Abstract

Peningkatan anggaran kesehatan pada Anggaran Pengeluaran dan Belanja Negara (APBN) sangat signifikan selama pandemi COVID 19. Alokasi belanja anggaran kesehatan diperoleh dari refocusing dan realokasi anggaran. Penelitian ini bertujuan untuk mengetahui besarnya peningkatan anggaran belanja kesehatan dan penggunaannya selama pandemi COVID 19 pada tahun anggaran 2020-2021 dibanding pada tahun anggaran 2019 dimana pandemi COVID 19 belum dimulai. Penelitian dilakukan secara kuantitatif dengan melihat laporan tahunan salah satu klinik kementerian pada tahun 2019 hingga 2021. Penelitian ini juga menggunakan wawancara kepada narasumber terkait penyusun dan pelaksana penggunaan anggaran. Hasil penelitian menunjukkan bahwa adanya peningkatan anggaran kesehatan hingga 11 kali lipat dibandingkan dengan anggaran sebelum pandemi berlangsung. Peningkatan anggaran ini dialokasikan untuk upaya pencegahan penularan COVID 19 serta tracing pegawai yang tertular COVID 19. Upaya pencegahan meliputi pemberian vitamin, susu, madu, hand sanitizer dan masker kepada pegawai secara berkala; penyediaan masker N95, gloves dan gown kepada petugas kesehatan; serta pemeriksaan swab secara rutin dan berkala bagi pegawai di lingkungan kementerian. Kesimpulan pada penelitian ini adalah ada peningkatan anggaran kesehatan yang sangat signifikan selama tahun 2020 hingga 2021 karena pandemi COVID 19 yang menunjukkan adanya upaya dari kementerian untuk melindungi pegawainya dari penularan COVID 19 secara maksimal.

Abstract

Komponen penting dalam menjamin mutu pelayanan pada ketersediaan obat di Instalasi Farmasi adalah pada tahap seleksi, perencanaan dan pengadaan Rumah Sakit. Penelitian ini merupakan studi kasus yang bertujuan untuk melihat gambaran pada tahapan seleksi, perencanaan dan pengadaan obat di RSUD Kembangan tahun 2021. Evaluasi dilakukan dengan mengukur nilai indikator terhadap standar yang berlaku. Penelitian ini bersifat deskriptif dengan data-data logistik farmasi pada tahap seleksi, perencanaan dan pengadaan Tahun 2021 yang dikumpulkan secara retrospektif. Daftar obat tersedia yang sesuai dengan DOEN (Daftar Obat Esensial Nasional) 60%. Jumlah item obat tersedia terhadap jumlah item obat pada kenyataan pemakaian 87%. Jumlah obat yang diterima secara riil terhadap jumlah yang direncanakan pada periode yang sama 98%. Ada beberapa nilai indikator dari hasil penelitian ini yang belum sesuai standar.

Abstract

The implementation of National Health Insurance in 2014 is expected to increase access in health services while reducing the economic burden due to illness. This study aims to see how the behavior of people in choosing treatment when experiencing health problems, whether they tend to select self-medication or outpatient treatment. Quantitative analysis was carried out on variables previously formed based on the KOR Data questionnaire found in SUSENAS 2017. The results of the study showed that more respondents chose to do self-medication than those who accessed formal services with outpatient care. Multivariate tests show all variables except secondary education significantly influence individual decisions to self-medication.

