Abstract

The National Health Insurance or JKN, which was launched on January 1 2014, is intended to increase public access to comprehensive and quality health services, so as to achieve universal health coverage (UHC). Effective promotive-preventive efforts will reduce the incidence of disease and have an impact on reducing the number of sick people and the number of people seeking treatment so that health financing is more efficient. Family planning (KB) services are part of promotive-preventive efforts. In the management of program contraceptive devices provided by the government in the last 3 years, various problems were still encountered, including shortages of contraceptive devices (stock out), accumulation of Alkon in storage warehouses, and Alkon distribution mechanisms that were not smooth. The research was conducted using a descriptive quantitative approach. Research location in East Lombok Regency taking into account the high vacancy of Alkon (Data F/II/KB online). Then, 31 First Level Health Facilities (FKTP) were taken, namely Community Health Centers, Doctor's Practices, Pratama Clinics and Type D Pratama Hospitals. The research was carried out for 6 months, namely May-October 2017. The results of the research show that in general the management of Alkon in the OPD-KB of East Lombok Regency has complied with technical instructions, but there are several mechanisms for providing, storing, distributing, recording and reporting as well as Alkon monitoring and evaluation requires clear and written Standard Operating Procedures (SOP) that refer to technical manuals, and are spread throughout the OPD-KB, UPT-KB, Health Faskes and Network areas, so that the management of the mechanism runs uniformly.

The search identified 342,847 articles, which were then screened for relevance based on document type, language, publication area, and open access status, narrowing the pool to 76 articles. Following a detailed eligibility assessment, 17 articles were included in the final analysis. These articles covered various aspects of hospital operations, including leadership and management practices, health promotion initiatives, technological innovations, and patient satisfaction. Key findings highlight the critical role of adaptive and strategic leadership in enhancing hospital efficiency and patient care quality. Health promotion initiatives were found to be essential for improving population health, especially when integrated with technological advancements. The implementation of advanced health information technologies, such as electronic health records and digital alerts, significantly improved service efficiency and patient outcomes. Patient satisfaction, influenced by the quality of care, personalized interactions, and supportive environments for healthcare professionals, emerged as a fundamental component of effective healthcare delivery. The implications of this study suggest that hospital administrators and policymakers should prioritize adaptive leadership, integrate health promotion with technology, invest in advanced health information systems, and focus on enhancing patient satisfaction through improved care quality and supportive environments. These strategies are critical for achieving sustainable improvements in hospital health services and overall patient outcomes.

Abstract

This study aims to identify the dominant factors that influence the performance of health services at the UPT BLUD Puskesmas Pemenang. This study used a cross-sectional design with 60 respondents. Data collection was carried out through questionnaires. Data analysis used Spearman correlation to test the relationship between variables such as competence, discipline, and service facilities, and their impact on service quality. The results showed that (1) the competence and discipline of health workers were the most significant factors influencing service performance; (2) competence showed a strong positive correlation with service performance (ρ = 0.828, p < 0.001), which means that skilled staff can provide more effective services; (3) discipline had a substantial positive impact (ρ = 0.760, p < 0.001), which means that the importance of compliance with protocols and schedules; (4) the availability of adequate facilities had a moderate effect on service performance (ρ = 0.342, p = 0.008). This confirms that improving the performance of health services in Community Health Centers requires a comprehensive approach, so that continuous professional development to improve staff competence, discipline enforcement, and strategic investment in service infrastructure is very important.

Abstract

Background: Self-medication is an act of self-medication without medical consultation, the practice of self-medication in the people of West Nusa Tenggara reaches 66.62% and the community in Bima City is indicated to be 60.44%. This practice risks causing serious side effects and drug resistance. Purpose: To explore why the practice of hard drug self-medication is high in the community of Bima City. Method: Employing a descriptive qualitative approach with an emphasis on exploratory research. Conducted in Bima City starting in August 2024, it involved a total sampling method, selecting 10 participants who served as key informants from the National Agency of Drug and Food Control. Data collection was carried out through triangulation, integrating multiple sources, methods, and theories to enhance result validity. In-depth interviews were used as the primary data collection technique, and the data were analyzed using thematic analysis to extract key insights and interpret the underlying meaning of the findings. Results: Self-medication practices, such as buying antibiotics without a prescription, risk causing drug resistance and side effects. Monitoring drug distribution, especially online purchases, is a challenge for the Health Office and National Agency of Drug and Food Control. People choose practical and cost-effective ways, while pharmacists face pressure in selling drugs without a prescription. Collaboration and education are needed to ensure safe and rational drug use. Conclusion: The high cost of medicine and queues at health facilities encourage self-medication, exacerbated by poor Social Security Administration services. 24-hour pharmacies and online purchasing make it easier to access drugs without a prescription, with weak distribution oversight. Lack of accurate information and distrust of formal health services increase the risk of side effects and drug abuse.
