Abstract

Diabetes melit us (DM) adalah penyakit kronis yang merupakan masalah kesehatan yang serius didunia. Masalah yang sering terjadi pada pasien DM adalah penurunan sensitivitas kaki yang dapatdipengaruhi oleh sirkulasi darah kurang opti mal yang berakibat lanjut ke neuropati sampai ulkus diabeti k. Preval ensi neuropati pada DM tipe 1 berkisar antara 8-54% dan pada DM tipe II 13-46%.Upaya meningkatkan sensitivitas kaki dan supaya tidak terjadi komplikasi dapat dilakukan dengan perawatan kompl ementer yaitu dengan terapi pijat refleksi telapak kaki. Penelitian bertuj uan untukmengetahui pengaruh t erapi pijat refleksi telapak kaki terhadap sensitivitas kaki pada pasi en diabet esmelitus tipe 2. Penelitian ini menggunakan rancangan pra-eksperimen dengan pre- and post-testdesign. Sampel penelitian adalah 44 responden penderita DM tipe 2 yang dit etapkan dengan tekni kpurposive sampling. Sebelum dan sesudah dilakukan intervensi pijat refleksi telapak kaki selama 3hari berturut-turut dilakukan pengukuran sensitivitas kaki menggunakan monofilament 10 gram.Tekni k analisa data menggunakan uji Wilcoxon. Hasil menunjukkan sesudah intervensi terapi pijatrefleksi telapak kaki terdapat peningkatan sensitivitas kaki yang signitifi kan (p-value 0,000). Meansensitivitas kaki kanan 3,04 dan mean sensitivitas kaki kiri 2,90. Terapi pijat refleksi telapak kakidapat diterapkan untuk mengurangi resi ko neuropati dan dapat mencegah komplikasi yang berakibatulkus diabetik maupun amputasi.Kata kunci : terapi pijat refl eksi telapak kaki, sensitivitas kaki, diabetes melitus tipe 2

Abstract

AbstrakPasien yang dirawat di ruang Intensive Care Unit (ICU) merupakan pasien kritis yang dalam keadaan terancam jiwanya karena kegagalan atau disfungsi pada satu atau multipel organ yang disertai gangguan hemodinamik. Pasien kritis dalam keadaan penurunan kesadaran memiliki keterbatasan dalam mobilisasi, yang berdampak terhadap tekanan darah dan saturasi oksigen yang tidak stabil. Salah satu intervensi yang dapat dilakukan untuk menangani hal tersebut dengan mobilisasi progresif level I berupa head of bed, ROM, dan rotasi lateral. Penelitian ini bertujuan untuk mengetahui pengaruh mobilisasi progresif level I terhadap tekanan darah dan saturasi oksigen pada pasien kritis dengan penurunan kesadaran di ruang ICU. Metode penelitian ini menggunakan pra eksperimental dengan rancangan pre-test and post-test one group design. Responden penelitian ditetapkan dengan non probability sampling dengan metode total sampling. Penelitian dilaksanakan di ruang ICU pada 15 responden yang terdiri dari 10 responden perempuan dan 5 responden laki-laki dan memenuhi kriteria inklusi. Uji dependent t-test menunjukkan ada pengaruh mobilisasi progresif level I terhadap tekanan darah sistolik (p = 0,024), tekanan diastolik (p = 0,002), dan saturasi oksigen (p = 0,000). Mobilisasi Progresif Level I dapat meningkatkan tekanan darah dan saturasi oksigen pada pasien kritis dengan penurunan kesadaran. Mobilisasi Progresif Level I dapat dijadikan salah satu intervensi keperawatan untuk meningkatkan tekanan darah dan saturasi oksigen pasien kritis dengan penurunan kesadaran dengan tekanan darah di bawah normal. Kata kunci: Mobilisasi progresif level I, tekanan darah, saturasi oksigen, pasien kritis. AbstractThe effect of progressive level I mobilization on blood pressure and oxygen saturation in critical patients with decreased awareness. Patients who are treated in the Intensive Care Unit (ICU) are critical patients who are in danger of failure or dysfunction in one or multiple organs accompanied by hemodynamic disturbances. Critical patients in a state of reduced consciousness have limitations in mobilization, which have an impact on blood pressure and unstable oxygen saturation. One intervention that can be done to deal with this is with progressive level I mobilization in the form of head of bed, ROM, and lateral rotation. This study aims to determine the effect of progressive level I mobilization on blood pressure and oxygen saturation in critical patients with decreased awareness in the ICU. This research method uses pre-experimental design with pre-test and post-test one group design. Research respondents were determined by non-probability sampling with total sampling method. The study was conducted in the ICU room in 15 respondents consisting of 10 female respondents and 5 male respondents and met the inclusion criteria. The dependent t-test showed that there was an effect of progressive level I mobilization on systolic blood pressure (p= 0.024), diastolic pressure (p= 0.002), and oxygen saturation (p= 0.000). Level I Progressive Mobilization can increase blood pressure and oxygen saturation in critical patients with decreased consciousness. Level I Progressive Mobilization can be used as one of the nursing interventions to increase blood pressure and oxygen saturation of critical patients with decreased consciousness with below normal blood pressure. Keywords: Progressive level I mobilization, blood pressure, oxygen saturation, critical patients

