Abstract

Periodontitis is periodontal inflammation in response to plaque bacterial antigens, causing damage to periodontal ligament and alveolar bone resorption. Bone graft material combination i.e. demineralized freeze-dried bone allograft (DFDBA) and hydroxyapatite (HA) using sandwich bone augmentation (SBA) method will support each other and will be beneficial to be used as a scaffold. The body takes long time to resorb HA so this could complement DFDBA which is more easily dissolved. This study aimed to reveal the effect of bone graft addition using SBA method on the treatment of infrabony pocket with open flap debridement in terms of probing depth, relative attachment loss, alveolar bone height, and density. This study was carried out to 20 infrabony pockets, where 10 of them were treated using open flap debridement with HA addition, while the other 10 groups were treated using open flap debridement with DFDBA and HA using SBA method. Probing depth  and relative attachment loss were measured on days 0, 30 and 90. Bone height and density were measured using cone-beam computed tomography (images on day 0 and 90). The study showed that probing depth reduction on SBA group was greater than HA group. There were significant differences in probing depth and relative attachment loss examinations. However, bone height and bone density reduction did not show any significant difference. The conclusion from this study is open flap debridement using SBA method yields better regeneration in terms of probing depth and relative attachment loss than open flap debridement with HA addition. There is no difference in bone height and bone density between the two groups.

Abstract

Orbital cellulitis is uncommon sequelae of the spread of odontogenic infection to adjacent maxillary sinuses or to distant sites such as the orbits. Once it happened, the spread of such infection can be of serious complication such as complete blindness or even more serious and life threatening situations as cavernous sinus thrombosis, intra-cranial abscess, or even death. This paper aimed to expose the guideline treatment of orbital cellulitis as a complication of odontogenic infection. It reported one case of orbital cellulitis in the emergency unit of Hasan Sadikin hospital Bandung, with complaints of pain, swelling at cheek and periorbital region, and the history of toothache. The treatment include incision drainage, extraction of tooth, and parenteral administration of antibiotic and analgesic. Odontogenic infections are derived from dental infection and can potentially spread rapidly to be ascending infection like orbital cellulitis. Odontogenic infection and orbital cellulitis should be adequately treated with incision drainage, extraction of tooth and parenteral administration of antibiotic, steroid and analgesic. Without immediate treatment, odontogenic infection can lead to ascending infection. Orbital cellulitis due to spreading of odontogenic infection is a rare case. In this case report, the patient had a significantimprovement due to immediate and proper treatment.

Abstract

Denture base material should have a good level of biocompatibility. Acrylic resin is frequently used as a denture base material, however it has a disadvantage of producing residual monomer. Residual monomer is known to have a cytotoxicity effect. Titanium dioxide (TiO2) nanoparticles are used as fillers due to their biocompatibility and ability to enhance the mechanical properties of acrylic resin. The addition of the material to acrylic resin could affect the amount of residual monomer. The aim of this study was to examine the effect of the addition of TiO2 nanoparticles as acrylic resin denture base filler on the cytotoxicity in fibroblast cells. The samples consisted of 24 heat cured acrylic resins in disc shape (5 mm in diameter and 2 mm in thickness), divided into 4 groups (n = 6): three groups given treatment with0.5%, 1%, 2% TiO2, respectively and one control group. Cell viability was measured with MTT assay. The results were tested with one way ANOVA with 95% confidence level followed by LSD post hoc test. The results showed that the highest percentage of cell viability was found in the treatment group of 0.5% TiO2 with value of 91.83 ± 1.75%, while the lowest value was seen in the treatment group of 2% TiO2 with value of 79.38 ± 3.34%. Significant differences were shown between the treatment groups of 0.5% and 2% TiO2, as well as between the control and treatment group with 2% TiO2. The conclusions of this research are the addition of TiO2 nanoparticles as acrylic resin denture base filler has an effect on cytotoxicity; the addition of 0.5% TiO2 nanoparticles filler results in lower cytotoxicity on fibroblast cells compared to the addition of 1% and 2% TiO2.

