Abstract

Patients diabetes mellitus type 2 have the potential to suffer from various complications, both acute and chronic complications. The family in addition to providing support also has an important role in the process of preventing these complications. So the purpose of this study was to determine the effectiveness of endurance  diabetes foot exercise based on family centered care (EDFE-BFCC) in reducing blood sugar levels patients diabetes mellitus type 2 in the work area of Bengkuring Public Health Center, Samarinda. This study used a pre-experimental research design with a pre and post test research design without a control group. Researchers only intervened in one group without a comparison. The population in this study were all patients with Type 2 Diabetes Mellitus in the working area of Bengkuring Public Health Center, Samarinda. The sample used was 13 people by taking all members of the population according to the inclusion criteria. Diabetic foot exercise is modified with endurance movement, which is then through family empowerment to train family members with diabetes mellitus type 2. The effectiveness of the treatment was assessed by comparing the value of blood sugar levels during pre and post intervention. The results showed that p value = 0.001. Endurance diabetes foot exercise based on family centered care (EDFE-BFCC) is effective in lowering blood sugar levels during patients diabetes mellitus type 2.Keywords: diabetes foot exercise; endurance; family centered care; diabetes mellitus type 2

Abstract

The communication method of Situation, Background, Assessment, and Recommendation (SBAR) are effective communication techniques that aim to improve patient safety. This study aims to analyze the application of SBAR communication to nurses to improve the safety culture of patients who are part of the service at A.W. Sjahranie Hospital, Samarinda, Indonesia. This study used a cross-sectional design with a population of 452 people spread over 14 service units. The sample size was 77 people. Variables in this study were SBAR communication methods and patient safety culture. The instrument used to assess the application was the SBAR communication method in the form of an observation sheet for the procedure for reporting patients' conditions during handover while collecting data on variable patient safety culture used a questionnaire from the agency for Healthcare Research Quality (AHRQ). Data of SBAR communication technique variables were collected using the observation method, while the patient safety culture variable was collected by distributing patient safety culture questionnaires. Data were analyzed using a Chi-Square test. The results of this study showed that nurses who used the SBAR communication method had a better patient safety culture (p = 0.000). The conclusion is the SBAR communication method to nurses can use to improve patient safety culture in the hospital. Keywords: SBAR communication method, Patient safety culture

Abstract

The important factor affecting patients’ treatment and safety service quality is patients’ safety culture. This research aimed to identify the most effective strategy to develop the patients’ safety culture to prevent nursing error in the highest referral hospital in East Kalimantan Province. This research is a mix method. The population in this study were all nurses in the inpatient room totaling 342 people and a sample of 119 people obtained by proportional random sampling. In quantitative research, data were obtained using questionnaires and then analyzed using multiple linear regression. While qualitative data were obtained through in-depth interviews with 3 informants, namely the implementing nurse, the head of the inpatient room and the chairman of the patient safety committee. The results of the analysis show that there are two environmental factors that influence patient safety culture, namely lifesaving control (p value = 0.002) and patient safety (p value = 0.041). Through in-depth interviews, it is known that individual, organizational, and environmental factors are effective in developing a patient safety culture.Keywords: strategy; development; patients safety culture

Abstract

Pendahuluan : Pneumonia merupakan salah satu masalah kesehatan dan penyumbang terbesar penyebab kematian balita di dunia. Upaya perlu dilakukan untuk menyiapkan keluarga dalam asuhan maupun pemberian informasi sebagai persiapan pulang kepada keluarga tentang cara perawatan anak dengan pneuomonia. Hal tersebut menjadi dasar bagi keluarga menindaklanjuti pelaksanaan perawatan dan cara pencegahan pneumonia di rumah secara mandiri. Tujuan: Penelitian ini bertujuan untuk menilai kesiapan ibu dalam merawat anak dengan pneumonia pasca hospitalisasi. Metode: Desain penelitian ini adalah deskriptif analitik dengan metode cross-sectional, dengan sampel dalam penelitian ini adalah ibu yang memiliki balita pneumonia usia 2 sampai dengan 59 bulan. Hasil: Hasil analisis uji fisher exact didapatkan p value < 0,05 yang artinya ada hubungan yang bermakna antara pengetahuan, persepsi, dan psikomotor/keterampilan ibu dalam merawat balita dengan pneumonia pasca hospitalisasi. Hasil analisis juga didapatkan nilai OR yang bermakna ibu dengan pengetahuan dan sikap rendah memiliki risiko 88,0 kali lebih besar untuk tidak siap merawat anak dengan pneumonia, dan ibu yang tidak terampil berisiko 16,0 lebih besar untuk tidak siap merawat anak dengan pneumonia pasca hospitalisasi. Kesimpulan: Sehingga penting dilakukannya edukasi kepada orang tua terutama ibu saat berada di rumah sakit untuk meningkatkan pengetahuan, sikap, dan keterampilan dalam rangka kesiapan merawat anak dengan pneumonia pasca hospitalisasi

