Abstract

Intrauterine growth restriction (IUGR) is a baby born at term but has a low birth weight (LBW) due to impaired fetal growth while in the mother's womb. Intrauterine growth restriction (IUGR) is closely related to severe preeclampsia (PEB). In PEB there is interference with placental implantation, thereby reducing blood flow to the fetus. Reduced blood flow causes inadequate nutrition for fetal growth. Intrauterine growth restriction (IUGR) is a fairly common complication, where the prevalence reaches 3-7% of all pregnancies in developing countries. This study is a case report where Mrs. Ni, 41 years old with G3P2A0 34 weeks pregnant came to PONEK with clear discharge and heartburn up to the waist. On the os, it was found that the conjunctiva was anemic and the height of the uterine fundus did not match the gestational age. Laboratory examinations showed hemoglobin 10.4 g/dL, Hematocrit 32%, erythrocytes 3.7 million/ul, proteinuria ++. Based on the data above, the patient was diagnosed as Mrs. Ni, G3P2A0, 41 years old, 34 weeks gestation, single live fetus, intrauterine, breech presentation, right back, not yet inpartu with IUGR.

Abstract

Hypertension is one of the degenerative disease. Degenerative disease is a chronic non-communicable disease due to deterioration in organ function due to the aging process. The prevalence of hypertension in lampung reached 7,95%. Through identifying the risk factors, clinical problems, conduct patient management holistically by applying family doctors according to evidence based medicine through a patient center, family approach, and community oriented. This study is a case report. The primary data obtained through anamnesis, physical examination, and home visits to complete the family data, psychosocial and environmental data. Assessment based upon a holistic diagnosis of early, process, and the final quantitative and qualitative studies. Based on a holistic diagnosis, the patient had a history of hypertension and type 2 diabetes mellitus for more than 10 years, as well as hypercholesterolemia and obesity. Patients routinely seek treatment at the clinic and routinely take medication every day. The patient's diet is still irregular and the patient rarely does physical activity. The diagnosis and managemet of the patient has been done holistically, patient centered, family approach and in accordance with national guidelines and based on evidence based medicine. Changes in patient and family knowledge and behaviour occur after patient center and family approach interventions.

Abstract

The excessive uric acid levels in the body can increase the risk of hyperuricemia. The formation of uric acid starts from the synthesis of purines which produce inosine, adenine and guanine monophospate that will be degraded into uric acid. Peanuts contain purines so if it is consumed in excess, it can increase blood uric acid levels. To know the difference in the average blood uric acid levels of rats given peanuts at a dose of 1gr/200grBB, 2gr/200grBB, 4gr/200grBB and not given peanuts. This research used experimental research with post test control group design approach. The sample in this research used 28 rats that divided into 4 groups; K1 were only given standard feed, P1 were given 1gr / 200grBB peanuts, P2 were given 2gr/200grBB peanuts and P3 were given 4gr/200grBB peanuts in everyday for 28 days. Then measuring the blood uric acid levels of rats. The average blood uric acid level of rats before intervention (K1) is 3,28mg/dl, and after intervention is 3,87mg/dl (P1), 4,20mg/dl (P2) and 5,03mg/dl (P3). There is a significant difference between P2 and K1 (p=0,005 ), P3 and K1 (p=0,000 ). There is no a significant difference between P1 and K1 (p=0,130 ). There is a significant difference in the average blood uric acid level of rats given peanuts at a dose of 2gr/200grBB, 4gr/200grBB and not given peanuts. There is no significant difference in the average blood uric acid level of rats given peanuts at a dose of 1gr/200grBB and not given peanuts.

