Abstract

Thyroid disease is quite common. The cardiovascular clinical manifestations of hyperthyroidism are palpitation, systolic hypertension, fatigue, or with the basis of existing heart disease, angina or heart failure. In men, the disease is more frequently to develop into congestive heart failure than in women, thus more exploration is needed. This case report discussed a 42-year-old male patient who was admitted to the emergency department due to palpitations, shortness of breath aggravated with activity and lie down position, and alleviated with resting, cough with white sputum, epigastric pain, and constipation since the past 3 days. He was diagnosed with a history of hyperthyroidism and congestive heart disease 1 year ago and routinely consumed propylthiouracil (PTU). He had a history of herniotomy 10 days before admission. The patient did not have a history of hypertension, diabetes mellitus, or hypercholesterolemia. The patient has a smoking habit of up to 3 packs/day since a teenager. The patient was diagnosed with hyperthyroid heart disease (congestive heart failure, atrial fibrillation, and coronary heart disease) with comorbid of electrolyte imbalance, hypoalbuminemia, and thrombocytopenia. The patient was treated in the Intensive Care Unit (ICU) and was given oxygen therapy, crystalloid infusion, antithyroid drug, beta-blocker, diuretics, digitalis, anti-angina, anti-thrombotic, and adjunct therapy. The patient was treated for 8 days in ICU, followed by 2 days in the ward with a good outcome. Early detection and intervention followed by close monitoring is key management for the patient with hyperthyroid heart disease, especially in a male patient, to achieve a better outcome.

Abstract

Understanding the human immune response toward Mycobacterium tuberculosis infection is important for controlling its infection. Its transmission through the air consists of "droplets nuclei" containing TB bacilli. After initial infection, the human body will provide diverse immune responses and will determine different clinico-histopathologic finding. This response starts from innate immunity that consists of phagocytosis by distal alveolar macrophages or nasal microfold cells, then will be continued by dendritic cells to be transferred to mediastinal lymph nodes to induced adaptive immune responses. This response is mediated by cells through IFN- γ signaling which will enhance phagocytosis. If this response is effective, there will be a latent infection with an initial histopathological finding of caseosa granulomas and predominantly followed by chronic granulomas. In a few cases, it can be reactivated via the IL-10 activation pathway and exogenous factors, it will induce a great adaptive immune reaction and provide more severe clinico-histopathological manifestation. The existence of the human body's immune response to Mycobacterium tuberculosis, etiher innate or adaptive immunity will determine the clinical course and pathology that will occur.

Abstract

Coronavirus Disease 2019 (Covid-19) has now become a concern around the world. Until 6 September 2020, there were 190,665 cases in Indonesia, and it was in the third rank of most cases in the Asian region. It is important to determine the criteria of return to work for healthcare workers with Covid-19 positive in Indonesia. This case report discussed a female patient, 32 years old with chief complaint of fever and itchy throat since 2 days ago. She is a health care worker in the hospital, with a history of treating patients with positive confirmation of Covid-19 for 2 days in the ordinary ward before the case was confirmed with a history of contact approximately 5 days ago. The patient was diagnosed with positive confirmation of Covid-19 and was required for self-isolation. The patient still had positive results of RT-PCR Covid-19 up to the third examination in 30 days after the initial RT-PCR testing, although received specific antiviral treatment with oral oseltamivir and oral supplementation. Based on growing evidence that Sars-Cov-2 may not infectious for more than ten days in the mild-moderate disease, this case report is strengthening the use of symptom-based strategic approach to determine when to return to work in Indonesian healthcare workers with positive COVID-19.

Abstract

In the urogenital system, simvastatin is associated with interstitial cystitis adverse effects, but the exact mechanism is not yet clearly defined. This study aims to determine the effect of simvastatin on eosinophilic inflammation of bladder tissue in vivo. Laboratory experimental research design with the post-test only control group using 24 female Wistar rats aged 8-10 weeks were randomly divided into simvastatin 50mg/kg BW (n=12) or placebo carboxymethylcellulose 0.5% (n=12). All groups received treatment through oral gavage for thirty days. After that, each group was divided equally into three subgroups: control rat, day 0 Interstitial Cystitis (IC) rat (IC0), and day 3 IC rat (IC3). Control or IC0 rats each received intravesical instillation of buffered saline or protamine sulfate (PS), respectively, and were terminated immediately less than 3 hours after instillation. The IC3 rats received intravesical PS instillation and were terminated three days post-instillation. The bladder tissue was made in Hematoxylin-Eosin histology preparations. As in previous studies, the results showed successful desquamation of the urothelium after PS instillation. Tissue eosinophil counts were significantly higher in the simvastatin group than in the placebo group in the IC3 model (15.50±5.92 vs. 4.00±2.83, p=0.013). It can be concluded that the mechanism of the adverse effect of simvastatin on bladder tissue is through increased tissue inflammation mediated by eosinophils along with urothelial layer destruction by the protamine sulfate.

