Abstract

Acne vulgaris is a multifactorial disorder characterized by seborrhea, microcomedones, comedones,papules, pustules, nodules and possibly scarring. The pathogenesis of acne is not completelyunderstood. Recent studies reported the possible role of reactive oxygen spesies (ROS) especiallylipid peroxides in mediating acne inflammation. Malondialdehyde (MDA) is one of the final productsof the lipid peroxidation that commonly used as a lipid peroxidation marker. This study was conductedto evaluate the correlation between serum MDA levels and severity of acne on male patients. A total60 patients with acne who attended in Polyclinic of Department of Dermatology and Venereology,Dr Sardjito General Hospital and met the inclusion and exclusion were involved in the study. Thepatients were then classified using combined acne severity classification method into three group asmild, modrate and severe acnes. Blood sample were then taken from all subjects and serum MDAlevels were determined spectrophotometer. The mean serum MDA levels of severe acne group(1.85 ± 0.368 μmol/L) were significantly higher than that moderate acne group (0.79 ± 0.159μmol/L) (p = 0.000) and that mild acne group (0.52 ± 0.239 μmol/L) (p = 0.000). Moreover, themean MDA levels of moderate acne group were also significantly higher than that mild acne group(p = 0.002). Significantly positive correlation was observed between serum MDA levels and acnelesions total count (r = 0.714; p = 0.000). It can be concluded, there is positive relationshipbetween serum MDA levels and severity of acne vulgaris on male patients.

Abstract

Chronic Rhinosinusitis (CR) is disease by incidence and prevalence of high and is expected to rise to load against the economy and lowering the quality of life of patients.with CR involved many factors and an impact of highly significant on the quality of life and affect the economy.The quality of life patients done to assess the effects and result of surgery.Sino nasal outcome test-20 ( snot-20 ) was one of the instruments specific measurement the quality of life of patients CR. 10 patients who surgery action , obtained improving the quality of life based on snot-20 observed 28 days after done the act of surgery. Rate the SNOT- 20 before surgically obtained 37,3 with DS ± 7,87 and 28 days after done surgically obtained rata-rata snot-20 is 6 with DS ± 2,49. The SNOT-20 pre and post action there are different of significant (p = 0,024 ) .

Abstract

This study was aimed to understand differences of ondansetron serum concentrationin each antiemetic responses, polymorphisms of 5HT3B receptor, CYP2D6 and ABCB1genes in Indonesian cancer patients treated with high emetogenic cytostatics. We recruitedcancer patients in Dr Sardjito Hospital treated with cisplatin (≥ 50 mg/m 2) as monotherapyor combination therapy. Patients were treated with ondansetron 8 mg intravenously anddexamethasone 8 mg intravenously and metoclopramide (10 mg orally) after cytostaticadministration until 5 days after chemotherapy. We cathegorized the nausea and vomitinggrade according to the National Cancer Institute Common Toxicity Criteria v.3. We alsodetermined some SNPs of ABCB1, 5HT3B and CYP2D6 genes using realtime PCR. Werecruited 191 cancer patients in this study with the average of ondansetron serumconcentration reached 33.48 ng/ml (SD: 18.54). According to the patients’ response tothe antiemetic, during the acute phase, 21.8% patients experienced acute nausea and30.2% patients experienced acute vomiting. Only the haplotype of CTG-CTG of ABCB1which have significant association with ondansetron serum concentration. EM patients ofCYP2D6 and patients with haplotype of delAG of 5HT3B had lower ondansetron serumconcentration. However, IM patients of CYP2D6 showed higher ondansetron serumconcentration and lower grade of nausea and vomiting. Variations of ABCB1, CYP2D6and 5HT3B may be used as pharmacogenetic marker in predicting antiemetic response incancer patients receiving highly emetogenic cytostatic.

