Abstract

Abstract Chronic Obstructive Pulmonary Disease (COPD) is a chronic airway inflammation with resulting progressive airflow limitation that has a high incidence, morbidity, and mortality. Roflumilast is an oral phosphodiesterase-4 inhibitor as a therapy to decrease the risk of COPD exacerbations in patients with moderate-severe COPD with a history of chronic bronchitis. Roflumilast can be given orally once daily as a single or combination drug. It can be used as COPD moderate-severe but also more beneficial as COPD mild treatment. The efficacy of Roflumilast can prevent exacerbations of repetition and can fixed lung function both in terms of FEV1 and vital capacity to force. The incidence of side effects, which are diarrhea and digestive disorders. The cost-effectiveness showed that Quality of life patient with Roflumilast is better than the group without additional. Abstract

Abstract—Pharmacists’ involvement in the human immunodeficiency virus/acquired-immunodeficiency syndrome (HIV/AIDS) prevention and management program has been globally advocated. However, there is currently a lack of information on whether pharmacists’ roles in the program have been clearly articulated in the national policy records in countries with high scores on power distance and collectivism, such as Indonesia. This study aimed to explore the roles of pharmacists in the prevention and management of HIV/AIDS in Indonesia using a systematic review method. We systematically searched for relevant studies in three large medical databases (CINAHL, EMBASE, and PubMed) and for gray literature in both the Indonesian Pharmacist Association and the Ministry of Health websites, and the national regulation database. Only two studies gave a glimpse of pharmacists’ role in the prevention and management of patients with HIV/AIDS in Indonesia. However, activities conducted by pharmacists in each of these roles were not explicitly described. We found no official national policies mentioned the role of pharmacists in HIV/AIDS prevention and management. Since the practice of pharmacists is closely related to medications, including antiretroviral drugs, more research is needed to explore the role of pharmacists in the prevention and management of HIV/AIDS in a real-world context in Indonesia. Keywords: AIDS, HIV, pharmacist, indonesia Abstrak—Keterlibatan apoteker dalam program pencegahan dan tata laksana human immunodeficiency virus/acquired-immunodeficiency syndrome (HIV/AIDS) di dunia telah direkomendasikan. Akan tetapi, saat ini terdapat keterbatasan informasi mengenai apakah keberadaan peran apoteker dalam program tersebut telah dinyatakan dnegan jelas dalam dokumen kebijakan nasional di negara-negara yang memiliki skor tinggi pada power distance dan kolektivitas, seperti Indonesia. Penelitian ini bertujuan untuk menggali peran apoteker dalam pencegahan dan tata laksana HIV/AIDS di Indonesia menggunakan metode kajian sistematis. Secara sistematis, peneliti melakukan pencarian artikel penelitian terkait di tiga basis data medis yang besar (CINAHL, EMBASE, dan PubMed) dan pencarian gray literature di laman Ikatan Apoteker Indonesia dan Kementerian Kesehatan Republik Indonesia, serta di basis data peraturan nasional. Hanya dua penelitian yang memberikan secercah informasi mengenai peran apoteker dalam pencegahan dan tata laksana pasien dengan HIV/AIDS di Indonesia. Namun demikian, aktivitas spesifik yang dilakukan oleh apoteker tidak secara eksplisit digambarkan lebih lanjut. Peneliti tidak menemukan dokumen kebijakan nasional yang menyebutkan peran apoteker dalam pencegahan dan tata laksana HIV/AIDS. Oleh karena praktik kefarmasian berhubungan erat dengan obat, termasuk obat-obat antiretrovirus, maka dibutuhkan lebih banyak penelitian untuk menggali peran apoteker dalam pencegahan dan tata laksana HIV/AIDS pada konteks dunia-nyata di Indonesia. Abstract

Pendahuluan: Hipertensi termasuk sebuah penyakit kronis yang memerlukan penanganan konsisten dalam jangka waktu panjang karena mampu mengakibatkan masalah serius termasuk stroke, gagal jantung, serta gagal ginjal. Salah satu cata terbaik menangani hipertensi yaitu mematuhi petunjuk konsumsi obat antihipertensi. Namun, tingkat kepatuhan pasien masih menjadi tantangan, dipengaruhi oleh berbagai faktor seperti persepsi kesembuhan, efek samping obat, kejenuhan dalam terapi jangka panjang, serta kurangnya edukasi. Penelitian ini bertujuan mengetahui hubungan kepatuhan minum obat dan tekanan darah pasien hipertensi Klinik Sahabat Sehat Desa Blayu. Metode: yang digunakan yaitu kuantitatif non-eksperimental melalui pendekatan korelasional berdesain cross-sectional. Dimana diperoleh 30 sampel pasien hipertensi melalui teknik purposive sampling. Tingkat kepatuhan diukur dengan 2 instrumen yaitu metode Pill-count dan kuesioner Morisky Medication Adherence Scale-8 (MMAS-8), sedangkan tekanan darah diukur 2X (hari ke-1 dan hari ke-7). Analisis data menggunakan uji korelasi Kendall’s Tau. Hasil: ditemukan bahwasannya terdapat hubungan signifikan kepatuhan minum obat dan tekanan darah. Pada metode Pill-count, nilai signifikansi (2-tailed) sebesar 0,00 dan koefisien korelasi 0,780 menunjukkan hubungan yang kuat. Pada metode MMAS-8, nilai signifikansi (2-tailed) 0,001 dan koefisien korelasi 0,569 menunjukkan hubungan sedang. Simpulan: ada hubungan signifikan diantara tingkat kepatuhan minum obat dengan tekanan darah pasien hipertensi.

