Abstract

Job satisfaction among doctors is the main key to getting better health services. Changes in payment methods in the JKN era led to the dissatisfaction of specialists and general practitioners with the medical services received in BPJS patient care. This study analyzes the effect of perceptions about the distribution of medical services for BPJS patients on the job satisfaction of doctors. The research design used in this study was a descriptive analytic non-experimental study with a cross-sectional approach. The study was conducted at the Royal Prima General Hospital located in Medan City from May 2021 to August 2021. The sample in this study was selected using the purposive sampling method so that a sample of 77 people was obtained with the inclusion criteria of having worked for at least 2 years. Data were analyzed using the Chi-Square test and multiple logistic regression. The results showed that the total medical services received by general practitioners per month was 10 million rupiah. Meanwhile, the total number of medical visits and services received by specialist doctors per month is 25 million rupiah. Based on the results of the Chi-Square test, it can be seen that doctors' perceptions of medical services (number of medical services (p = 0.000), timeliness of delivery (p = 0.000), suitability of medical services with performance (p = 0.003), distribution system (p = 0.004), and transparency (p=0.011)) had a significant effect on doctor's job satisfaction. The results of multiple logistic regression test show that the variable with the greatest influence is the number of medical services. The variable number of medical services has a value of Exp(B) = 29,830 (95% CI 3,037-293,034) that means that respondents who state the number of good medical services are 29 times more likely to be satisfied than those who state that the number of medical services is less. Doctor's job satisfaction can be increased by the management's efforts to be more open to doctors regarding doctor's medical services, giving doctors the opportunity to improve their career paths, and conducting periodic evaluations of doctor's satisfaction through questionnaires.

Abstract

The role of specialist doctors in hospital services is large, making the bargaining position of specialist doctors high against hospitals. Various literature states that loyalty plays an important role in the formation of a person's decision in choosing a place of work and giving commitment. This study analyzes the effect of motivation and organizational climate on the loyalty of specialist doctors. This explanatory survey research uses a cross-sectional design. The study was conducted at the Royal Prima General Hospital Medan for 1 (one) month starting from July 2021 to August 2021. The sample was 50 people who were determined by the inclusion criteria were specialist doctors who had worked for at least 2 years. Data were analyzed using the Chi-Square test and multiple logistic regression. The results of statistical tests using Chi-Square show that motivation has a significant effect on the loyalty of specialist doctors, whereas the results of statistical tests show that physical and esteem needs have a p-value of less than 0.05. The organizational climate also affects the loyalty of specialist doctors, where the results of statistical tests show that flexibility, responsibility, standards, and commitment have p values less than 0.05. The results of the multiple logistic regression test show that the variable that has the greatest influence on the loyalty of specialist doctors is flexibility. With an Exp (B) value of 18,444 (95%CI 2,052- 165,779), Respondents who stated good flexibility had 18 times more loyal than those who stated poor flexibility. In conclusion, good flexibility will increase the loyalty of specialist doctors. This study concludes that motivation and organizational climate have a significant effect on the loyalty of specialist doctors. The aspect of flexibility in the organizational climate has the most influence on the loyalty of specialist doctors.

Abstract

Efforts to strengthen the health services provided to patients rely heavily on the use of high-quality, complete, accurate, and timely data to inform decision-making at the clinical, facility, and policy levels in hospitals. However, evidence of gaps in the quality of medical records is often found. At RSU Royal Prima Medan there are still some incomplete data such as writing the actions that have been done to the patient. The purpose of this study was to analyze the factors that affect the quality of medical records as part of the initiative to strengthen the health care system. The research was conducted using a survey method with an explanatory research design. The research population was 612 people and as many as 100 samples analyzed were taken by stratified random sampling. The univariate test showed that most of the respondents had filled out medical records completely (78.0%) and on time (72.0%), but there were 41.0% medical records that were inaccurate and did not meet the legal requirements of medical records (59 ,0%). Bivariate analysis with Chi-Square showed that the variables of knowledge (p=0.001), procedures (p=0.006) and supervision (p=0.003) had an effect on the quality of medical records, while equipment had no significant effect. Of the three factors, the most dominant factor influencing the quality of medical records is the knowledge obtained through multiple logistic regression tests with an OR value of 4 times the risk of affecting the quality of medical records. This research is an indication that poor knowledge will affect the quality of medical records so that the Royal Prima Hospital Medan needs to conduct socialization and training to increase the knowledge of health workers (doctors, nurses, medical recorders).

