Abstract

The eye is an organ that has many functions and is a vital organ in the body. The eyes have a part called photoreceptor which functions as light detector that will be stream to the retina of eyes according to its function. Based on data obtained from WHO, around 217 million people in the world have visual impairments. Refractive errors have several ways to correct them, one of which is using LASIK or with Refractive Lenticule Extraction, Small Incision Lenticule Extraction (RELEX SMILE). In a study conducted at the Eye Center Hospital, Hangzhou, Zhejiang Province, China in 2015, the incidence of dry eye complaints after RELEX SMILE surgery was 11% - 24%. A hospital in Semarang, reported that the incidence of dry eye complaints after RELEX SMILE surgery was 12%. This research is a Literature review which is a type of research that gathers theories that are relevant to the material that is currently researched, namely regarding dry eye syndrome. Researchers collected a number of articles from several databases like MEDLINE/PubMed, Google Scholar, and national also international journals. The result of the study found several complications in the procedure for healing eye refractive errors using RELEX SMILE, one of which is dry eyes. Dry eyes after RELEX SMILE and LASIK surgery is caused by damage to the sensory nerves in the cornea which may be caused by incisions during surgery. The incidence of dry eye after RELEX SMILE surgery can be prevented by a thorough examination by looking for signs and symptoms of dry eyes, or using drops without preservatives to reduce the impact of dry eyes.

Abstract

According to the Brain Injury Association of America, a head injury is damage to the head, not congenital or degenerative, but caused by an external physical attack or impact, which can reduce or change consciousness which can cause damage to cognitive abilities and physical function. Traffic accidents are the main cause of head trauma or head injuries. A 23 year old man came to the emergency room at Jendral Ahmad Yani Metro Hospital accompanied by his family with complaints of a single post-KLL headache in the Pekalongan area, East Lampung on August 20 2023, because he hit a monument and fell from a motorbike without wearing a helmet. History of fainting for 30 minutes, vomiting without spraying 3x after waking up. Complaints accompanied by difficulty opening the left eye, swelling and pain. On physical examination, vulnus excoriation, vulnuslaceratum and vulnuscontussum were found. The diagnosis of moderate head injury (CKS) accompanied by secondary injury and sinistra oculi hematoma can be confirmed by history taking, physical examination, neurological examination, and supporting examinations in the form of laboratory examinations, CT scans and MRI. Management in this case includes primary survey, secondary survey, observation of general condition and vital signs. The patient received further treatment in the Neurology ward for 3 days.

Abstract

Stroke is a functional disorder of the brain that occurs suddenly with clinical signs and symptoms, both focal and global, that last more than 24 hours, or can cause death, caused by impaired cerebral blood circulation. Hemorrhagic stroke is a rupture of a brain blood vessel causing blood to leak into the brain parenchymal tissue, cerebrospinal fluid space around the brain or a combination of both. A 62 year old man came to the emergency room at Jendral Ahmad Yani Metro Hospital accompanied by his family with complaints of sudden loss of consciousness since 3 hours ago. Previously, the patient complained of severe, sudden headaches, which did not decrease at rest or after taking headache medication, vomited twice, vomited without nausea, and finally the patient fainted. Vomiting contains liquid with food. The patient has a history of uncontrolled hypertension since 10 years ago. Brain pons bleeding was found with an estimated bleeding volume of +1.75 ml, but the family refused surgery. The patient's general condition continued to decline so that after 3 days of treatment in the ward of the Department of Neurology, the patient was declared dead.

Abstract

Stroke is defined as a neurological deficit caused by an acute focal injury to the central nervous system, whether in the brain, retina, or spinal cord, caused by blood vessels. More than 13.7 million people suffer strokes every year, and around 70% of strokes are ischemic strokes. A 66-year-old man came to the RSAM emergency room with complaints of decreased consciousness since 3 hours of SMRS and weakness of the left limb. Physical examination revealed a Babinsky reflex (-/+), and a CT scan of the head without contrast showed thromboembolism in the right middle cerebral artery. The patient was treated with thrombolysis and medical therapy. The patient was discharged after one week of hospitalization and then scheduled for re-control at the polyclinic, and the patient's National Institute of Health Stroke Score (NIHSS) score was assessed. A total score of 5 was obtained with the interpretation of mild neurological deficits, which concluded that the patient's clinical condition had improved. Establishing diagnosis and therapy as early as possible is important for the key prognosis of ischemic stroke. Thrombolysis is the right choice as a treatment for ischemic stroke in patients who meet the inclusion criteria.

