Abstract

Precision medicine, particularly pharmacogenetic-guided approaches, has emerged as a promising tool to optimize treatment strategies for cardiovascular disease. However, the economic evaluation of these approaches in Asian populations remains underexplored. This review study aimed to synthesize evidence on the implementation, methodology, research gaps, and limitations of pharmacogenetic-guided precision medicine in cardiovascular disease among the Asian population. A comprehensive search of electronic databases (Pubmed, Embase, Google Scholar and Cochrane Library) was conducted to identify relevant studies. The data were extracted and synthesized to address the study objectives A total of 12 studies were included in the analysis. Economic evaluation studies of pharmacogenetic-guided precision medicine in cardiovascular disease management in Asian countries are still limited, with precision medicine interventions often requiring high resources. The sampling process remains a challenge in conducting economic evaluation studies, and there are limitations in obtaining clinical outcome descriptions from real-world evidence, as most studies rely on literature reviews or modeling-based approaches. Furthermore, most studies use a provider or healthcare cost perspective, limiting the comprehensiveness of cost information. This evidence synthesis highlights the potential for pharmacogenetic-guided precision medicine in cardiovascular disease management among the Asian population, as well as the current challenges and limitations in conducting economic evaluations. Further research is needed to address these issues, develop more robust sampling strategies, obtain real-world clinical outcome data, and provide comprehensive cost information to inform clinical practice and healthcare policy in the region.

Abstract

X Hospital is one of the hospitals in Surabaya that continues to improve its medical services following several negative reviews found on Google Business between January and April 2024. These reviews suggest that human resource performance, particularly in patient care, needs improvement. In response, the hospital is focusing on internal development by cultivating a workforce ecosystem that includes organic employees, non-organic employees, and interns. This study aims to analyze the development of the workforce ecosystem at Manyar Hospital by evluating employee performance, skill training, and on-the-job training among 116 organic employees, 88 non-organic employees, and 20 interns, with a sample size of 65 participants. Descriptive statistical analysis was employed, with samples selected through convenience sampling. The results indicated that, for employee performance, the highest mean value was found in supervisor social support, while the lowest was found in empowerment. For skill training, the highest mean value was found in the ability to ask, while the lowest was found in self-control. For on-the-job training, the highest mean value was found in the effect on the job, employee training, and deductive and inductive on-the-job training strategies, while the lowest mean was found in structured on-the-job training in developing nations.

Abstract

This manuscrip present the perceptions from intern doctors regarding their abilities, confidence and experience. A mailing survey was conducted with the question of how respondents would perceive if they did not undergo the internship program. Analysis using simple bivariate logistic regression. The results there were 6830 responden, Average ability= 6.31 Confidence=6.27 dan Experience=5.95. 46.8% of respondents felt they had abilities, 47.5% felt confident and 54.6% had experience without having to participate in an internship program The variable that statistically significantly affects the perception of ability is gender (Man p <0.01 OR=0.679 95%CI=0.612-0.754), while for self-confidence also gender (Man p <0.01 OR=0.667 95%CI=0.601-0.741); Experience (Man p <0.01 OR=0.721 95%CI=0.650-0.799) Conclusion. Perception of newly graduated doctors almost same beetween the groups that feel capable, confident and have experience and the group without having to go through the internship process. Suggestions need to be done cost benefit studies to assess whether the internsip program is useful for all parties.