Abstract

Tetralogy of Fallot is a congenital heart defect consisting of pulmonary valve stenosis, ventricular septal defect, right aortic valve deviation (overriding aorta), and right ventricular hypertrophy. TOF abnormalities constitute 7%-10% of congenital heart defects. The incidence is 0.23-0.63 cases per 1000 births. Tetralogy of Fallot is a congenital heart defect which is one of the most common cyanotic heart defects. The incidence of cerebrovascular disorders and brain abscess in TOF patients were 9.6% and 13.7%, respectively. In patients with tetralogy of Fallot who require surgery, it is important to know the proper anesthetic management. The goal is to perform anesthetic management according to the patient. The study conducted is a case report. Data were obtained through alloanamnesis and physical examination of patients. Assessment based on interview diagnosis and observation. A female patient aged 40 years came with complaints of decreased consciousness, vomiting and no appetite since 4 days ago. Previously, the patient had a history of VP Shunt insertion surgery 1 month ago due to hemorrhagic stroke. On physical examination, the general condition appeared to be severe, apathetic consciousness. On examination of the thorax and heart, abnormalities were found in the form of widening of the heart boundaries. On the superior and inferior extremities clubbing of fingers was found. Neurological examination revealed left limb weakness and motor speech disturbances. The patient underwent a craniotomy with prior anesthesia considerations. Preoperative, intraoperative, and postoperative management makes the success of surgery better

Abstract

The number of people with hyperthyroidism according to the World Health Organization (WHO) worldwide in 2000 was estimated at 400 million, and it was more common in women than men with a ratio of 5: 1.7. This disease is important to be managed comprehensively because this disease is chronic so the patient must know about the disease and have the awareness to make lifestyle modifications so the treatment goals can be achieved and needs to be carried out through a family approach. This case report describes a woman Mrs. J 60 years old who was diagnosed with hyperthyroidism since 2 years ago came with complaints of heart palpitations, shaking hands, accompanied by complaints of frequent sweating even in a cold room. Mrs. J has internal risks in the form of inappropriate diet and eating habits of the patient, lack of knowledge about the causes, dangers and prevention of the disease suffered, as well as the treatment pattern of patients who only take medicine when there are complaints. External risk factors in patients, namely the lack of family support in reminding patients to manage a good lifestyle. So that pharmacological and non-pharmacological management is carried out holistically in accordance with the approach of family doctors and evidence based medicine. Enforcement of diagnosis and management of these patients has been carried out holistically, patient centered, family approach and community oriented based on several theories and recent research. In the process of behavior change, the patient has reached the trial stage.

Abstract

After the baby agrees 6–12 months breastfeeding is no longer enough to meet nutrition. Inadequate fulfillment of nutrition will result in poor nutritional status of children. The impact of malnutrition status will cause an increase in physical growth and intelligence development as well as a decrease in endurance which results in an increase in mortality in infants. One cause of malnutrition is the help of parents in providing complementary feeding. This study aims to determine the relationship between the behavior of complementary feeding with the nutritional status of infants aged 6–24 months in working area of ​​the Bandar Lampung Satelit Health Center. This research uses quantitative method with cross sectional approach. The sample consisted of 64 mothers who had babies aged 6–24 months in working area of ​​Bandar Lampung Satelit Health Center. Sampling was done by consecutive sampling and the data were analyzed by chi square test. As many as 45 infants (70.3%) had good nutritional status and 19 (29.7%) infants had undernourished status. A total of 25 people (39.1%) had good complementary feeding behavior, while a total of 39 people (60.9%) have poor complementary feeding giving behavior. Statistically, complementary feeding giving behavior is related to infant nutritional status with p value = 0,000. Based on this study, we can concluded that there is a relationship between maternal behavior in the provision of complementary feeding and nutritional status of infants aged 6–24 months in working area of ​​Bandar Lampung Satelit Health Center.

Abstract

Head injury is a cause of morbidity and mortality worldwide, can occur due to mechanical trauma either directly or indirectly that occurs due to impact, shock, blow or penetrating injury to the head that interferes with neurological function, namely physical, cognitive, psychosocial functioning temporarily. nor permanent. Epidural Hematoma (EDH) is the presence of an extra-axial collection of blood in the potential space between the internal tabula and the dura mater due to rupture of the bridging vein. A 16-year-old boy who is a referral from the Ryacudu Hospital Kotabumi came to the ER Abdul Moelok Hospital with a post-traffic accident that he fell from his motorbike about 7 hours before entering the hospital. The patient had decreased consciousness 10 minutes after the accident. Later, the patient regained consciousness and complained of headache and projectile vomiting. On physical examination, the general condition was moderate apatis consciousness, GCS E3V5M5, blood pressure 110/70 mmHg, pulse 78x/minute, respiration 20x/minute, axillary temperature 36.8 °C, body weight 45kg. On neurological examination, pupillary isocor, direct light reflex (+/+) and indirect light reflex (+/+). On examination of the head found a vulnus laseratum in the parietal region with a size of 7 cm x 0.1 cm which has been heacted. The patient underwent a craniotomy to evacuate the hematoma under general anesthesia and with due observance of the principles of neuroanesthesia during the surgical procedure.

