Abstract

Glaucoma is a chronic progressive optic neuropathy characterized by elevated intraocular pressure, cupping of the optic disc and visual impairment with visual field defects. Every year the number of glaucoma is increasing and become the second common cause of blindness. This study aims to determine what factors related to the incidence of glaucoma at the Eye Hospital Lampung Eye Center Bandar Lampung. This study used an analytic observational design with a cross sectional approach. The data used who were sourced from secondary data in the form of medical records that are recorded data were patient characteristics, main complaint, history of hypertension and incidence of glaucoma. This respondents consisted of 134 glaucoma patients who showed the results that most respondents are in the age group >60 years (43.3%), male (53%), had high education status (64,9%), had no history of hypertension (73.1%), the main complaint with visual impairment (59,7%) and the incidence of primary glaucoma type of glaucoma (47.8%) . There were no correlation among age (p = 0,468), gender (p = 0.148), education status (p = 0.116), a history of hypertension (p = 0.259) and main complaint (p = 0.087) with the incidence of glaucoma at Eye Hospital Lampung Eye Center Bandar Lampung. There were no correlation among the characteristic features, history of hypertension and main complaint of glaucoma with the incidence of glaucoma.

Abstract

Surgical site infection is a type of nosocomial infection with the most common occurrence, which is 38% of all cases of nosocomial infection. The distance of sutures affects the local hemodynamics around the surgical wound and can cause the inhibition of thrombus formation which plays a role in the wound healing process so that it will inhibit wound healing which will lead to increase of bacterial growth and cause infection and tissue damage. This study is an unpaired comparative analytic method with a cross-sectional research design. The sampling technique is total sampling. The research subjects were patients with perforated appendicitis at RSUD Dr. H. Abdul Moeloek, Lampung in 2020-2021, totaling 37 people. The independent variable in this study were age, gender, and the distance between the sutures, while the dependent variable was the incidence of surgical site infection. The study showed that the distance of sutures had a correlation with the incidence of surgical site infection with a p value of 0.00 (<0.05), where the distance of sutures that <2 cm has a higher frequency of surgical site infections than the distance of sutures that ≥2 cm. Age also has a correlation with the incidence of surgical site infection with a p value of 0.041 (<0.05). Meanwhile, gender did not have a correlation with the incidence of surgical site infection with a p value of 0.728 (> 0.05). We can conclude that the distance of sutures and age have a correlation with the incidence of surgical site infection. Otherwise, gender is not associated with the incidence of surgical site infection.

Abstract

Congenital cataract is a disorder of the loss of transparency of the lens, sometimes causing a change in its refractive index that occurs during the first year or childhood of life. Congenital cataract is a rare eye disease, with a variable prevalence of 0.6-9.3/10,000 live births. However, this disease remains one of the main causes of visual loss in children. This disease is considered a preventable cause of blindness. However, late diagnosis and treatment will also have an adverse impact and can cause severe permanent blindness. Congenital cataracts have a significant impact on affected patients due to the low age of the target population and the deprivation amblyopia that can occur as a complication of this disorder. The method used in compiling this literature review is through searching literature from various sources using online searches with Google Scholar, Pubmed, and NCBI instruments within the last 5 years and using the keywords congenital cataract, diagnosis, management, management, treatment data that have been collected were analyzed scientifically and systematically. The data analysis technique used is descriptive and argumentative. The results of this study indicate that the diagnosis of congenital cataracts can be made through anamnesis, physical examination, and supporting examinations. The main treatment with good visual results is through early intervention with surgical procedures. The surgical techniques used include anterior Capsulotomy, Lens Aspiration & Vitrectomy anterior/Lansectomy, and IOL Implantation.