Abstract

Depression can be caused by a variety of factors, including genetics, mental health, personality, mental health, unemployment, and intelligence. Another common cause of depression in children is lack of family support and proper encouragement. The problem solving process is a useful skill because it is a process of managing mental illness (both external and internal) and requires real effort and effort in the form of suppressing emotions. The problem solving process is the body's response to change, the need to solve problems, and internal and external damage to the mind. Basically, parents have a big role in controlling the growth and development of each child from birth to adulthood. Therefore, parents who ignore their children's needs, especially if the child experiences mental disorders (depression), leave the child empty and without support. Parents can play an advocacy role if they know what to do, including understanding the problems their children face, managing family relationships, and making contributions. a person's level of self-confidence. Mental illness is not something to be ashamed of or hidden, but it is an illness that requires proper treatment. Just like wounds on the hands and feet, wounds in the heart must also be healed. Parents promote good career opportunities for teenagers. The higher the parents' education, the easier it is to help children who are experiencing depression. Supporting children with depression is more than just raising awareness.

Abstract

Geographically, Indonesia is in the tectonic ring, where 3 tectonic plates meet, including the Indo-Austalian Plate, the Eurasian Plate and the Pacific Plate. The meeting of these 3 plates carries the risk of an underwater earthquake causing a tsunami. Tsunamis are a threat to residential areas in coastal areas. This is the biggest problem for coastal communities because it can cause a lot of casualties and very serious damage to buildings. The largest tsunami that occurred in Indonesia was a tsunami measuring 9.3 SR (Ritcher Scale) in Aceh Province on December 26 2004, which resulted in many fatalities. This article aims to understand and describe pre-tsunami management. This article uses a qualitative approach with discussion using paragraph descriptions of the data. The results of this article explain that there are several efforts that can be made to minimize tsunamis, namely pre-tsunami management consisting of mitigation, preparedness and early detection. According to Law no. 24 of 2007, Disaster management is a dynamic, continuous and integrated process that aims to improve the quality of things related to disaster analysis and observation as well as steps for prevention, mitigation, preparedness, early warning, emergency management, disaster rehabilitation and reconstruction. . Therefore, pre-tsunami management is very important for all communities, especially communities living in areas close to the sea such as beaches and ports.

Abstract

Type 1 diabetes mellitus (DMT1) is a lifelong chronic disorder of glucose homeostasis characterized by autoimmune destruction of insulin-producing pancreatic beta cells, which progressively leads to insulin deficiency and results in hyperglycemia. This disease causes chronic complications that require ongoing treatment management and education for patients and their families. Uncontrolled disease will cause various metabolic complications, macrovascular and microvascular disorders that cause a decrease in the quality and life expectancy of the patient. The purpose of writing this article is to applying the principles of a holistic and comprehensive family doctor approach in detecting internal and external risk factors and solving EBM (Evidence Based Medicine) problems that are family approach and patient centered. This study is a descriptive study of case reports. Primary data were obtained through autoanamnesis and alloanamnesis, physical examination and home visits. Secondary data obtained from the patient's medical record. Patient An.J, aged 17 years, complained of feeling weak and tingling in his legs. The patient has a history of type 1 diabetes on insulin treatment. Patient complaints do not recur and can return to normal activities. Clinically the patient was diagnosed with type 1 diabetes mellitus and diabetic neuropathy. Internal risks are family history of DM, lack of knowledge about type 1 diabetes, high calorie and sugar diet behavior, smoking habits, and alcohol consumption and rarely exercise. External risk with a lack of family knowledge about diabetes mellitus, its triggers and complications faced by the patient. Furthermore, holistic management is carried out, namely non-medical management and mentose medicine with intervention using poster media. In the evaluation, the results obtained in the form of a better understanding of the disease and changes in behavior that have an impact on improving the quality of life. Holistic management with a family medicine approach is needed for patients with type 1 diabetes mellitus with complications of DM neuropathy to identify problems in various aspects in order to support the success of therapy and change patient behavior so as to prevent aggravation of complications.

