Abstract

Type 2 Diabetes Mellitus (T2DM) is one of the leading risk factors in developing Pulmonary Tuberculosis (PTB) and associated with a higher risk in recurrence, treatment failure, and MDR-TB. Duration of PTB treatment usually takes six months with first line regimen, however in uncontrolled blood glucose confirmed by HbA1c, Fasting Blood Glucose (FBG), and Postprandial Glucose (PPG) the treatment takes longer than usual because of the difficulty to achieve an optimal management in both diseases. The aim of this study was to assess the correlation between glycemic control and duration of anti-tuberculosis treatment at Persahabatan General Hospital, Jakarta in 2019-2021. It was a non-experimental study with analytical observational design and retrospective approach by using medical records. Data were analyzed descriptively and by the chi square method. Odds ratio and relative risk measure the association between duration of treatment in PTB patients and their gylcemic controls. The results showed that 57 PTB patients with T2DM (69.5%) received nine months course of anti-tuberculosis therapy. Most patients tend to have poor glycemic control shown by HbA1c level >7% (79.3%), FBG >130 mg/dL (72%), and PPG >180 mg/dL (80.5%). Correlation between glycemic control and duration of PTB treatment are significant shown by p-value result 0,001. The OR result was found to be 8.74 (95% CI 2.45-31.11) which indicate that patients with poor glycemic control have a greater risk to experience longer duration of PTB treatment. In conclusion, duration of PTB treatment are mostly done in more than six months due to poor glycemic control.

Abstract

Penggunaan obat Antidiabetik Oral pada pasien gagal ginjal kronik stage 1(satu) sampai stage 3 (tiga) dengan terapi tunggal dan terapi kombinasi memilki perbedaan penurunan gula darah puasa sebelum pemberian terapi dan sesudah pemberian terapi. Tujuan dari penelitian ini untuk mengetahui dan menganalisa penggunaan obat antidiabetik oral  terapi tunggal dan kombinasi pada pasien penyakit gagal ginjal kronik yang disertai diabetes melitus tipe 2 pada tahap 1 (satu)  sampai tahap 3 (tiga) di Rumah Sakit Universitas Hasanuddin. Penelitian dilakukan di Rumah Sakit Universitas Hasanuddin mulai tanggal 16 Mei sampai 16 Juli 2016. Penelitian ini merupakan studi kohort crossecsional dengan teknik pengambilan sampel secara repeated survey dengan sampel berjumlah 30 orang yang dikelompokkan menjadi 2 kelompok yang menggunakan obat terapi tunggal antidiabetik oral dan menggunakan terapi kombinasi antidiabetik oral dengan obat antidiabetik golongan lain. Kemudian masing-masing kelompok dibandingkan antara efek pemberian terapi tunggal dan kombinasi  dan kejadian efek samping menggunakan kuesioner. Perbedaan terapi tunggal dan kombinasi berdasarkan perbedaan penurunan gula darah dan HbA1c  menggunakan uji mann whitneyt U dan uji chi Wilcoxon, p<0,05 dinyatakan bermakna secara statistik. Hasil penelitian menunjukkan bahwa  perbandingan efek terapi penggunaan obat antidiabetik oral terapi tunggal dan kombinasi bedasarkan penurunan gula darah dengan nilai p< 0,05 yang berarti bermakna signifikan secara statistik dimana setelah pemberian terapi  yang memberikan hasil penurunan yang efektif  yaitu pada terapi kombinasi. Sedangkan presentase kejadian efek samping terbanyak  terjadi pada kelompok terapi kombinasi yaitu pusing (26.67%).