Abstract

Inflammation is a biological response of the immune system that can be triggered by various factor including pathogens, physical trauma and toxic compounds. Prolonged consumption of NSAID as an anti-inflammatory can caused some serious side effects for the body. As a result, many people has switched to using plants as therapy. Another reason for choosing therapy with plants is more affordable, as well as family traditions trust in the efficacy of a plant. The purpose of this review article is to provide information about several medicinal plants that have anti-inflammatory  activity. The method in this study was a systematic literature review which is traced through database, Google Scholar and Pubmed. The literature used was published in 2019 to 2024 and 11 journals were obtained as primary data. Some plants studied as anti-inflammatory are red spinach (Amarathus tricolor L.), Beligo (Benincasa hispida (Thunb.) Cogn.), Pineapple (Ananas Comosus (L.) Merr.), Kecapi (Sandoricum koetjape (Burm.f. ) Merr., Sesewanua (Clerodendrum squamatum Vahl.), Turmeric (Curcuma domestica Val.), Aloe vera (Aloe vera L), Tamarind (Tamarindus indica L.), Green grass jelly (Cyclea barbata Miers.) and Galangal (Kaempferia galangal L.). In research on the anti-inflammatory activity of several plants studied, it was proven to be able to improve several inflammatory parameters, but at different doses because it depended on the response of the test animals and the secondary metabolite content of each plant.

Abstract

Dermatophytosis is a dermatophyte fungal infection that attacks keratinized tissues. This disease can be caused by various internal and external factors. The internal risk factors causing dermatophytosis include age, gender, and occupation. In addition to that, this study aimed at determining the relationship between age, gender, and occupation toward dermatophytosis cases. This study used an analytical observational design with a cross-sectional approach. The sample in this study consisted of 114 patients with dermatophytosis sourced from the secondary data in the form of medical records. The research data were conducted by using total sampling technique in RSUD Dr. H. Abdul Moeloek Lampung Province. The recorded data included the patients’ age, gender, occupational, and diagnosis. Then, the data were analysed using chi-square test presented in tabular form. The results showed that the majority of dermatophytosis patients were adults, female, and employees who worked in government/private institutions. The results of the chi-square test showed a significant value for the age variable (p = 0.035) and the gender variable (p = 0.041), while for the work variable there was no significant relationship (p = 0.615). There is a significant relationship between age and gender with the dermatophytosis cases and there is no significant relationship between occupation and the dermatophytosis cases.

Abstract

Trachoma is a neglected tropical disease. Trachoma is an infection of the eye caused by bacteria Chlamydia trachomatis. This disease often attacks people who live with low economic income with a bad environment. This infection is transmitted through physical contact and from animals to humans. Trachoma can be classified into an active or infectious phase and a chronic or non-infectious phase. According to its level, trachoma has six types of grades in the assessment of clinical symptoms of trachoma namely stage Normal Tarsal Conjunctiva (N), stage Trachomatous inflammation-Follicular (TF), stage Trachomatous inflammation-Intense (OF), stage Trachomatous Scarring (TS), stage Trachomatous Trichiasis (TT), and stage Corneal Opacity (CO). Treatment that can be done to treat trachoma is the SAFE program developed by WHO, including: surgery (surgery),antibiotics (antibiotics),facial cleanliness (facial cleansing), and environmental improvement (environmental improvement). This article uses the method literature review from various national and international journal references with the following search keywords: trachoma, etiology, classification, management, and Chlamydia trachomatis.

Abstract

Breast cancer is the most common cancer suffered by women in the world, 15-20% is a subtype of triple negative breast cancer (TNBC). TNBC is a subtype of breast cancer that does not express genes for estrogen receptors (ER), progesterone receptors (PR) and HER2 overexpression, which is clinically more aggressive and unresponsive to standard therapy, thus it can worsen the prognosis. It suggests that TNBC is associated with BRCA-1 gene mutation, moreover in African-Americans. So this study aims to determine the association of TNBC with BRCA-1 gene mutation and ethnicity. International journal literature review. BRCA-1 gene mutations increased 45%-85% of the risk of breast cancer growth and 15% -25% of TNBC have BRCA-1 gene mutations which resulted in activation inefficiencies of the HR DNA repair pathway. TNBC occurred in 60% of the black race, disproportionate incidence of TNBC greater in African ethnicity compared to other ethnic groups in the world and has the worst prognosis among all races. There is asignificant association between TNBC with BRCA-1 gene mutations and ethnicity especially in African-Americans.

