Abstract

Imminent preterm is the presence of a threat in pregnancy with emergence signs in 20 weeks-37 weeks. Preterm premature rupture of membrane is rupture of chorioamniotic membrane before the onset of childbirth and occurs in gestational age less than 37 weeks.. This study is a case report. A 34 years old woman with preterm pregnancy come with complained of the release of amniotic fluid that is odorless and can not be held from seven hours before entering hospital and accompanied by two days before entering hospital. It is accompanied by pain that intermitently radiating to the waist but still rare. From physical examination found moderate sick in general condition, compos mentis, BP 120/70mmHg, pulse 100 x/min, respiratory rate 20 x/min, temperature 36,6oC. General examination within normal limits. In obstetric examination found that fundus uteri is 3 finger under proccesus xiphoideus (26 cm), his (+) 2x in 10 minutes duration 10-12 seconds, fetal heart rate 144 x/min. In inspekulo examination found that portio is livid, OUE closed, amniotic fluid (+) not active, erosion/laseration/polip (-), lakmus (+) red to blue. Vaginal toucher not performed in this patient. The diagnosis are G4P3A0 in 30 weeks pregnancy, not inpartu yet, with imminent preterm and premature rupture of membrane. Patient planned conservative therapy such as observation of his, fetal heart rate, and vital signs mother, IVFD RL gtt x x/min, dexamethasone 2x10 mg, nifedipine 4x10 mg, cefadroxil 2x500mg.

Abstract

The growth of social media usage has increased almost 15 times, in line with the growth of internet usage in Indonesia in the last 10 years. Social media usage has a relationship with learning motivation, even though long duration of social media usage has impact on the decrease of learning motivation. Investigated relationship of social media usage duration with student learning motivation in class of 2015 Faculty of Medicine, the University of Lampung. Cross sectional design was applied in this research by taking primary data during period of August to September 2017, with the sample of 185 students class of 2015 Faculty of Medicine, the University of Lampung. Analysis model used was Chi-Square Analysis. Based on the statistic value of significant alpha more than 0,05, there was no relationship between social media usage duration and learning motivation. Moreover, the research result shows that learning motivation in term of intrinsic dimension was in low category with the amount of 55,14%. Beside this, leaning motivation in term of extrinsic motivation external regulation was in high category with the total amount of 56,76%. The duration of the use of social media has a positive relationship on student learning motivation because the duration of the use of social media by students of the Faculty of Medicine, University of Lampung class of 2015 is less than one hour per day and student learning motivation is in the aspect of extrinsic motivation of external regulation.

Abstract

Acne Vulgaris is an inflammation of the polysebaseal follicles indicated by the appearance of blackheads, papules, pustules, cysts and nodules, which often occur mainly in the chest (15%), face (99%), and back (60%). In Indonesia around 95-100% of men and 83-85% of women aged 16-17 years suffer from acne. One of the microorganisms that cause acne vulgaris is Propionibacterium acnes bacteria which is a normal flora on the skin. In treatment, acne vulgaris wide-spectrum antibiotics have been commonly used as a treatment for acne, as the time goes by, it has been found there are increasement of Propionibacterium acnes resistance to antibiotics like erythromycin, clindamycin, and tetracycline. Sulfur has been widely used in anti-acne products which have an antibacterial function and can inhibit the growth of Propionibacterium acne as a cause of acne. This study aims to test sulfur solutio with concentrations that are often used in anti-acne products, namely 2-10% in inhibiting P.acne bacteria. This type of research is an experimental laboratory with a well method. Solutio sulfur is divided by various concentrations of 10%, 8%, 6%, 4%, and 2%, with K (-) is distilled water, and K (+) is a clindamycin gel antibiotic. The data is based on the results of the measurement of the inhibition zone formed and tested with One Way Anova. The results of this study indicate the diameter of the inhibition zone formed sequentially, namely: 12.70 mm, 10.55 mm, 7.70 mm, 5.80 mm, 4.67 mm, positive control of 30,075 mm and negative concentration of 0 mm (p value = 0,001). There is an antibacterial effect of sulfur solutio with a concentration of 10%, 8%, 6%, 4% and 2% against Propionibacterium acnes bacteria.