Abstract

Background: Chronic obstructive pulmonary disease refers to chronic diseases, such as emphysema, chronic bronchitis, asthma, or a combination of them. The observed symptoms of chronic obstructive pulmonary disease include breathlessness, wheezing, repeated respiratory tract infection, losing weight, and prolonged fatigue. Objectives: This research determined the influence of a 6-principle energy conservation technique on the respiratory frequency and oxygen saturation of Chronic Obstructive Pulmonary Disease (COPD) patients. Methods: This quasi-experimental research applied one group pretest-posttest design. The research design was useful to measure the respiratory rate and oxygen saturation before and after the implementation of 6-principle energy conservation. The researcher involved 29 respondents taken by accidental sampling. Results: The result showed the frequency mean before the intervention was 26.34 times per minute and after the intervention was 23.83 times per minute. The oxygen saturation, SpO2, before the intervention, was 97.17% while after the intervention was 96.76%. The Wilcoxon test found the influence of a 6-principle energy conservation technique toward the respiratory rate, a p-value of 0.000. On the other hand, the researcher did not find the influence of the 6-principle energy conservation technique on the COPD patient (p-value 0.944). Conclusion: The researcher recommends the implementation of 6-principle energy conservation as the non-pharmacological intervention alternative to manage the abnormal respiratory rate of COPD patients.

Abstract

Background: critical patients often experience sleep disturbances, namely the time that is too short to sleep. Changes in the characteristics and sleep time of critical patients can cause worsening of the disease and increase the risk of death. Sleep in critical patients has a very important function in the repair mechanism of damaged tissue cells (natural healing mechanism).Purpose: to determine the relationship between disease response factors, environmental factors, and nursing procedure factors on the sleep quality of patientsMethods: this cross-sectional study took place at a tertiary hospital, from March to May 2023, including 50 adults. We collected demographic and clinical data and utilized the Richards-Campbell Sleep Questionnaire (RCSQ) and the Sleep Disorders Factors Questionnaire (KFGT) scale to evaluate sleep qualityResults: the study showed a significant relationship between physical factors and sleep quality in patients with sig. 0.001 (sig. 0.05) and shows a strong relationship (p value = 0.84). Abstract

