Abstract

Karsinoma paru merupakan salah satu jenis karsinoma dengan angka kejadian serta angka kematian cukup tinggi di Indonesia dengan persentase 8,6% dan mortalitas 12,6% dari seluruh kejadian kanker pada tahun 2018. Studi epidemiologi ini dilakukan untuk mempelajari karakteristik pasien kasus karsinoma paru di Rumah Sakit Umum Pusat Sanglah, Denpasar pada tahun 2018-2019. Penelitian ini merupakan penelitian deskriptif observasional dengan desain stufi potong lintang, serta menggunakan data sekunder dari hasil pemeriksaan sitopatologi dan mutasi EGFR pasien karsinoma paru yang terdapat di Laboratorium Patologi Anatomi Rumah Sakit Umum Pusat Sanglah. Variabel yang diteliti adalah usia, jenis kelamin, lateralisasi, tipe sitopatologi, dan mutasi EGFR. Metode total sampling digunakan untuk memperoleh sampel sebanyak 98 sampel. Hasil yang didapatkan berupa kejadian karsinoma paru tertinggi pada rentang usia 50-59 tahun (29,6%), lebih banyak ditemukan pada laki-laki (56,1%), dengan lokasi tersering pada paru dekstra (69,4%). Tipe sitopatologi terbanyak adalah adenokarsinoma (89,8%). Karsinoma paru dengan mutasi EGFR dan tanpa mutasi EGFR ditemukan masing-masing sebanyak 50%. Mutasi EGFR paling sering terjadi pada tipe adenokarsinoma (44,9%). Sementara sisanya terjadi pada tipe karsinoma sel skuamus, adenoskuamus dan NSCLC unclassified yaitu masing-masing sebanyak 1%. Mutasi EGFR paling banyak terjadi pada ekson 19 (31,6%), diikuti oleh ekson 21 (11,2%), ekson 20 (4,1%) dan ekson 18 (3%). Hasil penelitian ini diharapkan dapat digunakan sebagai dasar penelitian dan dikembangkan lebih lanjut untuk membantu mempelajari perkembangan karsinoma paru khususnya di Bali. Abstract

ABSTRACT Hirschsprung's disease is an abnormal condition of the colon with characteristics of the Meissneri submucosus, myenteric plexus and Aurbachi plexus without parasympathetic ganglion cells. Manifestations of Hirschsprung's disease are seen in the discharge of meconium that does not come out within 24 hours. Then there are signs of obstruction to the gastrointestinal tract, vomiting bile, constipation, and irregular bowel movements. In 2017, 2.5 million children died in the first month of life worldwide. 47% of deaths of children under the age of 5 years occur in newborns, which is about 7,000 deaths. Cases of Hirschsprung's disease itself occur in 1:5000-7000 births, with cases in boys 4 times more than girls. This study aims to determine the clinicopathological characteristics of Hirschsprung's disease at Sanglah Hospital Denpasar in 2019-2020 based on clinical symptoms, age, gender, and location of the aganglionic bowel segment. This study was carried out in 2021 at the Sanglah Hospital Denpasar Anatomical Pathology (PA) installation, using data from the years 2019-2020. It was conducted using a cross-sectional method, with secondary data in the form of patient data available at the Sanglah Hospital Denpasar PA Laboratory, and total sampling. SPSS version 26 was used to analyze the data. The results of this study showed that there were 78 patients with Hirschsprung's disease at Sanglah Hospital Denpasar in 2019-2020 and all of them met the criteria of inclusion. Most cases of Hirschsprung's disease in Sanglah Hospital Denpasar experienced the most flatulence, namely 38 cases with a percentage of 36.5%. Cases were dominated by patients within age of 0-12 months with total of 64 people with 82.1% percentage, and dominated by men as many as 50 people with 64.1% percentage. The aganglionic most frequently found in the rectum as many as 49 people with a percentage of 62.8%. It was concluded from the study result that Hirschsprung's disease patients at Sanglah Hospital in 2019-2020 were mostly male, mostly in the age range 0-12 months, with the most clinical symptoms being flatulence, and the most common aganglionic location found in the rectum. Abstract

