Abstract

Menopausal women have a risk of suffering from several diseases due to decreased levels of the hormone estrogen in the body. One effort to reduce this risk is to use Hormone Replacement Therapy (HRT). Objective: To analyzed  the effect of HRT use on postmenopausal women. Methods: This research was a systematic review by searching articles in the PubMed electronic journal with keywords used [“Premenopausal women” OR “Postmenopausal women”, OR “Menopause”] AND  [“Hormone Replacement Therapy” OR “Menopausal Hormone Therapy”] AND [“Increased risk disease” OR “Decreased risk disease”]. Results: There were nine studies analyzed; six studies discussed the use of HRT having a positive effect in reducing the risk of several diseases, and as many as five studies discussed the use of HRT having a negative effect, namely, it could increase the risk of several diseases. The studies discussing the factors that influence the effect of HRT use are one study discussing how to use HRT, three studies discussing the type of HRT used, two studies discussing when to start using HRT, and three studies discussing the duration of using HRT which has a significant relationship to the effect resulting from HRT use. Conclusion: The use of HRT in postmenopausal women has negative and positive effects. This effect can be influenced by how HRT is used, the type of HRT used, when HRT was started, and the duration of using HRT.Keywords:  duration of use, how to use, HRT, type, time to start using

Abstract

Mesenchymal Stem Cells Wharton's Jelly (MSC-WJ) has become a new hope for Alzheimer's disease due to its capability to suppress Amyloid-beta (Aβ) plaques formation. It is found that people with Alzheimer's disease have a lower expression of Sirtuin-1 because of the oxidative stress that was produced by Aβ plaques. Objective: To observe the Sirtuin-1 gene in Alzheimer's disease rat models that AlCl3 was inducing after being given MSC-WJ. Methods: This was an experimental study with a post-test-only control group design, including three groups with a total sample of 18 samples of Alzheimer RNA rat models. Group-1without AlCl3 and MSC-WJ; Group-2 with AlCl3; Group-3 AlCl3 and MSC-WJ. Mean value of the Sirtuin-1 gene was obtained by the Image-J application using a semiquantitative method which was then tested using the One Way ANOVA test. The study was significant if the p-value < 0,05. Results: This study found that the mean ± standard error value of the Sirtuin-1 gene in three groups were 0.18±0.04; 0.16±0.03; and 0.36±0.06 respectively. The statistical test showed a significant difference between the three groups (p = 0.014). Conclusion: There was an increased Sirtuin 1 gene expression in Alzheimer’s rat Model after being given Mesenchymal Stem Cells Wharton’s Jelly. Keywords:  Alzheimer, MSC-WJ, Sirtuin-1

Abstract

Malnutrition has many adverse effects, including a decrease in hemoglobin levels, or known as anemia. Malnutrition caused by nutritional deficiencies can affect the decrease in hemoglobin levels or anemia. Objective: To determined the correlation between nutritional status and hemoglobin levels in Markidam elementary school-age children in Cilame Village Methods: This cross-sectional study uses primary data from research subjects who have agreed. The number of subject research that has been taken was 143 respondents. Anthropometric examinations using weight scales and microtoise were then carried out in a plotting process to determine the nutritional status of respondents. The hemoglobin level was used as a rapid diagnostic test tool with the Easytouch brand type GCHB. Spearman test to find out the results of this research hypothesis. Results:  Hemoglobin levels did not have a significant correlation with nutritional status. This could be seen from the significance value (p-value) of 0,172, where this value is greater than 0,05. Conclusion: There is no correlation between nutritional status and hemoglobin level at the age of elementary school children. Keywords: anemia, hemoglobin, nutritional status

Abstract

Nasopharyngeal carcinoma is a malignancy that originates from the epithelial tissues lining the nasopharynx and is considered one of the most common malignant cancers which cause mortality in Indonesia. Neutrophils and lymphocytes are the first lines to provide body immunity against foreign substances. Neutrophil-to-Lymphocyte Ratio (NLR) is a prognostic factor in various malignancies. Objective: To investigated the correlation between pre-therapy Neutrophil-to-Lymphocyte Ratio (NLR) and survival rate among patients with nasopharyngeal carcinoma at Abdoel Wahab Sjahranie Regional Public Hospital in 2018. Methods: The Neutrophil-to-Lymphocyte Ratio (NLR) data were obtained from outpatient medical records at Abdoel Wahab Sjahranie Regional Public Hospital in 2018. In addition, the data on mortality status were collected based on the information after directly contacting the patients. A purposive sampling technique was used to collect the data. Thirty-three data of patient medical records were obtained and analyzed using Kaplan-Meier survival analysis with a significant p-value of <0.05. Results: The analysis result showed p-value of 0.495. Conclusion: There is no correlation between pre-therapy Neutrophil-to-Lymphocyte Ratio (NLR) and survival rate among patients with nasopharyngeal carcinoma. Keywords:  nasopharyngeal carcinoma, neutrophil-to-lymphocyte ratio, survival rate

Abstract

Hepatobiliary ultrasound is the most common initial diagnostic modality in the early detection of biliary atresia. It is not the gold standard, but the hepatobiliary ultrasound examination can be used to determine the appropriate further management. Objective: To determined the diagnostic value of ultrasound examination as a predictor of biliary atresia in cholestasis of infants. Methods: This was an observational study with a diagnostic test to determine the validity of hepatobiliary ultrasound as a predictor of biliary atresia with cholangiography as the gold standard. The research subjects were 40 infants taken by total sampling from January 2017 until December 2021. Data were obtained from medical records, hepatobiliary ultrasound images were accessed from PACS, and cholangiography results from SIMARS. The hepatobiliary ultrasound images were scored by two observers using a scoring table. The variables assessed included: gallbladder morphology, absence of common bile duct, triangular cord sign, gallbladder contraction index and liver morphology. Data analysis consisted of interobserver reliability test, and relationship assessment analysis. Diagnostic tests to determine the optimal cut-off score, sensitivity, specificity, positive predictive value, negative predictive value and accuracy. Results: At cut-off≥ 5, the number of patients with radiologically impressive biliary atresia was 26 and not biliary atresia was 14. The validity analysis showed a sensitivity 87.5%, specificity 68.8%, positive predictive  80.8%, negative predictive 78.6%, and accuracy 80%. Conclusion: The diagnostic value of hepatobiliary ultrasound as a predictor of biliary atresia in cholestasis of infants is good. Keywords:  biliary atresia, hepatobiliary ultrasound, neonatal cholestasis

Abstract

Chronic rhinosinusitis (CRS) is defined as chronic inflammation of the nose and paranasal sinuses with two or more symptoms, one of which can be nasal congestion/obstruction/congestion or rhinorrhea (anterior/postnasal drip), and/or with facial pain, reduced or loss of smell. Elongated uncinate is one of the anatomical variants of the uncinate process where the uncinate process is curved or bent, which causes sinus problems and is one of the risk factors for CRS. It has been reported a 19-year-old man with the diagnosis of chronic rhinosinusitis with third-grade bilateral nasal polyp from computed tomography of the paranasal sinusitis. There was an impression of the sinonasal polyp with inverted papilloma as a differential diagnosis. The patient was treated through microdebrider-assisted functional endoscopic sinus surgery. This was chronic rhinosinusitis with grade III bilateral nasal polyps, and microdebrider-assisted fungsional endoscopic sinus surgery was performed with great results. Keywords: chronic rhinosinusitis, elongated unsinatus,  functional endoscopic sinus surgery, nasal polyp
