Abstract

Abstrak: Penurunan fungsi tubuh lansia yang disebabkan oleh berkurangnya jumlah sel secara anatomi diantraranya fungsi kognitif, jantung, pembuluh darah, ginjal dan persyarafan. Hal ini mengakibatkan beberapa masalah kesehatan yang sering kali kemunculannya tidak dapat diprediksi dan baru diketahui setelah berkomplikasi. Mitra yang memiliki riwayat dan keluhan gejala penyakit hipertensi, diabetes mellitus, paska stroke, dan kelemahan fisik perlu mengelola proses perawatan dan pemeliharaan kesehatannya. Kegiatan pengabdian ini bertujuan untuk mendeteksi secara dini dan monitoring status kesehatan melalui pemeriksaan kesehatan pada mitra….. yang merupakan salah satu lembaga keagamaan di wilayah Palembang. Hasil pemeriksaan menunjukkan sebagian besar memiliki tekanan darah pada kategori pre-hipertensi dan hipertensi derajat 2, namun masih memiliki tekanan arteri rata-rata dan tekanan nadi yang normal, serta gula darah sewaktu yang normal dan meningkat (hiperglikemia).Abstract: Decreased body function in the elderly can then result in several health problems that often appear unpredictable and only known after complications. Partners who have a history and complaints of symptoms of hypertension, diabetes mellitus, post-stroke, and physical weakness need to manage the process of care and health maintenance. Therefore, this service activity aims to detect early and monitor health status through health checks at partners who are one of the religious institutions in the Palembang area and education based on the results of the examination. The results showed that most had blood pressure in the category of pre-hypertension and grade 2 hypertension, but still had normal average arterial pressure and pulse pressure, as well as normal and elevated blood sugar (hyperglycemia).

Abstract

Chronic Kidney Disease (CKD) is a progressive condition characterized by a gradual decline in kidney function, often requiring hemodialysis therapy. One of the most common complications experienced during hemodialysis is muscle cramping, particularly in the lower extremities, which can reduce patient comfort and interfere with the effectiveness of treatment. Intradialytic Stretching Exercise is a non-pharmacological intervention that has been shown to help reduce muscle cramps in hemodialysis patients. To analyze the implementation of Intradialytic Stretching Exercise in reducing muscle cramps among patients with CKD undergoing hemodialysis. This case study used an Evidence-Based Practice (EBP) approach involving a CKD patient on hemodialysis who experienced muscle cramps. The intervention consisted of Intradialytic Stretching Exercise performed for 20 minutes, twice a week during hemodialysis sessions. Muscle cramp levels were assessed using the Cramp Questionnaire Chart before and after the intervention. The implementation of Intradialytic Stretching Exercise resulted in a reduction in muscle cramp severity. Previous studies supporting this EBP intervention demonstrated significant improvements in the frequency, duration, and intensity of muscle cramps following regular intradialytic stretching exercises. Intradialytic Stretching Exercise is an effective non-pharmacological intervention for reducing muscle cramps in patients with CKD undergoing hemodialysis. Hemodialysis nurses are encouraged to incorporate Intradialytic Stretching Exercise into routine nursing care and provide patient education to promote regular exercise participation, thereby preventing and reducing muscle cramps during hemodialysis.

Abstract

Introduction: Nursing care documentation serves as the cornerstone of effective interprofessional communication, continuity of care, and patient safety, continuity of care, and defensible legal practice. Despite its important role, the quality of nursing care documentation remains a constant challenge in various clinical settings. While traditional methods for improving nursing documentation can result in some initial improvements, they often fail to achieve sustained and meaningful change.  Given these persistent problems, coaching became one of the innovative strategies to improve nursing documentation practices and foster a culture of continuous improvement in healthcare settings. Purpose: This study aims to explore the effectiveness of coaching as a strategy to improve the quality of nursing care documentation. Method: 16 nursing documentation measures used the Q-DIO before-after coaching implementation. Collected data in this study analyze using Wilcoxon test to find significance of the difference before-after measurements. Result: The study found that there was improvement in each dimension of nursing documentation quality as measured by the Q-DIO instrument, and that the dimensions significantly differed in overall quality (p0.001). Discussion: Therefore, further implementation of the coaching method with longer and more intermittent sequences gives certain promise as an alternative strategy for enhancing the quality of nursing care documentation in the future.
