Abstract

Abstract

Pandemics have historically disproportionately impacted the poor and disadvantaged. Poverty, the external environment, and race or ethnicity can all significantly impact COVID-19 consequences. Those barriers, called social determinants of health (SDOH), are significant for many people's health. This study aimed to investigate the social determinants of COVID-19 risk among children in Jakarta, Indonesia. We recruited parents whose children were between 6 and 12 and were admitted to a general public hospital in Jakarta, Indonesia. Logistic regression was used to examine the relationship between socioeconomic status and COVID-19 risk. This analysis includes 200 parents of children aged 6 to 12 years old (60%) retrospectively recruited. About half of the parents had undertaken primary education level. No significant correlation was found between parent education level, occupation, and monthly income with COVID-19 risk among children. The number of house occupants more than two was positively associated with a higher risk of COVID-19 in children. In conclusion, poor housing conditions increase the probability of COVID-19 infection in Indonesian children. This implies that parental reinforcement of anti-household transmission strategies is necessary.

Abstract

The COVID-19 pandemic had an impact on nursing students’ academic and clinical performance. Nursing institutions have provided remote learning to continue the learning process. However, students have met several obstacles that caused a decrease in the quality of learning. Remote learning for education requires the support of information and communication technology. However, studies related to how the COVID-19 pandemic affected nursing education in Indonesia are still rarely done. Accordingly, this study was conducted to evaluate the implementation of remote learning for nursing education during the COVID-19 pandemic. A cross-sectional study was conducted with 331 members of the Association of Indonesian Nurse Education Center (AINEC). The instrument used was the Guidelines of Filling Out Remote Learning. The results showed that 51% of the study programs implemented remote learning, while 69% of the regulations for remote learning were created by rectors. The most common teaching materials were visual, text, and audio-visual content. These materials were distributed through WhatsApp Group, Learning Management System (LMS), and video conference. To support the process of remote learning, nursing institutions need to improve information technology staff and cooperate agreement with other institutions. Overall, the remote learning process needs to be further improved to increase the students’ learning quality.

Abstract

Background: The challenge of fatigue significantly affects children undergoing cancer treatment, impacting their overall well-being and quality of life. Unlike their healthy counterparts, pediatric cancer patients grapple with the demanding impacts of chemotherapy, leading to anemia and nutritional deficiencies that exacerbate fatigue. Beyond fatigue, chemotherapy alters appetite, nutrient utilization, and mental function. Fatigue's multidimensional impact, marked by reduced energy and increased need for rest, can detrimentally affect the quality of life for pediatric patients. Understanding these challenges is essential for devising tailored strategies that comprehensively address the unique needs of children undergoing cancer treatment, enhancing their overall well-being and quality of life. Objective: This research aims were to determine the relationship between anemia and nutritional status with the occurrence of fatigue in children with cancer undergoing chemotherapy at Dharmais Cancer Hospital in Jakarta. Methods: An analytical cross-sectional study conducted in August 2022 included a sample of 24 children selected through accidental sampling. Demographic and disease characteristics, such as age, gender, cancer type, cancer stage, and treatment duration, were collected through a questionnaire. Anemia was determined by hemoglobin levels (12 mg/dl), and nutritional status was assessed using Body Mass Index. Fatigue was measured with tailored instruments for different age groups: the Parent Fatigue Scale (PFS) for children under 7 years and the Child Fatigue Scale (CFS) for those aged 7 years and older. Statistical analyses included univariate assessment, bivariate analysis with the chi-square test, and logistic regression. Results: Demographic analysis revealed a predominantly male sample (70.8%) with a significant adolescent presence (41.7%) and a prevalence of leukemia (37.5%). Most participants were in early cancer stages (58.3%), undergoing treatment for over a year (62.5%). Anemia was common (54.2%), and 58.3% were underweight. High fatigue prevalence was observed (62.5%). Chi-square analysis found no significant gender or cancer stage differences in fatigue. Age and treatment duration showed no significant associations with fatigue. Logistic regression revealed a significant association between anemia and fatigue in the first model (p = 0.022, Exp. B = 0.104). The second model, incorporating nutritional status, showed a non-significant relationship with anemia but a notable association with normal nutritional status and fatigue (p = 0.022, Exp. 0.059). This underscores the crucial role of nutritional interventions in mitigating fatigue. Conclusion: This study highlights a significant relationship between anemia and nutritional status with fatigue in children undergoing chemotherapy. These findings underscore the importance of considering anemia and nutritional interventions when providing care to pediatric cancer patients undergoing chemotherapy. Nurses play a vital role in addressing these concerns and enhancing the overall well-being of children with cancer.