Abstract

The prevalence of Toxoplasma gondii and Cytomegalovirus (CMV) infections are high in the world. Indonesia is one of the countries with high prevalence of these infections varied from 20 to 63%. The T. gondii and CMV infections infections can be chronic and cause maternal and fetal deatht as well as infant defects. Previous clinical study reported that chronic infections can cause somatic symptoms indicating psychological stress. The aim of this study was to evaluate the correlation between T. gondii and CMV infections with somatic symptoms. This was an observational study with a cross sectional design involving 103 eligible patients with seropositive IgG T. gondii or and CMV from six cities in Java, Indonesia. The presence of somatic symptoms was detected by using somatic symptoms inventory (SSI) questionnaire. Logistic regression analysis was used to evaluate the correlation. The percentage of patients with somatic symptoms (SSI score >48) in seropositive groups of IgG anti- T. gondii, anti-CMV, anti- T. gondii and CMV were 70.0; 62.2 and 36.2%, respectively. In addition, the prevalence ratio (PR) for each group were 1.333, 1.178, and 0.954, respectively. No significantly different in PR was observed in this study (p>0.05). In conclusion, the T. gondii and CMV infections are not correlted with the somatic symptoms.

Abstract

Physical exercise is important in preventing and adjunctive therapy certain diseases. However,over physical exercise for beginners may cause oxidative stress and muscle injury leads to generatechemoattractants whic enables to attract neutrophil and monocyte towards the injury and stimulatesleucocytes activation. The use of antioxidant-rich fruits to mitigate exercise induced oxidative stresshas been applied. This study was conducted to evaluate the effect of guava juice (Psidium guajavaL) consumption during aerobic exercise on leucocytes, neutrophils and superoxydes dismutase(SOD) level of beginners. Sixteen students of National Land College (Sekolah Tinggi PertanahanNasional/STPN), Yogyakarta who met the inclusion and exclusion criteria were involved in thisstudy. Subjects were divided into two groups i.e. a group given mineral water as control andanother group given 240 mL guava juice 0.67 g/mL daily for 27 days as treatment group. Bothgroups then underwent aerobic exercise for 30 minutes every day for 27 days. Blood samples weretaken before and after exercise on day 1, 7 and 14 for leucocyte and nutrophil count as well as SODanalysis. The leucocyte count and SOD level before and after exercise in both Guava juice andMineral water groups were not significantly different (p>0.05). However, the percentage of neutrophilon day 1 and 7 were significantly higher than that in Mineral water group (p<0.05). Moreover,during the exercise, the percentage of neutrophils in Guava juice group significantly increased(p<0.05), whereas in Mineral water group was not significantly different (p>0.05). It can beconcluded that consuming guava juice during exercise for the beginners does not influence leucocytecount and SOD levels. However, it can increase percentage of neutrophil.

Abstract

Dyspepsia is a common disorder characterized by heartburn, nausea, vomiting, bloating, feelingof fullness or early satiety, and belching. Functional dyspepsia (FD) or non-ulcer dyspepsia is asyndrome without any physical or endoscopic abnormalities underlying these symptoms. Theimpacts of FD on psychological disturbances and quality of life in patients have been postulated.However, it has not much been a concern. This study was conducted to evaluate the relationshipbetween symptoms of depression and quality of life in patients with FD.This was an observationalstudy with cross-sectional design involving 30 patients. The symptoms of depression weremeasured by beck depression inventory (BDI) and the quality of life of patients were measuredby SF-36. The results showed that FD were more suffered consecutively by female patients(66.7%), age over 45 years (53.4%), housewives (56.6%), educated graduates (56.6%) andmarried (90.0%). Moreover, patients who experienced symptoms of clinical depression withlow, moderate and severe levels were 16.3%, 33.3% dan 10.0%, respectively. A significantcorrelation between depressive symptoms and the overall of patients quality of life was observed(r = 0.481; p<0.05). Furthermore analysis showed that among eight domains of health in SF-36analyzed, a negative significant correlation was observed between the depressive symptoms andthe three domains of health i.e. general physical function, limitation of motion caused byphysical problems, and vitality. In conclusion, there is a negative correlation between depressivesymptoms and quality of life in patients with FD

Abstract

Cervical cancer is a woman’s health problem that is still widespread throughtout the world, especially in developing country such as Indonesia. The high incidence of cervical cancer related with early detection program. Visual inspection of acetic acid (VIA) is used as an alternative screening method because it is easier, cheaper and effectiveness is not much different from the Pap test. The most participant in urban and rural areas was the age group 31-40 years old, low parity, and first intercourse at ≥ 17 years old. There was asscociation between cervical cancer screening coverage with education level, knowledge and distance of service facilities. There were significant difference of education level (p = 0.000), knowledge (p = 0.000) and distance of service facilities of urban and rural participant (p = 0.000). There was no significant differences in family support of urban and rural participant (p = 0.224).

