Abstract

Pengembalian berkas rekam medis rawat jalan di Rumah Sakit Tingkat III Baladhika Husada Jember masih mengalami keterlambatan. Pada triwulan pertama tahun 2019 total berkas rekam medis rawat jalan yang terlambat sebanyak  25,32% berkas dari  25,32% berkas. Keterlambatan pengembalian berkas rekam medis dapat mempengaruhi pelayanan rekam medis dan akan menghambat kegiatan selanjutnya, seperti kegiatan assembling, koding, analisis, indeks. Penelitian ini bertujuan untuk menganalisis penyebab keterlambatan pengembalian berkas rekam medis rawat jalan di Rumah Sakit Tingkat III Baladhika Husada Jember. Jenis penelitian ini menggunakan kualitatif dan pengumpulan data dengan wawancara, observasi, dokumentasi dan brainstorming. Hasil penelitian yang didapatkan oleh peneliti pengetahuan dan sikap yang masih kurang baik, prasarana atau fasilitas yang kurang memadai. Tidak ada motivasi yang diberikan oleh kepala rekam medis, serta belum pernah dilakukannya sosialisasi SOP tentang pengembalian berkas rekam medis rawat jalan. Upaya yang dilakukan adalah, Menginformasikan tentang buku ekspedisi dan kegunaannya dan petugas juga harus mengikuti pelatihan atau seminar agar pengetahuan dan keterampilan petugas mengenai pengelolaan rekam medis, kepala rekam medis memberi teguran kepada pertugas yang terlambat mengembalikan berkas rekam medis, Melakukan perancangan buku ekspedisi, memberikan motivasi kerja dan melakukan sosialisasi SOP terkait pengembalian berkas rekam medis rawat jalan.

Abstract

Kegiatan penyelenggaraan rekam medis di Puskesmas Dringu masih belum optimal. Hal ini dapat dilihat dari belum adanya ruang filing, kurangnya rak rekam medis, serta proses penyimpanan yang belum sesuai dengan SOP sehingga dapat menyebakan terjadinya missfile. Tingkat kejadian missfile di Puskesmas Dringu sebesar 11% pada bulan Juni 2019. Missfile dokumen rekam medis dapat menyebabkan data rekam medis tidak berkesinambungan. Penelitian ini bertujuan untuk menganalisis faktor penyebab terjadinya missfile di Puskesmas Dringu Kabupaten Probolinggo. Jenis penelitian adalah penelitian kualitatif. Pengumpulan data menggunakan wawancara, observasi, dokumentasi dan kuesioner. Subyek penelitian berjumlah 4 orang yang terdiri dari 2 petugas rekam medis, kepala rekam medis serta kepala Puskesmas Dringu. Penentuan prioritas penyebab masalah terjadinya missfile menggunakan USG (Urgency, Seriousness, Growth) dan penentuan solusi menggunakan brainstorming. Hasil penelitian menunjukan bahwa faktor penyebab missfile yaitu ketidakpatuhan petugas dalam menjalankan SOP, kurangnya pengetahuan, tidak adanya tracer dan buku ekspedisi, pendidikan petugas belum sesuai kualifikasi, petugas tidak melakukan penyortiran DRM yang akan disimpan dan petugas tidak melakukan audit penyimpanan secara periodik. Hasil prioritas utama penyebab missfile menggunakan USG yaitu petugas yang tidak patuh terhadap SOP (Standart Operasional Procedure). Upaya penyelesaian masalah tersebut yaitu adanya sosialisasi SOP, pengawasan kinerja petugas, mengadakan evaluasi kinerja dan  adanya punishment dan reward.