Abstract

Tuberculosis cases in children at 2022 reach 1.1 million in the world and 100,726 cases in Indonesia. Management of Tuberculosis in children needs holistic treatment with a family doctor approach to achieve Tuberculosis elimination targets. The aim of this study is  to apply the principles of family medicine services to patients  and carry out holistic management based on the patient problem found with patient-centered approach, family focused, and community oriented based on Evidence Based Medicine. This study is a case report with Primary data obtained through history taking, physical examination and home visits to complete family, psychosocial and environmental data. Secondary data was obtained from the patient's medical record. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study. Diet and health behavior of patients classified as lacking. Knowledge about Tuberculosis disease and the implementation of healthy behavior in the family still poorly. After the intervention, there was an improvement in the diet and the patients diet behavior. Then there is an increase in knowledge through pretest and posttest as well as improvement in family life patterns to become healthier.  After holistic management was carried out based on the problems identified through family doctor approach, there was an increase in knowledge, nutritional intake, and better healthy behavior.

Abstract

General anesthesia is the standard technique for operation. Due to many complication of general anesthesia, thoracic spinal anesthesia (TSA) is widely used for several operation. Thoracic segmental spinal anesthesia has been proven safe and effective for a variety of laparoscopic surgeries. Thoracal spinal anesthesia is also proven usefully in maintaining hemodynamics and reducing side effects. Laparoscopic surgery is an alternative to the drainage of liver abscess in addition to laparotomy. A 58-year-old man with a diagnosis of posterior segment VI of liver abscess lead a laparoscopic surgery which became a laparotomy under thoracic spinal anaesthesia. Thoracic spinal anesthesia with a median approach at the level of 9-10 intervertebral space (T9-10) single shot using a combination regimen of Isobaric 5 mg and Hyperbaric 2.5 mg, Fentanyl 50 mcg, Dexmetomidine 10 mcg, and Ketamine 20 mg. During the operation there were no hemodynamic fluctuations. Postoperative monitoring found the patient's condition was stable, pain control was good, and there were no postoperative complications such as headache, nausea, and vomiting. Single dose thoracal spinal anesthesia (TSA) is one of the regional anesthetic techniques of choice for abdominal surgery, especially in high-risk patients. In this patient it was found that TSA was effective and safe even though the surgical technique used was different. Monitoring during and postoperatively showed good results without any complications and significant hemodynamic fluctuations.

Abstract

Benign Prostate Hyperplasia (BPH) is the second most common urology’s disease in Indonesia. This disease occurs due to hyperplasia of stromal cells and epithelial cells of the prostate. The clinical manifestations of this disease consist of obstruction symptoms, irritation symptoms, and post void symptoms. People with BPH often complain about the decrease of their quality of life. One of the factor that Impaired to their quality of life is erectile dysfunction. This study aims to identify the factors that impact the incidence of erectile dysfunction in BPH patients at Yukum Medical Centre Hospital. There were 67 men with bph was obtained from Yukum Medical Centre Hospital in January-September 2021. Data were collected with International Index of Erectile Function-5 questionnaire and medical records. Data were analysis with chi-square test using SPSS and a p-value (<0,05 was considered significant). The result showed that 39 BPH patients had erectile dysfunction. Mean age of BPH patiens with erectile dysfunction was 66,74 (SD±6,51) and mean IIEF-5 scores of the BPH patients was 19,41 (SD±4,23). Age and prostate volume had significant impact with p-value 0,004 and 0,002 respectively. Treatment and medication didn’t have significant impact with p-value 0,193, and 0,856 respectively. Age and prostate volume have a significant impact on the incidence of erectile dysfunction in BPH patients. However, treatment and the type of medication doesn’t impact the risk of incident erectile dysfunction in BPH patients.
