Abstract

Gastrointestinal Stromal Tumor (GIST) merupakan jenis tumor mesenkimal yang paling sering ditemukan pada saluran pencernaan. Terapi lini kedua saat ini menggunakan Sunitinib, namun obat ini memiliki profil efek samping hematologi dan non-hematologi yang cukup luas. Oleh karena itu, pengembangan terapi alternatif berbasis bahan alam seperti Benincasa hispida yang kaya akan senyawa bioaktif sangat diperlukan. Penelitian ini bertujuan untuk mengevaluasi potensi senyawa bioaktif dari B. Hidpida sebagai kandidat obat penarget GIST melalui pendekatan in silico. Penelitian dilakukan secara komputasi (in silico) dengan tahapan meliputi prediksi aktivitas biologis menggunakan PASS Online, uji drug-likeness berbasis SwissADME, serta simulasi molecular docking menggunakan PyRx dan Biovia Discovery Studio terhadap protein target Platelet-Derived Growth Factor Receptor Alpha (PDGFRA). Hasil analisis PASS Online menunjukkan bahwa senyawa Isomultiflorenyl acetate memiliki nilai aktivitas potensial (Potential Activity/Pa) tertinggi sebesar 0,913, mengindikasikan bioaktivitas yang sangat kuat. Uji drug-likeness memperlihatkan senyawa ini memenuhi kriteria Lipinski’s Rule of Five meskipun terdapat pelanggaran pada nilai refraktivitas molar (MR 144,6). Hasil simulasi molecular docking menunjukkan Isomultiflorenyl acetate memiliki afinitas pengikatan (binding affinity) terbaik sebesar −8,7 kcal/mol, yang lebih kuat dan stabil dibandingkan obat kontrol Sunitinib (B49) dengan nilai −7,2 kcal/mol. Kestabilan kompleks ligan-reseptor ini didukung oleh pembentukan ikatan Hidrogen Konvensional dengan residu ARG585, Pi-Sigma dengan TRP586, dan Pi-Alkil dengan PHE969. Senyawa Isomultiflorenyl acetate dari B. hispida berpotensi besar untuk dikembangkan lebih lanjut sebagai kandidat agen terapi penarget Gastrointestinal Stromal Tumor (GIST) berbasis bahan alam.

Abstract

Advancements in information technology have made significant contributions to supporting decision-making processes across various sectors, including healthcare. One of the major challenges faced by governments and related agencies is determining the most appropriate locations for healthcare facilities to ensure that healthcare services are delivered equitably, effectively, and in accordance with community needs. This study aims to implement the PROMETHEE method in a web-based Decision Support System (DSS) to assist in recommending suitable locations for healthcare facility establishment based on multiple evaluation criteria. The PROMETHEE method is employed to analyze and compare alternative locations using criteria such as distance to the health department office, population density, accessibility, establishment cost, and proximity to existing healthcare facilities. The developed system automatically calculates the leaving flow, entering flow, and net flow values, thereby generating an objective ranking of alternative locations. The results indicate that Alternative 2 (A2) achieved the highest Net Flow value of 0.56, outperforming Alternative 1 (A1) and Alternative 3 (A3). Consequently, the system successfully recommends the most appropriate location for establishing a healthcare facility based on the highest net flow value. Therefore, the proposed system effectively supports decision-making in a more objective, systematic, accurate, and efficient manner, serving as a reliable reference for planning the development of healthcare facilities in suitable locations.
