Abstract

The release of health information to third parties must use a power of attorney. The release of health information without using a power of attorney can have a negative impact. Power of attorney is needed so that there is no deviation in the provision of claims submitted by health service providers to the insurance company. The purpose of the study was to determine the suitability of information release activities from activity planning, strategies used in achieving goals, process of information release activities and success of health information release activities. This study uses the CIPP evaluation method, namely evaluating information release activities from the context, input, process and product aspects. Data was collected by observation, interviews and documentation. The results of the study indicate that there is a discrepancy between the release of information activities with the regulation of the minister of health. Inconsistency in the purpose of releasing information which is only for insurance, in its activities it was found that there was no power of attorney in requesting health information 44%, there was no clear name of the doctor 12.5% and there was no information on filling time of 36.5%. The decision obtained from the evaluation results is to modify the goals, objectives and strategies to achieve the goals. The final decision was to continue the information release activity by revising the SOP. The solution obtained from the discussion is the SOP for the release of health information that can be used for third parties in general. Revised SOP for releasing existing information with additional points regarding power of attorney and completeness of filling out.

Abstract

The BPIS claims process for inpatient admissions starts from registration until the patient file is entered into the lINA-CBGs application. Claim submission under the claims verification technique instructions is limited to the 10th of the following month. RSUD Genteng in filing a claim exceeds the 10th of the following month. The purpose f this research is to analyze the factors causing the delay in submitting BPIS claims for hospitalization to BPJS verifier in RSUD Genteng, determining the priority of the problem and the improvement efort to the problem. Type of this research is qualitative using a system approach based on 7M factor (Man, Method, Materials, Machine, Money, Market and Minute). Methods of data collection using observation, interview and documentation. Based on the result of NGT that has been done, the priority of problem causing delay is RM refund more than 2x24 hours, internet network is less stable, less discipline officer, no claim submission SOP, DPJD main diagnosis placement and BPJS verifator of different perception. Improvement eforts that can be done is to make SOP return of medical records of inpatient and SOP related to claims of inpatient BPJS, repair internet network, conduct coordination meeting among officer related to office discipline and make special form for correction from BPJS verifier. Abstract

Maternal Mortality Rate (MMR) turns out to be a problem that has not been solved in the world. Jember regency ranked second highest in east java in 2016. Puskemas Karang Duren in Jember regency has MMR in 2011, 2012, 2013. Promotive and preventive effort through the continuing approach has been implemented to make the MMR stable. For example by developing the maternal healthcare program with the use of mother card as their pregnancy medical record. This research intends to analyze motivation, opportunity, ability, and performance maternal healthcare program in Puskesmas. It uses analityc observational cross sectional design as the research method. There were 10 midwives as the research sample obtained by simple random sampling technique. It was analyzed by applying the data tabulation software. This result found out that most of the midwives were average in their motivation (60%), they were sufficient in the opportunity (60%), they were good in the ability (80%), and they were performance maternal healthcare program belongs to good category (80%). It shows that there were significant relationship between opportunity, ability, and the performance maternal healthcare program. Hence, the recommendation was formed by brainstorming activity involving the entire midwives and the head of the Community Health Center. It can be conducted by giving motivation (praising during meeting, warning, and guidance from the chiefs). It surely needs the uniformity of this medical recording format to make the supervision activity, monitoring, and evaluation goes easier later on. All of the sides are expected to be involved in maintaining the MMR.

Abstract

Pemanfaatan aplikasi eposyandu oleh Kader Posyandu Manggis 15 dan Manggis 18 di Desa Kemuning Lor, Jember masih belum optimal. Kader lebih memilih menggunakan cara konvensional karena masih kurang pemahaman teknologi informasi. Sehingga hal berdampak pada proses kinerja dan layanan kepada masyarakat yang kurang optimal karena muncul permasalahan seperti pengisian metadata pada buku form register tidak konsisten, pencarian data yang lama, perhitungan data stunting atau gizi buruk relatif lama dan pengelolaan laporan yang kurang optimal. Oleh karena itu, dalam kegiatan pengabdian masyarakat ini dilaksanakan pelatihan pemanfaatan aplikasi eposyandu dengan tahapan mulai dari persiapan, pengambilan informasi, analisis, desain, pengkodean dan proses training. Hasil dari kegiatan pengabdian ini berupa update pengetahuan kader dalam menggunakan aplikasi eposyandu. Serta pemahaman dalam pengelolaan laporan untuk deteksi dini stunting.

Abstract

Bed management is an inpatient bed processing activity by taking into account the 4 indicators of BOR, LOS, TOI and BTO. Based on observations, known that the number of inpatients from October - December 2021 continues to increase while the inpatient registration officers is only 1 officer. The bed management system is also still done manually. The purpose of this study was to design an information system interface design for Inpatient Bed management. The type of research used was the design of the bed management information system interface design. The method used was the prototype method with interview, observation and documentation data collection methods. The result was the design of the bed management information system interface that is tailored to the User's needs. The interface design can display room details to patients by showing facilities and room descriptions obtained by patients when treated according to the desired class, Users can also view and print the required reports including; inpatient visit reports, patient reports per payment type, and barber johnson indicator reports and this system can show graphs of total patient visits per room, graphs of total patient visits type of payment, and graphs of the number of patients per room.

