Abstract

Background: Acute bacterial meningitis is one of paediatric emergency cases which results in high morbidity and mortality. Diagnostic procedures using cerebrospinal fluid (CSF) are often unsatisfactory. A simple and rapid procedure is required for early diagnosis and prompt treatment.Objectives: The aims of this study were to assess total cerebrospinal fluid creatine-kinase (CSF-CK) level in childhood acute bacterial meningitis and to assess the diagnostic values of CSF-CK level compare with routine CSF analysis (i.e. cell count, morphology, chemical, C reactive protein and culture) which is accepted as diagnostic gold standard for acute meningitis.Materials and methods: All patients hospitalized in the Pediatric Department Sardjito Hospital for suspected acute bacterial meningitis in July-December 1998, underwent lumbar puncture. The level of CSF-CK and routine analyses were examined for diagnosis acute bacterial meningitis.Results: Forty five children with suspected meningitis were included in this study. CSF-CK 16 U/I had sensitivity 50%, specificity 80%, positive predictive value 42%, negative predictive value 85% and likelihood ratio 2,75. Mean of CSF-CK in childhood acute bacterial meningitis was 82,84 U/I.Conclusion : CSF-CK level was not sensitive, but it is a specific diagnostic test for childhood bacterial meningitis.Key words: acute bacterial meningitis - CK-CSF - diagnostic test

Abstract

Since the beginning of the Landsteiner era, genetics has been developing tremendously. Genetics now is used in genetic counselling, genetic screening and in the management of genetic disorders or diseases due to the fast technological development during the last two decades.Amniocentesis, one of the products of this technological development, is indeed valuable, as it can detect any chromosomal abnormality of the fetus in utero, thus genetic defects among babies, can be prevented.By taking 10-20 ml of amniotic fluid during 14-16 weeks of gestation by amniocen tesis, disposed fetal cells in the fluid can be immediately cultured. After 2-3 weeks, chromosomal analysis can be undertaken to make its karyotype. Other than the above benefit, amniocentesis can also be used for detection of any biochemical abnormality of the fetus, thus early prevention of biochemical disorders among babies can be done.With the application of amniocentesis as one of the diagnostic tools which continuously improves, its benefits help human beings in the prevention of birth defects and in counselling couples who are expecting healthy babies in their marriage.To extend the beneficial aspect of amniocentesis to grass-root level, improvement of knowledge and skills of amniocentesis of practicians is mandatory.Key Words: amniocentesis - amniotic cell culture - chromosomal analysis - genetic counselling -prevention of birth defects

Abstract

All open fractures especially open fracture of the lower leg grade III A and III B, should be assumed to be contaminated, therefore they tend to be infected. The time of intervention is one of the risk factors of infection complication.A descriptive analysis has been conducted aiming at evaluate the occurrence of infection and the time of treatment on patients with open fracture of the lower leg grade III A and III B. In this study 102 cases (80 males and 22 females) who had open fracture of the lower leg grade III A and III B were admitted to Dr. Sardjito General hospital, Yogyakarta. Patients were evaluated according to the inclusion and exclusion criteria. Out of fifty-four cases underwent intervention before 8 hours after injury (within golden period) 25 cases (46,29%) had been infected. The rest (48 cases) who had treatment after 8 hours since trauma, 83,33% (40 cases) were infected.A statistical analysis (X2 = 13,520, df=1 and p < 0,001) showed that there was significant difference of infection complication of open fracture of the lower leg grade III A and III B and the intervention between within and beyond golden period.Key words: open fracture - tibia fracture - infection - golden period - debridement

Abstract

Background: Psoriasis is a chronic skin inflammation and proliferation disease. Natural history of psoriasis depends on the genetic, age, sex, history of treatment and psychosocial stressor. Calcipotriol is suitable for long-term therapy with good efficacy and safety and clinical response variability but its activity is influenced by ultraviolet.Objective: To know the factors that may influence treatment response of calcipotriol ointment on mild to moderate plaque type psoriasis.Method: Longitudinal study of two times daily calcipotriol ointment 0.005% treatment in the psoriasis patients. Factors that may influence the natural course (age, sex, history of psoriasis in family, history of therapy, MED, working location and psychological stressor) were identified. Evaluation of therapy was performed in the 8th weeks and based on the PASI and PDI. Correlation between treatment responses with various factors were analyzed by using student t-test, Pearson and Spearman correlations and multiple linear regression.Result: At the end of study, PASI was shown to decrease 38.45%, while PDI was 28.70% (p

Abstract

The results of binocular cataract extraction with local anaesthetic are presented, which are good enough and not different compared with monocular cataract operations in The Eye Hospital of Gadjah Mada University. Abstract

Phendyne-diisothiocyanate 1.4. (JONIT) or Bitoscanate in a single, doseof 150 mg was given to 34 patients infected with hookworm. Besides habit-worm, they also harboured the common species of soil-transmitted helminths, such as Ascaris lumbricoides and Trichuris trichiura.The result of the study .shoWed-flat the drug was effective against hookworm and less effective against A. lumbricoides and T. trichiura' infections. The cure rates were 82.35%, 47.05% and 26.47 % for hookworm, A. lumbri-: coides and T. trichiura respectively.The side effects, including nausea, vomiting, diarrhoea,. giddiness orheadache and abdominal discomfort were mild and transient which did not require any treatment.

