Abstract

Tumor markers play an important role in cancer detection and guide patient management. In healthy asymptomatic individuals, biological markers can be used to identify sufferers at high risk of developing malignancy and in screening for early cancer. After diagnosis of malignancy, biomarkers can be used in determining prognosis, prediction of advance therapy, postoperative surveillance and monitoring response to ongoing therapy. Biomarkers are simply defined as components that can be used to distinguish abnormal and normal images, making for better diagnostic measures for lung cancer that facilitate early detection of disease, therefore enabling effective interventions, are needed to reduce lung cancer mortality. in both men and women, lung cancer is the most frequently diagnosed cancer and the leading cause of cancer death. In addition, the incidence rate is increasing in many countries. Many lung cancer patients have a poor prognosis because it is usually diagnosed at an advanced stage. Therefore, there is an urgent need to develop effective methods for early diagnosis of lung cancer. Several systematic reviews have evaluated the value of biomarkers for diagnosing lung cancer. However, it is still unclear which biomarkers have superior performance for early and accurate detection of lung cancer. This literature review presents the potential of biomarker detection which in the future can be developed into major biomarkers that are more beneficial for patients.

Abstract

Hypertension is an abnormal increase in blood pressure which can be a major cause of cardiovascular disease. It is a condition characterized by systolic blood pressure> 140 mmHg and / or diastolic blood pressure> 90 mmHg. The prevalence of hypertension in Indonesia occurs in the age group 31-44 years (31.6%), age 45-54 years (45.3%), age 55-64 years (55.2%). One of the risk factors associated with hypertension is obesity. Every 1 kg of increase in body weight has a HR for suffering from hypertension of 1.36 (95% CI: 1.29-1.45). In addition, based on the results of previous studies, it shows that every kilogram of weight loss can reduce systolic blood pressure by 1.05 mmHG and diastolic by 0.92 mmHg. Hypertension can be controlled by improving modifiable risk factors. This is the reason for comprehensive management of hypertensive patients with risk factors for obesity, in order to prevent serious complications in patients that lead to decreased quality of life of patients both within the family and community.

Abstract

Ocular trauma is one of ophthalmologic emergency cases. Trauma to the eye that is not treated immediately can leads to various eye injuries such as visual impairment that can leads to blindness. Mr. AM, 25 years old was admitted to hospital due to blurry vision on the left eye since 6 days ago after fell from height of 5 meters on his workplace. The patient also complaints pain, difficulty to open the left eye and headache. The ophthalmological status obtained anisocoria pupil, visual acuity was 6/60 on the right eye and 3/60 on the left eye. Examination on the right eye revealed superior and inferior palpebral hematoma and conjunctival injection. Examination on the left eye revealed ptosis, superior palpebral hematoma, and abrasion on inferior palpebral. Examination of the extra ocular movements revealed limit eye movement to superior, superolateral, medial, inferior, inferomedial and inferolateral of the left eye. Funduscopy examination shows normal findings. Orbital CT Scan result showed multiple fractures of medial wall of the orbit, frontal bone, and left temporal bone with left frontal intracerebral hematoma, left temporal subdural hematoma, intrasinus hematoma at maxillary, left ethmoid, sfenoid, and frontal sinus. The treatment given to this patient are bed rest with 30 degrees head elevation and warm compresses and the medication taken are ranitidin 2 x 50 mg and metilprednisolon 4 x 250 mg.

Abstract

Diabetes mellitus (DM) is a group of metabolic disorders characterized by hyperglycemia that occurs as a result abnormalities in insulin secretion, abnormalities in insulin action or both. Risk factors for type 2 diabetes include age and genetic actors. The increase in the prevalence of DM in families with DM compared to the population in general is caused by genetic factors. An increase in the prevalence of DM in families with DM compared to the population in general raises the suspicion that genetic factors play an important role in the etiology of DM. Evidence of genetic heterogeneity as a cause of DM is the presence of various kinds of certain genetic syndromes due to mutations in various genetic loci. Early diagnosis of glucose disorders in blood is carried out by examining the oral glucose tolerance test (TTGO). The design of this study was an observational analytic study with a cross-sectional approach to 40 first-generation respondents with type 2 diabetes. The data were taken in the form of primary data, namely the results of blood tests of first-generation respondents with type 2 diabetes and TTGO value. The results showed that most of the first generation of type 2 diabetes mellitus sufferers aged 30-39 years experienced impaired glucose tolerance (70%) and normal TTGO values ​​were mostly at the age of 20-29 years (71.4%). Abstract