Abstract

Introduction: Thyroid Neoplasm is a tumor originating from the thyroid gland. Thyroid neoplasms encompass both benign and malignant tumors. Thyroid malignancies are among the most frequently encountered malignancies of the endocrine glands. Thyroid neoplasms generally have a relatively good prognosis when detected early. This study aims to determine the characteristics of thyroid neoplasm patients in the Department of Anatomic Pathology, Aloei Saboe Hospital.Method: This study is descriptive research using secondary data from medical records. Data was collected using a total sampling technique with a sample size of 58 patients. The study was conducted in the Department of Anatomic Pathology, Aloei Saboe Hospital, from January 2021 to October 2022.Results: The distribution of thyroid neoplasm patients is highest in late adulthood (36-45 years) with 15 patients (25.9%), followed by early elderly (46-55 years) with 15 patients (25.9%). The gender distribution shows more female patients, accounting for 43 (77.6%). The most common histopathological type is Colloid Goiter, with 20 patients (34.5%).Conclusion: The distribution of thyroid neoplasm patients is most prevalent between the ages of 35 to 55 years, predominantly in females, with the most common histopathological finding being Colloid Goiter. This study is expected to serve as a valuable source of information, enhancing public knowledge and awareness regarding thyroid neoplasm patients' characteristics.

Abstract

Introduction: Medical students need to learn histology to understand the relation between microscopic structure and function of cells and tissues. Pictures of manual preparation made by students can be used as notes and can describe students' understanding of the observed structures. Producing good pictures in a short time is a challenge for medical students. The use of digital illustrator technology is expected to be a solution to overcoming these problems. This study aims to measure the perception of acceptance and satisfaction rate of medical students in Gorontalo towards digital illustrators in apprehend histology practicum.Method: Cross-sectional descriptive survey with modified questionnaires from previous studies. The sampling technique used purposive sampling and involved 54 first-year students. The raw data were analyzed using Microsoft Excel. The mean value calculated for each item was compared with the theoretical mean value of 2.50 to determine whether the respondents agreed with the statement.Results: All items have an average rating exceeding the theoretical mean of 2.50. Also, the mean of 3.91 (SD = 0.58) for all items substantially exceeds the theoretical mean. Items with the highest perceived rate of perceived utility, ease of use, intention to use, and actual use of digital illustrator teaching materials in learning practical skills will significantly increase students' motivation, interest, and acquisition of practical skills.Conclusion: the use of digital illustrators is perceived positively by medical students in helping histology practicum learning, which is indicated by the high rate of acceptance and rate of satisfaction with its use.

Abstract

Introduction: Impaired lung function is a functional disorder of the lungs that causes a slowing of airflow during respiration. Bentor drivers are a profession with a risk of decreasing lung function due to work activities that are always on the road and exposed to motor vehicle pollution. Exposure to cigarette smoke in active and passive smokers is also significant in causing several other respiratory diseases, such as chronic obstructive pulmonary disease (COPD). The purpose of this study was to describe the risk factors for impaired lung function in Bentor drivers in Gorontalo City in 2023.Method: The research was conducted in June 2023 on eligible Bentor drivers accidentally found in Gorontalo City. The research instrument used a questionnaire, stress perception scale (SPS), and spirometry.Results: The results obtained are the distribution of risk factors for impaired lung function in Bentor drivers in Gorontalo City in 2023, based on length of work found more work more than five years (63.33%), more work more than 8 hours a day (75%), had smoking habits with a smoking duration of more than ten years (58.3%) and smoked more than ten cigarettes a day (62.5%) and had a severe perception of stress (66.6%).Conclusion: Bentor drivers are one of the contributing factors in impaired lung function. There is a relationship between the risk factors for smoking, length of work, duration of exposure to pollution, and perception of stress with lung function in Bentor drivers in Gorontalo City. Key words: Bentor drivers, lung function, smoking, stress perception

