Abstract

Pasien stroke akut sebagian tidak terdeteksi afasia, hal ini dilihat dari angka kejadian afasia yang dilaporkan. Mereka yang ditemukan sudah menggunakan gaya berbicara telegrafis karena keterlambatan mengenali tanda dan gejala afasia. Tentu hal ini meningkatan hari rawat, biaya perawatan, pasien mudah depresi dan menurunkan derajat hidup. Alat ukur untuk menilai afasia menjadi salah satu faktor keberhasilan deteksi dini. Namun, dari yang ada tergolong lama dan tidak melaporkan sensitivitas dan spesifitas sehingga dapat diragukan kesahihannya. Tujuan dari penulisan artikel ini adalah menemukan sensitifitas dan spesifitas alat ukur serta cepat dalam mendeteksi afasia pasien stroke akut. Metode yang digunakan literature review dari penelitian sebelumnya dengan mengekstrak 3 artikel Tahun 2010-2019 dalam bahasa Inggris yang membahas tentang identifikasi skrining tes afasia yang dipilih dari beberapa database. Language Screening Test sangat kuat dalam mendeteksi afasia stroke dilihat dari nilai sensitivitas dan spesifitas serta cepat dalam penilaiannya dengan waktu hanya 2 menit, namun instrumen ini tidak melaporkan onset stroke.

Abstract

Among stroke ischemic survivor, disability was contributed the stroke prevention and respon in medical treatment engagement when onset. Prevention stroke also was influenced the stroke knowledge. Stroke konowledge invarious population have studied. Meanwhile the the knowledge of stroke, risk factor,  symtom warning stroke, and respon to medical treatment engagement was not established in Indonesia. The aim of the study was to investigate the stroke knowledge among stroke ischemic survivor. Method was descriptive study. The sample were 78 samples. Quetionaire was Stroke Knowledge Test (SKT). Ethic was conducted by Ethic Committe of Medical Faculty Diponegoro University. The Result Showed that the knowledge of stroke, risk factor,  symtom warning stroke, and respon to medical treatment engagement were poor. This result gain the good insight in developing nursing intervention.

Abstract

Pasien kanker stadium lanjut atau tahap terminal secara umum mengalami kesulitan dalam penyembuhannya dan berakhir dengan kematian. Peran perawat sangat diperlukan dalam mempersiapkan kematian yang damai bagi pasien dengan kondisi ini dan keluarganya melalui perawatan menjelang ajal (end-of-life care). Sudut pandang terhadap kesiapan perawat dalam merawat pasien menjelang ajal dapat dijelaskan melalui sikapnya. Sikap perawat dapat mempengaruhi emosi dan perilaku perawat dalam merawat pasien terminal dan keluarganya. Penelitian ini bertujuan untuk mendeskripsikan sikap perawat terhadap persiapan kematian padapasien kanker stadium lanjut. Jenis penelitian ini adalah kuantitatif noneksperimental deskriptif survei. Teknik pengambilan sampel yang digunakan dalam penelitian ini adalah total sampling dengan jumlah sampel sebanyak 145 orang responden. Data penelitian ini dikumpulkan menggunakan kuesioner The Frommelt Attitude Toward Care of the Dying Scale (FATCOD). Hasil penelitian ini menunjukkan 76 orang dari 145 perawat (52,4%) memiliki sikap positif terhadap persiapan kematian yang damai bagi pasien kanker stadium lanjut. Hasil analisis lebih lanjut menunjukkan kedua domain sikap perawat mayoritas positif yakni 53,8% perawat memiliki sikap positif terhadap persiapan kematian bagi pasien kanker tahap terminal dan 57,2% perawat memiliki sikap positif terhadap persiapankeluarga menghadapi kematian pasien kanker stadium lanjut. Intervensi keperawatan bagi pasien kanker stadium lanjut menjelang ajal dan keluarga perlu ditingkatkan. Sangat penting untuk perawat berkomunikasi dengan pasien dan keluarga mereka dalam membahas tentang masalah akhir kehidupan untuk meningkatkan kualitas hidupnya dan mempersiapkan kematian yang damai sertaberkualitas.Patients with advanced cancer are generally difficult to be healed and end up in death. Nurses' roles are needed in preparing the patient's death through end-of-life care. The nurses' viewpoint of readiness in caring for a dying patient can be explained through their attitudes. Nurses' attitudes will affect the nurses' emotions and actions while conducting end-of-life care for dying patients and their families. This study aims to determine the nurses' attitudes toward death preparation in advanced cancer patients at RSUP Dr. Kariadi Semarang. The type of this researchis descriptive quantitative non-experimental. The sampling technique used was total sampling with a sample of 145 respondents. Data was collected by using The Frommelt Attitude Toward Care of the Dying Scale (FATCOD) questionnaire. The results showed 76 of 145 nurses (52.4%) had positive attitudes toward death preparation. The result of the analysis of two domains showed 53.8% of nurses had positive attitudes toward death preparation in advanced cancer patients and also 57.2% of nurses had positive attitudes toward death preparation in families of advanced cancer patients. Nurse interventions are still needed to improve communication with patients and their families in discussing about end-of-life issues for improving the quality of life and good death.

