Abstract

In order to guarantee the availability, fair distribution, and sensible use of medications, drug management at the District/City Health Office Pharmacy Installation is essential. This study uses a literature review methodology to examine several national publications over the last five years in order to investigate various issues and difficulties in drug management at the District/City Health Office Pharmacy Installation. In order to complete the review process, literature pertinent to the subject of drug management in healthcare institutions was chosen using inclusion criteria. Using the terms medication management, pharmaceutical installation, and health office, ten publications were retrieved from Google Scholar and the Garuda Portal (Garda Rujukan Digital). According to the study's findings, planning and procurement, storage and distribution systems, recording and reporting techniques, supervision and assessment, budgetary concerns, and human resource difficulties are the primary obstacles. The implementation of an integrated drug logistics information system, the development of human resource capacity, and more regionally sensitive policies are the goals of improvement initiatives. The study's findings highlight the necessity of a more all-encompassing management approach to ensure the successful, efficient, and sustainable operation of pharmaceutical services at the District/City level.

Abstract

Pasien dengan penyakit ginjal kronik yang menjalani terapi hemodialisis umumnya memiliki berbagai kondisi medis penyerta, seperti hipertensi dan gangguan metabolik lainnya yang menyebabkan penggunaan banyak obat secara bersamaan. Penggunaan sejumlah besar obat ini dapat menyebabkan polifarmasi, yaitu kondisi ketika pasien mengonsumsi lima atau lebih jenis obat yang pada gilirannya meningkatkan risiko terjadinya interaksi obat. Penelitian ini bertujuan untuk menganalisis prevalensi polifarmasi serta mengidentifikasi potensi interaksi obat pada pasien hemodialisis. Penelitian ini dilakukan menggunakan metode narrative review dengan menelaah 10 artikel yang relevan dalam 10 tahun terakhir. Hasil penelitian menunjukkan bahwa sebagian besar pasien mengalami polifarmasi dengan tingkat konsumsi obat bervariasi antara lima hingga lebih dari sepuluh jenis obat per pasien. Mayoritas artikel menunjukkan tingginya prevalensi polifarmasi (>80%) dan potensi interaksi obat kategori sedang hingga berat, terutama pada pasien lansia dengan penyakit penyerta seperti hipertensi dan diabetes. Dari analisis interaksi obat, ditemukan bahwa potensi interaksi yang terjadi mencakup kategori ringan, sedang hingga berat dengan beberapa kombinasi obat menunjukkan risiko signifikan terhadap penurunan efektivitas terapi maupun peningkatan toksisitas. Temuan ini menegaskan pentingnya evaluasi menyeluruh terhadap regimen pengobatan yang digunakan oleh pasien hemodialisis.

Abstract

Patients with stage 5 chronic kidney disease (CKD) who undergo routine hemodialysis (HD) often receive a complex treatment regimen (polypharmacy) to control comorbidities. This condition increases susceptibility to drug-related problems (DRPs) due to pharmacokinetic and pharmacodynamic changes after nephron damage. This narrative review aims to evaluate the impact of polypharmacy on the incidence of drug problems (DRPs) and its implications on various clinical outcomes, quality of life, and cost burden in stage 5 CKD patients with routine HD. This writing used a narrative review method through searching literature from various scientific data sources such as Google Scholar, PubMed, and Scince Direct, as well as national journals with a publication time span of the last 5-10 years. Literature analysis showed that polypharmacy was strongly correlated with a high incidence of moderate to major drug interaction potential (DDI) (49–89%) and drug side effect multiplication (ADRs). The use of drugs without proper dosage adjustment to the dialysis circuit triggers therapy failure. Monitoring of therapeutic outputs has been proven to integrate the dimensions of clinical data, laboratory biomarkers, economic aspects, and drug burden reduction through deregulation. Polypharmacy in ESRD-HD patients poses massive clinical and financial problems. The active involvement of clinical pharmacists through treatment reconciliation and selection of drug types with safe non-renal elimination is crucial to minimize the risk of toxicity, optimize clinical outcomes, and improve patients' quality of life.

Abstract

Tuberkulosis (TB) merupakan penyakit menular dengan prevalensi tinggi di Asia Tenggara, termasuk Indonesia, dan menjadi salah satu penyebab utama kematian akibat infeksi. Kompleksitas regimen pengobatan TB yang melibatkan penggunaan obat jangka panjang berisiko menimbulkan Drug Related Problems (DRPs), seperti interaksi obat, efek samping, dan ketidakpatuhan pasien terhadap terapi. Tinjauan ini bertujuan untuk mengeksplorasi peran apoteker dalam identifikasi dan penanganan DRPs pada pasien TB berdasarkan literatur terkini. Metode yang digunakan adalah narrative review terhadap 10 artikel ilmiah dalam rentang waktu 2019–2024 dari database bereputasi. Hasil kajian menunjukkan bahwa peran apoteker mencakup identifikasi interaksi obat, edukasi pasien, pemantauan efek samping, serta penguatan kepatuhan melalui intervensi farmasi seperti Medication Therapy Management (MTM) dan program DOTS. Bukti ilmiah menunjukkan bahwa keterlibatan aktif apoteker dapat meningkatkan efektivitas terapi dan keselamatan pasien TB. Oleh karena itu, diperlukan penguatan sistem dan kebijakan kesehatan untuk mendukung optimalisasi peran apoteker dalam praktik klinik pengelolaan TB.