Post-Stroke Depression (PSD) is a common psychiatric symptom after stroke. PSD is a condition in which stroke patientsexperience traumatic events that damage their physical and mental integrity, autonomy and self-esteem, and social values.Lack of psychological coping mechanisms, as well as premorbid personality, are determinants of PSD. PSD patients mostlypresent with fluctuation in mood, retardation, irritability, or apathy. PSD is a major factor that can inhibit the recovery ofneurological function and daily activities in stroke patients, and is associated with increased mortality. A recent meta-analysisshowedthattheprevalenceofPost-StrokeDepression(PSD)was29%.PSDdealswiththedisruptionofmonoamine circuits either directly or indirectly. In cytokine theory, inflammation of PSD may be related to the production of“depressogenic” cytokines by the inflammatory response to ischemia. This may be related given that inflammatorycytokines, such as IL-1, IL-6, IL-18, and tumor necrosis factor (TNF) -alpha increase significantly after stroke. Biological andpsychological conditions cause PSD either due to lesions, biogenic amines, inflammatory cytokines, or geneticpolymorphisms. PSD must be distinguishable from post-stroke apathy, post-stroke anxiety (PSA), post-stroke fatigue (PSF),and post-stroke psychotic disorder (PSPD). The goal of PSD treatment is to achieve complete remission of the symptoms ofa depressive episode, which may have a beneficial impact on the recovery of neurological deficits. Management can bedone with pharmacological and non-pharmacological interventions in the form of neuromodulation and psychosocial. TheThe purpose of this article is to provide information regarding the Post-Stroke Depression (PSD).

Abstract

Bilirubin is a sequence product of red blood cells in the reticule-endothelial system. At excessive concentrations, about 5 mg/dL bilirubin will appear clinically as a yellow color on the skin and mucous membranes which is called jaundice. Jaundice can be physiological and pathological or commonly known as hyperbilirubinemia. Hyperbilirubinemia is a pathological condition that arises at the first day of birth and increasing of bilirubin levels 5 mg/dL per day rapidly, ABO incompatibility is the most common risk factor causes hyperbilirubinemia in newborns. ABO incompatibilities included in the main blood group antigen are blood group A and B which are the most common cases in newborns. This study aims to determine whether there is a relationship between incidences of hyperbilirubinemia with ABO incompatibility in newborns at Abdul Moeloek Hospital, Lampung. This is observational analytic study using cross sectional method. Total sampling method was done. Medical record was data source of this study. Data analysis was performed by chi-square test with p <0.05. The study used 325 medical records with 20% hyperbilirubinemia of newborn, 80% without hyperbilirubinemia, ABO incompatibility incidence is 20.9% and 79.1% without ABO incompatibility. Data analysis showed significant relationship of hipebilirubinemia with ABO incompatibility in newborns at Abdul Moeloek Hospital Lampung (p<0, 05). There study showed relationship between hypebilirubinemia with ABO incompatibility in newborn at Abdul Moeloek Hospital Lampung.