Abstract

Schizophrenia is a common health problem in the world, including in Indonesia.Polymorphism of gene disrupted in schizophrenia 1 (DISC1) Leu607Phe is allegedly relatedto the predisposition to schizophrenia. However, studies on the relationship betweenpolymorphism of DISC1 Leu607Phe and schizophrenia in various etnics provided differentresults. The purpose of this study was to determine the frequency of DISC1 Leu607Phegenepolymorphism and its association with treatment response in patients with schizophreniaat Department of Psychiatry, Dr. Soetomo General Hospital Surabaya. In this study, thenumber of male patients with schizophrenia was more than that of the female patients.The mean age of male patients with schizophenia was lower than that of the femalepatients. Schizophrenia patients were primarily came from Javanese ethnic with positiveand negative symptom score (PANSS) lower in male patients than that in the femalepatients. In conclusion, no DISC1 gene polymorphism at codon 607 is observed inschizophrenia patients at Dr. Soetomo General Hospital Surabaya, but the G nucleotidevariation at the number 196.339 in intron regions is found instead.

Abstract

Breast reconstruction is such a unique procedure that might potentially be adjustedin line with time and all advances in technology and instruments (endoscopy,silicone implants, alloderm grafting). In addition, it adapts in accordance to better,more sophisticated knowledge of breast anatomy (vascularization, perforator, andinnervation), breast subunits, and systemic changes. Surgical techniques and thecurrently available list of materials might be utilized to reconstruct the breast insuch a way to yield cosmetic satisfaction. Combination and modification of thosetechniques might be adjusted to the patients’ requests without crossing over thepaths of the disease being corrected (cancer, benign tumor, infection, or otherbreast anomalies).Breast reconstruction’s prerequisite is the absence of cancer approximately 1cm from the edge of incision, adjusted by shape and size of breasts, and sizeof cancer. Several techniques might be applied to conserve and reconstruct thebreasts when the disease has been diagnosed. Oncoplasty applies all techniquesavailable to reaffirm the principles of oncology, by increasing the distance fromthe edges of cancer and proceeding with reconstruction by reduction/mastopexy(volume displacement), or adjacent/distant flaps (volume replacement). Thedisadvantage of BCS/BCT is the short distance from the edge of incision to thetumor due to the risk of post-operative breast deformity, especially when surgeryis followed by adjuvant radiation.

Abstract

This study aimed to investigate the association between human leukocyte antigen-B(HLA-B) alleles and nevirapine allergy in HIV patients in Indonesia. A case control studywas conducted involving 147 HIV patients comprising of 50 patients with and 97patients without nevirapine allergy as control. The HLA-B allele typing was conducted byusing polymerase chain reaction-sequence specific oligonucleotide probes (PCR-SSOP),followed by sequencing. Bivariate analysis using Chi-square (X2) test and multivariatelogistic regression with significance level at p<0.05 were applied to analysis the data.Among 147 subjects, 34 with HLA-B alleles were identified. Bivariable analysis showedthat HLA-B*58 allele was the most significant risk factor for the nevirapine allergy (OR= 3.67; 95% CI: 1.79 to 7.54), while HLA-B*35 allele was a protective factor (OR =0.18; 95% CI: 0.08 to 0.42). Multivariate logistic regression analysis showed that youngmen and HLA-B*58 allele were the significant risk factors of nevirapine allergy (OR: 4.63;95% CI: 2.02 to 10.61), while older women with the HLA-B*35 was able to reducethe risk of nevirapine allergy approximately 81% (OR: 0.19; 95% CI: 0.08 to 0.49). Inconclusion, young male with the HLA-B*58 allele are the high risk factor for nevirapineallergy in Indonesian HIV patients.