Abstract

One of the hospital's quality is determined by a complete medical record document. The completeness of medical records at the Purwogondo General Hospital in Kebumen only reached 86% of the 100% standard determined by the minimum service. Completeness of medical records is needed in order to create continuity of treatment for patients and strong legal force. This study aims to analyze the completeness of medical records based on SNARS elements. The research subjects were 6 people, namely doctors, nurses, quality team representatives, medical recorders, internal supervisors, MIRM working group officers. Data collection methods are observation, interviews and brainstorming. The results of the research on the element of man are officer discipline and communication. The money element is the absence of a budget for the monitoring and evaluation team. The method element is the absence of an SOP for reviewing the completeness of medical records. The material element is the form design that the PPA has not yet understood. The machine element is the absence of an incomplete medical record control book, no monitoring and evaluation team for the completeness of medical records. Efforts to optimize the completeness of medical records are monitoring medical records on a regular basis and optimizing the monitoring and evaluation team.

Abstract

The return of medical record files is an activity to return the completed medicalrecord file to the medical record unit after the patient returns home. The delay inreturning the outpatient medical record file at the Besuki Hospital in the thirdquarter of 2021 reached 63.87%. This study aims to analyze the factors that causedelays in returning outpatient medical record files at Besuki Hospital. This type ofresearch is qualitative. Data collection techniques using interviews, observation,documentation and ultrasound. The results of this study are individual factors thatcause delays in returning outpatient medical record files, namely officers do notunderstand the SOP for returning outpatient medical record files and officershave not been able to complete medical record files because the doctor's workshift is only briefly at Besuki Hospital. Psychological factors that cause delays inreturning outpatient medical record files are the lack of awareness andresponsibility of officers towards returning medical record files and medicalrecord distribution officers have never been motivated by the head of the medicalrecord. The organizational factor causing the delay in returning outpatientmedical records is that there is no compensation given by the hospital to theofficer and the head of the medical record has not been so firm in handling casesof late return of outpatient medical record files. Besuki Regional GeneralHospital is expected to provide rewards or rewards to officers; make SOPs forreturning outpatient medical record files and socialize them to officers; sanctionsare given to officers who are late in returning outpatient medical record files; thehead of the medical record is even more assertive to his subordinates andprovides motivation.

Abstract

Delay in returning medical records is one of the things that can affect the quality of medical record services and have an impact on the continuity of the hospital. This study aims to determine the factors causing the late return of inpatient medical records at the hospital based on Lawrence Green Theory which consists of: predisposing, enabling and reinforcing factors. A total of 22 articles met the inclusion and exclusion criteria. The results of the study show that predisposing factors include: knowledge, attitude, discipline, lack of training, level of education; Enabling factors include: the absence of special officers, the long distance between the inpatient room and the medical record installation, and the not yet optimal utilization of facilities and infrastructure; reinforcing factors include: the absence of rewards and punishments, and the lack of dissemination of SOPs to officers. Efforts that can be made are increasing knowledge, level of discipline and holding training and seminars to make officers understand more about the importance of returning medical records before 2 times 24 hours, bearing in mind that not all officers have a medical record background and need socialization of the SOP for returning inpatient medical records on a regular basis because along with the development of knowledge and guidance information on quality services is needed.Keywords: medical record; inpatient; return delayABSTRAK Keterlambatan pengembalian rekam medis merupakan salah satu hal yang dapat mempengaruhi mutu pelayanan rekam medis dan berdampak pada kesinambungan rumah sakit. Studi ini bertujuan untuk mengetahui faktor penyebab keterlambatan pengembalian rekam medis rawat inap di rumah sakit berdasarkan Teori Lawrence Green yang terdiri atas: faktor predisposing, enabling dan reinforcing. Sebanyak 22 artikel memenuhi kriteria inklusi dan eksklusi. Hasil studi menunjukkan bahwa predisposing factors mencakup: pengetahuan, sikap, kedisiplinan, kurangnya pelatihan, tingkat pendidikan; enabling factors mencakup: belum adanya petugas khusus, jauhnya jarak antara ruang rawat inap dan instalasi rekam medis, dan belum optimalnya pemanfaatan sarana dan prasarana; reinforcing factors mencakup: belum adanya reward dan punishment, serta kurangnya sosialisasi SOP kepada petugas. Upaya yang dapat dilakukan ialah meningkatkan pengetahuan, tingkat kedisiplinan dan diadakan pelatihan dan seminar untuk membuat petugas lebih memahami tentang pentingnya pengembalian rekam medis sebelum 2 kali 24 jam, mengingat tidak semua petugas berlatar belakang rekam medis dan perlu sosialisasi SOP pengembalian rekam medis rawat inap secara berkala karena seiring berkembangnya pengetahuan dan informasi tuntunan akan pelayanan yang berkualitas sangat diperlukan.Kata kunci: rekam medis; rawat inap; keterlambatan pengembalian

Abstract

The time of providing medical record documents to the polyclinic is one of the indicators in measuring patient satisfaction. The results of the preliminary study showed that 12 of the 13 observed medical record documents experienced delays in providing medical record documents. This is not in accordance with service standards, the minimum time for providing outpatient medical records is less or equal to 10 minutes. The purpose of the study was to analyze the factors causing the delay in providing medical record documents for outpatients at Bima Hospital. This research was a qualitative research, the research subject consisted of 5 informants. Collecting data through interviews and observations. Data were analyzed by content analysis. The results showed that the predisposing factor was that some officers did not attend training and did not know the standard time for providing outpatient medical records. Enabling factors were caused by the distance from the medical record room to the poly that is quite far, the lack of infrastructure such as the availability of shelves is still lacking, the use of tracers has not been maximized, and there are no trolleys. The reinforcing factor is caused by the unavailability of standard operating procedures for the provision of medical records. This study suggests hospitals to provide training to officers, socialize standard operating procedures for returning and providing medical records, adding trolleys as a means of distributing medical records, and using tracers to reduce missfiles.Keywords: medical record; delay; provision