Aim: To assess the association between Ki67 index and various clinicopathological parameters of breast cancer patients in Bali. Methods: This was a cross-sectional study of breast cancer patients at Prof. Dr. I. G. N. G Ngoerah Hospital, a referral hospital in Bali, from 1 January 2021 to 31 December 2021. Clinicopathology data was taken from SIMARS and archives in the Anatomical Pathology Laboratory. Ki67 index was assessed by immunohistochemical examination, categorized into low (Ki67 index <20%) and high (Ki67 index?20%). Association between Ki67 index with clinicopathological parameters was analysis with X2 test with significance level determined at ?=0.05. Results: There were 75 cases of breast cancer patients with a mean age was 50.74 ± 10.05 years old. Low Ki67 index was found in 9 (12%) cases, while high Ki67 index in 66 (88%) cases. There was a significant association between Ki67 index and histological grade of the tumor (p=0.007), nodal status (p=0.014), and molecular subtype of tumor (p=0.007). There was no significant association between Ki67 index and the patient's age (p=0.268), histological subtype (p=0.086), and T stage (p=0.511). Conclusion: Ki67 index has significant association with histological grade of the tumor, nodal status, and the molecular type of the tumor in breast carcinoma patients in Bali.

Abstract

Background: Breast carcinoma is a heterogeneous disease, morphologically and molecularly. GATA binding protein 3 (GATA3) is an important marker in breast cancer origin. GATA3 expression and its prognostic value in breast carcinoma are inconsistent. Tumor budding is a small cluster of malignant cells in the invasive edge of the tumor. Breast carcinoma with highgrade tumor budding has an unfavorable prognosis. The objective of this study was to determine whether there is a relationship between GATA3 expression and clinicopathological parameters in breast carcinoma including with tumor budding grade. Another objective of this study was to determine the association between GATA3 expression and other clinicopathological factors with nodal status. Methods: This was a cross-sectional study involving breast carcinoma patients at Prof. Dr. I.G.N.G Ngoerah Hospital during the year 2022 who underwent mastectomy and axillary node dissection. GATA3 expression was determined by immunohistochemistry and categorized into weak and strong expression. Tumor budding was evaluated in Hematoxylin Eosin slides and categorized into low and high grades. The association between GATA3 expressions and clinicopathological parameters, and the association between GATA3 expression and clinicopathological parameters with nodal status, were assessed with a chi-square test with a significance level determined at p 0.05. Results: There were 61 breast carcinoma patients with mastectomy and axillary node dissection that fulfilled inclusion and exclusion criteria. GATA3 expression was significantly associated with the histological type (p = 0.012) and molecular subtype (p 0.001) of the tumor. There was an association between nodal status and molecular subtype of tumor (p = 0.012), tumor stage (p = 0.005), and tumor budding grade (p = 0.024), but there was no association between GATA3 expression and nodal status (p = 0.066). Conclusion: GATA3 expression is associated with the histological type and molecular subtype of tumor in breast carcinoma, but not with nodal status. Nodal status was associated with molecular subtype, T stage, and tumor budding grade of tumor.

Abstract

Introduction: Oncocytoma is a benign tumor of the salivary glands consisting of oncocyte cells, occurring in approximately 2% of all salivary gland neoplasms, most of which occur in the parotid gland. This tumour most commonly occurs in the sixth to eighth decades of life, with a mean age of 64 years and without gender predilection. The clinical presentation is often misdiagnosed as pleomorphic adenoma, cystic lesion, and inflammation.Case Presentation: A 63-year-old male, complained of a lump under the right ear, for 7 years ago. It feels painless and slightly enlarged since a month before surgery. Based on clinical examination suspected as benign cystic lesion due to inflammation and ultrasound suggesting a benign tumour mass with multiple lymphadenopathies at the right and left submandibular and right parotid. Then, the resected tumour continued for histopathological examination. Microscopically, the tumor cells are confined to the tissue. It consists of a uniform large polygonal cell with granular eosinophilic cytoplasm without any mitosis and necrotic area.Conclusions: Oncocytoma is a rare benign tumor of the salivary glands, especially affecting the parotid gland. Clinical and radiologically it is often misdiagnosed, so histopathological examination is needed for diagnostic confirmation for the best patient outcomes

Abstract

Introduction: GISTs usually occur in older adults with a median age of about 60–65 years. The incidence rate between men and women is equal (1:1). The most common location of GISTs is the stomach which accounts for 60%, followed by the jejunum and ileum around 30%, duodenum 5%, and colorectal 5% and a small percentage of cases occur in the esophagus, appendix, gallbladder, and extraintestinal areas such as the mesentery, omentum, and retroperitoneum. Case Presentation: A 59-year-old male, suffered from a lump under the umbilicus 3 months before admission to the hospital. The results of the ultrasound examination showed a mass in the superior area of the bladder, and the mass did not appear to infiltrate the intestines, bladder, umbilicus, and surrounding organs. The biopsy tissues showed spindles and round-to-oval morphology with eosinophilic cytoplasm. Mitoses 65/50 High Power Field In addition, immunohistochemistry examination showed positive stains in DOG 1, CD117, SMA, CD 34, and negative for S100.Conclusion: Extragastrointestinal stromal tumor is a rare tumor. Based on, clinical, imaging, histopathological, and immunohistochemical examination, this case was diagnosed as an extragastrointestinal stromal tumor, spindle cell type.

