Abstract

Background. Minahasan culture has an important role in shaping community attitudes and behavior, including in health services, with cultural values If All Children Tumou Too, has principles that are very relevant in improving the quality of hospital services. This design aims to integrate local Minahasan wisdom into the hospital service system to increase patient satisfaction. The aim of this research is to determine the quality of patient satisfaction based on Minahasa culture at RSUD Dr.  Sam Ratulangi Tondano  Method. The research design used was qualitative phenomenological. The source of informants or participants in this study were 10 people consisting of patients or families of patients who were undergoing treatment in the surgical room at RSUD Dr. Sam Ratulangi Tondano. Primary data was obtained through in-depth interviews using interview guides conducted with informants. Data analysis in this research uses content analysis  Results. The research results showed that there were 5 themes that emerged, namely: Responsiveness in accordance with Minahasa culture, Malinga-lingaan (listen to each other), Empathy, that is  Mangenang-genangan (feeling what the patient feels, praying for each other, visiting). Tangible, that is Maesa-esa’an, (must be united), Maesa-esa’an (remind each other), Reliability that is Matombo-tombolan (support each other) and Assurances that is  Mapalus, (help and please help). In providing health services, especially at RSUD Dr. Sam Ratulangi Tondano Minahasan culture continues to be preserved, especially in context ma-saali (service), where a nurse provides good service to patients, both in terms of appearance and friendliness.  Conclusion. Aspects of Minahasan culture applied at RSUD Dr. Sam Ratulangi Tondano, namely Malinga-lingaan (listen to each other), Maleo-leosan (attitudes and behavior that love each other), Maesa-esa’an, (must be united), Mangenang-genangan (remind each other), Matombo-tombolan (support each other) and Mapalus, help each other and help each other.  Suggestion. For the hospital to make signboards to rooms or corridors, providing announcements using language Tolour or Tondano language, and wearing work clothes/uniforms using local batik (batik bentenan) and for nurses and medical personnel to provide hospital services with Minahasan cultural dimensions, magenang-genangan, masawang-sawangan, matombo-tombolan, malinga-lingaan dan maleo-leosan

Abstract

Gagal ginjal kronis telah diakui sebagai salah satu masalah kesehatan dengan prevalensi terbanyak di seluruh dunia dengan 13,4% dari total populasi. Kondisi medis saat ini penatalaksanaan masih dengan hemodiafiltrasi jangka Panjang. Pasien dengan pengalaman pertama kali hemodialisa muncul rasa cemas karena ketakutan terhadap managemen terapi bahkan sampai adanya penolakan. Pemasangan dan pemeliharaan arteriovenus fistula merupakan salah satu masalah yang sering pada pasien. Pasien mengalami berbagai tahap kesedihan yang berbeda selama proses berduka, dan tidak semua orang dapat mencapai keadaan pikiran yang positif. Saat ini belum ada penelitian yang mengeksplorasi pengalaman pasien hemodialisa yang terpasang arteriovenous fistula (AVF). Tujuan penelitian untuk menggali pengalaman pasien hemodialisa yang terpasang arteriovenus fistula berdasarkan perspektif Kubler Ross di RS ‘Aisyiyah Kudus. Penelitian ini adalah penelitian kualitatif dengan pendekatan studi fenomenologi. Pengambilan data menggunakan  in-depth interview, partisipan 11 orang yang terdiri partisipan pasien dan keluarga. Partisipan pasien merupakan pasien hemodialisa yang terpasang AVF satu tahun terakhir. Dari hasil penelitian didapatkan 4 tema yaitu (1) partisipan berespon menyangkal dan menerima  sebagai pasien hemodialisa yang terpasang Avf Shunt, (2) Membatasi makan, minum dan perawatan Avf shunt di rumah, (3) Membutuhkan dukungan keluarga dan (4) Hambatan yang dialami keluarga pasien dalam psikologis, sosial dan ekonomi kemudian ditindaklanjuti dengan pembuatan buku panduan perawatan arteriovenus fistula dalam tahap penolakan.Kata Kunci : Arteriovenous fistula, hemodialisa, Kubler Ross, Pengalaman

Abstract

Poverty in Central Mamberamo Regency is exacerbated by the low daily malnutrition rate (PoU) per capita, which is only 26.1% of the AKG in 2023. This region is of particular concern because it contributed 35.42% of the highest stunting prevalence among 28 other regions in Papua in the same year. The Neuman System Model encourages the role of nurses in maintaining the stability of the client system (individual, family, group, or community) through primary, secondary, and tertiary interventions. This study aims to explore the pattern of approaches to the effectiveness of local government policies and the supporting factors and obstacles to the success of programs in reducing stunting rates in Central Mamberamo Regency based on Betty Neuman's theory. This study used a qualitative approach with a case study or applied phenomenology method, focusing on an in-depth understanding of the experiences and perspectives of various stakeholders (pregnant women, postpartum women, mothers with stunted toddlers, nurses, and heads of agencies) selected through a combination of purposive sampling and snowball sampling methods at the Kobakma Community Health Center in Central Mamberamo from June to July 2025, with sample size determined based on data saturation. The results showed that the success of the ANC program in Central Mamberamo was evident in the increase in pregnant women's weight and normal births. This is supported by Betty Neuman's theoretical approach with structured interventions (education, supplements) to prevent stunting, along with cross-sector collaboration. However, this is hampered by a shortage of professional health human resources. Government strategies, which include nutrition education, iron supplementation, and nutritional rehabilitation for stunted toddlers, have shown effectiveness in improving maternal and infant health, although human resource challenges need to be addressed urgently. This study highlights the program's success in improving ANC adherence among pregnant women and healthy birth outcomes, with cross-sector collaboration as a key contributing factor. However, inadequate human resources (HR) for health professionals was identified as an urgent inhibiting factor that needs to be addressed.
