Abstract

Medical records can be maintained both manually and electronically, the transition process from manual and electronic medical records is certainly not easy because it must have a strategy and readiness, both from facilities and infrastructure. Of course, using electronic medical records is still obliged to pay attention to legal aspects that cover not only for the sake of achieving patient safety but also for health service providers. Socialization is needed that discusses overarching regulations, strategies, and challenges that must be faced and prepared in the process of change. So that the migration process from manual to electronic can be carried out well according to the expectations of users and policymakers.

Abstract

Era digitalisasi 4.0 menuntut masyarakat melek teknologi salah satunya dibidang kesehatan. Pemanfaatan aplikasi bidang kesehatan tidak hanya memberikan informasi kesehatan namun menstimulasi kesadaran masyarakat untuk hidup sehat. Banyak berkembang aplikasi kesehatan yang diakses secara gratis melalui mobile. Tujuan pengabdian masyarakat mengedukasi pemanfaatan aplikasi mobile health (M-health) yang tersedia di Android atau IOS mendukung kesadaran masyarakat untuk bisa mengikuti pola hidup sehat dan memiliki kemudahan akses informasi kesehatan. Aplikasi m-health tentang kesehatan reproduksi untuk remaja ataupun keluarga, masyarakat tidak perlu malu dan ragu untuk mendapatkan informasi perihal kesehatan organ vital reproduksi, edukasi perawatan dan informasi pra-post pernikahan dan lain halnya. Sasarannya adalah ibu PKK Dawis Kenikir, remaja di dusun Mejing Lor, RT 04, kel. Ambarketawang, Kec. Gamping, Kabupaten Sleman. Kegiatan ini diselenggarakan secara langsung atau luring dengan tetap mematuhi protokol kesehatan dimasa Pandemi Covid-19. Pelaksanaan kegiatan ini pada tahap pertama melakukan koordinasi dengan RT 04 Mejing Lor, Ambarketawang, Gamping, Sleman, tahap kedua penyampaian materi secara daring dan luring. bagi peserta yang tidak hadir dan luring bagi yang berkenan dengan batasan peserta 10 orang dan wajib disesuaikan dengan protocol kesehatan ketat. Pemateri pertama membahas pentingnya kesehatan reprodoksi dan pemateri kedua tentang pemanfaatan aplikasi mhealth, dilanjutkan diskusi dengan peserta.

Abstract

Era digitalisasi sangat berpengaruh terhadap keberlanjutan dan keberlangsungan kehidupan, tidak hanya pada sektor ekonomi namun sektor pendidikan juga dituntut untuk melek teknologi dan informasi. Sejak 2020 Indonesia terdampak Pandemi Covid-19, sehingga diperlukan kerjasama semua lapisan pendidikan dari setiap level dituntut untuk melaksanakan pendidikan secara daring atau tatap maya. Beberapa penelitian sebelumnya menjelaskan bahwa pemanfaatan aplikasi berbasis dekstop atau mobile akan memberikan dampak yang banyak, tidak hanya kemudahan akses, namun bisa meningkatkan kreatifitas pendidik dalam pembuatan video pembelajaran. Mitra PKM SMK Kesehatan Bumantara Magelang. Bahwasannya dengan kondisi pandemik saat ini menuntt para pendidikan semakin kreatif sehingga siswa tidak merasa bosan. Kegiatan ini akan diselenggarakan secara langsung atau luring dengan tetap mematuhi protocol kesehatan dimasa Pandemi Covid-19. Metode pelaksanaan kegiatan adalah koordinasi dengan pihak SMK Kesehatan Muntilan Magelang Jawa Tengah. Pelaksanaan kegiatan ini pada tahap pertama dengan koordinasi dengan pihak Admin SMK Bumantara, tahap kedua penyampaikan materi secara luring di Laboratirium SMK Bumantara. dilanjutkan diskusi dengan peserta serta pendampingan proses pembuatan video pembejelaran.