Abstract

Jukut pendul (Kyllinga nemoralis) digunakan sebagai antioksidan, antiinflamasi, dan penyembuh luka. Nanoemulsi sebagai sistem pengiriman obat berukuran nano dapat meningkatkan stabilitas dan efektivitas ekstrak herbal. Penelitian ini bertujuan menilai karakteristik, stabilitas, serta mutu fisik nanoemulsi ekstrak jukut pendul yang diformulasikan menggunakan surfaktan Tween 80 pada konsentrasi 25%, 30%, dan 35%. Homogenizer dengan kecepatan 13.000 rpm digunakan dalam penelitian ini dengan waktu pengadukan 20 menit. Hasil menunjukkan bahwa Formula 3 dengan konsentrasi Tween 80 35% memiliki kualitas fisik dan stabilitas terbaik. Formula ini menunjukkan kejernihan tinggi dengan nilai transmisi ≥90%, stabilitas pH untuk kulit dalam rentang (4,5–6,5), dan viskositas yang sesuai (57,9 cP). Nilai transmisi tinggi 100% menunjukkan bahwa ukuran partikel dalam sistem telah mencapai skala nano. Kesimpulan yang diperoleh adalah bahwa nanoemulsi ekstrak jukut pendul dengan konsentrasi Tween 80 35% menunjukkan potensi terbaik sebagai sediaan topikal yang stabil, jernih, dan efektif.

Abstract

The COVID-19 pandemic has significantly increased public interest in traditional herbal medicines, particularly instant temulawak (Curcuma xanthorrhiza Roxb.), as immune-boosting agents. This systematic narrative review evaluates the immunomodulatory and antiviral potential of temulawak's bioactive compounds, especially curcuminoids, in the context of instant herbal formulations. A comprehensive literature search was conducted across PubMed, Google Scholar, Scopus, and Science Direct using Boolean operators. From 247 initial records, 38 articles met the inclusion criteria. Evidence demonstrates that curcuminoids exhibit broad-spectrum antiviral activity against SARS-CoV-2, influenza, hepatitis B/C, dengue, and HIV through inhibition of viral entry, replication, and modulation of NF-κB, MAPK, and JAK/STAT signaling pathways. Immunomodulatory effects include enhanced macrophage function, T-cell modulation, and cytokine balance regulation. A notable case report showed C. xanthorrhiza consumption led to HBsAg seroconversion in a chronic hepatitis B patient. However, critical gaps persist regarding the specific efficacy of instant formulations, bioavailability challenges of curcumin in processed forms and the absence of rigorous clinical trials on temulawak instan specifically. Future research should prioritize standardized formulations, bioavailability optimization, and clinical validation to bridge the translational gap between in vitro evidence and practical herbal applications.

Abstract

The poor water solubility and low oral bioavailability of various phytochemical compounds (BCS Classes II and IV) continue to be major obstacles in the development of modern therapeutics, despite their remarkable therapeutic potential. This narrative review critically evaluates the use of nanoscale drug delivery systems to overcome the biopharmaceutical limitations of hydrophobic phytochemicals and pinpoints key challenges in clinical translation and industrial-scale production. The review provides a comprehensive analysis of the latest literature on the effectiveness of nanotechnology-based physicochemical modifications, including solid lipid nanoparticles (SLNs), nanostructured lipid carriers (NLCs), nanosuspensions, polymeric micelles, liposomes, and phytosomes. The review's results indicate that nano-carrier engineering significantly enhances solubility, avoids first-pass metabolic degradation, and enables targeted delivery. Specifically, nanoformulations have proven effective at penetrating the dense matrix of the tumor microenvironment (TME) in cancer therapy, crossing the blood-brain barrier (BBB) in neurodegenerative diseases, and protecting drugs from stomach acid in infectious and metabolic conditions. However, the transition toward commercialization is hindered by issues of long-term physicochemical stability (e.g., lipid polymorphic transitions) and challenges in standardizing botanical raw materials. These issues affect the reproducibility of large-scale production, ultimately delaying the availability of these promising therapies to patients in need. In conclusion, while nanoformulations offer transformative solutions for overcoming phytochemical biological barriers, standardizing protocols, innovating green synthesis, and fostering interdisciplinary collaboration are essential for overcoming scalability barriers and preparing these innovations for clinical application.

Abstract

Antioxidants are compounds capable of slowing or preventing oxidation, even in relatively low concentrations, thereby contributing to various physiological functions in the body. The antioxidant components found in plants act as free radical scavengers by transforming them into less reactive molecules. Jukut Pendul (Kyllinga nemoralis) contains several bioactive substances, including flavonoids, tannins, and alkaloids, which are recognized for their potential as natural antioxidants. Flavonoids, for instance, exhibit antioxidant activity by donating hydrogen atoms or through their metal-chelating properties. These compounds can be present as glycosides (with glucose moieties) or in the free form known as aglycones. This research aimed to evaluate the antioxidant capacity of Kyllinga nemoralis extract. The plant material was sourced from Banyuwangi, East Java. Extraction was performed using the maceration method with 70% ethanol as the solvent. The free radical scavenging activity was assessed using the DPPH (1 radical) method. Ascorbic acid was used as positive control. Measurement of absorbance was carried out through UV-Vis spectrophotometry. The extract was tested at concentrations of 20, 40, 60, 80, and 100 μg/mL. The findings revealed that the value of Kyllinga nemoralis extract was 11.351 μg/mL, whereas ascorbic acid exhibited an IC50 of 3.43 μg/mL. The antioxidant activity of Kyllinga nemoralis ethanol extract falls into the very strong category, ranging <50-100 μg/mL. These results suggest that although the antioxidant strength of Kyllinga nemoralis is lower than that of , the extract still possesses detectable radical-scavenging activity.