Abstract

The Enhanced Recovery After Cesarean Section (ERACS) method is an effort to improve the quality of cesarean delivery service through standardization of perioperative management, supported by evidence-based medicine. This study assessed the impact of implementing ERACS method on hospital service quality, focusing on patient recovery time and satisfaction. Furthermore, it also measures the influence of age and educational background in moderating the effect of service quality. A prospective cross-sectional study was conducted using a consecutive sampling method on 75 patients who underwent ERACS and were selected according to the inclusion criteria. Documentation on patients’ perception of service quality and satisfaction was reviewed post-surgically before discharge. The hypothesis test showed that ERACS has a significant positive effect on patient satisfaction (β= 0.9, p= <0.05), and it has a significant negative effect on length of hospital stay (β= -0.29, p= <0.01). Age and educational background did not moderate the effect of ERACS on patient satisfaction (β= 0.001, p= >0.05 and β= 0.01, p= >0.05, respectively). The education factor also did not moderate the effect of ERACS on the length of hospital stay (β= 0.01, p= >0.05). In contrast, the age factor moderated the effect of ERACS on the length of hospital stay (β= 0.31, p= <0.05). The ERACS method is feasible to be carried out at a level two public hospital with good patient outcomes and satisfaction. Young aged group of patients were good candidates for ERACS accelerated recovery time protocol.

Abstract

The administration of booster vaccines has been implemented in a number of countries. Unfortunately, there are limited studies of the immune response after booster to recipient with CoronaVac inactivated primary vaccine. This scoping review aims to determine heterologous booster profiles for recipients of the CoronaVac inactivated primary vaccine. This study obtained data from PubMed, Cochrane Library, Medrxiv, Google Scholar and Grey literature. The inclusion criteria were Sars-Cov-2 article, heterologous boosters and CoronaVac vaccine published from 2020 to 2022, written in English, and in a form of an original research articles, project reports, articles with RCT research designs, cohorts and articles with human research subjects. The keywords were Sars-cov-2, COVID-19, heterologous boosters and CoronaVac. Profiles assessed were heterologous vaccine types, immune responses, intervals between the booster administration and the primary vaccine, and heterologous booster doses. The number of articles that met the inclusion criteria was 19 articles. Types of vaccines that could be used as heterologous boosters for recipients of CoronaVac inactivated primary vaccine were BNT162b2, AZD1222, mRNA-1273, Ad26.COV2-S. The administration of heterologous booster vaccines for recipients of the CoronaVac inactivated primary vaccine was able to increase antibody levels against of the Sars-Cov-2 variants of Mild, Beta, Gamma, Delta and Omicron. The doses of booster vaccines (Ad26.COV2-S, BNT162b2, AZD1222, mRNA-1273 (Moderna) and CoronaVac) for the CoronaVac recipients respectively were 0.5 mL, 0.3 mL, 0.5mL, 100 µg and 0.5 mL. In conclusion, the heterologous boosters for the CoronaVac recipients could enhance immune responses against the Sars Cov 2.

Abstract

Risperidone is an atypical antipsychotic for acute and chronic schizophrenia and mania. In this case report, we report a rare side effect of risperidone, namely nonpuerperal lactation, where the common side effects of risperidone are insomnia, anxiety, headaches, and extrapyramidal symptoms. Lactation nonpuerperal is one of the rare side effects of risperidone, with an incidence in adults < 1%. We report the case of a 21-year-old woman. This woman was diagnosed with bipolar affective disorder with major depression and got risperidone, fluoxetine, trihexyphenidyl, and valproic acid for treatment. The first visit was at a teaching hospital on April 21, 2022. She had to control it within a month. At the first control on July 1, 2022, her condition improved. She had no complaints. On August 4, 2022, she came to the hospital and told the doctor that she had nonpuerperal lactation. We do a causality analysis that the side-effect is due to risperidone. From Naranjo’s analysis, we scored 6 for risperidone (probable). From the Liverpool algorithm, we concluded that risperidone probably caused lactation nonpuerperal. In this case, risperidone probably caused lactation nonpuerperal.