Abstract

Tooth extraction is a common treatment in dental practice. However, complications such as bleeding or dry socket may occur during healing process. Pangas catfish (Pangasius djambal) gelatin contains glutamine which could increase the proliferation of fibroblast, accordingly collagen synthesis increases. The study aimed to demonstrate collagen density after the application of pangas catfish gelatin on post-tooth extraction wound of albino rats (Rattus norvegicus). This wasan experimental study with randomized posttest-only control group design which involved male Wistar strain albino rats. The rats were selected using a random sampling method and divided into four groups, namely day-3 control group (n=7),day-7 control group (n=7), day-3 treatment group (n=7) and day-7 treatment group (n=7). Pangas catfish gelatin was administered into the socket once after extraction. The variable measured in the study was collagen density, determinedby collagen scoring of hematoxylin eosin–stained histopathological preparations. Collagen formation were determined according to the assessment criteria of collagen distribution and density. Data analysis showed there was a significantdifference of collagen synthesis between the control and treatment groups according to Kruskal-Wallis test of which the result of control group day-3 was 1.13 ± 0.34, control group day-7 was 1.76 ± 0.67, treatment group day-3 was 1.66 ±0.47 and treatment group day-7 was 2.93 ± 0.25. In addition, the day-7 treatment group showed the highest collagen scoring values as compared to the control groups according to the Mann-Whitney test (p<0.05). In conclusion, pangascatfish gelatin increased collagen density during dental socket healing process after tooth extraction in albino rats.

Abstract

Periodontal tissue damage requires regenerative material to repair the damage. Platelet-rich plasma (PRP) is known as a regenerative material from blood which contains high level of growth factor that plays a role in wound healing and tissue remodeling. However PRP has a weakness, i.e. it is too watery so it is easily dissolved in the oral cavity, and should be used immediately after preparation. Therefore PRP storage method is needed to increase the benefits of PRP. The addition of collagen to PRP serves as a scaffold as well as an activator that stimulates the release of growth factors. One method of storing PRP is by freeze-drying process. The purpose of this study was to analyze the effect of freeze-drying process of collagen-activated PRP (PRP+C) on transforming growth factor-β1 (TGF-β1) levels. Transforming growth factor-β1 is a cytokine content in PRP, that plays a role in bone remodeling and is an important stimulator for osteoblast formation, causing chemotaxis, osteoblast proliferation and differentiation. In this study, PRP was produced from peripheral blood probandus. Platelet-rich plasma was then activated with collagen (PRP+C), and divided into two groups: freeze-dried PRP collagen (FD PRP+C); and non freeze-dried PRP+collagen (PRP+C). Transforming growth factor-β1 levels were measured using the ELISA method, followed by independent t-test. The TGF-B1 level of FD PRP+C group was significantly higher than PRP+C group (p<0.05). From this study it can be concluded that freeze-dried collagen-activated PRP has an effect to increase TGF-β1 level.

Abstract

A good sealing ability of apical part of root canal system is needed to prevent microleakage thus resulting long-lasting successful treatment. Chitosan nanoparticles as nanofillers could be added to epoxy resin sealer to increase adaptation to dentinal wall. The purpose of this study was to evaluate the apical sealing ability of chitosan nanoparticles addition to epoxy-resin-based sealer. Thirty mandibular premolars were used in this study. The root canals were prepared using rotary files up to #30/0.09. The canal was irrigated with 2.5% NaOCl and 17% EDTA, then was rinsed with sterile water. All teeth were randomly divided into 3 groups (n=10). Group I was obturated with gutta-percha, Group II was obturated with gutta-percha and epoxy resin sealer, group III was obturated with gutta-percha and epoxy resin sealer that was added with chitosan nanoparticles. All specimens were stored in an incubator for 7 days at 37 °C. After that, specimens were tested with centrifuging dye penetration method with methylene blue 2% solution. The specimens were longitudinally sectioned, observed under stereomicroscope (8x magnification) and measured in millimeters (mm). The data were analyzed using one-way Anova. The results of one-way Anova showed that dye penetration in apical part of the root canal in all groups was statistically significant difference. The addition of chitosan nanoparticles to epoxy resin sealer increases the apical sealing ability of root canal obturation material.

Abstract

Patients, especially those who use health insurance have felt that the quality of primary care services tends to be less satisfying in terms of both public services and dental care services. Based on the results of the annual report at the Kahuripan Health Center, the number of patient visits to BPG decreased from 2016 to 2017 by 802 people. This study aimed to determine the quality of dental care services and the satisfaction of patients visiting BPG Kahuripan Health Center. This case study used a qualitative method and an explanatory research design. The sample in this study consisted of 40 patients as the respondents responding to the questionnaires about service quality and patient satisfaction, and 12 people were interviewed. The results showed that all dimensions of the quality of dental care services at the Kahuripan Health Center were categorized as very good i.e. real dimensions (82.71%), reliability (83.21%), responsiveness (87.12%), assurance (84.93%), and empathy (83.54%). As for patient satisfaction, three stages of patient satisfaction were categorized as very satisfied, namely the anamnesis and diagnosis stage (83.02%), action (83%), and completion of action (82.62%). The stages categorized as satisfying were the registration and waiting room stage (78.87%), and the stage of preparation of treatment (72.50%). The result of interview showed that the average respondents stated that the quality of dental care services was very good. All the five dimensions of the quality of dental care services were categorized as very good. Patients who visited BPG expressed satisfaction with the five stages, namely they were very satisfied with the three stages and satisfied with the other two stages.