Abstract

Severe preeclampsia is a disorder that occurs at the age of 20 weeks of pregnancy until approaching labor characterized by systolic blood pressure ≥160 mmHg and diastolic blood pressure ≥110 mmHg, proteinuria ≥2+ on dipstick, platelets <100.000/μL, elevated levels of liver enzymes; AST and ALT, oliguria <400 ml/24 hours, creatinine serum >1.2 mg/dl, epigastric pain, pulmonary edema, headaches in the frontal region, diplopia or blurred vision, and retinal bleeding. Pre-survey results in 2016 at H. Abdul Moeloek Regional Hospital in Lampung Province as many as 46 cases of maternal deaths caused by preeclampsia. Delay in handling the patient can cause worsening conditions for the mother and fetus so that many emergency conditions that force labor must be done by sectio caesarea (SC). Sectio caesarea is a method for delivering babies through incisions in the abdomen and uterus. In sectio caesarea, regional spinal anesthesia is generally chosen because it has many advantages. The method of management of spinal anesthesia is obtained by injecting local anesthetic drugs directly into the cerebro spinal fluid in the subarachnoid space. Mrs. L, age 28 years came with complaints of heartburn since 20 hours before being admitted to the hospital accompanied by high blood pressure that appeared since the age of 7 months of pregnancy. Patients performed sectio caesarea with some consideration of previous anesthesia. Good preoperative, intraoperative, and postoperative management will make this caesarean section successful and unobstructed.

Abstract

Total cholesterol levels in body play an important role in the process of degenerative diseases. Total cholesterol levels are affected by age, intake of fiber, fat, carbohydrates and protein, physical activity and body mass index. This study aimed to determine the relationship between fiber intake and body mass index (BMI) with total cholesterol levels in students of Biology Department University of Lampung. This study used analytic-correlative research method with cross sectional approach. Sampling was taken by disproportionate stratified random sampling and obtained 43 respondents, aged 18-22 years. This research was conducted on November 2017 in Biology Department of Matematics and Sains, Faculty University of Lampung. The results showed that the average intake of fiber was 4,20 grams/day; the average of body mass index was 21,719; the average of total cholesterol levels was 180,4 mg/dl. Results showed significant relationship between fiber intake and total cholesterol levels with medium correlation (r=-0,470, p = 0,001) and significant relationship between body mass index and total cholesterol levels with medium correlation (r=0,510 p =0,000). There was relationship between fiber intake and body mass index (BMI) with total cholesterol levels in students of Biology Department University of Lampung.

Abstract

Learning motivation is important in the learning process, especially in a problem-based learning discussion or tutorials that require high learning motivation to achieve learning goals. Lerning motivation in a tutorial is influenced by a variety of factors both internal and external. Learning motivation of final year students decreased, especially during the tutorial process. This study uses a qualitative research design with a phenomenological approach. The main informants in this study were students from the class of 2015 as final year students. Data Collection through Focus Group Discussion (FGD), then the researchers also used in-depth interview techniques as data triangulation. Research data were analyzed by thematic analysis. The results of the study found various factors that influence the motivation of students learning at the final level of Faculty of medicine Lampung University in the tutorial process. We found five internal factors that increase student motivation in the tutorial process, i.e the level of intelligence, ideas, the desire to graduate immediately, the will of oneself, and the ability to communicate well. Then we found five internal factors that decrease student motivation in the tutorial process, i.e decreased health conditions, lazyness, bored, lacking self-readiness. For external factors we found five external factors that can increase or decrease student learning motivation in the tutorial process, i.e facilitators, learning materials, facilities and infrastructure, parental support and learning environtment. Then we found another one external factors that increase student motivation in the tutorial process, i.e scenarios. Then we found two external factors that reduce student motivation in the tutorial process i.e academic burden and the influence of friends.

Abstract

Syringomyelia is a rare condition characterized by fluid-filled cavities called syrinx in spinal cord. Enlarging syrinx often cause progressive neurological damage, through combination of direct pressure on neural tissue and ischemia. Syringomyelia divided into primary and secondary based on it’s cause. More than half of secondary syringomyelia cases are caused by Chiari Malformation (CM) type I. Prevalence of CM is higher in children and younger adult than older adult. Diagnosis is effectively determined by Magnetic Resonance Imaging (MRI) and management are based on the main cause of syringomyelia. In this study, we describe a case in a 59 year-old women pesented with progressive weakness of all extremities for the past three months associated with respiratory failure. In physical examination we found flaccid type tetraplegia and absent of reflexes in all extremities. MRI result found syringomyelia from cervical 2 until 6 with MC type I. Foramen magnum decompression become primary management in syringomyelia caused by CM type I in order to return the abnormal flow of cerebrospinal fluid. Foramen magnum decompression in adults can improve the symptoms with minimal complication.
