Abstract

Introduction: Creatinine is an examination parameter for kidney function and can be measured by several methods. Quality control needs to be done to ensure the results of the creatinine examination are correct. Objective: This study aims to determine the results of the quality control analysis of creatinine examination with Westgard multirules and Six Sigma in the clinical pathology laboratory at Wangaya Hospital, Denpasar, in 2023. Methods: This study used a descriptive research design, secondary data, and quality control results. Data analysis was done by determining the mean value, SD, CV, d%, sigma, and making the Levey-Jennings graph. The results showed that the bias value (d%) was still below the maximum value; the CV value in January, March, April, and July exceeded the maximum limit. Evaluation of Westgard's rules obtained the results of violations of trends, shifts, and 1- 2s, then variations in sigma values each month, namely <3 and >4 from June and September to December 2023. Results: Based on the results of the study, it can be concluded that creatinine examination obtained a good accuracy value, low precision value in certain months, there are random (1- 2s) and systematic (trend and shift) errors, and a low sigma value, which is <3 (unacceptable performance and poor performance categories). However, the sigma value has increased from June to September to December, which is >3 (marginal performance, good performance, and world-class performance categories). Conclusion: Accuracy and precision values must be maintained because good accuracy and precision values will increase the sigma value, especially in creatinine testing.

Abstract

Hasil kritis laboratorium adalah hasil yang menunjukkan adanya resiko yang dapat mengancam nyawa pasien dan segera dilaporkan. Pelaporan hasil kritis laboratorium berhasil tercapai dengan baik dengan memperhatikan waktu pelaporan ≤ 30 menit, capaian indikator mutu 100%, dan informasi pelaporan lengkap terdokumentasi. Rumah Sakit menerapkan sistem LIS yang terintegrasi dengan Hospital Information System (HIS). Tujuan penelitian ini adalah untuk mengetahui tingkat capaian pelaporan hasil kritis laboratorium sebelum dan sesudah penggunaan LIS di RS Bangli Medika Canti. Jenis penelitian ini menggunakan penelitian analisis deskriptif dengan pendekatan cross-sectional. Sampel dari penelitian ini adalah seluruh capaian kegiatan pelaporan hasil pemeriksaan laboratorium tahun 2023 dan 2024 di RS Bangli Medika Canti. Hasil analisa sebelum penggunaan LIS adalah waktu pelaporan 36 menit, capaian indikator mutu 94,3%, dan informasi tidak terdokumentasi lengkap. Hasil analisa sesudah penggunaan LIS adalah waktu pelaporan 10 menit, capaian indikator mutu 100%, dan informasi lengkap terdokumentasi.Berdasarkan hasil tersebut tingkat capaian pelaporan hasil kritis laboratorium tercapai dengan baik dengan penggunaan LIS. Peran LIS dapat membantu meningkatkan efisiensi komunikasi untuk pelaporan hasil kritis laboratorium karena sedikit melibatkan pihak lain dan informasi lengkap pada sistem

Abstract

Latar belakang: Kualitas spesimen sputum berperan krusial dalam pemeriksaan mikroskopis basil tahan asam (BTA). Pada pasien tuberkulosis paru, dahak yang melewati tenggorokan sering mengandung debris mikroskopis yang dapat mengganggu kualitas pewarnaan. Penggunaan bleach sebagai dekontaminan dinilai mampu meminimalkan gangguan tersebut dengan membersihkan lapang pandang mikroskopis. Tujuan: membandingkan skor BTA pada pewarnaan Ziehl Neelsen konvensional dan dengan penambahan bleach 0,5%. Metode: eksperimental yang dilakukan di UPTD Puskesmas Gonenggati Donggala pada Januari -April 2025. Sampel pada penelitian ini adalah sputum dari pasien dengan suspek TB paru di UPTD Puskesmas Gonenggati Donggala sebanyak 67 sampel. Pemeriksaan BTA dilakukan dengan metode pengecatan Ziehl Nelseen. Hasil: hasil pemeriksaan BTA dengan metode Ziehl Nelseen konvensional didapatkan skor negatif (83,6%), scanty (0 %), positif 1 (10,4%), positif 2 (3,0%) dan positif 3 (3,0%). Sementara itu hasil pemeriksaan BTA dengan metode Ziehl Nelseen dengan penambahan bleach 0,5% didapatkan skor negatif (82,6%), scanty (1,5%), positif 1 (10,4%), positif 2 (3,0%) dan positif 3 (3,0%). Hasil uji beda dengan Wilcoxon Range Test didapatkan hasil nilai p > 0,665. Kesimpulkan: bahwa tidak ada perbedaan hasil skor basil tahan asam pada pewarnaan Ziehl Nelseen konvensional dengan Ziehl Nelseen dengan penambahan bleach 0,5%.