Abstract

The government has a responsibility to ensure that every citizen gets health services. Minimum Service Standards Indicators aim to be a benchmark of achievement in hospital performance. The indicator that is often used is the Bed Occupancy Rate (BOR). The state of the inpatient installation is an internal determinant of increasing satisfaction from consumers (patients). The aim of the research is to help local governments to improve the level of public health. As well as helping Royal Prima General Hospital Medan provide the best health services, and high quality standards. This research is a quantitative analytical-observational study with a cross sectional approach. Performed at the private public hospital Royal Prima Medan. Primary and secondary data collection was carried out in March-May 2021. Data analysis used a quantitative analysis approach; Data editing, data coding, univariate analysis, bivariate analysis.This analysis was carried out by using the Analysis of Variance (ANOVA) test on the program IBM SPSS. The research subjects came from inpatient room respondents from grades 1, 2, and 3. Based on the results of the study, the attitude of medical personnel was significant (P=0.04) towards services. Medical and medical services were not significant (P=0.13) on health services. Public facilities and infrastructure are significant (P=0.03) on health services. Distance and location of settlements are not significant (P=0.18) to health services. In conclusion, the attitude of medical personnel and public facilities affects the patient's interest. Meanwhile, distance, residential location, and medical services did not affect the patient's interest. Suggestions from this research are to improve the performance of medical personnel and hospital workers, by conducting training and training gradual coaching, to support more conducive health services at Royal Prima General Hospital Medan

Abstract

Antioxidants are chemical compounds that can donate one or more electrons to free radicals, so that these free radicals can be quenched. There are two sources of antioxidants, namely natural and synthetic antioxidants. Andaliman (Zanthoxylum acanthopodium DC.) is one of the many spice plants found in Toba Samosir and North Tapanuli Regencies, North Sumatra. Andaliman plants grow at an altitude of 1,500 m above sea level. Apart from North Sumatra. This research using method experimental research when collecting and processing andaliman fruit, making simplicia, making ethanol extracts and examining antioxidant activity tests using the ABTS free radical scavenging method (2,2'-azino-bis(3-ethylbenz-thiazoline-6-sulfonic acid) and antibacterial activity. using the minimum inhibitory concentration method. Testing the antioxidant activity of the ethanol extract of andaliman fruit with the ABTS method using a UV-VIS spectrophotometer with a wavelength of 734 nm. the regression equation (y = 0.4619 + 20.226) obtained an IC50 value of 64.4598 mg/mL which can be classified as a strong antioxidant activity. The results of the antibacterial activity test of the ethanol extract of the andaliman fruit showed that the extract had antibacterial activity in the moderate category at a concentration of 300 mg/ml with an inhibition zone of 10.

Abstract

Covid-19 is an infectious disease caused by a new type of coronavirus with symptoms of acute respiratory distress such as fever, cough, and shortness of breath. The type of research is comparative research with secondary data collection of as many as 280 comorbid patients and 280 non-comorbid patients in the month of august 2021 – January 2022. Data were analyzed univariately, bivariate with test Mann Withney. The results of this study are: Based on the mean length of stay for comorbid patients was 12.22 higher than that of non-comorbid patients, which was 5.28. The results of the laboratory examination of leukocytes in comorbid patients were higher at 13.17 than in non-comorbid patients, namely 10.07. In contrast, platelets were smaller, namely 268.87 than non-comorbid patients, namely 278.91. Neutrophils in comorbid patients were 76.28 higher than non-comorbid patients, namely 61.13. The lymphocyte of comorbid patients was 13.28 smaller than that of non-comorbid patients, which was 25.93. The D-dimer of comorbid patients was not too much different, namely 1259.85 than that of non-comorbid patients, namely 557.16. The conclusion in this study is that there is a significant difference between length of stay and laboratory tests (leukocytes, platelets, neutrophils, lymphocytes, D-dimer) for comorbid and non-comorbid diseases in Covid-19 patients at Royal Prima Hospital Medan.
