Abstract

Background: Aplastic anaemia is a disease that occurs when the body fails to produce enough blood cells. Oral lesions, such as canker sores, are often used as an early marker of this disease condition. In the COVID-19 pandemic era, a teledentistry method is expected to assist in the management of patients with various diseases including aplastic anaemia. Purpose: This case report discusses the management of patients with aphthous-like ulcers related to aplastic anaemia in a 34-year-old woman, based on history, clinical examination, and blood tests through teledentistry. Case: The patient complained of a bleeding wound on the inner left cheek and corner of the left lip that appeared one month ago. There was an ulcer lesion that was bleeding easily in the buccal mucosa sinistra and the angular sinistra, as well as macules lesion on her hand and foot. The complete blood count test exhibits a low platelet level. Case management: The diagnosis was an aphthous-like ulcer related to aplastic anaemia. Patient was treated with topical antiseptic and anti-inflammatory mouthwashes for the oral lesions, as well as collaboration with the medical team. The lesions improved after the patient received a blood transfusion and drug therapy from the internist while she was hospitalized, but the lesions reappeared in other locations in the oral cavity when the platelet level dropped. Conclusion: Management of oral manifestations in patients with aphthous-like ulcers related to aplastic anaemia in the COVID-19 pandemic era can be done through teledentistry. However, this requires sensitivity from the dentist, patient compliance in following instructions, and the involvement of a multidisciplinary approach, such as collaboration with internists to achieve recovery. Delay in detecting and treating aplastic anaemia will lead to death, as in this case.

Abstract

Stomatitis aftosa adalah suatu kondisi inflamasi dengan etiologi yang belum diketahui, ditandai dengan ulserasi sangat nyeri di mukosa rongga mulut selama 7 hingga 14 hari. Berbagai faktor berpengaruh dalam pembentukannya, salah satunya adalah trauma lokal. Perawatan diberikan untuk meredakan nyeri, mengeliminasi sumber trauma, dan penyembuhan ulserasi. Tujuan laporan kasus ini adalah untuk melaporkan stomatitis aftosa yang diperparah oleh iritasi kimiawi obat tradisional. Seorang laki-laki berusia 64 tahun mengeluh sariawan pada gusi kiri bawah terasa nyeri sejak kurang lebih 1 minggu yang lalu. Pasien mengoleskan suatu obat cair tradisional untuk meredakan rasa nyeri tapi kemudian rasa nyeri timbul kembali dan belum mereda. Tanda vital normal kecuali skor VAS (visual analog scale) nyeri mulut 6/10. Pemeriksaan ekstra oral normal, pemeriksaan intra oral didapatkan ulkus soliter berukuran 1x1,5 cm pada lipatan bukal regio 34, 35 dengan peninggian tanpa indurasi dan nyeri. Tata laksana meliputi anamnesis untuk mengetahui faktor yang memperparah, pemeriksaan klinis, debridement lesi untuk menghilangkan jaringan nekrotik dan aplikasi obat topikal gel mengandung klorin dioksida. Pasien diinstruksikan untuk berhenti mengoleskan obat tradisionalnya dan menggunakan obat yang diresepkan. Lesi sembuh dalam waktu 13 hari. Stomatitis aftosa dapat diperparah oleh iritasi kimiawi. Anamnesis, pemeriksaan klinis, debridement lesi dan pemberian obat yang tepat dapat membantu tata laksana dan penyembuhan yang efektif.

Abstract

Background: Dysplasia is malignant transformation of stratified squamous epithelium characterized by cellular atypia and loss of stratification and maturation. It can caused by cigarette smoke. Measuring the activity of cell proliferation as the beginning of dysplasia is very useful to know the development of malignancy. One of proliferation marker is Ki67 that expressed in all cell cycle phase except G0.Methods: 30 male wistar rats divided into: control grup (n=10), exposed to cigarette smoke for 4 weeks (n=10), 8 weeks (n=10) as much as 20 cigarettes/group/day/exposure used smoking pump. Specimen were stained HE to see dysplasia and Ki67 expression seen immunohistochemically. Statistical analyzed by Spearman correlation.Result: There was an increase in mean of Ki67 expression and significant differences between groups (p=0,000) and also significant difference of degree of dysplasia between groups (p=0,000). There was a strong correlation between Ki67 score to degree of dysplasia (p=0.001).Conclusion: Increased of Ki67 expression proportional to the Increase in degrees of dysplasia