Abstract

Indonesia faces a double burden of disease, with an increasing prevalence of non-communicable diseases (NCDs), such as diabetes mellitus (DM), alongside communicable diseases. This study investigates the determinants of DM prevalence among individuals aged 15 years and older in Indonesia. An ecological design was employed using secondary data from the 2018 Basic Health Research Report, 2018 Welfare Statistics Report, and the 2019 Provincial in Figures Report, encompassing 514 districts across 34 provinces. Data analysis included univariate descriptive statistics, Pearson correlation for bivariate analysis, and multiple linear regression with a backward method to identify significant predictors. The results revealed significant associations between DM prevalence and hypertension (p < 0.001), obesity (p < 0.001), routine blood sugar testing (p < 0.001), good nutrition (p = 0.016), the ratio of general hospitals (p < 0.001), public health centers (p < 0.001), and physicians (p < 0.001), as well as the interaction between hospital and physician ratios (p < 0.001). The model explained 61.5% of the variance in DM prevalence (R² = 0.615), meeting all multiple linear regression assumptions. Prevalence of hypertension, obesity, and higher ratios of general hospitals and physicians were positively correlated with DM prevalence, while good nutrition and greater availability of public health centers exhibited protective effects. The positive association of routine blood sugar testing likely reflects increased screening in high-DM areas. These findings highlight the need for targeted public health interventions, emphasizing obesity reduction, nutritional improvements, and equitable healthcare distribution to mitigate DM prevalence in Indonesia.

Abstract

Pregnant women and newborns are particularly vulnerable during public health emergencies in Indonesia, where there is a lack of specific guidance to support these groups when emergent threats (ET) arise. Health workers, crucial in delivering maternal and newborn health (MNH) services, often fail to anticipate and address the needs of these vulnerable populations adequately.A literature review analyzing 24 articles from PubMed, Dimension, and Web of Science published between 2016 and 2024 focused on emerging infectious diseases, maternal and newborn health, quality of care, referral systems, and small vulnerable newborns. The review identified substantial gaps in the skills and competencies needed to provide high-quality MNH services globally and in Indonesia. Key issues include non-compliance with existing guidelines, inefficient referral coordination, insufficient monitoring of sick mothers, and a lack of preparedness for emergent threats, all contributing to suboptimal MNH outcomes.These gaps could be addressed by adopting improved training and referral systems, as proposed in the new SVN conceptual framework. Essential actions to enhance MNH outcomes include improving guideline compliance, enhancing referral systems, and ensuring comprehensive training for healthcare providers. Developing an integrated tool tailored to Indonesia's needs that includes SVN, sick mothers, infrastructure, referral systems, and infectious disease management is vital. Implementing these strategies can significantly reduce adverse outcomes and enhance care quality for women and newborns during public health crises.

Abstract

In the sixth year, the implementation of JKN has resulted in achievements, especially community access to treatment at health facilities, but there are still many problems, especially in hospitals. This research is an observational study with qualitative methods. The informants are two government hospital directors and three private hospital directors, a triangulation of BPJSK officials. Hospitals have adaptively made adjustments, especially regarding the increase in the number of BPJSK participating patients, preparation of facilities and facilities, socialization of JPSBK regulations, and improvement of services to maintain the quality of services and to increase BPJSK patient satisfaction. Medical staff workload has increased but has yet to be accompanied by increased medical incentives, restrictions on medical authority, especially in administering drugs according to the BPJSK formulary and limiting medical support services and patient care days. This was all done to control costs due to reduced hospital income due to Ina-CBG payments from BPJSK. Hospitals must be innovative and creative in managing finances and generating revenue. The problem of late payments (claim pending) needs to be resolved immediately. Likewise, the satisfaction of internal hospital staff needs to be considered.

Abstract

Background: Health transformation in Indonesia is aimed at achieving one health data. Health data interoperability is the key to comprehensive health services. Integration can be done if a sistem is interoperable. This case study was conducted at a local government hospital in West Java Province. Concerning the vision of national interoperability, it is important to assess interoperability readiness.Objective: To explore the interoperability challenges of medical records in RSUD SMC Tasikmalaya.Method: A qualitative research design through in-depth interviews with 11 participants to explore the interoperability of medical records based on the framework of interoperability maturity aspects.Results: The interoperability of medical records at RSUD SMC is in the preparation phase. Device standardization as a sistem compatibility requirement has been implemented. Currently, hospitals are faced with technical interoperability challenges, both compliance with medical record data variables, sistem quality challenges, and medical record information quality, as well as data security in hospitals. For this reason, maturity support is needed in aspects of information technology leadership and governance to strengthen digital investment and vendor cooperation. The functionality of human resources in the implementation of information sistems through the support of human resource capacity building for front and back-end developers, medical record experts, and data engineers is in line with the strengthening of technology infrastructure. Conclusion: To improving  quality of the RME interoperability, management of the three aspects of leadership and governance, HR, and technology in hospitals is urgently needed for further strategies for organizational interoperability.

