Abstract

Introduction: Low birth weight (LBW) infants deal with various problems during transitional period from intra-uterine and extra-uterine because of immature organs’ functions. This leads to LBW as the second death cause in Indonesia, particularly in the fi rst seventh days of infants’ lifes. The problem continues to occur at home when the infants have discharged. This research was aimed to develop the nursing care services model for LBW infants and to test the model.Method: The research design was an action research using quantitative and qualitative approach. This design was chosen as it facilitated improvement in health care system, which was involving nurses and other health providers.Results: Nursing care services provided by the nursing team are hindered by several factors, such as various level of nurses’ knowledge, not optimal health education activities, incomplete standard operational procedure, ethical dilemma, paramedic functions, and documentation system. This model was developed based on conservation and becoming a mother/maternal role attainment theory, family-centered care principles, and input from the experts through focus group discussion.Conclusion: The result of this research is going to increase the quality of nursing care for LBW infants by achieving nurses’ and parents’ satisfaction in giving care for their infants which can lead to lower infant death rate.

Abstract

Anak kanker perlu dipasang infus sebagai akses kemoterapi. Pemasangan infus menyebabkan kecemasan dan nyeri pada anak. Model Adaptasi Roy sebagai pendekatan dalam asuhan keperawatan dapat mengoptimalkan metode distraki, yaitu dengan cara memodifikasi stimulus fokal, kontekstual dan residual. Tujuan penelitian ini untuk menganalisis distraksi sebagai upaya mengurangi kecemasan dan nyeri saat pemasangan infus, dengan metode studi kasus pada 5 kasus anak kanker yang dipasang infus. Penelitian dilakukan pada Januari-Maret 2020 di RSCM. Hasilnya menunjukkan pada anak usia muda distraksi yang dapat digunakan untuk mengurangi kecemasan dan nyeri adalah dekapan ibu, pada anak usia sekolah dapat menonton video sebagai metode distraki, dan pada remaja dapat dikombinasikan dengan teknik relaksasi. Distraksi terbukti mengurangi kecemasan dan nyeri saat pemasangan infus, dimana skala kecemasan dan nyeri menurun. Skala nyeri sudah 0 pada kelima anak, dan skala kecemasan pada empat dari 5-10 menjadi 2 dan satu anak dari 10 menjadi 5 . Namun, perlu teknik distraksi yang dapat digunakan pada rentang usia yang beragam, misalnya dengan alat bantu kombinasi vibrasi dan sensasi dingin berupa alat bantu buzzy.

Abstract

Abstract The priority of handling children with chronic illness is to improve the quality of life by paying attention the symptoms that arise during treatment, one of which is the sleep problem. The purpose of this study to identify the effect of music therapy for the sleep quality of children with chronic illness. This study used a quasi experiment. Sample size was 30 childrens were divided into experimental group and control group. Respondents were given music therapy in 30-45 minutes before bed for 4 days. Sleep quality is measured by using Pittsburgh Sleep Quality Index (PSQI). There was a significant mean difference regarding PSQI score in experimental and control group. It was found that the experimental group score was higher compared with the control group (p=0,001). Thus music therapy could be used as an alternative intervention to improve the sleep quality to improve the quality of life of children with chronic illness. Abstract

Mothers who have good knowledge will support the acceleration of pneumonia recovery of toddlers. The study aimed to examine the effectiveness of mobile phone-based audiovisual education on increasing knowledge and attitudes among mothers in treating toddlers with pneumonia. The quasi-experimental study, pre-test, and post-test design with the non-equivalent control group were applied in this study. Forty-six samples collected using consecutive sampling with questionnaires analyzed by the Fisher Exact Test. There are differences in the knowledge and attitudes of mothers in the intervention group before and after health education (knowledge: p= 0.017; attitude: p= 0.003). But in the control group, it is only knowledge that is significantly different (p= 0.030). Knowledge and attitudes in the intervention and control groups have different statistical values ??after health education (knowledge: p= 0.018; attitude: p= 0.384). Conclusion: There is a correlation between education and experience with knowledge of mothers in treating toddlers with pneumonia, but there is no difference in mothers' attitudes. Concerning age, family income, and information, there is no difference in mothers' knowledge and attitudes. Abstract

Objective: to illustrate the application of the Theory of Unpleasant Symptoms (TOUS) in children who experience nausea caused by chemotherapy and analyze their effectiveness. TOUS has three components: symptoms, factors affecting the client and the final appearance.  Methods: The application process begins with an analysis of the theory, preparation of nursing care format of assessment and evaluation, implementation of the application of theory to the three clients and analyzing the effectiveness of the use of theory. Results: Results of the implementation of applications, namely TOUS theory can be applied to the nursing care of children who have cancer pain and pain related to diagnostic procedures for the management of symptoms through TOUS based, needs a sense of comfort the child will be met despite the pain they still appear for hospitalized children.  Conclusion: Application TOUS on nursing care to children should pay attention to the psychological aspects of growth and development and child. Both aspects are proven to affect the shape of pain and symptom management may affect the implementation of nursing care in general.

