Background: Depression is the strongest predictor of quality of life for Parkinson’s disease (PD).Presentation of Case: A 56 years old man came with the main complaint of difficulty sleeping due tothinking about tremors in his left hand since 6 months. Complaints are accompanied by feelings of sadness,lack of enthusiasm, and decreased appetite accompanied by weight loss. The patient had been diagnosedwith PD 8 years previously and was on medication. One month before the visit, the patient underwent athalamotomy operation. After surgery, the patient received therapy in the form of a combination tabletof levodopa/carbidopa/entacapaone (100/25/200 mg) 4×1 tablet and 1×2 mg of ropirinole. On the BeckDepression Inventory (BDI) examination, a score of 23 was obtained and on the Montgomery AsbergDepression Rating Scale (MADRS) examination a score of 31 was obtained, both of which correspond tomajor depression. The patient was diagnosed with a major depressive episode without psychotic symptomsand was given escitalopram therapy 1×10 mg and lorazepam 1×2 mg. Patients also received biofeedbacktherapy with family psychoeducation and supportive psychotherapy. At the time of control, the patientadmitted to feeling calmer and able to sleep after receiving therapy. Conclusion: Escitalopram has goodefficacy for the management of depression in PD patients.

Although Bipolar Disorder (BD) is a common mental illness worldwide (1-3%), there was no data about the prevalence of BD or bipolar spectrum disorder in Indonesia. This study aimed to screen bipolar disorders in various communities Surabaya and its variations of symptoms characteristics. Through a cross-sectional design and non-random sampling survey (N = 1,104) was conducted using the Mood Disorder Questionnaire (MDQ), a screening tool for BDs, and validated self-report instrument. The SPSS 17.0 and chi square was used for analysis. Results showed the lifetime proportion of MDQ positive was 10.7% (n = 118). The proportions of MDQ positive by gender were 4.8% males and 5.9% females, no gender (p = .444) and educational background differences (p = .470). The highest proportions of MDQ positive were 4.4% in the 25-60 year group, 4% having an education level of senior high school and 6.7% having unmarried status. Among participants who had MDQ positive, 22% had an awareness of having psychological problems, unfortunately only 5.9% had visited a medical professional. Overall, the lifetime proportion of suspected bipolar disorder spectrum in Surabaya was higher than that reported in other studies. Conducting a periodic research regarding other psychosocial-cultural backgrounds will help clinicians and government identify the exact prevalence of bipolar disorder in the society and their risk factors. Furthermore, it will help to prevent the increased rate of bipolar disorders.

Schizophrenia is a common health problem in the world, including in Indonesia.Polymorphism of gene disrupted in schizophrenia 1 (DISC1) Leu607Phe is allegedly relatedto the predisposition to schizophrenia. However, studies on the relationship betweenpolymorphism of DISC1 Leu607Phe and schizophrenia in various etnics provided differentresults. The purpose of this study was to determine the frequency of DISC1 Leu607Phegenepolymorphism and its association with treatment response in patients with schizophreniaat Department of Psychiatry, Dr. Soetomo General Hospital Surabaya. In this study, thenumber of male patients with schizophrenia was more than that of the female patients.The mean age of male patients with schizophenia was lower than that of the femalepatients. Schizophrenia patients were primarily came from Javanese ethnic with positiveand negative symptom score (PANSS) lower in male patients than that in the femalepatients. In conclusion, no DISC1 gene polymorphism at codon 607 is observed inschizophrenia patients at Dr. Soetomo General Hospital Surabaya, but the G nucleotidevariation at the number 196.339 in intron regions is found instead.

