Abstract

The use of tobacco plant as a symbol of the area as well as batik motif typical of the area showed that tobacco plants also a local wisdom in Jember. But the last 4 years, productivity of tobacco has decreased, while economic growth in Jember steadily growing over the last 4 years. Therefore we draw two formulation of the problem is how the productivity elasticity of tobacco in Jember? and how the relationship with productivity GRDP Jember tobacco? Are there long-term relationship. From the test results we found that the elasticity of productivity tobacco has the same elasticity is inelastic, while Granger kausality test results showed that in both variables affect each other there is no relationship, but only variables that affect GRDP is productivity Tobacco.

Abstract

Diclofenac sodium (DS) is a nonsteroidalantiinflammatory drugs (NSAIDs), that inhibitscyclooxygenase-2 enzyme (COX-2). It has first passmetabolism by 40-50%, because of its shortbiological half-life, the drug has to be givenfrequently (Ganiswara, 2005). Dose of DS in gel is 1%(Sweetman, 2007). In peroral use, it may cause therisk of gastrointestinal bleeding, cardiovascular, hypersensitivity reactions and disorders of thecentral nervous (Katzung, 2002, Chuasuwan, et al.,2008).DS does not penetrate well through skin and cannotreach the effective concentration at the site ofaction after transdermal application (Mohammed,2001; Ozguney, et al., 2006). DS partition coefficientin n-octanol-buffer aqua (log P) was 1.1 (Chuasuwan,2008). Lipophilic nature of the stratum corneum andthe hydrophilic nature of the underlying tissuesshowed that the drug will penetrate the skin shouldhave an optimal balance between the lipophilic andhydrophilic properties. In general, the skin is morepermeable to the material which has a partitioncoefficient (P) in octanol-water between 10-1000(Michniac-Kohn et al., 2005).Oleic acid enhances the penetration of drugs intothe skin by increasing the fluidity of the stratumcorneum lipids through the establishment ofchannels (water channels) (Fang , et al , 2003)This study aimed to know the influence of OleicAcid (OA) in several concentrations (1%, 3% and5%) as penetration enhancer on the in vitropenetration of topical DS gel. Furthermore, thephysical properties of the topical preparation(organoleptic, pH and viscosity), and DS penetrationpercutaneously through rat skin . Then observedthat in spite of DS assay and penetrate through therat skin at certain intervals and observed by meansof HPLC.

Abstract

Intra-Hospital Cardiac Arrest (IHCA) incidence isquite high and varies with an average of 3.8 to 13.1per 1000 patient. Most IHCA (60%) due to heartproblems such as pulseless electrical activity (PEA),asystole, and the combined ventricular fibrillationventriculartachycardia. While, the cause IHCA byhypoxia about 30% (Becker, Aufderheide, &Graham, 2015). The mortality rate are recordedvery high. America Heart Association (AHA) statedthat only 14% of adults and 27% of children whosurvive after experiencing IHCA (Morrison, 2013).Cardiopulmonary resuscitation (CPR) is one of themost important emergency actions in lifethreatiningconditions. The performance ofcardiopulmonary resuscitation has an importantposition in the chain of survival, but despite newtechniques and technology the survival rates fromcardiac arrest are still low and the incidence of inhospitalcardiac arrest is rarely reported in theliterature. Cardiopulmonary resuscitation is acomplex of emergency procedures, if thatperformed correctly, can provide the necessaryminimum of circulation until return of spontaneouscirculation (ROSC) to patients with sudden cardiacarrest (Stiell, et. al., 2012). The high incidence, lowrate of survival, and unpredictability of cardiacarrest makes it a grave public health issue and amedical emergency. The application of CPR plays acritical role in saving lives from cardiac arrest in thehospital, and chest compression is the first part toplays a key role in CPR. (Chen, et al, 2015). Highqualitychest compressions of sufficient depth andrate with full recoil of the chest betweencompressions and avoidance of interruptions arecrucial to patient survival. The efficiency of CPR indecreasing the death rate from avertable causes isa very important factor. However there arerelatively few data available on the efficiency ofcardiopulmonary resuscitation in hospitals, whichplay an essential role in the chain of survival.Despite the fact that the cardiopulmonaryresuscitation was introduced more than 50 yearsago, the survival rate is still relatively low and thereis no trend of improvement of the situationobserved, although the technologies havedeveloped rapidly (Neumar, et.al., 2015).The survival of patients with cardiac arrestdepends on the quality of CPR is given. The successof CPR can be seen from the results whether thepatient is awake and back to life with normalfunction or only slightly deformed, conscious andreturned with moderate disabilities, unconsciouswith severe disabilities, coma or vegetative state, brain death, or death from other causes(Lallestedt, 2011).Although resuscitation guidelines provide a logical, sequential algorithmic approach, they have mainlyemphasized technical tasks performed byindividual rescuers and have not addressed issuesof adapting to the complex nature of most actualresuscitations. Part of this complexity relates tothe fact that in a healthcare environmentresuscitations are usually performed by teams ofresponders, not by isolated rescuers (Bhanji, et.al.,2011). Therefore, high quality CPR must be appliedin each treatment in cardiac arrest by usingmethods and best performance to maintain thequality of life of patients.