Abstract

Pendahuluan: Terjadinya lesi oral pada Orang Dengan HIV (ODHIV) telah banyak dihubungkan dengan peningkatan viral load, penurunan jumlah CD4+, dan konsumsi Highly Active Antiretroviral Therapy (HAART) yang walaupun penggunaannya dapat meningkatkan kondisi umum ODHIV, tetapi penggunaan dalam jangka panjang juga akan memicu berbagai perubahan secara sistemik dan lokal. Tujuan Penelitian: Penelitian ini bertujuan untuk mendata prevalensi terjadinya lesi oral pada ODHIV yang menggunakan HAART. Metode: Penelitian ini menggunakan jenis penelitian observasi cross sectional dengan total sampling sesuai dengan STOBE statement. Data dicatat pada formulir kuesioner dan formulir pemeriksaan klinis. Hasil: Sebanyak total 40 orang peserta dilaporkan dalam penelitian ini. Jenis HAART yang paling banyak dilaporkan penggunaannya dalam jangka panjang tanpa perubahan adalah TLD (27,5%), sementara jenis terapi dengan perubahan regimen adalah terapi awal dengan Duviral Neviral menjadi terapi dengan TLD (12,5%). Durasi terapi HAART selama lebih dari 3 tahun tercatat pada 29 peserta (72,5%) dan durasi terapi kurang dari 3 tahun tercatat pada 11 peserta (27,5%). Lesi oral yang paling banyak diobservasi adalah diffuse oral hyperpigmentation (47,62%), diikuti oleh coated tongue (23,81%), traumatic ulcer (4,76%), linea alba (4,76%), torus palatinus (4,76%), dan lesi-lesi lainnya. Kesimpulan: Lesi oral yang paling banyak ditemukan adalah oral diffuse hyperpigmentation dan korelasinya dengan penggunaan HAART masih perlu ditelaah lebih lanjut.

Abstract

Pendahuluan: HIV/AIDS telah menjadi masalah kesehatan masyarakat global. Infeksi HIV dapat menyebabkan gangguan kekebalan tubuh dan bermanifestasi di beberapa tempat baik secara sistemik maupun di rongga mulut. Manifestasi oral yang paling umum terkait dengan HIV/AIDS adalah Kandidiasis Oral dan Oral Hairy Leukoplakia. Sebagai orang yang hidup dengan HIV/AIDS (ODHIV) mungkin menemukan manifestasi tersebut, pengetahuan mereka dapat menentukan sikap mereka terhadap temuan tersebut.Tujuan Penelitian: Mengamati sikap ODHIV di kota Surabaya terhadap dua manifestasi oral HIV yang paling umum ditemukan, yaitu Oral Hairy Leukoplakia dan Kandidiasis Oral, serta menentukan hubungan antara pengetahuan dan sikap mereka terhadap hal tersebut. Bahan dan Metode: Sebanyak 30 responden diberikan kuesioner untuk menilai pengetahuan dan sikap mereka mengenai Oral Hairy Leukoplakia dan Kandidiasis Oral. Pengetahuan didefinisikan dengan kriteria baik, cukup, dan rendah. Sikap didefinisikan sebagai positif, netral, dan negatif. Hasil: Tingkat pengetahuan mengenai Oral Hairy Leukoplakia dan Kandidiasis  Oral menunjukkan hasil tinggi (33%), sedang (54%) dan rendah (13%). Responden menunjukkan sikap positif (30%) dan sikap tidak setuju (27%), sedangkan netral (43%). Terdapat hubungan positif yang signifikan antara tingkat pengetahuan responden dengan sikap mereka (p = 0,46). Kesimpulan: Terdapat hubungan antara pengetahuan yang baik dengan sikap positif pada masyarakat.