Abstract

Currently, many housewives decide to work, with these conditions it has an impact on limited time to interact directly with children. This causes children to be further away from parental supervision and assistance, especially in using gadgets. This study aimed to determine the relationship between working mothers and the level of dependency on gadget use in preschool-aged children. The research method used is an analytic study with a cross-sectional design. A sample of 303 mothers with preschool-age children in all public kindergartens in Samarinda City. The level of dependency on gadget use in preschool-age children is based on the standard length of time using gadgets in a day according to the Indonesian Pediatrician Association (IDAI). Then the data obtained were analyzed using the Chi-Square test. The results of the analysis obtained-valuable <0,05 (0,001), which means that there is a relationship between working mothers and the level of dependence on gadget use in preschool-aged children. The use of gadgets at preschool age must be supervised and assisted, especially by parents, not only mothers but also families.

Abstract

Background: Pneumonia is an acute respiratory infection that attacks the lungs. Pneumonia causes symptoms of difficulty breathing which if left untreated will lead to death. Families with problems with family members who experience illness or disease often make the family unable to meet health care needs or experience powerlessness which worsens the health condition of family members who experience health problems. Nursing intervention is needed to overcome the problem of family powerlessness. Objectives: This community service aims to increase mothers' knowledge and perceptions of caring for toddlers with pneumonia after being hospitalized at Pasar Rebo Hospital. Methods: The method used consists of identifying problems and needs, assessing problems, formulating plans, providing health education, and monitoring the ability of mothers to care for toddlers with pneumonia after being hospitalized. This community service was carried out for four months from June to September 2022. The health education activity was carried out for one day in the children's care room at Pasar Rebo Hospital. The target of this community service activity is mothers who have toddlers with pneumonia who are treated at Pasar Rebo Hospital as many as 10 people. Results: The results of the evaluation show that the average value of knowledge is post-test> pre-test and the average value of attitude is post-test> pre-test. Conclusion: There is an increase in the knowledge and attitude of mothers in caring for children under five with pneumonia after hospitalization. Health education needs to be carried out on an ongoing basis for mothers who have children under five with post-hospitalization pneumonia so they can carry out optimal care and prevent repeated treatments.

Abstract

Uremic pruritus is a common problem in renal failure patients undergoing hemodialysis (HD program). The purpose of this study was to see the effectiveness of the combination of stroking massage and sunflower oil on the degree of pruritus. This study is a Randomized Controlled Trials (RCT) study with 40 respondents divided into the intervention group (KI) and the control group (KK), where the intervention group was given a combination of cutaneous stimulation and sunflower oil (5 ml of sunflower oil and 7 minutes of stroking massage) given 2 times a week for 3 (three) weeks, while the control group was only given a stroke massage. Demographic data such as age, gender, duration of hemodialysis and access to hemodialysis were assessed using a questionnaire, while the degree of puritus was assessed using the Dynamic Pruritus Score (DPS). The results showed that there was a significant difference between the control group and the intervention group after giving a combination of stroking massage and sunflower oil with p value =0.000. So that giving a combination of stroking massage and sunflower oil can be an alternative to providing nursing care for kidney failure patients undergoing hemodialysis program with uremic pruritus.