Abstract

A holistic and comprehensive family medicine approach in detecting internal-external factors, prioritizing family-approached and patient centers is hoped to be able to improve the quality of life of patients. In this case, diagnosis and management were carried out with the concept of family medicine. Primary data were obtained through history taking, physical examination and home visits. Secondary data comes from patient medical records. The case of the patient, Mrs. 26 years old, had drug withdrawal TB (ICD X A15.0). The complaint was felt to have reappeared since three months ago, but the patient still had functional degree 2 in carrying out daily activities. Patients have internal risk factors, namely the patient's lack of knowledge of the disease, inappropriate lifestyle, curative treatment behavior and lack of medication adherence. External factors possessed by the patient are a family with low income (ICD10 – Z59.6), poor lighting, poor ventilation in the house, a densely populated home environment, inadequate cleanliness of the home environment and contact with patients with prolonged coughs. Handling and treatment efforts in a holistic and comprehensive manner using a family medicine approach in the form of education on causes, transmission, treatment and prevention of disease.

Abstract

Acute rheumatic fever (DRA) is an inflammatory condition that can affect various organs and systems such as the heart, joints, central nervous system and subcutaneous tissue due to group A β hemolytic Streptococcal infection. Rheumatic heart disease (CHR) occurs when cross reactions between carbohydrate cell walls bacteria with heart valve tissue causing chronic damage to the heart valves. The prevalence of PJR reaches 33 million cases and causes about 270,000 deaths each year. A 15 year old girl came for treatment complaining that she often felt weak since 3 months ago, especially every moderate-heavy activity and recovered when resting. Limp is exacerbated during menstruation. The patient also experienced shortness of breath during moderate to heavy activity. The patient had a history of cough and fever and a wound on the left leg. On physical examination of the patient, it was found that the general condition looked moderate, mental compost, pulse 112 times / minute, blood pressure 110/80 mmHg, breath rate 20 times / minute, temperature 37.5 degrees Celsius and body weight 52 kg. On examination of the head, it was found that the tonsils were enlarged with detritus. And on the chest examination found a systolic ejection murmur. The results of investigations showed positive ASTO immunoserology. Chest X-ray shows the heart is not enlarged. The patient was diagnosed with rheumatic heart disease. Patients are advised to reduce strenuous activities. Patients were given benzathine penicillin 1.2 million / IU IM once a day, prednisone 2mg / kgBW / day 2-6 mg, and captopril 2x 12.5 mg.

Abstract

Shingles is clinical manifestations due to reactivation of varicella zoster virus (VZV). in sensory ganglia that were previously exposed to primary varicella infection. The incidence of HZ increases with age and is rarely found in children. Shingles can occur in children who have primary primary infections, immunocompromised conditions, and who are related in the first year of life. The purpose of this study is to Identify internal and external risk factors and clinical problems in patients, as well as holistically managing patients by applying a family doctor based on evidence-based medicine through a patient centered approach and family approach. This study analysis is a case report. Primary data were obtained through history taking (autoanamnesis from the patient and alloanamnesis from the patient's family) physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment is based on a holistic diagnosis from the beginning, process, and end of study in a qualitative and quantitative manner. Patient A, 15 years old, who has been diagnosed with Herpes Zoster without complications has concerns about his health condition, the patient feels inferior and is afraid that his illness cannot be cured and is afraid of transmitting it to other family members. Patient's expectation is that complaints disappear completely and do not recur, so that they can be comfortable in their normal activities. Limitation of the patient's knowledge of the illness and bad lifestyle is a factor causing problems that occur in patients and families. The diagnosis of shingles in this case is in accordance with several theories and critical studies from recent research. Management has been given in accordance with management guidelines. Good developments were seen in patients ranging from decreased clinical symptoms to behavioral changes after a family-approach and patient-centered Evident Based Medicine intervention.

Abstract

Corona virus disease-19 (COVID-19) is a global health crisis problem. The clinical characteristics of disease progression and outcome in children and young adults appear to be much milder than in older people. The incidence of COVID-19 in children aged 10-19 years was 549 / 72,314 or 1% of all cases, while the age group <10 years was 416 / 72,314 (0.9%) cases. Compared to adults, the number of confirmed children cases is very low and the severity and mortality is even lower. Indonesia has 450 cases of COVID-19; 38 of them died as of March 21, 2020. The incubation period for COVID-19 in children based on case reports around the world ranges from 2 to 10 days. Data suggest 13-15% of virologically positive children may be asymptomatic. The most common symptoms described in the early phase of COVID-19 in children were fever (50%) and mild cough (38%). Other clinical features that can appear are sore throat, rhinorrhea, sneezing, myalgia, fatigue, diarrhea, dizziness, skin rash and vomiting.

