Abstract

Hypertension is a degenerative disease that eachyear has increased the number of patients. Numberof patients in Indonesia is estimated at 15 millionpeople. Nearly 50% of patients suffering fromhypertension are not aware that tends to be severehypertension (Syahrini et al., 2012).Diltiazem Hydrochloride (HCl) is an antihypertensivedrug used 3-4 times a day with doses of 30-60 mg(UBM Medica, 2012). Diltiazem HCl has a half life of3-5 hours (Sweetman, 2009) with the mainabsorption area in the upper stomach (Kapil et al.,2012) that can be formulated into sustained releasedosage gastro retentive system. The system created is a combination of floatingmucoadhesive. The combination of these systems isexpected to overcome the shortcomings of thesystem when used alone. Floating system floats overthe surface of gastric contents when the stomach isfull but at the time stomach is emptied and thetablet reaches the pylorus the bouyancy of thedosage may be decreased. If the stomach is full, mucodhesive dosage form not properly adheres maybe passage the pylorus due to peristalticmovements of the stomach (Gaykar et al., 2013).Polymer is one of the essential ingredients toproduce a floating-mucoadhesive system that meetsthe requirements. The polymer used is acombination of Carbopol and ethyl cellulose. Thecombination of hydrophilic and hydrophobicpolymers proved able to control the rate of drugrelease with high solubility (Nanjwade et al., 2011).

Abstract

Under five years old children is one of the group thatvulnerable to under nutrition, moreover this agegroup also susceptible to infectious disease due totheir immunity and their activity related to exposureof infectious disease. To achieve optimal growth, achildren must obtain adequate dietary intake, optimal care practice that include appropriatefeeding practices and less frequent of infectiousdisease (Black et al, 2008). Inadequate food intaketo Undernutrition among under five childrenbecome important issue since it can affect shortterm and also long term that go beyond child hood(Neufeld and Osendarp, 2014). Indonesia BasicHealth Survey revealed there was fluctuation ofunderweight prevalence from 18.4% in 2007,decreased to 17.9% in 2010 and increased to 19.6%in 2013 (Indonesia Basic Health Survey, 2013). Thereis a strong evidence that undernutrition thatoccurred during the first two years of life contributeto the increasing risk of non communicable diseasein the later life (Victora et al, 2008). Communicabledisease is one of the factors that contribute tonutritional status, particularly underweight inchildren since underweight is the indicator thatreflect an acute disturbance of nutritional status.Communicable diseases that often occur in childrenare diarrhea and upper respiratory infecti on. Indeveloping countries, diarrhea contributes to thechildhood morbidity and mortality, as it estimatedthat 1.5 million child die to diarrhea per year (Roy etal, 2011). There is a bidirectional relationship ofdiarrhea and malnutrition in children with themechanism of diarrhea episodes lead to anorexia, absorptive function impairment, the damage ofmucosal and the increase need of nutrients (Brown,2003). Malnutrition also increase the incidence andseverity of acute respiratory infection throughmechanism of immunity impairment, particularly incellular immunity (Bhutta et al, 1998). This studyaimed to analyze the association of underweight andmorbidity of diarrhea and acute respiratory infectionin under five years old children.

Abstract

People are more and more desire a youthful andattractive appearance. Aging of the skin can be theresult of a normal process that involves a lowerrate of metabolic activities and is calledchronologic aging. Photoaging is the consequenceof chronic sun exposure and manifests clinicallywith fine and coarse wrinkling, roughness, dryness, laxity, shallowness, pigmentary mottling, telangiectasia, and in some cases withpreneoplastic and neoplastic changes (Stefanaki etal., 2005). Tretinoin is a compound that is able toshow an effect of prevention and treatment ofwrinkles due to photo aging, acne, andinflammation of the skin. It is all-trans retinol isoxidized through retinal to its most activemetabolite (Kligman et. al, 1986). The effectivity ofan active ingredient is determined among othersby the ability to reach the site of action. To reachthe site of action the active ingredients used orallyor topically must first dissolve, release thenabsorbed or penetrates through the membraneand then diffuses into the site of action. Tretinoinpractically insoluble to increase it usednanoemulsion delivery as a vehicle. Solubility isone of the factors of drug absorption. Highersolubility of drug caused higher amount of drug toabsorb. Smaller droplet of emulsion caused rapiddrug release. In this study tretinoin loaded innanoemulsion using olive oil than it characterizedand test of tretinoin release compared withtretinoin in emulsion. Olive oil used as oil phase innanoemulsion as known it widely used in cosmetic.MATERIAL AND METHODSResearch MaterialsTretinoin (PT. Cortico Mulia), olive oil , Tween 80(Sigma Aldrich), Span 80 (Sigma Aldrich), ethanol96 % (E-Merck), NaH2PO4 (E-Merck) and Na2HPO4(E-Merck), aquademineralisata (PT Brataco).Research InstrumentsStirrer plate (Dragon Lab MS-Pro), ultrasonic(Branson 3510), shaker machine (Wine shake), pHmeter (Eutech Instruments pH 700), particleanalyzer (Delsa Nano C), Franz diffusion cell withcellophane membranes, spectrophotometer(Shimadzu UV-1800), Transmission ElectronMicroscopy (TEM-type JOEL JEM-1400), lightmicroscope.
