Abstract

In the case of Diabetes Mellitus, it will trigger some changes in lipid metabolism and lipoprotein composition, one of them is cholesterol. Cholesterol is a lipid that plays an important role in the structure of cell membranes. Seaweed contains bioactive components of flavonoids which reduce cholesterol levels. Brown seaweed contains bioactive components of alkaloids, flavonoids, triterpenoids, saponins, phenol hydroquinone, tannins, chlorophyll and β-carotene pigments. The bioactive component of brown seaweed also has strong antioxidant activity that can prevent damage caused by oxidative stress on pancreatic β cell function and acts as a trap for various free radicals and can reduce diabetic rat lipid peroxidation. This study to analyze the effect of giving brown seaweed extract on cholesterol levels in diabetic mice. This study was divided into 4 sample groups, namely the normal group, negative controls (diabetic mice), positive controls (diabetic mice + 1.3 mg / 20 gr BB metformin), K1 (diabetic mice + brown seaweed extract). The seaweed dose of 10mg / 20grBB is given on a daily basis for 21 days. Checking blood cholesterol levels in serum using the POD CHOD method and measured using biolysers. Diabetic mice with brown seaweed extract were significantly (p <0.05) lower cholesterol level of 68.94 mg / dL than diabetic mice 81.75 mg / dL. It was concluded that brown seaweed extract had an effect on reducing total cholesterol levels in diabetic mice. Keywords : Cholesterol levels, Diabetic mice, Brown Seaweed

Abstract

Various reports about service quality to social health insurance (Jamkesmas) participants pointed out that the given service to social health insurance was not compatible with the program’s purpose. As health professionals whom always keep patients company, nurses are required to be able to give primary care to all patients. Advocacy is one of nurse’s roles, in carrying out this role, nurses are helping patients making decision instead of making the decision for patients. This study is a qualitative study with phenomenological approach. Samples are acquired by purposive sampling method with the result 4 samples out of population. Data was obtained by interview using an interview guide as well as field notes. This data was analyzed with Colaizzi method. Results of study give rise to ten research themes which conclude a description of the particular goals. Those themes are the role of nurses in the inpatient admission procedure, provide information about the procedures of action, provide a suitable environment to the condition of the patient, assisting other health professionals, accompany the patient to undergo a medical examination, the type of actions taken, the role of nurses as patient managers, the nurse's role in planning after discharge, and JAMKESMAS service system. The nurses as care giver in Paru Hospital Jember should consider to the advocation role in all their actions. Keywords : patients’ perception, nurse advocation role

Abstract

Diabetes Mellitus (DM) is a chronic disease that requires a continous self-care. A low level ofknowledge about self-care can worsen health condition and cause diabetes distress. The continuingstress can cause hyperglycemia that lead to complications of DM. DSME/S is a structured healtheducation that facilitates DM patients in implementing and maintaining their behavior for sustainableself-care. This research aimed to analyze the effect of DSME/S on diabetes distress in patients withtype 2 DM. This research was quasi experimental with non randomized control group pretestposttest design. Thirty respondents in this study were divided into 15 respondents as theintervention group and 15 respondents as the control group by consequtive sampling technique.DSME/S was conducted in 6 sessions: 4 sessions in the hospital and 2 sessions at respondent'shouse. The data were analyzed by using dependent t test and independent t test with significancelevel of 0.05. The result of dependent t test revealed a significant difference of diabetes distressbetween pretest and posttest both in the intervention group (p=0.001) and in the control group(p=0.046). Furthermore, independent t test showed a significant difference between interventiongroup and control group (p=0.001). The decrease of diabetes distress was higher in the interventiongroup than control group. This result indicates that there was a significant effect of DSME/S ondiabetes distress in patients with type 2 DM. Nurses are expected to apply DSME/S as a dischargeplanning to reduce diabetes distress in patients with type 2 DM.Keywords: diabetes distress, DSME/S, Diabetes Mellitus

Abstract

Metformin hydrochloride (MH) is a first line therapy in the treatment of noninsulin-dependent diabetes mellitus. However, MH has a very short half-life, relatively low bioavailability, and gastrointestinal adverse effects. Controlled released formulations can overcome this problem by increasing gastro retention time (GRT). Drug delivery systems aimed at improving GRT can be designed in a single units (eg, tablets) and multiple unit (eg, microspheres). Stirring speed and time affect several aspects such as shape, size, and particle size distribution, the release rate, and entrapment efficiency (EE) of the microspheres.. This research was aimed to find the stirring speed and time to produce MH microspheres having high EE. The result showed that the microspheres stirred at 1,000 rpm for 2 hours produced the highest EE, 84.792 ± 0.975 %; had the drug loading of 12.067 ± 0.0649 %; and yield of 99.44 ± 0.123 % with the particle size 157.5 ± 1.007 μm, has the spheres shape and a relatively smooth and bright surface morphology. FTIR analysis results indicated that there were no changes in the functional groups on MH as an active ingredient.
