Abstract

Background: Acute coronary syndrome (ACS) becomes an important disease, because of its increased prevalence and mortality rates. Makers of systemic inflammation, including interleukin-6 and monocyte count, have been shown to predict future cardiovascular events. However, the association between these inflammatory markers with cardiac injury is still a controversy.Objective: To evaluate the relationship between IL-6 and monocyte count as inflammation markers and troponin I as cardiac injury.Methods: This was a cross-sectional study. Subjects were patients suspected of ACS admitted to the emergency department of Sardjito Hospital Yogyakarta. Diagnoses were performed based on WHO criteria. Troponin I and interleukin-6 levels were measured from sera using sandwich ELISA method (Roche). Monocyte counts were determined by automatic hematology analyzer (Sysmex). Kruskal-Wallis test and Spearman's correlation test were used to assess the difference and correlation between parameters.Results: Sixty one (61) subjects were included in the study. Age of the subjects were 51-60 years, and diagnosed as ST elevation myocardial infarct (STEMI). Significant differences of IL-6 and troponin I levels were determined among unstable angina pectoris (UAP), non-ST elevation myocardial infarct (NSTEMI), and ST elevation myocardial infarct (STEMI) subjects. There was a moderate correlation (r = 0.54; p = 0.001) between troponin I and Interleukin-6, but not with monocyte count.Conclusion: Interleukin-6, but not monocyte count, was correlated with troponin I in acute coronary syndromes.Key words: acute coronary syndrome - interleukin-6 - monocyte count - troponin I

Abstract

A discussion of the impact -of abnormal labor on the neonatal morbidity and mortality with special reference to birth trauma and perinatal mortality has been carried out. This report is a literature review of cases found in Sardjito Hospital and an illustration of obstetric cases during the first semester of 1987. The aim of this paper is to find out how far the negative impact of abhor.. LI labor on birth trauma and perinatal mortality is, as well as preventive measures to be taken.It is obvious that abnormal vaginal deliveries give rise to an increase incidence of birth trauma compared to spontaneous deliveries. The higher the birth weight, the higher the incidence of birth trauma. Caput succedaneum is the most frequent type of birth trauma. whereas broken bone is the least frequent.Transverse position is the most unfavorable position of the fetus, giving the perinatal mortality rate (PMR) 775/1000 deliveries. The second one is breech presentation, having the PMR 129/1000 deliveries. The very unfavorable obstetric operatives are vacuum and forcipal extraction and internal podalic version which altogether bring about the PMR 210.5/1000 deliveries. Prolonged labor gives rise to the PMR 21/2 times higher compared to non-prolonged labor. Although the Cesarean rate is quite high (12.5%), its PMR is still higher. This signifies that Cesarean section has not been adequately accomplished.Some efforts to reduce the PMR in this hospital are (1) expecting the high risk pregnant women especially during labor to be referred as soon as possible, (2) fast and appropriate management of such cases with sophisticated instruments provided, and (3) providing an intensive neonatal care.Key Words: high risk pregnancy -- abnormal labor and delivery birth trauma -- perinatal mortality rate -- neonatal care

Abstract

Background: Postmenopausal women tend to increase the body fat percentage. A decline in estrogen level cause decrease in lipolytic process.Objective: To know the effect of moderate intensity physical exercise on body fat percentage in elderly women.Method: Pretest and posttest control design. A total amount of thirty, healthy women, 60-70 years old, was divided into three groups. The first group was treated by exercise three times a week, the second group was treated by exercise two times a week and the third group was a control group. The exercise used was D series of fitness dance for elderly created by PERWOSI Yogyakarta which is a low impact aerobic dance with moderate intensity (70-79 % maximal heart rate), 40-54 minutes/day for eight weeks.Result: The result showed that the first group revealed a significantly decrease in body fat percentage (p< 0.05). The second group revealed decrease no statistically significant in body fat percentage (p> 0.05), however there was a significant correlation between the duration of exercise and the body fat percentage (r = -0.9042).Conclusion: The low impact aerobic dance with moderate intensity, 40-54 minutes/day, three times a week, for eight weeks decreased body fat percentage.Key words: moderate intensity low impact aerobic dance - excercise frequency - body fat percentage -elderly women

Abstract

Background: Previous studies showed that both systolic and diastolic blood pressures could be lowered to a modest degree with an aerobic type exercise.Objective: The present study was designed to explore the effect of resistance exercise training program on resting heart rate and blood pressure in males and females.Methods: Thirty-three normal healthy subjects participated in this study. Resting heart rate and resting blood pressure were measured before and after a weight-training program twice a week for eight weeks. Results: Data showed that resting heart rate was significantly (p) lower in males compared to that observed in females before and after the training program. A significant decrease in resting heart rate (p) was observed in both sexes as a result of the training program. Resting systolic and diastolic blood pressures were slightly (p = 0.08) lower in the female group compared to that observed in the male group before the training program. Resistance exercise training induced no significant change in resting systolic and diastolic blood pressures in both groups. After the training program, resting systolic and diastolic blood pressures were slightly (p=0.09) lower in the female group compared to that observed in the male group.Conclusion: Exercise stimulus in the form of circuit weight training was sufficient to elicit cardiovascular adaptations in response to training.Key words: resistance exercise training - heart rate - systolic and diastolic blood pressures -cardiovascular adaptations

