Abstract

Chronic Kidney Disease (CKD) is a health problem with increasing prevalence in Indonesia and globally. Patients with end-stage CKD generally undergo hemodialysis as replacement therapy, but this is often accompanied by complications such as Interdialytic Weight Gain (IDWG) due to excess fluid between dialysis sessions. Uncontrolled IDWG can lead to hypertension, pulmonary edema, and even heart failure. Therefore, effective nursing interventions are needed to help patients manage IDWG, one of which is through the application of Orem's Self-Care theory. This study aims to analyze the effectiveness of implementing the Orem Self-Care based protocol on IDWG in hemodialysis patients with chronic kidney failure at Dr. Loekmonohadi Hospital, Kudus. The study design used a quasi-experimental approach with a pre-test–post-test control group design . The sample consisted of 84 hemodialysis patients selected using a saturated sampling technique and divided into an intervention group (n=42) and a control group (n=42). Data collection was conducted by measuring the patients' Interdialytic Weight Gain (IDWG) before and after the intervention.  The intervention included fluid management education, weight monitoring, and fluid restriction strategies. Data analysis was performed using a paired t-test and a test for differences between groups with a significance level of p < 0.05. Before the intervention, the majority of patients were unable to control their IDWG (88.1 % in the intervention group; 81.0% in the control group). After the intervention, 90.5 % of the intervention group achieved IDWG control compared to only 26.2% in the control group. The mean IDWG decreased from 4.99 kg to 3.14 kg, with a significant difference of 1.85 kg (p < 0.001). The Orem Self-Care based protocol is effective in reducing IDWG in hemodialysis patients with CKD, increasing patient independence, and potentially preventing complications due to fluid overload.

Abstract

Manual femoral sheath removal following percutaneous coronary intervention (PCI) remains a major challenge in interventional cardiac nursing practice. Variations in manual pressure, prolonged compression duration, and nurse fatigue can lead to inconsistent hemostasis and potential peripheral complications. To address these issues, an innovative mechanical compression device Femovenart Mechanical Compression (Femopress) was developed as a locally engineered nursing tool designed to provide consistent, ergonomic, and safe compression after cardiac catheterization. This action research study was conducted at the Cathlab Unit of Dr. Loekmono Hadi General Hospital, Kudus, Central Java, from May to June 2025. The study involved four Cathlab nurses as collaborative practitioners and twenty post-PCI patients with femoral access. Data were collected through direct observation, peripheral safety assessment using six indicators (pulse, skin color, oxygen saturation, hematoma, paresthesia, and pain), and focus group discussions (FGDs). Triangulation was performed with three experts: an academic supervisor, an interventional cardiologist, and a senior Cathlab nurse. Quantitative data were analyzed descriptively, while qualitative data were processed using thematic content analysis. Prior to the innovation, manual compression required 15–20 minutes and often caused nurse fatigue and inconsistent hemostasis. Implementation of Femopress reduced compression time to 13–18 minutes, maintained optimal hemostasis, and improved peripheral safety. Based on the Abukays Peripheral Safety Scale, 90% of patients achieved “safe” scores (9–12), with no secondary bleeding or hematoma. Six FGD themes emerged: (1) manual practice limitations, (2) unavailability of special tools, (3) inadequacy of facilities, (4) need for dedicated compression devices, (5) positive evaluation of Femopress, and (6) expectations for future development. The Femopress innovation effectively improves procedural efficiency, reduces nurse workload, and enhances patient comfort and safety in femoral sheath removal after PCI. This device demonstrates strong potential as a standard nursing technology for Cathlab practice and a model of nursing-led innovation in cardiovascular care.

Abstract

Background: Clinical competency assessment is a core component of nursing governance and professional development. In many Indonesian hospitals, competency evaluations remain predominantly paper-based, resulting in administrative inefficiencies and limited data traceability. The implementation of the SICAKEP (Sistem Informasi dan Pencatatan Keperawatan) digital application at RSUD Welas Asih, West Java, represents an important step toward modernizing assessment processes; however, assessors’ experiences during this transition have not been fully explored. Objective: This study aimed to explore the lived experiences of clinical nurse assessors in conducting manual and digital (SICAKEP-based) competency assessments in a provincial teaching hospital in West Java, Indonesia. Methods: A qualitative descriptive phenomenological design grounded in a constructivist paradigm was employed. Four certified clinical nurse assessors with experience in both manual and digital assessments were recruited using purposive and snowball sampling. Data were collected through in-depth semi-structured interviews, brief observations, and document reviews between March and May 2025. Data analysis followed Colaizzi’s seven-step method, supported by reflexive journaling and member checking to enhance credibility. Results: Five themes emerged: adaptation to new assessment roles, multidimensional implementation barriers, constraints related to time and readiness, competency assessment as a driver of professionalism and quality, and transformation from manual to digital systems. Digital assessment improved efficiency and transparency but required stronger digital literacy and infrastructure support. Conclusion: Digital competency assessment represents a meaningful advancement in nursing governance, though sustained institutional support is essential to optimize its implementation

Abstract

Nursing services in the Emergency Department (ED) require fast and accurate actions, demanding effective communication amid high work pressure. This study aimed to explore nurses’ experiences, identify ideas, challenges, and suggestions in implementing a therapeutic communication model based on Hildegard E. Peplau’s theory, and examine its impact on the quality of nursing services in the ED of RSUD dr. Loekmonohadi Kudus. This qualitative study employed an Action Research design and involved eight ED nurses selected through purposive sampling. Data were collected using a Focus Group Discussion (FGD) module and were analyzed through thematic analysis, following workshops and expert consultations. The results indicated that nurses maintained effective communication despite high mobility demands by strategically tailoring their approach to patient conditions, fostering trust, and integrating holistic nursing care principles. Communication effectiveness was further strengthened through soft skills training and leadership support. The study recommends ongoing training, the development of standardized communication modules, and systemic support to reinforce therapeutic communication practices. These findings contribute to advancing humanistic and patient-centered nursing practices in high-acuity emergency care settings.