Abstract

Background: Cervical cancer is a malignant tumor of the cervix with a high mortality rate, primarily identified by vaginal bleeding. Human papillomavirus (HPV) infection was found in 99.7% of cervical cancer cases. Other risk factors include age at first sexual intercourse, parity, smoking, weak immune system, use of oral contraceptives, and a high number of sexual partners. This study aims to analyze and describe the clinicopathological features of cervical carcinoma, including patient demographics, primary symptoms, histopathological types, and tumor staging, at Prof. Dr. I.G.N.G. Ngoerah Hospital from 2021 to 2022.Method: This study employs a descriptive research method with a retrospective approach, utilizing secondary data and comprising 283 samples. Inclusion criteria included patients diagnosed with cervical carcinoma confirmed by histopathological evaluation and complete medical records documenting age, primary complaints, histopathological classification, and TNM staging. Data were processed and presented using descriptive statistics. Results: The results showed that the age group with the highest number of cervical carcinoma cases was the 40-49 years age group (36.0%), the most common main complaint was bleeding (68.9%), the most common histopathological type was squamous cell carcinoma (78.8%), and the most common TNM stage was T1 (52 cases) and N0 (87 cases).Conclusion: This study highlights that cervical carcinoma predominantly affects women aged 40–49 years, with abnormal vaginal bleeding as the most common symptom. Squamous cell carcinoma was the most common histopathological type (78.8%), with the majority of cases staged at T1 (18.4%) and N0 (30.7%).

Abstract

Obesity is a condition where excessive fat is accumulated, which poses a health risk. Several treatments for obesity have been carried out, ranging from lifestyle adjustments to medication to surgery. Corrosolic acid (CA) is a natural compound that has the potential to act as an obesity agent through many mechanisms, including through reactive oxygen species (ROS). This study aims to prove that CA can reduce MDA levels and HMG-CoA reductase levels and increase leptin levels and GLP-1 levels in male Wistar rats with obesity. Wistar obese rats were orally treated with CA compound induction at a dose of 10 mg/Kg BW. MDA, HMG-CoA reductase, leptin and GLP-1 levels were examined using serum and plasma from mice in both groups of mice before and after treatment, using the ELISA method. CA to the treatment group can reduce the body weight of rat MDA levels significantly at p<0.0001, HMG-CoA reductase levels significantly at p<0.0001 and increase leptin and GLP-1 levels significantly at p<0.0001. CA's mechanism for treating obesity is through the ROS mechanism because CA has antioxidant levels that can capture free radicals in the body.CA has been proven to be an alternative drug in treating obesity by reducing body weight, MDA, and HMG-CoA levels and increasing leptin and GLP-1 levels in obese Wistar rats.

Abstract

Background: Metastasis is the most common cause of death in breast cancer and nodal metastasis is one of the most important predictive factors for distance metastasis. One of the intrinsic properties of cancer cells is that they resemble stem cells which are capable of self-renewal, resistance to chemotherapy and radiation, and metastasis. CD90 is a stem cell marker found in several malignancies, including breast carcinoma. The objective of this study was to prove that CD90 can predict the occurrence of axillary nodal metastasis in breast carcinoma. Methods: The study design was a case-control study. The subjects of this study were breast cancer patients at Prof. Dr. I. G. N. G. Ngoerah Hospital who underwent mastectomy and axillary lymphadenectomy in the year 2019. CD90 immunohistochemistry was performed and its association with metastasis, along with various clinicopathological markers, was assessed with chi-square and logistic regression tests with a significance level determined at α=0.05. Results: There were 25 cases of breast carcinoma with axillary nodal metastasis and 25 cases without metastasis. There was a significant relationship between high CD90 expression and the occurrence of nodal metastasis (p=0.010). There was no relationship between patient age, histologic grade, histologic subtype, molecular subtype, and T-stage with axillary nodal metastasis. Breast carcinoma patients with high CD90 expression have a 7.25 times higher nodal metastasis risk compared to patients with low CD90 expression. Conclusion: High CD90 expression could predict axillary nodal metastasis in breast carcinoma.