Abstract

The increasing demands of the community for quality and affordable health services make hospitals have to always try to survive amid increasing competition. This article is a systematic review where data is taken from online databases such as ProQuest, Wiley Online, JStore, ScienceDirect, Embase, Scopus, and local journals (Garuda Portal, Jurnal Promkes) using the keywords "Strategy", and "Hospital Marketing", "Marketing". The search engine obtained 6234 international journals and 6 local journals. Then again sorting by year (starting from 2012 and above), duplication, type of article (journal, not systematic review), and full article only, then the final result is 7 titles. The result of this literature review article may provide experiences for directing future investigations regarding the marketing strategy of a teaching hospital. Thus, the findings of this article could help governments, universities, health providers, and academia to concentrate their efforts and policies on the development of the current teaching hospital, especially using marketing tools. Directions for improving the healthcare systems should mainly focus on reducing diseases, disabilities, and mortality among the population; improving the quality and accessibility of healthcare; and developing a competitive market of innovative medical services. The creation of an effective healthcare system could lead to improving the quality of life. Hospital marketing is an important element in introducing the hospital to the wider community. It is necessary to combine and coordinate related elements so that marketing programs can be carried out as effectively as possible.

Abstract

Penelitian ini bertujuan untuk mengkaji dampak tarif Indonesia Case Base Group (INA-CBG) terhadap pasien yang menjalani operasi caesar, serta mengidentifikasi hambatan dan solusi potensial untuk meningkatkan akses dan ketersediaan perawatan kesehatan di Indonesia. Melalui ulasan sistematis yang mencakup 6 studi terkait yang diterbitkan dalam 10 tahun terakhir, penelitian ini menemukan bahwa tarif INA-CBG mempengaruhi pasien secara signifikan. Tarif ini sering tidak mencerminkan biaya nyata yang dikeluarkan oleh profesional perawatan kesehatan, yang membebani penyedia dan pasien. Selain itu, kompleksitas dan variasi dalam prosedur operasi caesar tidak diperhitungkan dalam tarif tetap, potensial mengurangi tingkat perawatan yang disediakan. Skema pengembalian juga mendorong peningkatan operasi caesar yang tidak perlu, dengan dampak negatif pada kesehatan ibu dan pengeluaran kesehatan secara keseluruhan. Upaya perlu dilakukan untuk meninjau ulang struktur tarif, mempertimbangkan kompleksitas prosedur, dan mempromosikan pengambilan keputusan berdasarkan bukti. Hasilnya adalah peningkatan kesehatan ibu dan alokasi anggaran perawatan kesehatan yang lebih efektif. Studi tambahan diperlukan untuk memahami aspek lain dari implementasi INA-CBG di Indonesia dan konsekuensi jangka panjang dari intervensi ini.

Abstract

One of important things in managing Coronavirus Disease 2019 (COVID-19) pandemic is managing medical waste production. Medical waste management start from waste sort until waste manage by licensed parties, both of patients’ waste and health workers’ waste. Sentra Medika Hospital Cikarang is one of the referral hospitals that handles COVID-19. This research is a descriptive analytic study. Researchers analyze hazardous and toxic waste management before and during a pandemic by conducting interviews with the nursing unit and using secondary data. Secondary data was obtained from the hospital environmental health unit. There is no significant differences medical waste before and during pandemic. This happens because there are decreasing of patients amount which come to the hospital. Besides, health services waste is patients waste such as food or drink waste and personal protective equipment such as hazmat, masks, gloves and others. The weight of waste does produce is lighter. Medical waste at Sentra Medika Hospital has been managed in accordance with regulations.

Abstract

Surgery service is one of the health services in a hospital that can describe the quality of the hospital. Sanglah Hospital, Denpasar is a hospital at the provincial level which has a long waiting time for surgery. The long waiting time for surgery is generally related to factors such as the availability of facilities and infrastructure, human resources, and the length of time for approval from patients and families for surgery. Research is needed to determine the factors that significantly form the basis for determining policies at the Sanglah General Hospital level, especially in the Emergency Room, to improve operating services. observational approach and cross-sectional analysis design. This study involved 54 respondents in quantitative research and 7 informants in qualitative research. The result showed bed block events is 38,9 % from December 2017 to February 2018. Agreement following informed consent is obtained in ≥ 1 hour for 17 respondents (31,5%) and < 1 hour for 37 respondents (68,5%). Tools and equipment are complete and available in 94,4% cases and incomplete in 5,6% cases. Time consumed waiting for operation is categorized delay if ≥ 5 hours in 33,3% cases, categorized not delay if < 5 hours in 66,7%. Bivariate analysis using Chi Square showed p-value 0,000 for correlation between bed block and delay to operation, p-value 0,000 for correlation between time consumed to obtain agreement for surgery, p-value 0,012 for correlation between tools and equipment with delay to operation. The most influencing factor is operation agreement with p-value 0,005 using logistic regression. From this study, we conclude there is significant correlation between bed block, time consumed for obtain operation agreement, tools and equipment availability with delay to operation. This result is a base in making strategy to improve quality of surgery services in emergency department.