Abstract

A complete of medical record and the accuracy of clinical coding is reflect to the quality of medical record documentation. In 2021, the number of pending claim for medical record patient’s covid-19 is 41%, as cause the quality of medical record incomplete. The research aim is to measure the quality of medical records by covid-19 documentation for pending claims and the accuracy of coding Covid-19. The research method is mixed-method sequential explanatory. We used the checklist observation to measure the quality of clinical coding for patient covid-19 and interview guideline to explore the factors of pending claims. The total sampling are 889 of medical records as secondary data was analyze with STATA Version 13. The characteristic of administrative data by gender is man 571 (64,45%), woman 315 (35,35%). the number of classification covid-19 case is probable 25 cases (2,71%), suspected 214(24,15%) and confirmed 648 cases (73,14%). In another hand, the quality of accuracy coding covid-10 use code B34.2 is 377 (42,55%), use code U07.1 is 295(33,30%) and use code Z20.8 is 215 (24.14%). The accuracy of coding covid-19 is 295 (33,30%) and inaccurate 591 (66,70%). we observe the number of pending claims is 222 due to incomplete of signature and full name of physician items in discharge summary form.

Abstract

In 2014, primary health centres Lendah II was implemented health information system for primary care (Simpus). the use of simpus can help the physician and health practicioners entry data of patient, import dan export data and also improved theirs performance. The implementation of medical records for primary health centres an important role in managing patient data. The use of technology and information systems in the health sector has an impact on optimizing services. One of them is by utilizing a barcode system to support storage at primary health centres Lendah II This study aims to help the storage tracking system in the filing room using barcodes. Data capture with FGD for needs analysis, then application design and trials. This research uses research and develompent. The results of the study by making class diagrams, barcode applications and examples of barcodes applied to poly service names and medical record numbers. Barcodes will be printed and affixed to the patient's medical record to facilitate tracking of the The barcode of the patient's medical record number will also be affixed to the patient's BPJS card, making it easier for officers to check the patient's medical record number on the simpus. There is a scanner reader tool that will make it easier for officers to read barcode codes.

Abstract

Claims payment for health services is one of the factors that affect the financial health of a hospital. Indonesia itself has experienced several changes to the existing payment system. BPJS-K started an operational study on the implementation of the INA-CBGs and the Global Budget mixed-method payment system since 2018 with three stages in five districts/city in Indonesia. This study aims to evaluate the implementation of the Global Budget system at RSUD Wates in Kulon Progo Regency that appointed as a BPJS-K pilot project. This research is qualitative research and data collection was carried out by studying documents and interviews. As a result, it is known that in 2021 RSUD Wates experienced a budget surplus and on the contrary, in 2022 the hospital experienced a significant deficit. However, the efficiency of health service financing at RSUD Wates cannot be evaluated as a whole in the implementation of the global budget trial. The implementation of the global budget at RSUD Wates was carried out under uncommon conditions where the COVID-19 pandemic occurred in 2020-2022 so that the data on the number of patients became less relevant as a reference for budget calculations. In addition, all stages in the trial have not been fully implemented. Coordination and the role of policy makers are expected to be improved so that the competence and efficiency of hospital health services can be optimized.

Abstract

Objective: This community service aimed to educate healthcare workers at Aisyiyah Pandaan Inpatient Clinic on the process of medical record data, fostering data-driven decision-making in patient care. Method: In October 2024, a training program was conducted for 20 healthcare workers, focusing on medical record data analysis, interpretation, and presentation. Participants were trained on how to utilize medical data to support clinical decisions.  Results:  The training program led to a significant improvement in the participants' knowledge and abilities in medical record data analysis. The results were statistically significant with p-values: p < .001, p < .001, p = 0.003, p < .001, and p = 0.002. Novelty: This activity represents a novel initiative in empowering healthcare workers by enhancing their competencies in using medical record data for clinical decision-making, promoting efficiency and accuracy in patient care.

Abstract

Satu sehat merupakan aplikasi yang diluncurkan pemerintah Indonesia per Maret 2023, namunbelum semua lapisan masyarakat mendapatkan sosialisasi aplikasi satu sehat yang berbasis mobile. Aplikasitersebut memberikan manfaat terkait informasi rekam kesehatan, jaminan kesehatan, edukasi kesehatan.Posyandu di RT 04 merupakan layanan pemeriksaan primer tingkat desa oleh para nakes dan kader, kegiatanrutinnya adalah monitoring tumbuh kembang bayi. Beberapa penelitian sebelumnya menjelaskan pentingnyaedukasi pada implementasi sebuah aplikasi kepada masyarakat memberikan peran positif pada peningkatanpengetahuan dan manfaat dari aplikasi tersebut. Kegiatan ini akan diselenggarakan secara langsung atau luringdengan tetap mematuhi protocol kesehatan Metode pelaksanaan kegiatan adalah koordinasi dengan pihak RTdan RW di Mejing Lor, Gamping, Sleman. Abstract