Introduction: This research is to determine the profile of parenting style with tendency of hedonism lifestyle in university students of Medicine Program Faculty Of Medicine Airlangga University Batch 2015.Methods: This study is a observational analytic research with cross-sectional time and used questionnare as research instrument given to 200 respondentsResults: : 16 university students raised with authoritarian parenting style (8.30%), 169 students raised with authoritative parenting style (87.56%), and 8 students raised with permissive parenting style (4.14%). Students with tendency of hedonism lifestyle in category low is 45 students (23.32%), in category intermediate is 146 students (75.64%), and in category high is 2 students (1.04%). Tendency of hedonism lifestyle is low in 2 students raised with authoritarian parenting style (1.04%), 42 students raised with authoritative parenting style (21.76%), and 1 students raised with permissive parenting style (0.52%). Tendency of hedonism lifestyle in category intermediate has 12 students raised with authoritarian parenting style (6.22%), 127 students raised with authoritative parenting style (65.80%), and 7 students raised with permissive parenting style (3.62%). In category high tendency of hedonism lifestyle is 2 students raised with authoritarian parenting style (1.04%). Using Kruskal-Willis test, it was found that p = 0.123 .Conclusion: Most parenting style used in the students is authoritative. Category of tendency of hedonism lifestyle in most students is category intermediate. Tendency of hedonism lifestyle in category low has most students raised with authoritative parenting style. Tendency of hedonism lifestyle in category intermediate has most students raised with authoritative parenting style. Tendency of hedonism lifestyle category high has most students raised with authoritarian parenting style. And it was found that there is no significant difference in proportion in parenting style with the tendency of hedonism lifestyle with value of p = 0.123 with α = 0.05 (p>α)

Fobia ketinggian/acrophobia tergolong sebagai fobia spesifik yang didefinisikan sebagai ketakutan terhadap ketinggian yang ekstrem. Acrophobia sering dihubungkan dengan penghindaran bermacam-macam situasi yang berhubungan dengan ketinggian misalnya, perjalanan dengan menggunakan pesawat terbang, menaiki tangga, menaiki lift, jembatan dan menghindari bekerja di gedung yang berlantai tinggi. Exposure based therapy merupakan salah satu terapi pilihan yang terbukti efektif selama beberapa dekade untuk menurunkan kecemasan, distress dan hendaya fungsional atau okupasional yang diakibatkan oleh penghindaran obyek atau situasi fobia.

Banyaknya layanan kesehatan yang tersedia di Indonesia dan beberapa negara berkembang lainnya, tetapi kehadirannya tidak dapat mencakup semua penduduk jiwa karena berbagai kendala membuat sebagian masyarakat memilih upaya penyembuhan tradisional untuk mencari pertolongan. Sulitnya pengukuran dan luasnya pandangan terhadap masing-masing pelaku studi, membuat peran penyembuh tradisional tidak banyak disorot dalam perlakuannya terhadap kesehatan masyarakat, sehingga belum diketahui pasti hasil intervensi terhadap masyarakat pada kasus gangguan jiwa. Pengetahuan akan peran penyembuh tradisional pada gangguan jiwa berat merupakan hal yang penting untuk ditelaah lebih lanjut karena berdampak pada upaya kesehatan jiwa masyarakat yang optimal, atau sebaliknya.

Background: Schizophrenia is a chronic and serious clinical syndrome with very aggravating psychopathology involving perception, cognition, emotion, and behaviour. Currently, cognitive dysfunction is seen as a core disorder of schizophrenia. Duration of Untreated Psychosis (DUP) refers to the period from the onset of psychotic symptoms to the first adequate administration of antipsychotics.Aims: This review aims to analyse the impact of DUP on cognitive function in schizophrenia.Methods: PubMed and Google Scholar were searched using the following keyword: (cognitive function) and (neurodevelopmental OR neurotoxicity hypothesis) and (duration of untreated psychosis or DUP) and (schizophrenia or psychosis or psychotic) using the journal publication filter for the 2014-2020 issue. We also used textbooks published in the last 10 years and were related to writing themes.Review: There are two different opinions about the impact of DUP on the cognitive function of schizophrenic patients: (1) the neurodevelopmental hypothesis says there is no impact of the length of DUP on the cognitive function of schizophrenic patients; (2) the neurotoxicity hypothesis says the length of the DUP will impact the patient's cognitive function. Despite differences of opinion about the impact of DUP on cognitive function in schizophrenia, early intervention in schizophrenic patients is important because DUP is associated with worse general disease symptoms, lower likelihood of remission, more severe positive and negative symptoms, and worse social functioning and overall outcomes.Summary: There are differences of opinion about the impact of DUP on cognitive function in schizophrenic patients.