Abstract

Objective: To find an adequate and efficient treatment for candidiasis patients with iron deficiency anemia. Methods: A 57year-old male patient came with several complains including the tip of the tongue feels thin but feels thick in the middle since ± a year ago and he feels its getting thicker and sore. Patients have smoking habit one pack of cigarettes per day since college (± 39 years ago). Patients doesnt have a diabetic disease but claim to suffer from cholesterol and high uric acid. The patient consumed turmeric and kenikir leafs to overcome his complains, but the symptoms did not decrease, ± a month ago, patient checked his complain to the dentist and given a medicine (Nystatin drop) but the complains had not subsided. Results This patient also has iron deficiency anemia. This condition can lead to candidiasis which causes disruption of oral ecology or changes in oral microbiology caused by iron deficiency. Conclusion: Candidiasis guillermondii in the oral cavity can be cured by anti-fungal treatment, KIE in patients, by instructing to improve oral hygiene and increase nutrient intake to speed up healing of candida infection

Abstract

Orofacial pain is a pain that involves the face and oral cavity wich characterized by sharp pain, such as electric shock. This paper reported 67 years old female patient complained pain in the right gingiva, right palatum and right side of the face since 6 months ago. To defined diagnosis we need accuracy in anamnesis, clinical examination and supportive examination. The treatment of orofacial pain is vitamins B1, B6, B12 and multidisciplinary collaboration. Patients use drugs that are given routinely, and regulary control to neurologist  and cardiologist then sorofacial pain begins to heal after 53 days.

Abstract

Objective: Thalasemia is an inherited blood diseases cause which abnormalities systemically and intraorally. This case report to discuss management of Aphthous-Like Ulcer in 24 year old man with Thalasemia β Intermedia and need blood transfusion every month. Ulcer occured 2 days before transfusion.Methods: Based on history, clinical examination, hematologi and ferritin tests.Results: The diagnosis was Aphthous Like ulcer induce by trauma. Patient was treated using anti-inflammatory mouthwash and recovery was achieved within 3 weeks follow-up. Conclusion: Management of oral manifestation in Thalasemia patient need multidiciplinary approach to obtain successful therapy.

Abstract

Objective: A 59-year-old woman with complaints of dry mouth, often feel thirsty, pain when swallowing, chapped lips, bleeding gums and bad breath. The patient had a history of breast cancer, underwent surgery and chemotherapy in 2010. The patient consumed 1 mg of Aromatase Inhibitors (AIs) tablets a day for 9 years.Methods: Patient underwent depression, anxiety and Stress Scale (DASS) questionnaire (Depression: normal; Anxiety: mild; and Stress: mild) and sialometry test (draining method) <0.1 ml/minute. Patient was diagnosed with xerostomia then instructed to use chlorine dioxide mouthwash 4 times a day. Management included anamnesis, clinical and laboratory examination: Complete Blood Count (CBC); Diabetic Profile; Lipid Profile; Renal Function Profile; and Tumour Marker test.Results: Patient’s sialometry test results in 1st, 2nd, 3rd and 4th visit are: 0.1ml/minute; 0.6ml/minute; 1.4ml/minute and 1.8ml/minute.Conclusion: Dentist has important role to evaluate and treat oral condition for optimal and comprehensive breast cancer treatment. Chlorine dioxide mouthwash is clinically proven to stimulate salivary gland in treating xerostomia.

Abstract

Objective: Recurrent aphthous Ulceration (RAU) is a common oral mucosal disease characterized by recurrence and painful ulcers in non-keratinized oral mucosa.. A teenage girl aged 18 years 7 months comes with complaints of a lot of ulcers in the oral cavity. The patient said that she had had ulcers since 3 years ago, had no history of allergies, and with a weight that dropped below normal according to BMI standards. From intra oral examination multiple ulcers appear on the labial mucosa, buccal mucosa, painful gingiva and atrophy on the tongue.Methods : Patients undergo complete blood tests with Hb results of 10.8 g / dL (N: 11.7–15.3 g / dL), Eosinophils 5% (N: 2-4%), Neutrophils 42%, lymphocytes 42%, monocytes 8%, hematocrit 33%, MCV 68 fL, MCH 21 pg,  MCHC 31 g / dL.Results : Based on history, clinical examination and laboratory examination, a diagnosis of recurrent aphthous ulceration related to iron deficiency anemia can be established with a differential diagnosis of minor type recurrent aphthous stomatitis. The results of the examination with a TIBC value of 437.1 H, Serum iron 48.61 L, and ferritin 62.71 L. Patients treated with non-steroidal anti-inflammatory mouthwash and multivitamins. Patients are instructed to increase intake of foods containing iron, fruits and vegetables and avoid fatigue or adequate rest.Conclusions : Therapy RAU with a food-based approach and a drug-based approach chlorine dioxide, Iron Polymaltose Complex, folic acid and vitamin c Key words : Recurrent aphthous ulceration, hemoglobin, iron deficiency anemia