Abstract

Probiotics is live micro-organisms which whenadministered in adequate amounts, confer ahealth benefit on the host (FAO / WHO, 2002).Probiotics may give therapeutic effect in minimalamounts 106-107cfu/g daily. Some reports in thelast decade have described that probiotic, such asLactobacillus acidophilus, is useful for treating skindisorder such as dermatitis, acne, cellulitis, andpsoriasis. This relates to the content of probioticsmetabolites that protect the skin from mostpathogenic bacteria, such as Staphylococcusaureus, and improve skin structure (Cinque et al.,2011).In topical application, the active ingredient shouldlast a long time on the skin and prolonged release.Meanwhile, Lactobacillus acidophilus unstable toenvirontment factors, such as the hightemperature, oxygen, and humidity (Al-Hurr,2011). Therefore, probiotics encapsulation isneeded. In addition, encapsulation also mayprovide prolonged release so that its effectivenessincreased as antimicrobial activity becomes longer.Encapsulation process that forms microparticles (1-1000 μm) called micro-encapsulation. Sphericalmicroparticles called microspheres (Sahil et al.,2011). The recommended size of microsphereswhich active ingredient site action is in epidermislayer is 5-300 μm (Chadawar and Shaji, 2007).Sodium alginate is widely used as a matrix ofmicroencapsulated probiotics. Microspheres madewith sodium alginate matrix may improve theviability of probiotics during storage. Gelatin alsooften used in microencapsulated probiotics(Solanki et al., 2013). Gelatin is biodegradable,non-toxic, and easy to experience a cross-linking sothat it can be used in the preparation of colloidaldelivery systems, such as microspheres andnanoparticles (Sailaja, et al., 2010).Type of matrix influence the characteristic ofmicrosphere. Microspheres are only made withsodium alginate tend to provide simultaneousrelease. Therefore for prolonged release, sodiumalginate should be combined with gelatin (Lee etal., 2014; Roy et al., 2009). Combination of sodiumalginate and gelatine in the ratio 1: 2 will providesmall particle size that not easily aggregate duringthe process of mixing and drying (Lee et al., 2014).In this study, microspheres were formed by crosslinking between combination of sodium alginateand gelatin (1% : 2%) with CaCl2 as cross linkerusing ionotropic gelation method.OBJECTIVEThe aim of this research was to investigaste theeffect of combination of sodium alginate andgelatin (1% : 2%) content in characteristics andantimicrobial activity of probiotic microspheresLactobacillus acidophilus.

Abstract

Breast cancer is one of the most commonmalignancy on woman in Indonesia (Kemenkes RI,2015). Breast cancer patients usually come todoctor in a late stage & the deaths are due to thecomplications of metastasis in vital organs (NationalCancer Institute, 2012). The death case mostprobably happened when cancer is aggresive &distant metastazed. One factor that causingaggresivity & metastasis is presence of HER2 protein.HER2 is found in 25% cases of Ca mammae. Her2status also influences preference of drug for cancertreatment. Besides of antibodi monoklonal &antireseptor of tirosine kinase, doxorubicin is stillused for treatment breast cancer with overexpressHER2 (Nielsen et al., 2009). Long term use ofdoxorubicin causes several side effects, resistanceand toxicity to normal tissues (Smith et al., 2010).Therefore, combining doxorubicin withchemopreventive agent is needed to increaseactivity of doxorubicin, to overcome the drugresistance and to reduce its side effect (Sarkar andLi, 2006). Chemopreventive agent used in thisresearch are temulawak (Curcuma xanthorrhiza) andawar-awar (Ficus septica). The leaves of Ficus septicahave been used to treat colds, fungal and bacterialdiseases, and various cancers. The ethanolic extractof Ficus septica showed a cytotoxic effect on breastcancer T47D cell lines with IC50 value of 13 μg/mL.The extract at 4.88 μg/mL also showed an optimumsynergistic effect in combination with doxorubicin(3.75 nmol) (Pratama et al., 2010). In addition, theextract induced apoptosis and down regulated theexpression of Bcl-2 protein in breast cancer cellsMCF-7 (Sekti et al., 2010). The major components ofC. xanthorrhiza are curcumin and xanthorrhizol.Curcumin suppresses a number of key elements incellular signal transduction pathways pertinent togrowth, differentiation, and malignanttransformation (Kunumakkara et al., 2008). Choi etal. (2005) observed that injection of 0.2-1.0 mg/kgbw xanthorrhizol had an antimetastatic effect in amouse lung metastasis model. It is a general assumption that combinationtreatment is sometimes more beneficial than singlecompounds. The previous study reported thatcombination of doxorubicin and chemopreventiveagent showed synergistic effect in cancer treatment(Lewandowska et al., 2014). The purpose of thisstudy is to prove antimetastasis effect ofcombination of ethanolic extract of rhizome ofCurcuma xanthorrhiza (ECx), ethanolic extract ofleave of Ficus septica (EFs), and doxorubicin solelyand its combination on MCF7-HER2 breast cancercell line through inhibition activity of MMP-9.Cytotoxic assay is used to determine the synergy ofcombinations. Antimetastasis effect was observedthrough inhibition of cancer cell invasion. Gelatinzymography signify the inhibitory activity of MMP-9,which plays an important role in cancer cell invasion.