Abstract

Background: Post-stroke depression is one of the psychological disorders that is often experienced by post-stroke patients. Depression is more than just sadness. People with post-stroke depression will experience a lack of interest and pleasure in daily activities, weight loss or excessive sleep, lack of energy, inability to concentrate, feelings of worthlessness or guilt and recurring thoughts of death or suicide. Based on several types of post-stroke depression management, psychosocial interventions have been shown to reduce the likelihood of developing post-stroke depression. One of the interventions to reduce post-stroke depression is cognitive behavioral therapy. Objective: This scoping review aims to identify the effectiveness of cognitive behavioral therapy to reduce depression in post-stroke patients. Design: This study used a scoping review through a review of articles on cognitive behavioral therapy that has been used to reduce depression in post-stroke patients. Data Sources: Search for articles accessed through a database, including: CINAHL, MEDLINE, Academic search ultimate, PubMed, and google scholar with a range of search years 2011-2020. Review Methods: Quality appraisal assessment for each selected study was conducted using the Preferred Reporting Items for scoping review (PRISMA) method. Results: The results of this literature review show that cognitive behavioral therapy to reduce depression in post-stroke patients can change thoughts, feelings and behaviors to influence each other. The relationship between thoughts, feelings and behavior greatly influences respondents in dealing with post-stroke conditions with a positive way of thinking so that it will accelerate or help the post-stroke recovery process. The way to implement cognitive behavioral therapy is to be given as many as 12 sessions in 25-35 minutes each session. Cognitive behavioral therapy can be done in a hospital and has a significant post-stroke depression rate with p <0.01. Conclusion: Cognitive behavioral therapy can be given as an intervention to reduce depression in post-stroke patients including cognitive behavioral therapy, duration of cognitive behavioral therapy and sessions in cognitive behavioral therapy.

Abstract

One of the psychological problems experienced by stroke patients is self-efficacy. Stroke patients on average lack confidence in themselves to do exercises or activities that will have an impact on the recovery process of stroke patients. Several factors that vary related to low self-efficacy in stroke patients are one of the causes. The scoping review aims to identify factors related to self-efficacy in a post-stroke patient. The methods use a systematic review through review articles relevant to the topic from CINAHL database, MEDLINE, Academic search ultimate, science direct, and Taylor & Francis with a publication year spanning 2015 to 2020 and with the keyword “self-efficacy” and “stroke” then analyzed using a synthesis matrix. The inclusion criteria in this study were articles in English with longitudinal study design or cross-sectional study and self-efficacy as the dependent variable. The results of a review of four journals that met the inclusion criteria found factors related to self-efficacy in a post-stroke patient including depression, age, vascular history, fear, physical activity, Body Mass Index (BMI) and emotional function. The study concludes that depression, age, vascular history, fear, physical activity, Body Mass Index (BMI) and emotional function are factors related to self-efficacy in post-stroke patients. The factor most strongly associated with post-stroke patients self-efficacy is depression.