Introduction. Type 2 diabetes mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia, which causes complications and affects quality of life. Environmental factors influence the quality of life of DM patients. This study aimed to determine the relationship between environmental factors and the quality of life of type 2 DM patients at the Manyaran Public Health Center, Semarang City. Method. This research was quantitative study with a cross-sectional approach. Sixty-three respondents were involved in this study, which was determined using a simple random sampling technique. The Perceived Neighborhood Environment questionnaire measures environmental factors, and DQOL measures quality of life. Result & Analysis. The analysis results showed that most respondents were >65 years old at 42.9%, 61.9% were female, and 38.1% had primary school as their last education. The analysis results also showed that most respondents have good environmental factors (98.4%) and a high quality of life (79.4%). Discussion. The results of the Spearman Rank test stated that there was a significant relationship between environmental factors and the quality of life of type 2 DM patients at the Manyaran Public Health Center (p-value 0.000), with the strength of moderate and positive (r = 0.467), where the better the environment, the higher the quality of life.

Abstract

Drug regimens for CKD with comorbidities and complications require long-term treatment so that it can interfere with lifestyle and can affect treatment adherence. The purpose of this study was to determine the relationship between regimen complexity and medication adherence in patients with chronic kidney failure in the hemodialysis room of Tidar Hospital, Magelang City. This study used a cross-sectional approach with a population of all patients who routinely undergo hemodialysis in the Hemodialysis Room of Tidar Hospital, Magelang City. The sampling technique used purposive sampling so that a sample of 60 respondents was obtained. Collecting data in this study using a questionnaire. Data analysis using the Kendall Tau correlation test b. The results showed that the majority were male, aged <45 years, the education level was mostly high school, with the most comorbid hypertension, the majority of respondents underwent hemodialysis for more than 24 months, the complexity of the regimen was mild, and the level of adherence to taking medication very obedient. The results of the Kendall Tau b test analysis found that the p value was 0.000, r = +0. 0. 673, so there was a strong relationship between regimen complexity and medication adherence in chronic kidney failure patients at Tidar Hospital, Magelang City. Based on this study, it is expected that patients with chronic kidney failure will continue to improve adherence to taking medication and undergo regular and disciplined hemodialysis therapy for the rest of their lives in order to achieve treatment success and minimize complications in patients with chronic kidney failure.

Abstract

Background: Hypertension is one of the most prevalent chronic diseases among the elderly, significantly affecting their quality of life. In Rejang Lebong, the prevalence of hypertension is 30.71%, with over 60% of the elderly population affected. Health-Related Quality of Life (HRQoL) in elderly individuals with hypertension includes physical, psychological, and social aspects. Family support is a form of interpersonal relationship characterized by attitudes, actions, and acceptance among family members. Families in Rejang Lebong often exhibit a patriarchal structure. Purpose: To examine the relationship between family support and the quality of life of elderly individual swith hypertension. Method: A cross-sectional approach, specially focusing on observational study. The study took place from July-August 2024, in Rejang Lebong, Bengkulu. The independent variable was family support the dependent variables is quality of life. Using purposive sampling and Lemeshow formula, 87 respondent were selected, who met specific inclusion and exclusion criteria. The analysis proceeded with a univariate test to determine the frequency distribution of respondents characteristics, followed by bivariate analysis using spearman rank test. Results: Family support respondents were mostly categorized as high 43 (49.4%) and had a moderate quality of life 64 (73.6%) and significance value (Sig.) <0.05. Conclusion: Indicates most respondent had high family support and moderate quality of life. There is a significant relationship between family support and all domains of quality of life in the elderly with hypertension.