Abstract

This research aims to analyze the application of Kolcaba Model Theory in meeting the needs of comfort in neonates with duodenal atresia. This research is a qualitative research with a case study approach on two neonates treated in the neonatal intensive care unit (NICU). Data were collected through direct observation, in-depth interviews with nurses and parents, and analysis of medical documents of two neonates as samples. The results showed that neonates' physical comfort was partially achieved with stable body temperature, although there were still problems with vital signs and skin condition, which required improvement in pain management and skin care. Environmental comfort was successfully maintained by keeping the baby's body temperature stable, although there were challenges in the room temperature which tended to be cold. These findings emphasize that nursing interventions based on Kolcaba Model Theory need to focus on improving pain management and skin care for physical comfort. The implementation of Kolcaba Model Theory in the care of neonates with duodenal atresia can improve the quality of nursing care in the NICU.

Abstract

Tuberculosis is still the main cause of death due to infectious diseases throughout the world. Adherence to antiretroviral (ARV) treatment for HIV infection is a major challenge because it involves many factors. This systematic review follows the PRISMA guidelines checklist and includes articles from various databases, such as Pubmed, Cochrane and Proquest. Quality assessment of each article is carried out using the Critical Appraisal Skills Program (CASP). The search was carried out using the keywords "ARV Therapy, HIV/AIDS Children, Monitoring System, ARV Compliance". Following the inclusion and exclusion criteria 10 relevant articles were identified, excluding duplicates, limited to full papers, open access, and written in English. These findings indicate that electronic-based monitoring shows an increase in medication adherence. Other studies also note that electronic monitoring devices can create a temporary increase in compliance but then decline after some time. These results indicate that electronic-based monitoring can have a significant clinical impact in improving medication compliance. Electronic monitoring is important, especially in children and adolescents who face unique challenges regarding compliance.

Abstract

Objective: This study aimed to ascertain the prevalence of frailty and associated psychological factors among hemodialysis patients. Methods: A cross-sectional study was conducted employing a convenience sample of CKD patients undergoing hemodialysis at a Jakarta hospital. A total of 151 participants were included in this study. Instruments included demographic and clinical characteristics. The Tilburg Frailty Indicator (TFI) assessed frailty, while the Self-Rating Depression Scale (SDS) and Self-Rating Anxiety Scale (SAS) evaluated psychological status. Data analysis encompassed univariate descriptive statistics, t-test, and Pearson correlation to identify the relationship among variables. A multivariate regression analysis studied frailty index associations with demographic, clinical, and psychological factors. Results: Results: This study indicates that the prevalence of frailty, assessed by the Tilburg Frailty Index (TFI), is 53.6%, primarily evident in the physical aspect. Positive correlations are found between frailty and age, unemployment, higher CCI, and a history of diabetes. Anxiety is correlated with female gender and a history of diabetes, while depression correlates with age, female gender, higher CCI, and a history of diabetes. Conclusion: The study highlights the complexity of frailty among hemodialysis patients. Significant correlations emerged, associating frailty with factors such as age, unemployment, comorbidity, and a history of diabetes. Frailty was linked to psychological aspects, specifically anxiety and depression. Tailored interventions are recommended, focusing on early identification, psychological support, and a holistic care approach. Educational initiatives for healthcare providers are crucial to enhance awareness and improve the quality of care for this vulnerable population, leading to better health outcomes.
