent thrombosis is a rare complication of PCI but associated with STEMI and sudden cardiac death. Intra procedural stent thrombosis (IPST) was defined new or increasing (compared with baseline) thrombus within or adjacent to a deployed stent occurring the index PCI procedure whether occlusive or nonocclusive. We describe a case with double vessel disease who has complication cardiac arrest and intra procedural stent thrombosis in LAD and Left Main coronary artery after deployed stent in bifurcation LAD-D1. Thrombectomy and rescucitation were performed, and the patient completed her hospital course without complications.

Abstract

Endogamy continues to occur among the Madurese people in rural areas of the island of Madura, especially those areas of the smallest islands around the mainland of Madura. Endogamy as seen from a genetic standpoint will increase the frequency of homozygote genotypes. With regard to genetic variations, STRs of nuclear DNA and polymorphisms in mtDNA are frequently examined. Genetic variations in human undergo an evolutionary process through the accumulation of changes in DNA sequence, i.e. through the process of nucleotide substitutions that evolves in number with the directional development of lineage. So far, the genetic variations among the populations in Madura Island have not been known. The present study was an observational analytical research with the purpose of determining the genetic variations in STR CODIS in the populations of Madura Island. Results indicated that, based on loci alelle: CSF1PO, THOI, TPOX, and vWA, there was homozygote genotypes. The allele variations is not specific for Madurese ethnic but this variations may represent married model in Madurese ethnic. According to Mustama (2007), a gene pool is not only a collection of genes but a dynamic system organized and containing the past history of a population. Any genetic information has certain historical, anthropological and statistical aspects necessitating an interdisciplinary coordination and collaboration.

Abstract

The number of cancer cases in every year has increased. One effort in reducing breast cancer cases is by using anti-angiogenesis which could prevent the growth and metastasis of cancer cells. Xxx the process of angiogenesis and metastasis in cancer cells are associated with the expression of Matrixmetalloproteinase- 9 (MMP-9) and Vascular Endothelial Growth Factor (VEGF). To reduce metastasis in cancer cells, it can be performed by giving plant extracts Cayratiatrifolia L (Domin), containing the active compound Resveratrol, delphinidin/malvidin and Quercetin. The content of the chemical compound of these herbs has immunomodulatory effects, which could potentially prevent angiopoitin, FGF and decrease angiogenesis and neovascularization. So, the metastatic cancer cells can be inhibited. Objective to analyze the effect of the ethanol plant extract Cayratiatrifolia L (Domin) toward the reduction of Matrixmetalloproteinase Expression.-9 (MMP-9) and Vascular Endothelial Growth Factor (VEGF) on white rat of breast cancer models. This study used pure experiment laboratory (True Experimental) with Randomized design study. Post Test was only applied on Control Group Design. This research used experimental female animal Sprague Dawley Rattus which consisted of 30 individuals divided into three groups, from aged 30-40 days. In this research the number of K0 consisted of 5 individual, 6 Kl and 8 KP. KP is a group of experimental animals with breast cancer who were given Cayratiatrifolia extract as much as 300 mg/kg for four weeks. To create animal cancer models, the researcher used DMBA a dose of 10 mg/kg given orally. The measured variables were cells expressing Matrixmetalloproteinase-9 (MMP-9) and Vascular Endothelial Growth Factor (VEGF), differential test applied in this study is Manova. Results based on the statistical analysis of MMP-9 and VEGF-A, between the KI to KP showed a significant difference (p <0.05). Cayratiatrifolia plant extract orally can decrease the number of cells that express MMP-9 and the number of cells that express VEGF-A. The ethanol extract of plants Cayratiatrifolia L (Domain) can reduce the expression Matrixmetalloproteinase-9 (MMP-9) and Vascular endothelial growth factor (VEGF) on white rat of breast cancer models.

Abstract

Tuberculosis developed differently in each individual. The role of HIV in the disease development is widely known unlike the role of Hepatitis B and Hepatitis C infection. Therefore, this study is aimed at to describe the role of those co-infections in the TB development. The study employed observational – case control design with two groups, case and control group. Data collection include TB condition, whether latent or manifest, and history of Hepatitis B and Hepatitis C co-infection. The study successfully recruited the total of 32 respondents in each group. At the case group, the number of male and respondent is 19 and 13. Meanwhile, at the control group the number male respondent is 10 and the female respondent is 22. The study found that 20 and 12 respondents were positive Hepatitis B at the case and the control group. Chi-square test resulted significant different between two groups (P=0.0455 and a =0.05) with odd ratio p=0.479. Meanwhile, the study only tested one respondent with positive Hepatitis C at case group and one at control group. Comparison analysis found no significant different between groups (P=1 and a =0.05). This study reveals that TB patients are more likely to have history of Hepatitis B 2.7 times than those at the control group with the same infection but without TB development. Therefore, the possibility of Hepatitis B co-infection has role in the TB manifestation, but not for Hepatitis C co-infection.

