Abstract

Abstract

Background: Patients with chronic kidney disease (CKD) who are undergoing hemodialysis therapy are important to limit fluids to prevent fluid buildup that can trigger complications. Increased fluid volume, both intravascular and interstitial, can cause hypervolemia. Telenursing is the use of technological resources and communication systems to promote nursing development. This study is to determine the application of telenursing and health education to improve adherence to fluid restriction in CKD patients undergoing hemodialysis. Methods: This research is a case study on three patients with hypervolemia. The interventions provided were health education twice and telenursing via short messages for six days. The instrument used in this study used an adaptation of the ESRD-AQ questionnaire to measure adherence to fluid restriction and was supported by clinical data on blood pressure, IDWG, UF goal, and degree of edema. Results: The results of this study are that the level of adherence before the intervention was given was 100% non-adherent. After being given the intervention, 66.7% (Mrs. N. and Mrs. S.) were compliant, and 33.3% of Mr. T. did not comply with fluid restrictions. Conclusion: Health education and telenursing in the form of short message reminders can improve compliance with fluid restriction in CKD patients with hypervolemia. Recommendations from research can determine effective health education methods and media as well as telenursing media for patients and families.

Abstract

Background: Patients with chronic kidney disease (CKD) undergoing hemodialysis must adhere to fluid intake restrictions to prevent fluid accumulation, which can lead to complications such as edema, hypertension, and cardiovascular events. Excess intravascular and interstitial fluid volume contributes to fluid overload, a major risk factor for morbidity and mortality in this population. Objective: To describe adherence to fluid restriction among CKD patients receiving hemodialysis and identify common barriers to adherence. Methods: This descriptive survey study was conducted from January to June 2021 at two hospitals. A total of 129 CKD patients undergoing hemodialysis were recruited through purposive sampling. Inclusion criteria were age ?18 years, receiving routine hemodialysis for at least 3 months, and ability to communicate verbally. Fluid restriction adherence was assessed using the End-Stage Renal Disease Adherence Questionnaire (ESRD-AQ), previously validated and culturally adapted. Descriptive statistics summarized adherence patterns and patient characteristics. Results: Most participants were male, in late adulthood, had completed college, were unemployed, had been on hemodialysis for approximately 5 years, and underwent treatment twice weekly. Overall, 66% adhered to fluid restrictions, with 36.4% reporting following restrictions all the time. However, 62.8% indicated barriers to adherence, including lack of interest, inability to control fluid intake, and not understanding how to follow the restriction. Conclusion: While most CKD patients complied with fluid restrictions, a substantial proportion faced challenges that could undermine long-term outcomes. Continuous education, motivational support, and individualized counseling are essential to improve adherence. Future research should explore the association between adherence, fluid overload, and clinical outcomes.

Abstract

Abstract

(1) Background: Health problems that are not caused by infection, especially heart and blood vessel problems, remain the main reason people die and experience medical emergencies in our environment. Because people lack understanding and are not prepared to face critical situations related to the heart, this can result in first treatment being delayed.. (2) Research Objective: This activity aims to assess the effectiveness of health education in increasing knowledge of cardiac emergency preparedness in the adult community in the Suka Indah Community Health Center working area.; (3) Method: Community service activities use a pre-test and post-test design without a control group. Health education is provided through interactive lectures, discussions and video media. Evaluation was carried out using a knowledge questionnaire for 40 respondents before and after education. (4) Results: The pre-test results showed that the majority of respondents had a level of knowledge in the poor category (75%). After being given education, there was an increase in knowledge, shown by the increase in respondents in the good category to 82.5% and there were no more respondents with poor knowledge. (5) Conclusion: Health education is effective in increasing public knowledge regarding cardiac emergency preparedness and can be a promotive-preventive strategy in primary health services.
