Abstract

Liver injury can be caused by coronavirus disease 2019 (COVID-19), characterized by increased levels of transaminase enzymes (aspartate aminotransferase/AST and alanine aminotransferase/ALT). Objective: To determined the correlation between transaminase enzyme levels and the severity of COVID-19 in Dr. M. Djamil Central Public Hospital, Padang. Methods: This was an observational analytic study with a cross-sectional design. Data were obtained randomly from medical records of confirmed COVID-19 patients hospitalized in Dr. M. Djamil Hospital from June to August 2021, with moderate, severe, and critical severity based on Indonesian Ministry of Health criteria. Data were analyzed using the Chi-square test to examine the distribution and correlation between variables and a simple logistic regression test to assess the level of risk between variables. Results: 57,80% had abnormal AST, and 44,95% had abnormal ALT. The median AST was highest in the critical group, while the median ALT was highest in the severe group. The distribution of sex and age group were significantly different based on AST and ALT levels, and the distribution of patients with coronary heart disease was significantly different based on ALT levels. Transaminase enzyme levels had a significant correlation with the severity of COVID-19 (p-value = 0,00). Patients with critical degrees had the highest risk for abnormal AST and severe degrees had the highest risk for abnormal ALT. Conclusion: Transaminase enzymes have a significant correlation with the severity of COVID-19.Keywords:  COVID-19, liver injury, transaminases, disease severity

Abstract

Percutaneous Coronary Intervention (PCI) is one of the essential methods for Acute Coronary Syndrome (ACS) treatment, but this procedure has serious complications. Early loading dose statin has a favorable prognostic impact on patients undergoing PCI. However, this effect is still unclear. Objective: To evaluated the effect of loading high-intensity statin on Major Adverse Cardiac Events (MACE) before PCI in patients with ACS. Methods: Randomized controlled trials were conducted using databases from  CENTRAL, PubMed, Science Direct, and British Medical Journal. Then, titles, abstracts, and full texts were screened using the inclusion criteria to filter out irrelevant studies. Quality assessments were undertaken using the Jadad scale, and the results were narratively synthesized and presented in tables. Results: Seven studies were included in this systematic review after screening 1037 records. Four studies demonstrated a statistically significant reduction in MACE in the groups that received loading dose atorvastatin or rosuvastatin before PCI, compared with the comparator groups (p < 0.05). Conclusion: Loading high-intensity statin before PCI seems to be highly effective in reducing MACE in patients with ACS.Keywords:  acute coronary syndrome, major adverse cardiac events, percutaneous coronary intervention

Abstract

COVID-19 (coronavirus disease 2019) is a pulmonary infection disease caused by SARS-CoV-2. Chest radiographic examination of COVID-19 cases performed to make a diagnosis and monitor therapy. Objective: To    evaluated the results of chest radiography examination in COVID-19 cases as a predictor of patient outcome after treatment at the Sultan Agung Islamic Hospital in Semarang. Methods: This research was an observational analytic study with a retrospective design using medical record data from January until December 2021. The results of chest radiographs were analyzed using the Brixia Score system with a maximum score of 18. Outcomes of COVID-19 patients were described as recovered patients or died. Data analysis uses the ROC curve to determine the cut-off point. Results: This study showed that out of 47 patients (43±14 years old), 39 patients (81%) recovered and nine patients died (19%). The levels of leukocytes, D-dimer, Neutrophil-lymphocyte ratio (NLR), and Brixia scores were significantly higher in dead patients (p<0.05). Sensitivity and specificity values predictor of leukocyte count (83%, 61%), D-dimer (83%, 84%), NLR (83%, 82%), and Brixia score (83%, 79%), respectively. Conclusion: Increased levels of leukocytes, D-dimer, NLR, and Brixia Score can be used as a predictor of patient mortality after hospitalization. Keywords: Brixia score, COVID-19, thoracic radiograph

Abstract

Benign Prostatic Hyperplasia (BPH) is the growth of non-malignant cells or hyperplasia of prostate tissue, which causes Lower Urinary Tract Symptoms (LUTS) in men. One guide to direct and determine the symptoms of obstruction due to prostate enlargement is the International Prostate Symptom Score (IPSS). Objective: To determined the relationship between prostate volume and IPSS score in BPH patients at Siti Khodijah Muhammadiyah Cabang Sepanjang Hospital. Methods: This was an analytic observational study using a prospective cohort method. The sample selection used consecutive sampling techniques in accordance with predetermined inclusion and exclusion criteria for collecting data using primary and secondary data, abdominal ultrasound data, and the IPSS questionnaire. The number of samples used was 30 patients from November 2022 to April 2023. Results: The prostate volume in grade II was 46.7%, while for grade I and grade III were 33.3% and 20%, respectively. For the results of the IPSS, the moderate category was 76.7%, with mild and severe IPSS scores at 10% and 13.3%, respectively. The Spearman test results showed that the p-value was 0.006 <0.05, the value of r = 0.486, and the correlation coefficient was positive, meaning there was a unidirectional relationship between prostate volume and the IPSS score. Conclusion: There is a reasonably strong relationship between prostate volume and IPSS score in BPH patients at Siti Khodijah Muhammadiyah Cabang Sepanjang Hospital. Keywords:  BPH, prostate volume, IPSS