Abstract

Melakukan stimulasi yang memadai artinya merangsang otak anak sehingga perkembangan gerak, bicara dan bahasa, sosialisasi dan kemandirian berlangsung secara optimal sesuai dengan umur anak. Kepemilikan Buku KIA berperan penting bagi orang tua maupun pengasuh terutama sebagai pedoman dalam memberikan stimulasi pada anak. Tujuan dari penelitian ini adalah untuk mengetahui hubungan kepemilikan buku KIA dengan perilaku orang tua dalam memberikan stimulasi dan tingkat perkembangan anak. Jenis penelitian ini adalah kuantitatif dengan desain cross sectional. Penelitian ini dilaksanakan pada bulan Januari sampai dengan Juni tahun 2021 di wilayah kerja Puskesmas Bengkuring kecamatan Samarinda Utara kota Samarinda. Sampel yang digunakan sebanyak 68 ibu yang memiliki anak usia 0-18 bulan dengan teknik purposive sampling. Pengumpulan data dilakukan dengan menggunakan kuesioner dan lembar observasi yang digunakan untuk mengetahui perilaku ibu dalam melakukan stimulasi. Kuesioner Pra Skrining Perkembangan (KPSP) digunakan untuk mengetahui tingkat perkembangan anak (0-18 bulan). Kemudian data dianalisis secara univariat dan bivariat menggunakan uji chi square. Hasil penelitian menunjukkan p value (< 0,05) yang artinya secara statistik ada hubungan kepemilikan buku KIA dengan perilaku orang tua dalam memberikan stimulasi dan tingkat perkembangan anak. Sehingga diharapkan ke depannya agar tenaga kesehatan mampu meningkatkan perannya dalam memotivasi orang tua dalam memberikan stimulasi pada anak sesuai dengan buku KIA sebagai pedoman.

Abstract

Background: The challenge of fatigue significantly affects children undergoing cancer treatment, impacting their overall well-being and quality of life. Unlike their healthy counterparts, pediatric cancer patients grapple with the demanding impacts of chemotherapy, leading to anemia and nutritional deficiencies that exacerbate fatigue. Beyond fatigue, chemotherapy alters appetite, nutrient utilization, and mental function. Fatigue's multidimensional impact, marked by reduced energy and increased need for rest, can detrimentally affect the quality of life for pediatric patients. Understanding these challenges is essential for devising tailored strategies that comprehensively address the unique needs of children undergoing cancer treatment, enhancing their overall well-being and quality of life.Objective: This research aims were to determine the relationship between anemia and nutritional status with the occurrence of fatigue in children with cancer undergoing chemotherapy at Dharmais Cancer Hospital in Jakarta.Methods: An analytical cross-sectional study conducted in August 2022 included a sample of 24 children selected through accidental sampling. Demographic and disease characteristics, such as age, gender, cancer type, cancer stage, and treatment duration, were collected through a questionnaire. Anemia was determined by hemoglobin levels (12 mg/dl), and nutritional status was assessed using Body Mass Index. Fatigue was measured with tailored instruments for different age groups: the Parent Fatigue Scale (PFS) for children under 7 years and the Child Fatigue Scale (CFS) for those aged 7 years and older. Statistical analyses included univariate assessment, bivariate analysis with the chi-square test, and logistic regression.Results: Demographic analysis revealed a predominantly male sample (70.8%) with a significant adolescent presence (41.7%) and a prevalence of leukemia (37.5%). Most participants were in early cancer stages (58.3%), undergoing treatment for over a year (62.5%). Anemia was common (54.2%), and 58.3% were underweight. High fatigue prevalence was observed (62.5%). Chi-square analysis found no significant gender or cancer stage differences in fatigue. Age and treatment duration showed no significant associations with fatigue. Logistic regression revealed a significant association between anemia and fatigue in the first model (p = 0.022, Exp. B = 0.104). The second model, incorporating nutritional status, showed a non-significant relationship with anemia but a notable association with normal nutritional status and fatigue (p = 0.022, Exp. 0.059). This underscores the crucial role of nutritional interventions in mitigating fatigue.Conclusion: This study highlights a significant relationship between anemia and nutritional status with fatigue in children undergoing chemotherapy. These findings underscore the importance of considering anemia and nutritional interventions when providing care to pediatric cancer patients undergoing chemotherapy. Nurses play a vital role in addressing these concerns and enhancing the overall well-being of children with cancer.