Abstract

Background: Age is one of the impaired glucose tolerance risks. Impaired glucose tolerance may be caused by the decrease of pancreatic B cells insulin secretion, or by the decrease of target tissue cell glucose uptake. Glucose tolerance tests which were conducted in the elderly, showed that plasma insulin concentrations at 120th minute were still higher than at minute-0 (at fasting time), while blood glucose levels were still high. It generated an impression that insulin inefficiency exist in the elderly.Objective: To reveal that the cause of insulin inefficiency in the elderly is due to the decrease of target tissue cell glucose uptake velocity and/or in decrease uptake ability.Methods: Elderly men subjects and young men comparable controls were given 130mU/kg L8M/hour insulin and 20g% dextrose infusion, while blood glucose level have to be maintained in euglycemic situations (euglycemic clamp test). In 30 minutes with stable blood glucose level, the amount of 20% dextrose infusion were calculated as glucose uptake in 30 minute (euglycemic clamp time). From this results the glucose uptake velocity and ability (glucose uptake ability = ratio between glucose uptake velocity and plasma insulin concentration = insulin sensitivity index) between the elderly and the young men were compaired by t-test.Results: Euglycemic clamp tests were conducted to 4 elderly men of 65-74 years old, and 4 young men of 21-30 years old as controls. The results showed that the velocity of glucose uptake by the target tissue cells in those elderly men were significantly (p<0.05) lower (10.08 f 2.34 mg/kg LBM/min.) than young men group (17.78 ± 5.49 mg/kg LBMlmin.). One elderly men showed the lowest difference (0) of insulin sensitivity index compared to one control subject. In the remaining three (75%) subjects the average of insulin sensitivity index (0.11 f 0.03) showed a significant difference (p<0.05) compared to young men group 10.19 f 0.04).Conclusions: The result of this study indicated that the cause of insulin inefficiency in the elderly men was due to the decrease of glucose uptake velocity and ability.Key words: impaired glucose tolerance - pancreas B cells insulin secretion - velocity of glucose uptake ability of glucose uptake - euglycemic clamp technique.

Abstract

Background: Dichlorodiphenyl trichloroetane (DDT), polychlorinated biphenyl (PCB) and polybrominated biphenyl (PBB) are carcinogenic substances and significantly decreased liver vitamin A in the animal studies. Liver vitamin A is influenced by dietary vitamin A and the condition of the liver. Aflatoxin Bi is a carcinogenic substance like DDT, PCB and PBB. Aflatoxin Bi is the environmental exposures problem in several tropical countries and liver are the main organ targets of the carcinogenicity of aflatoxin B1.Objective: To provide more extensive description of the effects of dietary vitamin A (deficient, adequate, fourfold adequate) against the effect of aflatoxin Bi (AFB1) on the vitamin A status of rats.Methods: Vitamin A status of animals was demonstrated by concentrations of liver and serum vitamin A. The subjects consisted of fourty eight male Rattus norvegicus rats in good general condition, 1-2 months old with body weight around 62-68.5 grams, were randomly divided into 12 groups of four rats each. Rats in group I to VI which were killed at the start of the experiment and were used as the initial condition of liver and serum vitamin A concentrations for the rats in group VII to XII. Vitamin A-deficient diets were given to rats in group VII and VIII. Vitamin A-adequate diets were given to rats in group IX and X, while vitamin A-fourfold adequate diets were given to rats in group Xl and XII. Rats in group VIII, X and XII were orally treated by an aplicator with 15g of AFB1 in 0.2 ml of propylene glycol everyday for 16 weeks. While rats in group VII, IX and Xl were treated without AFB1.Results: By analysis of variance, the results showed that the effects of interaction of dietary vitamin A and AFB1 demonstrated a very significant effect (p<0.001) on liver vitamin A and significant effect (p<0.05) in serum vitamin A concentrations. Dietary vitamin A itself demonstrated a very significant effect (p<0.001) in serum and liver vitamin A concentrations. While AFB1 demonstrated a significant effect (p<0.05) in liver vitamin A and significant effect (p<0.001) on liver vitamin A concentarations.Conclusions: Vitamin A status of rats given aflatoxin Bi were not only influenced by dietary vitamin A but also by allatoxin Bi treatment.Key words: dietary vitamin A - aflatoxin Bi - serum vitamin A -liver vitamin A - Rattus norvegicus rats

Abstract

The effectiveness of trabeculectomy has been studied in 65 primary glaucomatous patients consisting of 30 men and 35 women. The trabeculectomy was able to control the intraocular pressure in 89% of cases and it improved visual acuity in 67.69% of cases. Based on the iridocorneal angle appearances, the cases of primary closed angle glaucoma were more frequent than those of primary open angle glaucoma. The most frequent complication found was hyphaema which occured in 23.08% of cases, but it I usually disappeared in 7 days post-operatively. Anterior uveitis was the second complication and this happened in 6.15% of cases. Other complications such as flat anterior chamber and cataract were very rare. Conjunctival fistula, bleb rupture and intraocular infection were not found here. Key Words: trabeculectomy - glaucoma - control of intraocular pressure - visual acuity improvement - hyphaemia