Abstract

Continuous Ambulatory Peritoneal Dialysis (CAPD) therapy is one of substitute for renal therapy. Children undergoing CAPD therapy show good survivability. The roles of nurses in the CAPD unit are integral to this success. This study used a qualitative descriptive phenomenological approach and included as participants seven nurses with experience in CAPD therapy in children. Data were collected through in-depth interviews and were analysed by the Collaizi method. The results identified six main tasks CAPD nurses perform: optimisation of therapy in children, coordination and collaboration with other health care providers, training families to apply CAPD therapy, facing various challenges while providing care, strategising care for each individual child, and dealing with their own attitudes and emotions. Procedures need to be optimised in a comprehensive manner to care for children receiving CAPD therapy.

Abstract

Background: Adherence to antiretroviral therapy (ART) can be challenging since it needs to be continued for a lifetime. At their age, adolescents start to be responsible for their life, and this continued therapy might be a challenge for them. Objective: This study explored the experiences of adherence to ART in adolescents living with HIV in Jakarta, Indonesia. Methods: A qualitative study with an Appreciative Inquiry (AI) approach was employed to explore challenges adolescents face in ART adherence which focused more on positive aspects of the experience. In-depth interviews were conducted with ten adolescents who were selected purposively based on criteria including those aged 13-19 years, having been diagnosed with HIV infection and receiving ART for more than a year, and never having discontinued ART. All participants were registered in the outpatient clinic in one top referral hospital in Jakarta. The data were analyzed using thematic analysis. Results: Five themes were identified from the data: living a normal life, wanting to be healthy, taking medication on time, challenges in undergoing treatment, and there is hope. Conclusion: Adolescents with HIV want to live as normal and healthy as possible, like other adolescents. Even though adolescents face several challenges to comply with ART, they try to take the medication as prescribed. The findings of this study serve as input for nurses to maintain compliance with ARVs in adolescents who have HIV.

Abstract

Kekerasan seksual di Indonesia terus meningkat dan Cirebon termasuk wilayah darurat kekerasan pada anak. Tujuan penelitian adalah mengidentifikasi pengaruh edukasi pencegahan kekerasan seksual terhadap persepsi anak usia prasekolah. Desain penelitian ini kuantitatif dengan pendekatan metode quasi experimental dengan pre-post test with control design melibatkan 240 responden meliputi 120 responden pada kelompok intervensi yang mendapat intervensi edukasi pencegahan kekerasan seksual dan 120 responden pada kelompok kontrol. Hasil analisis chi Square menunjukkan adanya pengaruh edukasi seksual terhadap persepsi anak usia prasekolah (p value < 0,05). Edukasi pencegahan kekerasan seksual pada anak usia prasekolah dapat direkomendasikan untuk masuk dalam kurikulum pendidikan usia dini dan keperawatan

Abstract

Tujuan: Fatigue merupakan gejala paling sering dialami oleh anak dengan kanker selama pengobatan dapat terjadi dikarenakan oleh kondisi medis, faktor biokimia maupun psikologis. Tujuan studi kasus ini untuk menganalisis pelaksanaan asuhan keperawatan pada anak dengan kanker yang mengalami masalah fatigue dengan menggunakan pendekatan Teori Kenyamanan Kolcaba. Metode: Metode yang digunakan yaitu studi kasus terhadap asuhan keperawatan dengan mengaplikasikan Teori Kenyamanan Kolcaba. Hasil: Intervensi yang diberikan berdasarkan prinsip Teori Kenyamanan Kolcaba, standard comfort, coaching, dan comfort food for the soul memberikan hasil yang cukup baik terhadap penurunan skor kelelahan pada anak dengan kanker. Simpulan: Asuhan keperawatan berdasarkan Teori Kenyamanan Kolcaba dapat digunakan pada anak kanker yang mengalami masalah fatigue. Intervensi yang dilakukan dengan pendekatan standard comfort, coaching dan comfort food for the soul diharapkan anak mampu beradaptasi dengan setiap respon tubuh yang terganggu hingga anak mencapai tujuan transcendence. Abstract

Chemotherapy is a cancer therapy that can prevent the development and spread of malignant cells, but causes various side effects (loss of appetite, nausea, vomiting, mucositis, changes in taste, pain, fatigue, dry mouth, diarrhea, constipation. These side effects result in discomfort for children with cancer, in more severe conditions it can worsen the child's nutritional status due to inadequate nutritional intake. Nursing care using the Kolcaba comfort approach can be applied to overcome children's discomfort cancer who experience nutritional imbalance problems. The method used is a case study. The comfort needs in the five management cases are assessed based on the physical, psychospiritual, environmental and sociocultural contexts. The comfort interventions provided include standards of comfort, coaching, and comfort food for the soul . Nursing evaluation of the problem of nutritional imbalance which is a physical aspect of the patient, namely, the comfort type for the five children is ease comfort and three of the five children also have transcendence type comfort for other aspects. Kolcaba's Comfort Theory can be applied to pediatric cancer patients who experience nutritional disorders and provides opportunities for nurses to develop innovative interventions to improve the quality of nursing care.