Abstract

Background: Problems and uncertainty during online learning often cause negative emotions among nursing students, such as anxiety, anger, stress, and confusion. A comprehensive treatment is needed to help students cope with uncertain situations. One strategy that can be implemented is to regulate emotions using the Emotion Detection Information System (SIDE) application. SIDE contains some features of emotion detection and happy spiritual intervention for regulating emotions. Objective: This study aimed to determine the effect of the SIDE application on emotion regulation among nursing students. Methods: This research was a quantitative study with a one-group pretest-posttest design without a control group. This study described its data in one group before and after the intervention using a modified Emotional Regulation Questionnaire (ERQ). This study was conducted among 46 nursing students. Results: The paired sample t-test showed a significance level of 0.000 (p less than 0.005), indicating a significant effect of the SIDE application on the emotional regulation ability of nursing students. Conclusion: The SIDE application had a positive effect on emotional regulation among nursing students. The students had better emotional regulation abilities so that they could minimize the symptoms of mental disorders while facing uncertainty during the COVID-19 pandemic. Abstrak: Latar belakang: Problematika dan ketidakpastian selama pembelajaran daring seringkali menimbulkan emosi negatif seperti kecemasan, marah, stres, dan bingung pada mahasiswa keperawatan. Penanganan yang komprehensif diperlukan untuk membantu mahasiswa mengatasi situasi-situasi yang tidak menentu. Salah satu cara yang dapat dilakukan adalah pengaturan regulasi emosi dengan bantuan aplikasi Sistem Informasi Deteksi Emosi (SIDE). SIDE berisi beberapa fitur deteksi emosi serta intervensi happy spiritual dalam regulasi emosi. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh penerapan aplikasi SIDE terhadap regulasi emosi pada mahasiswa keperawatan. Metode: Penelitian ini merupakan penelitian kuantitatif dengan desain one group pretest-posttest without control. Dimana penelitian ini menggambarkan data pada satu kelompok sebelum dan sesudah diberikan intervensi dengan mengunakan alat ukur berupa kuesioner modifikasi The Emotional Regulation Questionnaire (ERQ). Hasil: Penelitian yang dilakukan kepada 46 mahasiswa keperawatan menunjukkan hasil uji paired sample t-test dengan nilai sig. 0,000 (p kurang dari 0,005). Hal ini menunjukkan adanya pengaruh yang signifikan dari penerapan aplikasi SIDE pada kemampuan regulasi emosi mahasiswa keperawatan. Kesimpulan: Penerapan aplikasi SIDE terhadap regulasi emosi mahasiswa keperawatan menunjukkan hasil adanya pengaruh yang positif, dimana mahasiswa keperawatan memiliki kemampuan regulasi emosi yang lebih baik sehingga dapat meminimalkan gejala gangguan mental selama menghadapi ketidakpastian di masa pandemi COVID-19.

Abstract

Stroke sufferers are faced with limitations both in terms of physical, psychological, and social aspects which cause sufferers to experience difficulties in carrying out activities. The cause of these difficulties is low self-efficacy which will have a negative impact on the health conditions and behavior of stroke sufferers. The purpose of the review is to describe how interventions related to self-efficacy and their effect on self-efficacy in stroke patients. The method used is scoping review by looking for relevant articles using keywords. The inclusion criteria in this review were participants with a diagnosis of stroke, the results of the study measured self-efficacy, the study was a randomized controlled trial, the year of publication was in the 2011-2021 period, and the study was in English. The results in the review are five articles that have significant results on self-efficacy and six articles have insignificant results. Articles that have significant results apply interventions with several strategies including education, training, motivation, and self-management. The conclusion of this review is an article that has significant results in implementing a complex intervention (SESSMP intervention, MSE training intervention, and self-management empowerment intervention) and a single intervention (Health Empowerment Intervention for Stroke Self-management and SFBT).

