Abstract

Based on research at RSUPN Dr.  Cipto Mangunkusumo, Central Jakarta, there was a delay in the distribution of medical record documents to arrive at the surgical clinic more than 30 minutes. The purpose of this research was to identify the causes of the delayed in the distribution of medical records documents to arrive at the surgical clinic based on 5 management elements, such as man factors, materials factors, machine factors, methods factors, motivation factors and the factors of arrange the recommendations effort for improvements in the supply of medical record documents at RSUPN Dr. Cipto Mangunkusumo. This type of the research uses qualitative analysis. Based on the results and discussion, in the man factor there are filing staffs who still lack discipline such as, often come late, the one of the filing staff often absen. The material factor there is still a medical record document which folder / cover is torn, and therefore the medical record number and the patient's name are not visible or unclear, so that the staffs has trouble when searching for the medical record documents. The machine factor, which is EHR, usually occurs loading or could not be a accessed so the tracer not coming out and when the EHR can be accessed, the tracer will come out with a lot. Ther methods factor is KPI percentage of the speed of providing medical records of patients without agreement arrived at the  polyclinic in 2019 still has not reached the target of 90%. After an evaluation, in 2020 the time of the document arrived at the polyclinic would be less than 30 minutes by reducing the target to 75%. Motivation factor there is no external motivation provided by the head of the medical record .The conclusion is the filing staffs did not come on time and did not come, the materials of the cover on medical record documents are easily torn, EHR is often difficult to access, the achievement level has not yet reached the target of providing medical record documents, and there is still no motivation from the head of medical record.

Abstract

Based on the results of preliminary studies conducted at the RSUP dr. Soeradji Tirtonegoro is known thehighest delay count is the Anggrek of 95% room and 4th Dahlia room of 80%. The delay has an impactwhich daily census returned is not timely, and be it can increase the work of officers for the next day andsending the recapitulation results to the reporting department is delayed. This study aims to observe theimplementation of the daily census and the factors causing the inaccuracy in returning the daily census ofhospitalization, the determination of the main causes of the problem are using a fishbone diagram. Thistype of research uses qualitative and are collected a data with observation, interview and documentation.The results of the main problem are Man (the work of the recapitulation officer is delayed and ineffective,the employee's indiscipline). Method (there is not written policies of Standard Operating Procedure, theofficer does not know the Standard Operating Procedure). Minutes (The deadline for retuning the dailycensus of hospitalization are not consistent). Machine (there are no features that support the daily censusof hospitalization in SINERGIS, daily inpatient census is manually). Money (there is no rewards).

Abstract

There were 482 medical file missfile incidents in 2019, with the highest number occurring in June 2019 with 61 medical record misfile, this could cause officers to experience difficulties in searching for medical record files and increasing the work of officers to more busy because they have to do a search on other number shelves that allow it to be misplaced, so the service process tends longer. This research aims to evaluate the event of medical record misfile using the FOCUS PDCA method at Sanglah Hospital Denpasar. Data collection techniques used in this research were interviews and observation. The data analysis stage in this research includes the identification of stage of find, organized, clarify, understand, select, plan, do, check, and action. The solution chosen to solve the problem is by designing the media information about the range of medical record storage on each shelf, carrying out the process of attached numbering information media to each storage shelf, and conducting information dissemination related to the attached media. The results of this evaluation are storing medical record files that are arranged according to the number on the shelf and re-monitoring at the end of every month all medical records in the place.

Abstract

National Center General Hospital Dr. Cipto Mangunkusumo functions as an educational hospital, so the medical record file is often used for research materials. It can be seen from the hospital data in 2019 that there were 53,963 requests for medical record files, but only 42,138 medical record files could be fulfilled by research officers, meaning that 22% of files could not be provided by research officers. If this is done continuously, it will cause a decrease in the quality of the research officer. Based on observations that have been made on 2 people from the Medical Records Research Center Dr. Cipto Mangunkusumo is known that the officer concurrently had several jobs. This causes the workload that is not in accordance with the number of officers. The purpose of this study was to analyze the workforce needs of the Central Medical Records research staff based on WISN theory at Dr. RSUPN. Cipto Mangunkusumo. This type of research uses qualitative research with 2 informants. Data collection methods are observation, interviews, and documentation. Based on the workload calculation using the WISN method, it is known that the number of workers needed in the research unit is 8 people. It is expected that with the results of the calculation of labor needs based on WISN theory can be used as an effort to plan the addition of officers to improve the quality of hospital services and in accordance with the existing workload.

