Abstract

Background: Clinical learning has a very important role in determining the quality of nursing students' learning achievements, because clinical knowledge and skills are the main foundation in clinical education. Nursing students are expected to have competency in the knowledge and skills to provide high quality and safe nursing care to patients, however there are still many nursing students who feel they are not ready to become nurses in the future because they feel they are not yet competent in carrying out nursing care to patients. The aim of the research is to see the correlation between self-efficacy and the clinical competence of Nurse Professional Education students. Methods: This research used correlation research and the sample in this research were clinical practice students of the Nursing Professional Education who had undergone 2 clinical learning stages in semester 1 which were taken using a total sampling technique. Data analysis using SmartPLS. Results: From the research results, it was found that self-efficacy had a positive effect on clinical competence (P Value 0.000, T Statistics 14.273, sample mean 0.686). Self-efficacy influences clinical competence by 46.3% (R Square 0.463), the remainder is influenced by other factors not studied. Conclusion: These findings emphasize the importance of self-efficacy in forming and improving students' clinical competence. By having high self-confidence in their abilities, students are more likely to be able to apply their knowledge and skills effectively in clinical practice.

Abstract

Background: The COVID-19 pandemic is still being felt by the world and Indonesia. The impact of the pandemic has also affected the education sector. Currently learning in Indonesia is conducted face-to-face, as well as the clinical practice of Nurse Professional Education. With changes in the situation raises its own constraints and challenges. The aim of the research is to explore the experiences of students practicing the clinical practice of Nurse Professional Education during the COVID-19 pandemic. Methods: This type of research is qualitative with a phenomenological approach. Total sample of 10 participants using purposive sampling technique. Data collection was carried out by in-depth interviews and data were analyzed using content analysis. Results: The results of the study obtained five themes, namely: (1) Feelings of fear and anxiety carrying out clinical practice, (2) Efforts to reduce the risk of being infected with COVID-19 during practice, (3) Violations in the use of PPE during practice, (4) Obstacles to guidance during a pandemic, and (5) Achievement of competencies that have not been optimal related to changes in methods and areas of clinical practice. Conclusion: The results of this study provide an overview of the need for educational institutions and practice areas to develop guidance methods and methods for carrying out clinical practice for Nurse Professional Education in a pandemic situation to increase student competency achievement.Keywords: Experience, clinical practice, nurse professional education, COVID-19

Abstract

Outpatient waiting time is one of the national quality indicators. Most patients visiting outpatient clinics in Indonesian hospitals complain about the long waiting time before receiving healthcare services. Over the decades, the persistent issue of long waiting times and unresolved patient dissatisfaction has led hospitals worldwide to adopt Lean Healthcare management systems. These systems focus on improving the efficiency and effectiveness of service operations by eliminating waste. Siti Rahmah Hospital is a private type-C hospital facing outpatient waiting time issues exceeding 60 minutes. This community service aims to enhance nurses' knowledge and reduce service waiting times. This community service was conducted using a quantitative approach with an experimental design, and the primary data collection technique was through questionnaires. The implementation consisted of four main stages: preparation, socialization of lean healthcare methods, identification of waiting times, and mapping types of time waste in services. Nurses' knowledge about lean healthcare management increased significantly by 56.70% after the training, as shown by a pretest score of 36.6% and a post-test score of 93.3%. The knowledge components that improved substantially included understanding the outpatient waiting time standard of less than 60 minutes, identifying time-wasting activities in services, and using electronic medical records as a lean healthcare strategy to reduce service waiting times. The lean healthcare method is effective in reducing patient waiting times in outpatient care.

Abstract

Commitment of nurses to hospitals remains a challenge in human resource management. One factor influencing organizational commitment is ethical climate, an individual's perception of values, norms, and moral standards in the work environment. A positive ethical climate is believed to increase nurses' loyalty and emotional attachment to the institution. This study aims to analyze the effect of ethical climate on organizational commitment among nurses in government hospitals in Padang City, Indonesia. This is a quantitative explanatory study with a sample of 69 nurses who met the inclusion criteria, taken by proportional random sampling. Data were collected using a questionnaire and analyzed with SmartPLS. The results showed a significant positive effect of the ethical climate of benevolence and principles (caring, independent, law and code, rules) on organizational commitment (T-statistic 7.715; P value 0.000; sample mean 0.745, R square 0.440), and there was no effect of instrumental climate on organizational commitment. A positive ethical climate that reflects caring, compliance with rules, and the principle that every action and decision is based on personal or professional standards can strengthen nurses' commitment to the organization. Strengthening ethical culture is important in building sustainable and quality-oriented health services.