Abstract

Total bilirubin examination serves to determine liver and bile duct function. Pathological hyperbilirubinemia can cause central nervous system disorders to death. Due to the importance of bilirubin results for supporting diagnosis, examinations must follow procedures. However, in the field, there are often issues like lack of control, damaged equipment, and others, resulting in delayed specimens. This study aims to determine the difference in total bilirubin levels with light exposure examined immediately and delayed for 3 hours. This experimental research used 36 sample units from 18 samples with 2 treatment groups: serum exposed to light examined immediately and serum exposed to light delayed for 3 hours. Sampling technique: accidental sampling. Data analysis used the Wilcoxon test. The results showed an average total bilirubin level of 3.1444 mg/dL for immediate examination and 3.1044 mg/dL for delayed examination. The data analysis result p-value 0.000, indicates a significant difference in total bilirubin levels between immediate and delayed examination. This difference occurs due to external and internal factors. External factors include light exposure, storage time, and inappropriate storage temperature. The blue light in lamps can affect the results, leading to a decrease in total bilirubin levels.

Abstract

Coffee contains bioactive compounds such as caffeine, cafestol, and kahweol that can influene lipid metabolism. Consumption of unfiltered coffee (boiled coffee) has the potential to increase cholesterol levels. This study aimed to determine the difference in total cholesterol levels between coffee consumers in Banjar Juwr, Abiansemal. The research employed a quantitative comparative method with an observational design. A total of 70 male respondents age 20-50 years (35 coffee consumers and 35 non-consumers) were selected using purposive sampling. Total cholesterol levels were measured using the test strip method (Easy Touch GCU). Data were analyzed using the Independent Sample T-Test. The mean total cholesterol levels of coffee consumers was 170.49 mg/dL, while that of non-consumers was 178.80 mg/dL. Statistical analysis showed p=0.103 (p>0.05), indicating no significant difference between the two groups. Conclusion Black coffee consumption in Banjar Juwet was not proven to significantly affect total cholesterol levels. Other factors such as dietary patterns, phhysical activity, and health history are presumed to play a greater role.

Abstract

This study aimed to analyze the relationship between blood lead (Pb) level, urine Pb level, blood pressure, and hemoglobin level among online motorcycle taxi workers in Denpasar. A quantitative cross-sectional design was used involving 96 drivers selected by purposive sampling. Data were collected through questionnaires, direct physical measurements, and laboratory examinations of blood Pb, urine Pb, and hemoglobin. Normality was tested using the Kolmogorov–Smirnov test. Because several variables were not normally distributed, the data were analyzed using Spearman rank correlation, Mann–Whitney U test, and linear regression. Blood Pb level was not significantly associated with systolic blood pressure, diastolic blood pressure, or hemoglobin level. Urine Pb level was not significantly associated with systolic or diastolic blood pressure, but showed a significant weak negative correlation with hemoglobin level. Blood Pb and urine Pb levels were not significantly correlated. Work duration was not significantly associated with Pb levels, whereas daily working hours showed a marginal association with blood Pb. These findings indicate limited hematological effects and support routine biomonitoring and preventive occupational health measures in traffic-exposed workers.