Abstract

Latar Belakang : Pasien gagal ginjal kronis yang menjalani hemodialisa dan memiliki komorbid akan mendapatkan obat dengan jumlah banyak sehingga menyebabkan ketidakpatuhan pengobatan pada pasien. Upaya untuk meningkatkan kepatuhan pengobatan salah satunya dengan memberikan edukasi berbasis health belief model (HBM) dengan media leaflet kepada pasien hemodialisa. Tujuan : penelitian ini untuk mengetahui pengaruh edukasi berbasis HBM dengan media leaflet terhadap kepatuhan pengobatan pasien gagal ginjal kronis di ruang hemodialisa RSUD dr. Gondo Suwarno Kabupaten Semarang. Metode : Penelitian ini menggunakan pendekatan kuantitatif dengan desain Quasy-Experiment serta menggunakan rancangan pre-posttest with control group. Lima puluh enam responden yang menjalani hemodialisa ditetapkan dengan teknik purposive sampling dan dibagi dalam 2 kelompok. Pengambilan data menggunakan kuesioner kepatuhan pengobatan serta analisis data menggunakan uji t berpasangan dan uji t tidak berpasangan. Hasil : Hasil penelitian menunjukkan mayoritas responden berusia 45-65 tahun (lansia) dan berjenis kelamin laki-laki, sebagian besar berpendidikan SMP, sudah menjalani hemodialisa > 12 bulan, dan mayoritas memiliki komorbid hipertensi. Terdapat peningkatan rata-rata nilai kepatuhan pengobatan pada kelompok intervensi sebesar 33,96 dari 85,61 menjadi 119,57. Ada pengaruh perbedaan edukasi berbasis HBM dengan media leaflet terhadap kepatuhan pengobatan pasien gagal ginjal kronis yang menjalani hemodialisa (p = 0,000). Simpulan dan Saran : Edukasi berbasis HBM dengan media leaflet memiliki pengaruh terhadap peningkatan kepatuhan pengobatan pasien hemodialisa sehingga dapat digunakan sebagai intervensi untuk pasien hemodialisa untuk meningkatkan kepatuhan pengobatan.

Abstract

Diabetes Mellitus is a disease indicated by high blood glucose. This disease is a chronic disease that an individual suffers in his or her life. The psychological changes, such as stress suffered by Diabetes Mellitus type-2 patients could influence their blood glucose content instability. Thus, the patients require adaptive coping mechanisms to manage and adapt the stress. One of the influential factors of individual coping mechanisms is social support from family, friends, and significant other. This research examined the correlation between social support and individual coping mechanism of diabetes mellitus type-2 patients. This cross-sectional research applied consecutive sampling on 63 respondents suffering from diabetes mellitus type 2 in Gunungpati health care center. The researchers took the data with the Multidimensional Scale of Perceived Social Support (MSPSS) and Cope Inventory. The Pearson Product Moment obtained a p-value of 0.000 (< 0.05) with an r-value of 0.430, indicating a significant, strong, and positive correlation between social support with individual coping mechanisms in diabetes mellitus type-2 patients. This result suggested high social support led to high individual adaptive coping mechanisms. The researchers recommended nurses educate the families, friends, and close friends of diabetes mellitus type-2 patients so they could provide social support. This effort could realize adaptive coping mechanisms

Abstract

Audiovisual education is important to increase foot care compliance to prevent diabetic ulcers in DM Type 2 patients. This study examines the influence of audiovisual education on foot care compliance to prevent diabetic ulcers in DM Type 2. This is a quantitative research with a one-group pre-post-test quasi-experimental design at the working area of the Ngemplak Simongan Community Health Center. The sample consisted of 44 respondents selected by purposive sampling. Variables involved in this research are the audiovisual education and the foot care compliance. Foot care compliance was measured using the foot care compliance questionnaire. Based on the univariate analysis, the majority of respondents are female (72.2%), late elderly aged 56-65 years (43.2%), work as housewives (54.5%), and primary schooled (68.2%), also 50% suffered DMT2 for >5 years, 56.8% had hypertension complications, 79% did not smoke, and 33% did not have hypercholesterolemia history. The univariate analysis also stated that only 4.5% of respondents had foot care compliance behavior before intervention, then increased to 97.7% after intervention. Bivariate analysis stated that there is a significant influence of audiovisual education on compliance with foot care in people with type 2 DM at the Ngemplak Simongan Community Health Center, with p-value of 0.000. Healthy living practices can be influenced by appropriate health information. People with type 2 diabetes can try to avoid diabetic ulcers by learning about good foot care.