Abstract

Recurrent aphthous stomatitis (RAS) is an oral mucosal disease which was more prevalent in women than men. Study has shown that zinc deficiency associated with the occurance of RAS. Other study also have shown that vitamin D deficiency contributed in RAS. However, there is no study yet that reveal the association between serum zinc and vitamin D/25 (OH)D in women with RAS. To investigate the association between serum zinc and vitamin D/25(OH)D in women with RAS. Thirty two women patients with RAS who meet the inclusion criteria participated in this study. The inclusions criteria was non pregnant women who have been diagnosed RAS and did not have other oral diseases. The exclusions criteria were those who have systemic diseases, taking medications including multivitamin, and have bad habits (i.e. smoking). Data concerning characteristic of subjects, severity of RAS, serum vitamin D/25(OH)D, and zinc were collected and presented descriptively. The correlation between serum zinc and Vitamin D/25(OH)D was analyzed using Pearson correlation test with 95% confidence interval. This study has been approved by Medical and Health Ethics Committe, Faculty of Medicine, Universitas Gadjah Mada, Yogyakarta. All subjects of this study have low mean value of serum vitamin D/25(OH)D (11.08 ± 3.11 ng/mL) and categorized with vitamin D deficiency. The subjects also have low mean value of serum zinc (54.78 ± 9.19 μg/dL) and seventy percent subjects have mean value of serum zinc below normal. The result of Pearson correlation test showed that there was a significant positive correlation between mean serum zinc and Vitamin D/25(OH)D (r= 0.351, p<0.05) in women with RAS. Serum zinc associated with vitamin D/25(OH)D and play role in pathogenesis of RAS.

Abstract

Panoramic radiography is a radiography technique which is commonly used in dentistry. X-ray radiation from panoramic radiography can cause biological damage, such as the increase of gingival crevicular fluid (GCF) volume. One of materials which was known having the ability to decrease the effect of panoramic radiography is β-carotene in mucoadhesive gingival patch form. The application of β-carotene mucoadhesive gingival patch reduce micronucleus increasing in previous study. The aim of this study is to determine the effect of β-carotene mucoadhesive gingival patch application on palatal gingiva of maxillary incisor teeth to the GCF volume of panoramic radiography exposed patients. This study involved 10 subjects (5 females and 5 males). The GCF sample was taken from the labial sulcus of 11, 12 teeth (treatment group) and 21, 22 teeth (control group) of each subject before and after panoramic radiography exposure. β-carotene mucoadhesive gingival patch was applicated on palatal gingiva before panoramic radiography examination. Sample was collected using an absorbent paper which is inserted to the labial gingival sulcus. The GCF on the absorbent papers were marked by adding ninhydrin 2%, then the GCF volume was measured using a sliding caliper. Paired t-test result shows a significant difference between GCF volume in each group before and after panoramic radiography exposure (p<0.05). Independent t-test shows no significant difference between the GCF volume difference in the control and treatment group (p>0.05). The result of this study shows that the application of β-carotene mucoadhesive gingival patch on palatal gingiva of maxillary incisor teeth shows insignificant effect in decreasing the increase of GCF volume in panoramic radiography exposed patients.

Abstract

People with down syndrome have a different craniofacial structure compared to normal individuals. Commonly, down syndrome patients have abnormal ear morphology with smaller size, and lower ear position compared to that of normalsubjects. These patients also have smaller nose, shorter maxilla structure and facial height. Other disorders are anomalies in tooth shape, numbers and size. The ala-tragus line is one of the anatomical landmarks to determine the occlusal plane orientation. Abnormalities in the anatomical structure of people with down syndrome can affect the parallelism between the ala-tragus line and the occlusal plane. The purpose of this research was to determine the differences in parallelism between the ala-tragus line and the occlusal plane of people with down syndrome and normal individuals. The sample in this study were people with down syndrome and normal individuals. The sample was photographed using a digital camera with a fox plane attached to the oral cavity. Parallelism of the ala-tragus line and the occlusal plane was analyzed by tracing these photographs. Photo tracing was done using the Autocad 2013 software. The mean value of angle on people with down syndrome (5.852° ± 5.367°) was greater than that of normal individuals (2.169° ± 2.557°). Based on the research, it was concluded that the parallelism of the ala-tragus line to the occlusal plane in people with down syndrome was different from that in normal individuals. The ala-tragus inferior line in normal individuals was more parallel to the occlusal plane compared to that in people with down syndrome.