Abstract

The Osing tribal region is demographically adjacentto the island of Java, Madura and Bali. The proximityof this demographic regions affects multiple systemsof their organization, culture, and arts. The Osing’sfamilial and societal patterns are similar to the otherJavanese tribes, including housing, food, lifestyle andhealth. Osing tribe is frequently compared toBalinese culture, such as traditional clothing, wedding dresses, and others [1]. One of the culturesassociated with the teeth is Pangur or asa i.e.reducing/flattening the occlusal surfaces of the teethespecially for Osing bride women. In addition, thereis also a tradition of Osing women called Nginang orbetel chewing [2]. This situation may lead to animbalance of dental occlusion and disruption of thenormal flora in the oral cavity alleging effect on oralhealthDental and oral health can be achieved if there is arelationship of harmony and balance between theorgans and tissues in the oral cavity. This is becausethe oral cavity is an integral stomatognatic complexsystem.Stomatognatic system is unitary organ that hasmutual-related functions. These organs include jawbones, temporo mandibular joint (TMJ), teeth, andother supporting structures e.g. masticationmuscles, facial muscles, head and neck. Thisstomatognatic system plays an active role in theprocess of mastication. In normal conditions, itoccurs a harmonious relationship of all componentsof the masticatory system i.e. teeth, muscles, TMJ,lips, cheeks, palate, tongue and salivary secretion[3]. The conditions of normal tooth anatomy andwell-structured on curved teeth will put bothcondyle joints at the center of the articular disc. Thissituation will create an effective mastication process[4].Abnormal condition of tooth structure will causemalocclusion [3]. Occlusion is an interocclusalrelation (intercups) between the teeth of each jaw inaccordance with the position and movement of themandible [5]. Occlusion pathological state is knownas occlusal disharmony. Occlusal disharmony can becaused by several factors such as the loss of teeth, caries, attrition, tooth anomalies, bruxism, and thehabit of chewing on one side [6].Occlusal disharmony is suspected to causeinflammation in periodontal tissues. Periodontaltissue disease is one of the dental and oral diseasethat most commonly occurs. Periodontal diseasescan be caused by bacteria or mechanical theoverloads. The overloads can be caused by theocclusal disharmony. Thus, occlusal disharmony issuspected to cause inflammation in the periodontaltissues. Abnormal oocclusion condition in thisocclusion disharmony leads into occlusal traumawhich will increase the load received by theperiodontal tissue [7]. If this condition is persistent,it will cause periodontal tissue inflammation.Inflammation undergoing locally to the periodontaltissues will induce the immune system to increasethe activity of the inflammatory cytokine secretion, one of which is TNF-α (tumor necrosis factor-alpha).TNF-α will play a role in activating osteoclasts, whichcan result in the destruction of the alveolar bone [8].This inflammatory cytokines can be found in gingivalcrevicular fluid (GCF), which are inflammatoryexudates collected in the gingival sulcus [9].TNF-α is a cytokine that plays a role in theoccurrence of periodontal disease through itsactivities, which triggers proliferation, differentiationand activity of osteoclasts resulting in boneresorption, and inducing production of proteinase inthe cells of the mesenchyme that is responsible inbond tissue destruction. In patients withperiodontitis it occurs increased levels of TNF-α,which will be decreased after periodontal treatment.Based on the description above, it is necessary toknow the levels of TNF-α in GCF Osing women as anindicator of periodontal tissue inflammation due toocclusion disharmony. It is important for preventionand treatment of abnormalities of occlusiondisharmony that will improve oral health and qualityof life, especially in the Osing community in Kemirenvillage, Glagah District, Banyuwangi Regency.