Abstract

The E-Pasti especially in the insurance department does not have a notification to the patient thatthe file for insurance has been processed. This resulted in patients having to call or come directlyto the hospital so that which becomes inefficient. This is caused by the absence of notification inthe poly system that the blank has been completed. The research method used in this research isto use the FOCUS PDCA method. The results obtained after carrying out the improvement phase,namely the Asuransu Submission Service, become more efficient because the patient no longerneeds to come or call the hospital to ask the status of the file.

Abstract

Workload analysis is an attempt to find out the time used by officers to complete a particular task which is expected to know the proportion of the number of officers needed in a particular work unit in an agency. The Importance of Workload Analysis must be carried out routinely in order to obtain information about the description of the workload of units in an agency. This research aims to analyze the workload of medical records officers so that the workload level of the officer is known using the WISN (workload indicator staff need) method, The type of this research is qualitati with data collection techniques in the form of observation, interviews, documentation, and library research. The subjects of this study were 9 staff officers which 1 head of filing affair and 8 implementing staff. Based on human resource qualification research, there was enough job description, but based on patient visit rates after RME implementation since early 2020 showed a drastic decrease in visits where before the RME was applied the average number of patient visits 20,000 patients per month, while after applying RME for January and February there were 6,485 and 6,116. The results of the calculation of the workload know that the number of officers needed is the ideal number of officers needed, which is 4.96 so that it is rounded up to 5 so that it can be concluded that there is a need for relocation of as many as 4 people to other work units so that Mangusada Regional Hospital can provide maximum human resources as efficiently as possible.

Abstract

Medical records according to Permenkes No. 269 of 2008 is a file containing notes and documents about patient identity. The problem that often arises in Klaten General Hospital is the length of the file search process due to the absence of applications that control the entry and exit of files, when officers borrow and return only to write in the control book. Supporting all the problems and objectives needed by the data, so the data collection methods used are observation and interviews. Analysis of the data obtained from the interview and observation process is to draw conclusions and identify the most urgent problems. The goal to solve this problem is to make an application that controls the borrowing and return of the RM file so that the officer knows quickly where the file will be sought. The application is made using the waterfall method in the workflow process and uses several programming languages such as PHP, HTML and CSS. The results of the application have been tested by several users using blackbok testing, but still do not know how effective and efficient when the application is tested in actual service.

Abstract

Klaim BPJS Kesehatan adalah pengajuan biaya perawatan pasien peserta BPJS oleh pihak rumah sakit kepada pihak BPJS Kesehatan yang dilakukan secara kolektif dan ditagihkan kepada pihak BPJS Kesehatan setiap bulannya. Setelah itu BPJS Kesehatan akan melakukan persetujuan klaim dan melakukan pembayaran untuk berkas yang layak, namun untuk berkas yang tidak layak klaim atau pending (unclaimed) harus dikembalikan ke rumah sakit untuk diperiksa kembali. Kejadian pending klaim di RSUPN Dr. Cipto Mangunkusumo disebabkan oleh beberapa hal diantaranya administrasi, medis, koding, tidak layak dan lainnya. Berdasarkan studi pendahuluan ditemukan berkas pending klaim pada bulan januari 2020 sebanyak 788 dari 2539 pasien rawat inap yang menggunakan Jaminan Kesehatan Nasional. Dari permasalahan tersebut maka dilakukan analisis terkait faktor penyebab pending klaim akibat koding melalui pendekatan fenomenologi yang meliputi input dan proses pada bagian Koding di RSUPN dr. Cipto Mangunkusumo. Hasil penelitian terkait SDM diperlukan penambahan jumlah sumber daya manusia serta sosialisasi pembaharuan dari aturan koding. Terkait Sarana dan Prasarana yaitu keterbatasan ruangan petugas koding diperlukan pertemuan secara rutin. Terkait Teknologi yaitu mendukung proses klaim berkas rekam medis pasien diintegrasikan dengan proses manajemen lain yang terdapat di SIMRS. Berdasarkan analisis proses terkait Perencanaan yaitu telah memiliki SOP penyelesaian permasalahan pending klaim. Terkait Pengorganisasian yaitu perlu dibuat job description khususnya pada petugas bagian coding. Terkait Pelaksanaan yaitu proses pengklaiman dilihat dari alur yang ada telah sesuai dengan SOP yang berlaku serta pada analisis proses terkait Evaluasi yaitu mengadakan pertemuan rutin atau rapat bulanan.