Abstract

The national TB control program target is TB elimination at 2035 and TB eradication at 2050. Hospitals must establish a DOTS Team who have responsibility to implementing the TB control program and must be record and report any disease cases. TB case reporting uses the Integrated Tuberculosis Information System (SITT) since 2020. Integrated Tuberculosis Information System has become the Tuberculosis Information System (SITB). Syarif Hidayatullah Hospital is one of the 3 hospitals that have not completed the report. This research is a descriptive analytic study with a qualitative approach. The data collecting data include documents review, observation, and in-depth interviews with key informants. The results showed that recording and reporting did not run optimally because 1) the lack of human resources such as nurses as the spearhead of manual record, 2) there was no funding for the TB DOTS program including for recording and reporting, 3) Hospital Information System was not integrated into all service units causing difficulties in data collection TB patients, 4) the internal network did not working properly because there is no socialize about TB recording and reporting flowchart. Increased management commitment in implementing the TB DOTS program so that recording and reporting can working properly.

Abstract

Nowadays, the competition in the hospital industry business is getting tougher. For this reason, hospitals are required to have innovations and strategies to attract patients. One of the necessary things to encourage patients to come to the hospital is a good hospital marketing strategy. This research aims to identify the impact of CODE STEMI program on the marketing strategy of the Cipto Mangunkusumo National General Hospital. This research uses quantitative methods by tracking medical records and administrative documents, as well as qualitative methods by observation and in-depth interviews. The results showed a decrease in mortality, MACE, door to balloon time, total cost, and length of stay after the implementation of the CODE STEMI program. These results have a good impact on the product marketing mix, such as price, distribution, people, processes, and physical facilities. Patients are satisfied with the services provided, and the number of STEMI patients coming to the hospital has increased since the implementation of the CODE STEMI program. Based on these results, it was concluded that the CODE STEMI program had a positive impact on the marketing strategy of RSUPN Dr. Cipto Mangunkusumo. Abstract

The policy developed by the government in the form of a National Health Insurance (JKN) program whose funding is managed by the BPJS, must be supported by all health service facilities both government and private. RSD Kol. Abundjani Bangko is a regional hospital belonging to the Merangin District Government that has collaborated with BPJS to serve BPJS participants in the health services sector in 2014. In the course of the increase there have been an increase in the number of BPJS participants who have been referred and treated to RSD Kol. Abundjani Bangko. The increase in patient visits on the one hand was pleasing to management and on the other hand raised concerns for the management of RSD Kol. Abundjani Bangko. This concern arose since the budget deficit managed by the BPJS, resulting in delays in disbursing claims for patient services to BPJS participants and this affected the operations of hospitals in giving and maintaining the quality of health services. To ensure the operation of health services in hospitals, management must have a marketing strategy in the BPJS era so that hospitals can continue to provide services and not cause complaints from customers or patients. This study tries to describe the marketing strategy with the marketing mix pattern that includes 7P (product, price, promotion, place, people, process, and physical facility). The purpose of this study was to see an overview of the application of the marketing strategy by means of the marketing mix (marketing mix) of the health services of RSD Kol.Abundjani Bangko era BPJS. The research results obtained are with the marketing mix strategy. RSD Kol. Abundjani Bangko can still provide quality services to patients with conditions in the era BPJS that have a budget deficit