Abstract

Stroke iskemik terjadi ketika arteri otak terhambat sehingga aliran darah berkurang menuju otak. Stroke iskemik membutuhkan perawatan yang lama dan pemakaian regimen obat yang komplek sehingga berpotensi terjadi Drug Related Problems (DRPs), kejadian yang tidak diharapkan berkaitan dengan terapi obat yang dapat menggangu keberhasilan dalam proses penyembuhan. Penelitian bertujuan mengidentifikasi penyebab terjadinya DRPs pada pasien geriatri yang didiagnosa penyakit stroke iskemik. Studi ini merupakan observasional dengan menggunakan desain penelitian cross sectional. Pengumpulan data dilakukan secara retrospektif melalui rekam medis tahun 2020 di Rumah sakit Bhayangkara Anton Soedjarwo, Kota Pontianak. Sejumlah 94 pasien memenuhi kriteria inklusi meliputi pasien dengan diagnosa stroke iskemik berumur 60 tahun atau lebih dan rekam medis lengkap. DRPs diklasifikasikan berdasarkan penyebabnya menggunakan PCNE (Pharmaceutical Care Network Europe) ver 9.0. Analisis deskriptif dilakukan untuk mendiskripsikan jumlah dan jenis DRPs. DRPs ditemukan pada 28 pasien (29,78%) dengan total kejadian DRPs sebanyak 44 kejadian. DRPs diklasifikasikan dalam penyebab yang berkaitan dengan pemilihan obat yang tidak tepat sebanyak 24 pasien (25,53%) dan dosis tidak tepat sebanyak 20 pasien (21,26%). Jenis obat yang banyak menimbulkan DRPs adalah Amlodipin, dan ditemukan sebanyak 14 (23,33%). Adanya DRPs pasien geriatri stroke iskemik menjadi tantangan bagi farmasis untuk dapat menyelesaikan masalah terkait obat sehingga meningkatkan optimasi terapi pasien stroke iskemik.

Abstract

Ventilator-associated pneumonia (VAP) is a pulmonary infection that occurs after more than 48 hours of mechanical ventilation or endotracheal intubation. VAP is the most common nosocomial infection in ICU (Intensive Care Unit), and of course the use of antibiotics will affect the treatment of the infection. This study aims to evaluate the quality of antibiotic use for VAP treatment in ICU Dharmais Cancer Hospital using Gyssens method, evaluate the different outcomes of antibiotic therapy that are rational and irrational according to the Gyssens method, and evaluating the distribution of rational (category 0) and irrational (category 1-5) antibiotic use. This prospective study was carried out from February to May 2017 with descriptive analyses. Antibiotic uses were documented prospectively by a pharmacist and analysis by using Gyssens method. Pharmacist was used total sampling method, which is included all adult VAP patients in ICU. A total of 29 patients (18,24%) were reviewed from 159 patients. The most dominant antibiotic use for VAP empiric treatment was meropenem and the most dominant antibiotic use for VAP definitive treatment was levofloxacin. Factors affecting the quality of antibiotic use include type of the therapy, number of antibiotics used by patient and length of stay (P>0,05). Factors affecting the therapy outcome of VAP patient was time of antibiotic use (P<0.05). The relationship between outcome therapy and quality of antibiotic use based on Gyssens method shows on category 0 which died 64,3%, category 2 died 66,7% and the category 4 died 75%. The relationship between outcome therapy and quality of antibiotic use based on Gyssens method shows more irrational use of antibiotics then the therapy outcome is not getting better.

Abstract

During the COVID-19 pandemic, the consumption of vitamin supplements has become a new habit. As a result of the epidemic, people's attitudes and behaviours had altered, particularly when it came to consuming vitamin supplements to boost immunity or maintain health. The aim of this study was to analyze the correlation between the attitude and behaviour of the community toward vitamin supplements during the COVID-19 pandemic. A cross-sectional study was conducted in two cities in Indonesia, Jakarta and Pandeglang Banten, in August-October 2021. Each subject filled out an attitude and a behavioural questionnaire, both of which were made by researchers based on many references. All data collected by google form software analyzed the Socioeconomi characteristics by chi Square test, while the correlation of attitude and behaviour tested by Spearman rho test. The characteristics of Socioeconomi such as education, consumption of vitamin supplements during the COVID-19 period, and people's residence were correlated significantly with attitude and behaviour (P<0.005). there was a significant correlation between attitude and behaviour in using vitamin supplements to increase immunity during the COVID-19 pandemic (P=0.001, correlation coef=0.465). The correlation was moderate strength with a positive path correlation. It means that the more support an attitude, the more behaviour will increase to use vitamin supplements during COVID-19 pandemic.