Abstract

The role of chronic tissue hypoxia in the keloid patho-mechanism has been widely accepted. Whereas, the pulsed-light heat energy (LHE) has been developed which has the capacity to generate reactive oxygen species on exposed skin. Although the supra keloidal flap technique has a high recurrence rate, it was used because of its capacity to prevent suturing hypoxia, thereby the formation of lager recurrent keloid after surgery.The combination of supra keloidal flap and pulsed light heat energy was done I the treatment of postvaricella keloid on the right ear lobe of a 9 year old girl. The keloid was excised two times a year ago, but observation one month after the surgery showed a recurrent larger keloid. Abstract

Objective: The aim of this study was to know the knowledge of Junior High School teacher upon adolescent reproductive health and the influencing factors.Materials and Methods: The study population is the teachers of Junior High Schools in Yogyakarta Municipality, 280 teachers were enrolled. Data on the characteristics and the knowledge upon the adolescent reproductive health were collected by questionnaire.Results: The teachers knowledge on the maturation of the female reproductive showed a statistical difference between the teacher group involved in the reproductive health teaching and the univolved group (p = 0.0004); the knowledge on the maturation of the male reproduction showed a statistical difference between the teacher group who had been in charge for more than 20 years compared to less than 20 year teacher (p = 0.015), likewise between the teacher group involved in the reproductive health teaching compared to the uninvolved (p = 0.0005); on the woman fertile period only 50% of the teachers had correct knowledge and there was a statistical difference between the teacher group involved in the reproductive health teaching and uninvolved group (p = 0.035). The score of the teacher's knowledge that a woman can become pregnant by a single sexual intercouse was inadequate (about 60% of full score) and there was a statistical difference between teacher group involved in the reproductive health teaching and the uninvolved group (p = 0.035); the score of the teacher's knowledge that a woman can become pregnant by interrupted sexual intercouse was inadequate (about 50% of full score) and there was a statistical difference between female and male teachers (p = 0.01), between the teacher group who had been in charge for more than 20 years compared to less than 20 year group (p=0.025). The teacher's knowledge that woman cannot become pregnant by vaginal rinsing soon after a sexual intercouse was inadequate (about 60% of full score) and the teacher group involved in the Religion, Biology and Physical Health teaching was less than the other group and there was a statistical difference (p = 0.037), likewise between the teacher group who had been in charge for more than 20 years compared to less than 20 year group (p = 0.019), between teacher group involved in the reproductive health teaching and the uninvolved group there was a statistical (p = 0.005). Concerning the knowledge on Herpes infection there was a statistical difference between teacher group involved in the reproductive health teaching and the uninvolved group (p = 0.007); concerning the teacher's behaviour on reproductive discussion with the students, the teachers involved in of Religion, Biology and Physical Health teaching or involved in guidance and councelling performed discussion more frequently compared to the other groups and a statistical differences was found (p= 0.00005). It was questionable that 64.8% teachers who were involved in the reproductive health teaching never discussed about the matter with their students. The teacher's knowledge on some diseases that can be transmitted by sexual activities was variable: on AIDS and Syphillis nearly 70% teachers knew, on Chlamydia 39.8% and on Herpes less than 15%. Logistic regression analysis to find the influencing factor on the teacher's knowledge showed: the involvement in the reproductive health teaching had influence on the knowledge upon the maturation of female and male sexual development more than the length of teaching, it influenced also the behavior to discuss sexual matter to the students. Gender and length of teaching did not influence the teacher's knowledge whether a woman can become pregnant by interupted sexual intercouse; the subject of teaching and the involvement that a woman will not become pregnant by rinsing their sexual organ soon after a sexual intercouse.Conclusion: The knowledge and behaviour or the Junior High School teacher's in Yogyakarta were inad- equate on some important issues and influenced especially by the subject teaching.Key word : adolescent reproductive health knowledge - Junior High School Teacher - influencing factors

Abstract

Background: The tuberculosis control efforts have been alarmed by trend of the widespread emergence of multidrugs resistant Mycobacterium tuberculosis. To anticipate the _problem it is very important to provide clinicians with data on drugs susceptibility of M. tuberculosis periodically.Objectives: This study reviewed the pattern of susceptibility of M. tuberculosis to various antituberculosis drugs, for monitoring of antituberculosis drugs resistant strains of M. tuberculosis, especially in Yogyakarta. Method: Eighty four isolates of M. tuberculosis - collection of the Laboratory of Microbiology Faculty of Medicine Gadjah Mada University - were evaluated for their susceptibility to various drugs, both the first line drugs (INH, streptomycin, rifampicin, and ethambutol), and the second line drugs (kanamycin). Suspension of M. tuberculosis at density of approximately Mac Farland no. 1 standard (108cfu/m1) were cultured on drugs containing medium as well as control medium of Midlebrook 7H10 and incubated for 3 weeks. The number of colony on the drugs containing medium and control medium was reported.Result: The results showed that range of resistance of M. tuberculosis to the overall drugs were 16% to 62%. Resistance to INH .was the lowest (16.66%), whereas the resistant isolates to streptomycin, ethambutol, rifampicin and kanamycin were 27.38%, 34.21 %, 62.50% and 45.78% respectively.Conclusion: 88.10% of isolates were resistant to one or more antituberculosis, 3.58% were resistant to all of the drugs, and only 11.90% of the isolates still sensitive to all of the drugs.Key words: Mycobacterium tuberculosis - resistance - antituberculosis