Abstract

It's Important to conduct child development screening regularly in primary health services, so child developmental delay cases can be detected and treated as early as possible. Child development screening program in Puskesmas should been carried out integrated with growth screening in Stimulation, Detection, and Early Intervention of Child Growth and Development (SDIDTK) Program. Aims of this study was to gathering informations about the implementation of child development screening activity conducted by Puskesmas Mojo. This was a cross sectional study with kualitatif method. Primary data taken from under 5 years old child's mothers, kindergarten teachers, cadres, and health staff of Puskesmas Mojo. Secondary data gotten from PWS KIA report of Puskesmas Mojo.Results of SDIDTK activities in Puskesmas Mojo according to PWS KIA report, achieve 88.1% in 2013 and 95.2% in 2014. Child development screening activities done infrequenly in Posyandu. Routine activities of Posyandu was growth/nutrition screening. Child development screening activities done by visiting kindergarten school every 6 month but not every child getting development screen, only those who suspicious have developmental delay because of minimum number of staff and aids. Results of SDIDTK activities in Puskesmas Mojo only represents child growth screening activities.

Abstract

The aim of this experimental study was to examine the effect of erythropoietin on rat model and particularly in an ischemia reperfusion (HR) protocol. The effect of that molecule was studied biochemically using blood mean glucose (Gl) levels. Materials and methods: 40 rats of mean weight 247.7 g were used in the study. Gl levels were measured at 60 min (groups A and C) and at 120 min (groups B and D) of reperfusion. Erythropoietin was administered only in groups C and D. Results were that Epo administration non-significantly increased the gl levels by 5.59% +6.46% (p=0.3208). Reperfusion time non-significantly increased the gl levels by 5.63%+6.45% (p=0.4098). However, erythropoietin administration and reperfusion time together produced a non significant combined effect in increasing the gl levels by 4.94%+3.81% (p= 0.1892). Conclusions: Results of this study indicate that erythropoietin administration, reperfusion time, or their interaction non-significantly increase the blood glucose levels in short-term. Opposite bibliographic data are considered more reliable, until a greater sample provide clearer results.

Abstract

Tumor growth and metastasis in NPC patients suspected role of several molecular biomarkers that have been identified in tumor specimens of patients with NPC. Recent research states intracellular Hsp70 is a stress protein that is a main factor for the growth, invasion and metastasis of NPC. The purpose of this study is to analyze intracellular Hsp70 protein expression of NPC patients which correlate with staging of NPC as clinical manifestation. This study used cross sectional design. In this study, formalin-fixed paraffin-embedded biopsy specimens were obtained from 18 NPCs with all of histopathology and clinically divided into N0, N1, N2, N3. The expression of intracellular Hsp70 was obtained with immunohistochemistry using monoclonal antibody Anti Human Hsp70 antibodi from Santa Cruz Biotechnology, California, USA. Assessment of the staining was performed with Remmele methode by Histopathologies doctor Consultant. The Spearman's test was used to determine the relationship between expression of intracellular Hsp70 protein and enlargement cervical nodul of NPC. Statistical significance was defined as p <0,05. There were 18 patients NPC patients that met the inclusion and exclusion criteria. Showed negative intracellular Hsp70 expression by 16,67% for all sample in which consist 1 patient of N0, 1 patient of N1, 1 patient of N2, 0 patient of N3. Followed by 27,78% mild positive intracellular Hsp70 expression which consist 0 patient of N0, 2 patients of N1, 0 patient of N2, 3 patients of N3. Followed by 44,44% moderate positive intracellular Hsp70 expression which consist 0 patient of N0, 0 patient of N1, 1 patient of N2, 9 patients of N3. Followed by 11,11% strongly positive intracellular Hsp70 expression which consist 0 patient of N0, 0 patient of N1, 0 patient of N2, 2 patients of N3. Spearman test results test scores p = 0.01 with a correlation coefficient 0.606. Correlation of intracellular Hsp70 protein expression with enlargement cervical nodul of lymphe (N0, N1, N2, N3) in patients with NPC showed significant (p<0.05). In conclusion, there was correlation between increase of the intacellular Hsp70 expression and enlargement cervical nodul of lymphe in nasopharyngeal carcinoma.