Abstract

Mental retardation is a state of incomplete or cessation of mental development that affects a person's level of intelligence. This is marked by impaired skills during the developmental period that affect cognitive, language, motor and social abilities. Mental retardation is caused by several factors such as genetic, environmental and psychosocial factors. Mental retardation is classified as mild, moderate, severe and very severe. The incidence of mental retardation is difficult to know accurately because it is generally only detected at an older age. However, it is estimated that 10-15 per 1000 children in developing countries are mentally retarded with 1.5 times more males than females. This case report discusses moderate mental retardation in a 9-year-old girl who came with complaints of difficulty following lessons at school since 2 years ago. The patient's parents said that the complaints were getting worse because of the increasing level of difficulty in learning and the online learning process at school. The therapy given is in the form of psychotherapy and psychoeducation to patients and families who care for patients.

Abstract

The tremendous increase in infections caused by viruses and their mortality rate has demanded producing effective and safe antiviral drugs. Symptomatic treatment and immune enhancement are the most efficient methods because no specific drugs are available for every viral disease. Several naturally occurring compounds available have demonstrated antiviral potential in preventing and debilitating viral diseases. A nutrient-rich diet can help reduce many viral infections. Nutrients such as vitamins (vitamins A, D, C, E, B6, and B12) and elemental groups provide the highest nutritional value for all body systems, including bones, cardiovascular, liver, skin, and support immunity. This research provides information on dietary-based scientific recommendations, evidence, and case studies worldwide concerning a particular focus on viral infections. In this study, the use of bioactive compounds in viral mechanisms, particularly SARS and respiratory viral infections, will be discussed, which may help in facilitating preintervention either directly as supplements/fortified foods or in combination with pharmaceutical drugs. This research method reviews the immune potential in the content of dietary supplements based on laboratory studies and human research through international journals, trusted websites, and books in the last ten years. The result is that bioactive compounds such as vitamins, elements, and minerals with proven antiviral properties, in addition to pharmaceutical drugs themselves as dietary supplements, have been shown to play an important role in treating various infections caused by viruses and strengthening the immune system.

Abstract

Maternal mortality rate (MMR) is one of several parameters that can describe the welfare of society in a country, especially developing countries such as Indonesia. One of the causes of maternal death is caused by prolonged delivery. Long labor is the 5th leading cause of maternal death both in Indonesia and in the world. The factors for the occurrence of prolonged labor are divided into two factors, namely causative and risk factors, causative factors: his, mal presentation and mal position, large fetus, narrow pelvis, cervical and vaginal abnormalities, fetovelvic disproportion, and premature rupture of membranes, and risk factors: excessive analgesia and analgesia, parity, age, dependent woman, stress response, restriction of mobility, and strict fasting. Mrs. NS was pregnant at term, 26 years old with complaints of heartburn since 18 hours before being admitted to the hospital. Initially, the heartburn was felt at 04.00 WIB and until 22.00 WIB there was still an 8 cm opening. On obstetric examination, external examination revealed that the height of the uterine fundus was 32 cm, the upper part of the fetus was palpable buttocks, the left side of the mother was palpated for the back, the lower part of the head was palpated into the pelvic inlet (PAP) or the fifth was 2/5. Fetal Heart Rate 136 x/minute and Estimated Fetal Weight 3100 grams. On internal examination, it was found that 90% flattened, 9 cm dilatation, hodge III, station 0. The management of this patient was vaginal termination of pregnancy.