Abstract

Stunting masih menjadi masalah kesehatan yang prioritas untuk diselesaikan di negara kita. Meskipun tren stunting terus menurun, namun angkanya masih di atas standar world health organization (WHO) yang menargetkan di bawah 20%. Di Gorontalo, angka stunting masih jauh di atas pencapaian nasional yakni berkisar pada 29% pada tahun 2021. Beragam upaya perlu dilakukan dengan melibatkan kerjasama lintas sektor untuk berkontribusi pada penurunan angka stunting ini. Meningkatkan pengetahuan tentang pencegahan stunting pada masyarakat, khususnya pada pasangan usia subur merupakan salah satu strategi yang dipandang efektif. Hal ini dapat dicapai, diantaranya melalui penyuluhan kesehatan yang intens dan menarik. Berbekal dengan pengetahuan ini, diharapkan pasangan usia subur dapat menerapkannya secara langsung pada anak yang dilahirkan dan bisa berdampak pada menurunnya kejadian stunting. Kegiatan ini bertujuan untuk meningkatkan pengetahuan pasangan usia subur mengenai pencegahan stunting di Desa Bube Baru Kabupaten Bone Bolango. Kegiatan ini dilakukan dengan cara penyuluhan kepada 30 pasangan usia subur di wilayah tersebut. Penilaian tingkat pengetahuan pencegahan stunting dilakukan sebelum dan sesudah penyuluhan melalui kuesioner yang berisi pertanyaan tertutup. Uji Wilcoxon digunakan untuk menganalisis perubahan persepsi pengetahun yang terjadi. Hasil dari kegiatan ini menunjukkan terdapat peningkatan dari skor persepsi pengetahuan pasangan usia subur yang bermakna pada semua dimensi pertanyaan, baik definisi stunting, gejala stunting, penyebab stunting, faktor resiko stunting, dampak stunting, dan pencegahan stunting (semua p <0,05). Pada kesimpulannya, kegiatan penyuluhan pencegahan stunting pada pasangan usia subur di desa ini telah terlaksana dengan baik yang bisa diamati pada perbaikan persepsi pengetahuan yang dicapai

Abstract

Introduction: Lower Urinary Tract Symptoms (LUTS) are common in older adults and significantly impact individuals, caregivers, and the broader healthcare system. As the elderly population with various comorbidities increases, the burden of LUTS will increase. The chronic disease management program (Program Pengelolaan Penyakit Kronis, PROLANIS) is a government program generally followed by the elderly. This study aims to describe the characteristics of LUTS in PROLANIS patients in Gorontalo Province.Methods: The analytical observational study design, using a cross-sectional approach, was conducted on PROLANIS patients living in rural and urban areas in Gorontalo Province is between July - October 2023. An incidental sampling technique was conducted on eligible patients and areas. LUTS symptoms were measured using the International Prostate Symptom Score (IPSS) questionnaire and categorized into mild, moderate, and severe. LUTS symptoms were considered significant if the IPSS score was ≥8.Results: Of the 52 participants, 38.50% experienced significant LUTS symptoms. The majority of symptoms felt were nocturia (score two, as many as 23.10%) and mild symptoms (53.80%). Geographical differences, gender, education level, and age were not associated with LUTS symptoms (p 0.10).Conclusion: The prevalence of LUTS in patients undergoing PROLANIS is relatively high. The majority of LUTS patients have mild symptoms, with nocturia being the most common complaint compared to other symptoms. Further studies with a larger sample size and involving multiple sites are needed to obtain a more valid description of the characteristics of LUTS epidemiology in the PROLANIS population.                                                                         Keywords: Aged; Disease Management; Lower Urinary Tract Symptoms; Nocturia

Abstract

Background: Histology is one of the essential subjects to be understood in medicine. Identifying the appropriate learning styles can be a solution to improve learning performance and achieve maximum learning outcomes.Aims: This study aims to determine the effectiveness of learning styles on histology practical exam results in medical students.Methods: This study used a cross-sectional design by involving students at the medical faculty of Universitas Negeri Gorontalo who contracted the biomedical course I in the 2022/2023 academic year. Learning styles are assessed using a visual-auditory-kinesthetic (VAK) questionnaire to categorize student learning styles. The results of the practicum exam in the form of numbers are obtained from the accumulated scores of the first practicum exam in histology with the topics of cytology, epithelium, and connective tissue. The one-way ANOVA test was conducted to analyze the data.Results: Out of 42 participants involved, visual, auditory, and kinesthetic learning styles were 18 people (42.9%), 23 people (54.8%), and 1 people (2.4%), respectively. Generally, the student practicum exam results were 63.62 ± 17.24. Based on the learning styles group of visual, auditory, and kinesthetic, the mean (SD) practicum exam results were 66.00 (14.60), 62.43 (19.29), and 48.00 (0.00), respectively (p-value = 0.541).Conclusion: Visual learning style shows marginally better practicum exam results than other learning styles. This result can be considered a learning strategy to improve the results of practicum exams, particularly in the subject of histology practicum courses.