Abstract

Diabetes mellitus (DM) is a chronic condition with an increasing incidence worldwide. Not only does it occur in adults, but the incidence rate in children is also quite high. Type-1 DM is an autoimmune disease characterized by absolute insulin deficiency due to pancreatic gland cell damage. Although it can occur at any age, type-1 DM most commonly appears in adolescence with a peak onset around puberty. Diabetes mellitus is often caused by genetic factors and a person's lifestyle. Symptoms of Type 1 DM in children are the same as in adults: polyphagia, polydipsia, polyuria and nocturia, weight loss, and about a third suffer from diabetic ketoacidosis. Complications of diabetes can be divided into microvascular and macrovascular. Microvascular complications include nervous system damage (neuropathy), renal system damage (nephropathy) and eye damage (retinopathy), but can also affect cognitive function, heart and other organs. HbA1c screening is recommended every 3 to 6 months. HbA1c levels reflect glycemic control over the previous 2-3 months. The target HbA1c is generally <7.0%. Other laboratory tests include lipid profile, serum creatinine, eGFR, and urine albumin to creatinine ratio. Management of Type 1 DM includes insulin injection, blood sugar monitoring, nutrition, physical activity, and education. The goal of insulin therapy is to ensure that insulin levels in the body are sufficient for 24 hours to meet metabolic needs due to the glycemic effect of food.

Abstract

Gallbladder disease is associated with a malfunctioning gallbladder and highly concentrated bile, leading to conditions such as the formation of gallstones or cholelithiasis. Cholelithiasis can occur due to high concentrations of cholesterol in the gallbladder. Obstruction of the cystic duct, either by stones or functional issues, can cause acute inflammation of the gallbladder. Acute cholecystitis attacks are often associated with gallstones, or cholelithiasis. Common symptoms include right upper quadrant or epigastric abdominal pain, especially after consuming fatty foods, with additional symptoms such as nausea, vomiting, fever, and diarrhea. The development of this disease is believed to involve many factors and likely arises from complex interactions between various genetic and environmental elements. An imbalance in bile composition, characterized by excessive cholesterol secretion, reduced gallbladder motility, and the accumulation of mucin gel, contributes to the formation of gallstones. Gallbladder disease related to cholesterol results from a biochemical imbalance involving lipids and bile salts in the bile stored in the gallbladder. A 27-year-old patient, Mr. Y, has several risk factors that increase the development of the disease, and the following case report discusses the clinical presentation and laboratory examination results related to the onset of this disease. Laparoscopic cholecystectomy was performed on the patient, and appropriate education was provided, illustrating the importance of intra-professional collaboration and positive outcomes for the patient.

Abstract

Gout arthritis is an increase in blood uric acid levels above normal which causes an inflammatory reaction in the joints. Prolonged gouty arthritis can damage the joints, soft tissues and kidneys. Dyslipidemia is a common metabolic condition characterized by a level of cholesterol in the plasma that exceeds normal values. Total cholesterol level is correlated as a strong risk factor for cardiovascular disease. Therefore, application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. so that treatment goals can be achieved, prevent complications that may occur, and improve the patient's quality of life. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. A Female patient, 55 years old, came with complaints of pain in both joints of the hands and feet and the back of the neck felt sore and tense. The patient has a history of high uric acid since the age of 36 years. In the knowledge assessment, the initial score before the intervention was 70 (patient), 50 (patient's family) and after the intervention became 100 (patient), 90 (patient's family). In terms of uric acid levels, uric acid levels decreased from 6.8 mg/dL to 6 mg/dL and blood cholesterol from 221 mg/dL to 188 mg/dL. In the aspect of diet there is a change in the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity, sports 30 minutes 4 times a week. Enforcement of diagnosis and management of these patients has been done holistically, patient centered, family approach and community oriented based on several theories and the latest research. In the process of behavior change, the patient has reached the trial stage.