Abstract

Based on a preliminary study at RSIA Srikandi IBI Jember, there was a delay in submitting claim health BPJS to the inpatient unit over the next ten months. The delay in submitting claim health BPJS as many as 28 days with 353 inpatient files (57,5%) in January 2019, while February as 31 days with 249 inpatient files (78,3%) and March 29 days with 313 inpatient files (75,0%).It would harm hospital finances because of delays in the payment of health BPJS funds. This research aimed to improve the delay claim of health BPJS in inpatient units at RSIA Srikandi IBI Jember. This research used a qualitative method with interviews, observation, questionnaires, and brainstorming to collect data methods. This research showed that the cause of delay in submitting claims health BPJS by material factor was incomplete inpatient claim such as medical resume, the result of examinations. The Plan made SOP submit requests claims BPJS to the inpatient unit, the checklist of BPJS document, and SOP to verify the completeness and accuracy to the inpatient claim. The Do stage accepts the SOP to the hospital director. Stage of Check is to see the result of implementation claim health BPJS to the inpatient unit in February-April 2020, and that showed a decrease in the number of delays in submitting claims health BPJS at the hospital. Action steps were known that these efforts could help the BPJS health claims process.

Abstract

AIDS (Acquired Immunodeficiency Syndrome) is a collection of symptoms obtain due to decreased immunity caused by HIV (Human Immunodeficiency Virus). The number of people with HIV/AIDS is increasing, and becoming a global pandemic in the world of child and adult mortality. The highest mortality rate is in Kencong with the number of deaths increasing every year, the highest was in January-July 2019 with 55,88% deaths. The research was aimed to provide an overview of HIV/AIDS mortality in Health Center Kencong, Jember. This research is quantitative descriptive. The samples used were 51 medical record documents of HIV / AIDS patients. The data collection method in this study is a checklist sheet. The data analysis used was univariate analysis. The results showed that most respondents aged 15-49 years were 98.2%, the majority of respondents were male at 67.2%, most respondents had low education <high school (74.5%). It is hoped that the Puskesmas can socialize and provide information to HIV / AIDS patients regarding risk factors for HIV / AIDS mortality so that patients pay more attention to their health.

Abstract

The incompleteness of informed consent in general surgery cases inpatients at dr. Abdoer Rahem Situbondo hospital has an increase every month in the first trimester of 2019. The highest increase occurred in March, which was 74% from the previous 64% in February. Based on the Minister of Health of the Republic of Indonesia No. 129 of 2008, the minimum standard for filling out informed consent is 100%. The purpose of this research was to analyze the factors causing the incomplete filling of informed consent in inpatient general surgery cases at Dr. Abdoer Rahem Situbondo using Simamora's performance theory. The type of this research is qualitative analysis. The research design used is action research. Data collection techniques used were interviews, questionnaires, documentation, CARL (Capability, Accessibility, Readiness, Leverage), and brainstorming. The preliminary study was carried out from January to February 2020. Data collected were an inpatient general surgical case informed consent form, which had been returned to the medical record unit. The results obtained from this research are that there has never been an evaluation of informed consent’s incompleteness. No award was given for the performance of completeness informed consent. There has never been any learning such as a seminar or training on filling out the informed consent.

Abstract

The quality of Puskesmas services was an important factor in the utilization of health services. Patient satisfaction was an important measure of the effectiveness of health care providers to fulfill patient needs and expectations. The purpose of this research is to determine the relation between the quality of health services (tangible, reliability, responsiveness, assurance, empathy) and the satisfaction of inpatients at first-level health facilities. The method used is a literature review by analyzing 10 articles from various sources. Based on the results of the literature study from various studies obtained, the researcher stated that 3 articles that the overall quality of health services (tangible, reliability, responsiveness, assurance, empathy) have a significant relationship with patient satisfaction. A total of 6 literatures show that the tangible dimensions are related and 1 literature shows that there is no relationship. There are 4 dimensions of reliability that show a relationship and 3 literature that show no relationship. The responsiveness dimension consists of 4 literature showing a relationship and 3 literature that is not related. The assurance dimension consists of 3 literature showing a relationship and 4 literature that is not related to patient satisfaction. The empathy dimension consists of 5 literatures that show a relationship and 2 literature that show no relationship with patient satisfaction. The assurance dimension is considered is not good so that it has no relationship with patient satisfaction. There are literatures that only writes the correlation test results without explaining the situation or facts found in first-level health facilities.