AbstractBased on observations and document studies at PMB Ummu Hani Bantul Yogyakarta, the medical record documentation system has not been optimal. PMB Ummu Hani lacks standardized medical record folders, KIB (Patient Identity Cards), and forms. For instance, postpartum records are kept in large folio paper books with batik covers, where the paper size of each form is not uniform. All healthcare personnel and facilities must maintain proper and accurate medical records to protect both patients and healthcare providers and to support the continuity of care. This study aims to design a medical record system that benefits PMB Ummu Hani. The research method used is Research and Development (RnD) with a case study approach. Data collection was conducted through observation and document review of existing medical record forms and Focus Group Discussions (FGD) with 9 midwives. The forms were designed using Canva and Epi Info applications and were tested through design presentations and a second round of FGD. The study resulted in the design of medical record folders, KIB, prenatal examination forms, delivery forms, medical procedure forms, partograph forms, postpartum forms, and child growth records. The medical record folder uses ivory paper, size F4, weighing 260 grams, cream-colored. The medical record forms use A4 paper, weighing 70 grams, white-colored, and the KIB paper size is 85.6 cm x 53.9 cm.Keyword: design, medical record, primary care of Ummu Hani AbstrakBerdasarkan hasil observasi dan studi dokumen di PMB Ummu Hani Bantul Yogyakarta, sistem pencatatan dan pendokumentasian rekam medis belum berjalan optimal. PMB Ummu Hani belum memiliki map rekam medis yang standar, KIB, dan formulir yang terstandar. Contohnya, pencatatan nifas menggunakan buku besar berukuran kertas folio bergaris dengan sampul batik, di mana ukuran kertas setiap formulir tidak seragam. Setiap tenaga kesehatan dan fasilitas pelayanan kesehatan wajib menyelenggarakan rekam medis yang baik dan benar untuk memberikan perlindungan bagi pasien dan tenaga kesehatan, serta mendukung keberlanjutan pemeriksaan dan pengobatan. Penelitian ini bertujuan untuk mendesain rekam medis yang bermanfaat bagi PMB Ummu Hani. Metode penelitian yang digunakan adalah Research and Development (RnD) dengan pendekatan studi kasus. Pengumpulan data dilakukan melalui observasi dan studi dokumen pada formulir rekam medis yang ada, serta Focus Group Discussion (FGD) dengan 9 bidan. Desain formulir dibuat menggunakan aplikasi Canva dan Epi Info, kemudian diuji coba melalui presentasi hasil desain dan FGD tahap 2. Hasil penelitian menghasilkan desain map rekam medis, KIB, formulir pemeriksaan kehamilan, formulir persalinan, formulir tindakan medis, formulir patografi, formulir nifas, dan buku tumbuh kembang anak. Map rekam medis menggunakan kertas jenis ivory ukuran F4 dengan berat 260gram berwarna krem, sementara kertas formulir rekam medis berukuran A4 dengan berat 70gram berwarna putih, dan kertas KIB berukuran 85,6 cm x 53,9 cm.Kata Kunci: desain, rekam medis, PMB Ummu Hani

Abstract

Indonesia is a tropical country vulnerable to infectious diseases, including dengue fever caused by mosquito bites. Dengue fever cases are a burden on the world because they can cause death. WHO reported that in May 2024, there were a total of 465 cases in districts/cities in 34 provinces, the national death rate until May 2024 was 777 deaths. Recording a complete medical record can support good disease reporting. This study aims to determine the distribution of dengue fever cases in Jepara district, Central Java in 2023-2024. The method of this study is descriptive. The population is disease reports in 2023 and 2024, and the sample used is the dengue fever disease report for the period January - May 2024 with random sampling techniques. Data imported from Microsoft Access is processed with a stata application and narrated descriptivelyThe number of dengue fever sufferers spread across 16 sub-districts is 3,549 with 1,728 male and 1,821 female sufferers. Of the 16 sub-districts, the highest number is in Pecangaan District 543 cases, and the lowest cases are in Karimunjawa District. The category of patients based on the highest age group in category 1 (0-4 years old) was 2,136 (60.17%), with the highest mortality rate in category 2 (5-14 years old) as many as 12 people and the highest CFR at Kartini Hospital with a total of 19%, namely 12.9% in male patients and 7.24% in female patients.