Abstract

This study aims to determine interventions that can be carried out to reduce the risk of falls and the incidence of falls in patients. The research method used is scoping review through databases, namely CINAHL, MEDLINE, Scopus, and Proquest. The results showed that out of 14 articles, 13 articles showed a reduced risk of falling or falling events. Interventions provided to prevent the risk of falling use technology, video media, posters, and e-learning modules. Forms of giving information include providing education, training, and webinars. Fall risk interventions are provided to patients, caregivers, multidisciplinary teams, families, and communities. The material provided varies, such as the nursing care process, medication fall safety, fall risk prevention strategies, forms of physical exercise, and technology systems. In conclusion, the method for preventing patients at risk of falling and falling must get support. Support can be done through training, education, and organizational support. Abstract

Hemiparesis and hemiplegia are motor deficits found mostly in stroke patients. If the deterioration in muscle strength that occurs is not handled properly, it will cause permanent disability. ROM therapy is a therapy to increase muscle strength which has undergone various developments according to the research that has been done. The aim of this article was to identify interventions using ROM therapy to improve muscle strength in post-stroke patients. The databases used were JCS, PMC, Science Direct, and PubMed. The search was limited to articles published in 2014 to 2019. The study criteria was for adult, quantitative comparisons between before and after intervention ROM therapy and full text article, published in peer-reviewed journal and written in English and Indonesian. This article used the methodological framework by Arksey and O'Malley for the scoping review. The research has conducted a study of 4 articles, in which all of the items discussed the application of ROM therapy to increase muscle strength in post-stroke patients. The results from each articles showed that ROM therapy has helped improve muscle strength in post-stroke patients.

Abstract

Stroke karena penyumbatan aliran darah otak sehingga terjadi perubahan sistem saraf dengan gejala hemiparase dan hemiplegi.Rehabilitasi stroke dengan terapi terintegrasi, salah satu akupresur.Tujuan penelitian ini mengidentifikasi pengaruh akupresur padapenderita stroke.Metode scoping review melalui tinjauan artikel relevan dengan topik dari database Scopus, EBSCO (CINAHL, MEDLINE), Science Direct dan Proquest, kriteria 10 tahun publikasi 2013-2023, kata kunci Stroke AND Acupressure kemudian dianalisis menggunakan matriks sintesis. Kriteria inklusi artikel berbahasa Inggris dengan desain randomized controlled trial, case study, quasy experimentdanfull text. Hasil review didapatkan 5 artikel yang ditelaah dengan pedoman PRISMA (Preferred Reporting Items for Systematic Reviews and MetaAnalyses). Artikel dianalisis dan dikelompokkan dalam matriks tabel.Akupresur dipilih berdasarkan titik akupuntur dan dimodifikasi sesuai pengalaman terapis.Terapi + 20 menit (1-2x/hari), 3-5x/minggu selama 4-8 minggu.Titik ST 36 (Zusanli), LV 3 (Taichong), GB 34 (Yanglinquan) dan UB 60 (Kunlun) meningkatkan fungsi motoric ekstremitas bawah.Titik titik Zusanli (ST36) dan titik Zulinqi (GB41) meningkatkan kekuatan otot, rentang gerak dan kontrol motorik. Titik Yifeng, Lianquan, Tiantu dan Taixi meningkatkan respon menelan. Titik auricular (3 titik) menurunkan kecemasan, depresi dan perbaikan kualitas hidup. Titik Baihui (GV20), Susanli (ST36), Hegu (LI4), Shenmen (HT 7) dan Quchi (LI11) meningkatkan kualitas hidup dan menurunkan kecemasan. Kesimpulan telaah jurnal menunjukkan akupresur memberikan efek perbaikan kondisi fisik dan psikososial stroke.