Abstract

National Quality Indicators (INM) at RSGMP X in Semarang haves not reached the target in the aspect of 6-step handwashing compliance and the use of personal protective equipment among doctors, nurses and young dentists. Other research has found that education and training for employees is important in improving their technical skills and professional expertise. This study aims to determine the relationship between education and training as one of the elements of TQM and the achievement of INM at RSGMP X in Semarang City. This study is an observational quantitative study with a cross sectional approach. Data collection using a questionnaire by google form with a total sample of 75 of employee. Data analysis was performed bivariately with chi square. There is a relationship between perceptions of education and training aspects and the achievement of INM at RSGMP X in Semarang City. In addition, respondents who have good perceptions of education and training aspects tend to support the achievement of good INM (57.3%) compared to those who are not good (25.3%). Respondents with unfavorable perceptions of education and training tended to be less supportive of INM achievement (12%) compared to those who were supportive of INM achievement (5.4%). Most of the study respondents had good perceptions on education and training aspects (82.6%). In improving the achievement of National Quality Indicators, hospitals need to emphasize education and training efforts.Keywords : TQM, National Quality Indicators, Indikator Nasional Mutu, Education, Training

Abstract

This study explores how patient experience influences patient loyalty, with trustworthiness and outcome quality as mediators. In Indonesia, health clinics have been segmented since introducing the national health insurance scheme called BPJS. While universal healthcare was achieved, some patients are willing to pay a premium for better care experiences. Understanding patient loyalty and its antecedents is crucial for maintaining a competitive advantage in private clinics. The study was conducted at a private clinic in Batam, involving 117 patients who visited between October and December 2023. Data were collected using a modified version of the GS-PEQ (Generic Short-Patient Experience Questionnaire) and further analyzed with Partial Least Squares Structural Equation Modeling (PLS-SEM) via SmartPLS4® software. Ten hypotheses were tested to explore the relationships between patient experience, trustworthiness, outcome quality, and patient loyalty. The findings reveal that patient experience positively impacts patient loyalty, which is mediated by trustworthiness. Outcome quality mediates the relationship between patient experience and trustworthiness but does not significantly mediate the relationship between patient experience and patient loyalty. This study highlights the importance of patient experience and trustworthiness in private clinics and suggests the use of GS-PEQ to measure patient experience and achieve competitive advantage.

Abstract

Coastal communities in Indonesia face interconnected challenges from climate change and public health vulnerabilities, yet the relationships between these factors remain poorly understood. We investigated these connections across eight Indonesian provinces by analyzing marine dependency, disease susceptibility, and flood risk projections using Data Envelopment Analysis (DEA) and multiple datasets. Our analysis revealed that South Kalimantan's coastal communities are particularly vulnerable, demonstrating the highest dependency on marine resources while simultaneously facing significant health and environmental challenges. This heightened vulnerability is evidenced by elevated Marine Capture Fisheries Households (RTPT) scores and fish capture data, indicating substantial economic reliance on marine resources. The challenges are further intensified by projected flood risks expected to impact these areas within six years, alongside varying health vulnerabilities including tuberculosis, HIV, dengue fever, and COVID-19 cases. Through our DEA estimation, which incorporated six output and four input variables along with demographic information, we found that South Kalimantan's marine resource dependency significantly exceeds that of other provinces. These findings provide crucial insights for policymakers and health administrators, establishing a data-driven foundation for developing targeted interventions and resource allocation strategies. Our study underscores the necessity of implementing integrated approaches that simultaneously address environmental and public health concerns in coastal communities.