Abstract

A Medical resume is a summary of the whole medical patient's treatment. The incomplete medical resume can cause the patient's medical history to not continue with the patient's previous data. The various factors cause an incomplete medical resume. The research aimed to analyze the aspects related to the completeness of a medical resume. The study is a literature review with database from Google Scholar and Garuda. This research's keywords are factors related AND completeness OR complete AND medical resume OR medical resume OR discharge summary OR discharge summary. The manpower elements are 21,4 percent of good knowledge, 28,5 percent of long working tenure, 21,4 percent of civil servant employment status, 7,1 percent of good attitude, and 7,1 percent of higher education. The motivation elements are 14,2 percent of adequate supervision (monitoring and evaluation), 7,1 percent of good leadership, and 7,1 percent of reward and punishment. Media elements are 7,1 percent of how to pay for BPJS patients and 7,1 percent of directly related work conditions (unique table and chair to fill medical records) and other work environments (lighting and noise room to fill medical resumes). The main factor related to medical resume's completeness is tenure, and other high factors are knowledge and employment status. The writer suggests that health facilities should facilitate patient data completeness, especially for doctors with a new term, bad experience, and non civil servant employment status.

Abstract

The maternal mortality rate in East Java has increased over the last two years due to restrictions on antenatal care visits. In 2020 Jember Regency was ranked first in the maternal mortality rate (MMR) in East Java as many as 61 people out of 175.53 per 100,000 live births. The Jember Regency Government in 2017 has implemented an information system for recording pregnant women to suppress MMR in Jember Regency but currently, its use is not maximized because there are still obstacles such as users finding it difficult to use it. The purpose of this study was to analyze the information system for recording pregnant women in Jember Regency. This research is a quantitative study with a cross-sectional approach, this approach is used to see the relationship between one variable and another with the Technology Acceptance Model (TAM) method and the research subject is a midwife who uses information systems. The results of the study show that there is a significant relationship between the usability variable and perceived benefits (sig = 0.001, r = 0.483), privacy and confidentiality variables and perceived benefits (sig = 0.023, r = 0.879), and the perceived benefit variable on user intentions. (sig = 0.001, r = 0.468). The suggestion in this study is that the Health Service should make an SOP that requires users to use an information system for recording pregnant women for services to be more intensive and maximal in their use.

Abstract

Permasalahan terkait kepuasan pasien masih terjadi dalam penyelenggaraan pelayanan kesehatan. Salah satu institusi yang menyelenggarakan pelayanan kesehatan adalah klinik. Klinik dr. M. Suherman merupakan klinik pratama yang bekerja sama dengan BPJS Kesehatan dalam memberikan pelayanan kepada pasien. Data laporan bulanan jumlah kunjungan pasien BPJS yang mendapatkan pelayanan rawat jalan di Klinik dr. M. Suherman tahun 2015 secara garis besar mengalami peningkatan dari 40,73% pada bulan Januari menjadi 62,52% pada bulan Desember. Akan tetapi, peningkatan angka kunjungan pasien tidak selalu disertai dengan peningkatan kepuasan pasien. Observasi awal terhadap 14 pasien BPJS yang mendapatkan pelayanan rawat jalan menunjukkan bahwa 9 diantaranya menyatakan bahwa pelayanan kurang memuaskan. Tujuan penelitian ini adalah mengidentifikasi persepsi dan harapan berdasarkan lima dimensi mutu, mengukur serta menganalisis tingkat kepuasan pasien BPJS terhadap pelayanan rawat jalan. Metode pengolahan data dalam penelitian ini menggunakan metode SERVQUAL dan Customer Satisfaction Index (CSI) untuk mengukur tingkat kepuasan pasien. Metode Importance Peformance Analysis (IPA) digunakan untuk menganalisis prioritas masalah demi perbaikan kualitas layanan. Hasil pengukuran dengan metode SERVQUAL diperoleh bahwa gap tertinggi berada pada dimensi Daya Tanggap (Responsiveness). Metode Customer Satisfaction Index (CSI) menyatakan bahwa persentase kepuasan pasien adalah 79,958%. Hasil analisis dengan metode Importance Peformance Analysis (IPA) menyatakan bahwa terdapat 5 atribut yang menjadi prioritas utama.