Abstract

Background: Pharmacovigilance is an essential instrument for protecting public health after drugs are marketed. However, the implementation of this system in Indonesia was still hampered because the periodic adverse drug reactions (ADR) reporting rate was recorded as very low. Objective: This review was aimed to evaluate the regulatory changes in Indonesian pharmacovigilance. Additionally, this study reviewed the competency levels, psychological conditions of healthcare professionals, operational barriers on the national reporting platform, and compared systems across Southeast Asia. Method: This study was conducted using a narrative review method. Literature sources were collected from BPOM regulations and recent original articles regarding ADR reporting practices by healthcare professionals. Evaluations were also conducted on the challenges of using the e-MESO application in various healthcare facilities across Indonesia, alongside a comparative study of pharmacovigilance governance in Southeast Asia. Results: Indonesia was found to have strengthened its legal framework for pharmacovigilance progressively to achieve WHO Listed Authority (WLA) status. Nevertheless, knowledge-practice gaps and the phenomenon of low ADR reporting were identified to be still widely found in the field. These barriers were triggered by a blame culture, a lack of local guidelines, the absence of an autosave feature, and the administrative burden on the e-MESO system. The comparative study demonstrated that Indonesia needs to implement proactive methods, such as trigger tools, and optimize the role of structured consumer reporting by the public. Conclusion: This narrative review concluded that strengthening of pharmacovigilance in Indonesia was supported by new regulations, but was hindered by systemic underreporting and psychosocial challenges that also occurred in Southeast Asia. Therefore, system improvements could not rely solely on e-MESO 2.0, but instead demanded a no blame culture reform in healthcare facilities, the establishment of local Standard Operating Procedures, the application of proactive methods, early education in pharmacy institutions, and active community involvement.

Abstract

Background: Tuberculosis (TB) remains a global health problem with high incidence and mortality rates. The success of TB therapy is greatly influenced by patient adherence to long-term treatment. Medication Therapy Management (MTM) has been developed as a pharmaceutical care approach focused on optimizing medication use and improving patient outcomes. Objective: This review aims to analyze the impact of Medication Therapy Management implementation on treatment outcomes in tuberculosis patients and identify opportunities and challenges for its implementation in healthcare practice. Method: This study was conducted using a narrative review method by searching scientific articles from the PubMed, Google Scholar, Frontiers, and ScienceDirect databases. Articles published between 2016 and 2026 that discussed the implementation of MTM in tuberculosis patients were selected based on predetermined inclusion criteria. A total of 15 articles met the criteria and were analyzed descriptively. Results: The review indicates that the implementation of MTM has contributed to improving treatment adherence, identifying and addressing drug-related problems, improving monitoring of drug side effects, and supporting the success of TB patient therapy. Pharmacist interventions through MTM components have also been reported to have the potential to improve patient quality of life and strengthen collaboration among healthcare professionals. However, MTM implementation still faces various obstacles, such as limited human resources, suboptimal documentation systems, and poor patient understanding of therapy. Conclusion: Medication Therapy Management has demonstrated benefits in supporting successful tuberculosis treatment by improving adherence, safety, and effectiveness of therapy. Strengthening the role of pharmacists and providing support from the health system is needed to optimize MTM implementation in TB patients. Abstract

Peredaran kosmetik ilegal dan berbahaya di Indonesia masih menjadi ancaman serius bagi kesehatan masyarakat, terutama remaja yang menjadi target utama pemasaran produk viral di media sosial. Kegiatan pengabdian masyarakat ini bertujuan meningkatkan pengetahuan dan keterampilan siswa SMK HKTI 1 Purwareja Klampok, Banjarnegara, dalam memilih kosmetik yang aman dan legal. Metode yang digunakan terdiri dari diskusi interaktif dan praktik pengecekan produk menggunakan website cekbpom.pom.go.id dan aplikasi BPOM Mobile, dengan melibatkan 132 siswa. Hasil kegiatan menunjukkan peningkatan pemahaman siswa tentang definisi kosmetik legal, identifikasi bahan berbahaya (merkuri, hidrokuinon, asam retinoat), serta keterampilan memverifikasi izin edar BPOM. Siswa juga menunjukkan perubahan sikap menjadi lebih kritis terhadap klaim viral dan lebih selektif dalam memilih produk berdasarkan keamanan. Kegiatan ini menegaskan bahwa kombinasi edukasi teoritis dan keterampilan praktis efektif membekali remaja dengan literasi keamanan kosmetik, sekaligus mendorong mereka menjadi agen edukasi bagi teman sebaya untuk menekan peredaran produk ilegal di masyarakat.