Masalah utama yang belum teratasi di Indonesia adalah masalah gizi, diantaranya adalah anak kurus dan gemuk. Pada tahun 2017, kejadian anak kurus dan gemuk di Indonesia masing-masing sebesar 12.8% dan 4.6%. Pengetahuan ibu berkaitan erat dengan masalah gizi pada anak, semakin tinggi tingkat pengetahuan ibu maka akan semakin baik status gizi anak. Berdasarkan beberapa penelitian masih didapatkan hasil yang berbeda antara hubungan pengetahuan ibu dengan status gizi anak berdasarkan indeks BB/TB. Penelitian ini bertujuan untuk menentukan hubungan pengetahuan ibu dengan status gizi anak berdasarkan indeks BB/TB. Penelitian ini menggunakan observasi analitik dengan pendekatan cross sectional. Jumlah sampel sebanyak 94 ibu beserta anaknya yang diambil secara random sampling berdasarkan data sekunder yang diperoleh dari puskesmas. Selanjutnya data yang terkumpul akan dianalisis secara univariat menggunakan distribusi frekuensi. Hasil dari penelitian ini adalah status gizi anak terbanyak (79.79%) adalah status gizi normal dan pengetahuan ibu terbanyak (52.13%) adalah baik. Pada ibu dengan pengetahuan yang baik terbanyak memiliki anak dengan status gizi normal (83.7%). Pengetahuan ibu tidak berhubungan (p>0.05) dengan status gizi anak berdasarkan indeks BB/TB. Dapat disimpulkan bahwa pengetahuan ibu tidak berhubungan dengan status gizi anak berdasarkan indeks BB/TB.

Introductions: In Indonesia, a median of 6.1% of people diagnosed with depression disorder are people over 15 years old. Only 9% of that amount underwent medical treatment, while the rest, 91%, did not undergo treatment for their depressive conditions. Inadequate and inappropriate treatment of depression will lead to Treatment-Resistant Depression (TRD). Using ketamine as a pharmacotherapy opens up new possibilities for TRD treatments. Methods: This study uses a retrospective observational study design with a systematic review approach, in which all variable data were collected from previous studies aimed at measuring the effectiveness of ketamine pharmacological therapy in patients diagnosed with treatment-resistant depression (TRD) using placebo as a benchmark of the effectiveness of ketamine in reduced clinical symptoms of TRD using secondary data in the form of study results and analyzes from published studies of the effectiveness of ketamine therapy. Results: Administration of ketamine at doses of 0.4 mg/kg and 0.5 mg/kg is more effective as an antidepressant compared to placebo in adults and is effective in the elderly at doses above 0.2 mg/kg with a maximal effect at 24 hours post-administration and disappeared by about 7 days post-administration. Conclusions: The administration of ketamine therapy is more effective at reducing depressive symptoms in diagnosed patients (TRD) than the use of placebo and repeated administration of ketamine can increase the likelihood that TRD sufferers respond to therapy and experience remission.

Introduction: The prevalence of tumors/cancer in Indonesia is increasing. Many cases of cancer at an advanced stage experience severe symptoms and cause suffering that has not been handled properly. Palliative care helps a cancer survivor and his or her family to live a more comfortable life to have a better quality of life. Methods: For this reason, a team that works in an integrated manner includes both medical and non-medical personnel, including volunteers. The role of volunteers in palliative care teams varies according to need and can be involved in hospital care, or at home, expected to bridge between health care institutions and patients. Psychiatrists at Dr. Soetomo General Hospital Surabaya and their residents try to give four sessions of volunteer training to raise awareness, provide health education, or even provide some simple type of medical care and work as counselors. Results: After giving four training sessions every week, the ability of palliative teams had increased. They have the capability to communicate more effectively and efficiently, which is therapeutic, so it is necessary to provide them with counseling skills to listen to, understand, and respond positively when communicating with cancer patients and their families.