Abstract

Chronic energy deficiency (CED) is a state of chronic energy and protein intake shortages that persists (chronic) resulting in health problems for the mother and fetus. Implementation of evidence based medicine based family doctor services by identifying risk factors, clinical problems, and managing patients holistically with a patient centered approach, family approached, and community oriented. Primary data obtained through history taking, physical examination and home visits, psychosocial and environmental data. Assessment is based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. The patient, a 25-year-old woman pregnant with her fourth child, 24 weeks gestasional age with chronic energy deficiency, has concerns about growth, development and the fetus and herself. Complaints feel weak, no appetite at the beginning of the trimester, and difficulty to gain weight. The patient also has a history of miscarriage twice. The risk factor obtained is the lack of adequate nutritional balance in patients. In the process of behavior change, Mrs. T has reached the trial stage. The process of behavior change for a balanced nutritional diet, addition of food portions, routine vitamin intake and Supplementary feeding (SF)-mild physical exercise is seen after the patient is given an intervention.

Abstract

Nosocomial infection is an infection that is seen in a patient while in a hospital or other health facility, where the infection is not visible when a patient is admitted to the hospital. This research was conducted using descriptive observational laboratory methods. The study was conducted by taking air samples and wiping the floor to be planted on the media and viewed microscopically. To determine air quality and determine the type of microorganisms on the Intensive Care Unit (ICU) floor of the Regional General Hospital Dr. H. Abdul Moeloek Bandar Lampung. From the five points taken as a sample, the highest germ index was 244 CFU / m3. Gram-positive bacteria in the form of coccus are the most bacteria in the Gram staining of air samples. The growth of bacteria on the floor, which is Gram negative bacteria in the form of bacillus in all samples. The results of bacterial identification from biochemical tests showed a variety of bacteria. The quality of air microbiology in the Intensive Care Unit of the Regional General Hospital Dr. H. Abdul Moeloek Bandar Lampung is good with an average germ index of 153.4 CFU / m3.

Abstract

Paraquat dichloride is an active ingredient of the herbicide gramoxone which is often used for weed control and crop maintenance in the agricultural sector, paraquad dichloride is a broad spectrum contact herbicide which is geneally applied post growth. This herbicide is able to kill all types of weeds in the part that provides the herbicide solution direcly, because the dose and time of application used are correct so that the herbicide is effective in controlling weeds. Paraquat herbicide has a danger to organs such as eyes, skin, respiratory system, liver, heart, kidneys, and digestive tract. A man agreed to 56 years, a farmer, came with complacency since 3 days ago. Moisture with hoarseness, recovery, difficulty in carrying, and burning sensation in the chest. 5 days before admission, the patient inadvertently swallowed poison while working in the fields. On physical examination, hyperemic mucosa appears in the oral cavity, pharynx, and tonsils, as well as erosion in the posterior tongue and uvula. In indirect laryngoscopy, hyperemic mucosa and edema appear in the larynx and epiglottis. Investigations urea 276 mg / dl and creatinine 9.72 mg / dl. Patients diagnosed with acute laryngopharyngitis suspected corrosive esophagitis and post-intoxication of herbicides with acute kidney injury. Management of patients, namely: 1) 8 lpm oxygen with NRM, 2) IVFD RL 500 ml / 8 hours, 3) ceftriaxon 1 gr / 12 hours, 4) Furosemide / 8 hours, 5) Ranitidine 1 amp / 12 hours, 6) Metilprednisolone 125 mg / 12 hours, 7) Ketorolac 1 amp / 8 hours, 8) HD cito, 9) Bicnat 3 x 1000 mg, 10) Vit B12 3x1. The prognosis for paraquat poisoning greatly affects the amount of paraquat absorbed by the body, whereas the patient in this case is considered a good prognosis because the patient drank <20mg / kg of poison which could cause mild poisoning.