Abstract

Berdasarkan survey pendahuluan di Rumah Sakit Umum Bhakti Husada Krikilan terdapat permasalahan terkait kinerja pada unit filing yaitu beberapa job description belum terlaksana dan belum dikerjakan maksimal, waktu penyediaan dokumen rekam medis lebih dari 10 menit, petugas filing berlatar belakang pendidikan sekolah menengah kejuruan dan sekolah menengah ekonomi atas. Penelitian ini bertujuan untuk melakukan analisis faktor penyebab rendahnya kinerja petugas rekam medis pada unit filing kemudian dilakukan analisis faktor penyebab kinerja rendah dari faktor individu, organisasi dan psikologi dengan metode Problem Tree Analysis. Jenis penelitian yang digunakan adalah penelitian kualitatif dan pengumpulan data dengan wawancara, observasi dan dokumentasi. Hasil penelitian ini menunjukkan bahwa pada penyebab utama faktor individu dan demografi, adanya latar belakang pendidikan petugas yang tidak sesuai sementara perekrutan tenaga baru belum dilaksanakan karena masih dalam proses perencanaan manajemen; pada faktor organisasi penyebab utama desain pekerjaan terdapat kurangnya koordinasi terkait job description dan SOP antar petugas dan atasan, terbatasnya jumlah tenaga, dan petugas kurang memahami pentingnya job description; pada faktor psikologi penyebab utama sikap yaitu adanya jam kerja yang cukup padat namun tenaga kerjanya terbatas sehingga penyediaan berkas terlambat dan pada faktor penyebab utama motivasi yaitu adanya motivasi dalam diri petugas (bukan pengakuan dari orang lain yang mendorong kerja petugas) serta  motivasi dari luar diri petugas (pihak luar kurang memahami pentingnya job description dan masih  dalam proses perencanaan manajemen).

Abstract

Pengelolaan rekam medis di Rumah Sakit Mitra Sehat Situbondo kurang optimal. Hal tersebut dibuktikan pada Juni 2019, terjadi ketidaklengkapan pengisian rekam medis di bagian assembling sebesar 66,67%. Selanjutnya, pada bagian coding terjadi pengembalian klaim BPJS pada triwulan 1 tahun 2018 sebanyak 131 berkas. Selain itu, pada bagian filling ditemukan bahwa ruang penyimpanan tidak cukup untuk menampung dokumen rekam medis, sehingga ada dokumen yang ditumpuk di lantai. Tujuan penelitian ini menganalisis pengelolaan rekam medis melalui pendekatan sistem. Jenis penelitian ini adalah kualitatif dengan teknik pengumpulan data wawancara, observasi, dan dokumentasi. Subjek penelitian yaitu kepala rekam medis, petugas assembling, petugas coding, dan petugas filling. Penelitian ini menggunakan metode USG untuk prioritas masalah. Hasil penelitian didapatkan bahwa jumlah petugas rekam medis masih kurang dan ada petugas yang tidak berkualifikasi pendidikan rekam medis, serta ada petugas yang belum mendapatkan pelatihan. Hasil penelitian juga didapatkan bahwa buku ekspedisi rawat inap dan komputer untuk pelaporan belum tersedia. Selain itu, ruang penyimpanan terasa panas dan jumlah rak penyimpanan masih kurang. Standar prosedur operasional rekam medis juga kurang disosialisasikan. Hasil penelitian pada proses assembling ditemukan terjadi ketidaklengkapan pengisian rekam medis. Selain itu, pada proses coding terjadi kekosongan pengisiandiagnosa dan tindakan serta tulisan dokter tidak terbaca. Hasil dari prioritas masalah dapat disimpulkan bahwa yang menjadi masalah utama dalam pengelolaan rekam medis adalah kurang optimalnya fasilitas filling, dimana ruang penyimpanan terasa panas dan rak penyimpanan masih kurang. Saran yang diberikan adalah melakukan pengadaan AC dan pencatatan grafik suhu ruangan secara rutin serta meningkatkan dukungan manajemen dalam penyediaan rak penyimpanan.