Abstract

Healthcare facilities must adhere to patient safety principles to reduce the risk of harm. Prof. Dr. M.A. Hanafiah SM Batusangkar Hospital is among the healthcare providers in West Sumatra, necessitating endeavors to enhance and sustain service quality, especially regarding patient safety. The effectiveness of the patient safety culture depends on the 4 S supervision elements, including Structure, Skill, Support, and Sustainability. The 4S supervision model guides and aids nurses in advancing their skills related to patient safety. Survey findings at the hospital revealed that the promotion of 4S supervision was suboptimal, manifesting as nurses' limited grasp of the concept. On July 28-29, 2023, a structured socialization, education, and mentorship program was conducted for nursing executives and managers. The objective was to empower nurses to incorporate multilayered supervision into patient care and oversee the implementation of patient safety in inpatient, outpatient, and emergency room settings, aiming to enhance the patient safety culture.

Abstract

Penyakit jantung koroner (PJK) merupakan penyakit kronis dengan angka morbiditas dan mortalitas tertinggi di dunia bahkan di Indonesia. Untuk mengurangi gejala kekambuhan, meningkatkan atau mengelola kesehatan dalam kehidupan sehari-hari pasien memerlukan self care yang optimal sehingga kualitas hidup dapat meningkat. Tujuan penelitian ini adalah untuk mengidentifikasi hubungan antara self care dengan kualitas hidup pada pasien dengan PJK masa pandemi COVID-19. Jenis penelitian ini adalah korelasi dengan pendekatan cross sectional study dengan jumlah sampel sebanyak 86 responden. Sampel dipilih menggunakan teknik nonprobability sampling dengan pendekatan accidental sampling dan dianalisis menggunakan uji korelasi pearson product moment. Instrumen yang digunakan Self-Care of Coronary Heart Disease Inventory (SC-CHID) dan World Health Organization Quality of Life (WHOQOL-BREF). Hasil penelitian menunjukkan nilai rata-rata self care 55,24 dan kualitas hidup 53,94 dengan interpretasi self care dan kualitas hidup berada pada tingkat sedang. Hasil analisis statistik ditemukan terdapat hubungan self care dengan kualitas hidup (p =0,001) dengan korelasi kuat (r = 0,719) dan arah positif.

Abstract

Pendahuluan : Bidang kesehatan tentunya selalu berupaya untuk meningkatkan mutu pelayanan fasilitas kesehatan. Indikator mutu pelayanan keperawatan dapat ditingkatkan dengan cara salah satunya ialah penerapan standar pelayanan keperawatan professional seperti penerapan sistem Pemberian Pelayanan Keperawatan Professional (SP2KP). Penerapan sistem SP2KP tersebut melibatkan kerjasama yang professional antara kepala ruangan, perawat primer (PP) dan perawat asosiet (PA) serta CCM (Clinical Care Management). Motivasi kerja merupakan sebuah kondisi yang berpengaruh untuk mengarahkan, membangkitkan, dan memelihara perilaku yang erat hubungannya dengan lingkungan kerja. Perawat tentunya akan selalu mengembangkan karir yang bertujuan meningkatkan moral kerja dan mengurangi kebutuhan karier. Terdapat 4 indikator sebagai dasar pengaturan pengorganisasian yaitu pembagian sesuai tugas, sesuai pekerjaan, penentuan relasi dan penentuan mekanisme untuk mengintegrasikan aktivitas antar bagian dalam organisasi atau koordinasi. Secara optimal, pengorganisasian dapat menentukan mutu pelayanan. Pembagian tugas harus diperhatikan adanya keseimbangan wewenang dan tanggung jawab staff. Serta pembagian tugas dan pendelegasian wewenang dapat diketahui hubungan organisator antara suatu staf satu dengan yang lainnya dalam organisasi.Tujuan Penelitian: mengidentifikasi dan menganalisis faktor-faktor yang berkontribusi terhadap peningkatan kualitas pelayanan di bidang keperawatan, dengan fokus pada penerapan Sistem Pemberian Pelayanan Keperawatan Professional (SP2KP). Bahan dan Metode: Desain penelitian ini adalah kuantitatif dengan desain Cross-Sectional Study, teknik pengambilan sampel menggunakan proporsional random sampling, sampelnya adalah 67 perawat praktik yang bekerja di ruang rawat inap dengan menggunakan Metode Professional Nursing Service Delivery System. Penelitian dilakukan di salah satu rumah sakit Swasta di Padang. Data dianalisis menggunakan Smart PLS Structural Equation Modeling (SEM) dengan outer model melalui CFA 3rd Order. Analisis Faktor Konfirmatori terhadap variabel faktor Internal untuk meningkatkan kualitas pelayanan melalui penerapan metode Professional Nursing Service Delivery System. Hasil: Berdasarkan hasil analisis, Dimensi yang paling dominan berhubungan dengan faktor peningkatan kualitas pelayanan adalah dimensi pengakuan dengan nilai loading faktor sebesar 0,849. Kesimpulan: Dimensi pengakuan memiliki peran yang sangat dominan dalam mempengaruhi peningkatan kualitas pelayanan di bidang keperawatan. implementasi kebijakan atau strategi yang memperkuat aspek pengakuan dalam lingkungan kerja perawatan dapat dianggap sebagai langkah yang krusial dalam meningkatkan mutu pelayanan.
