Abstract

Background: Lifecycle-based health services are a strategic approach to primary care transformation in Indonesia, positioning the Integrated Health Post (Posyandu) as a crucial community-based service. However, the implementation of Posyandu still faces various obstacles, such as a lack of trained cadres, suboptimal health education, and limited educational facilities and media, particularly in reaching target groups. Purpose: To evaluate the implementation of integrated Posyandu and to increase the capacity of cadres through outreach and the development of educational media. Method: This community service activity was conducted in Kebon Baru Village in January 2025. Eight participants participated. The first stage involved identifying Posyandus based on the lifecycle, encompassing a systems approach, evaluating input, process, and output aspects, and observing the service process directly at the Posyandu. The second stage involved providing education and developing educational media to enhance cadre knowledge and facilitate knowledge transformation. Results: Data obtained showed that the level of knowledge of cadres before the outreach activity was categorized as good (4%, 50%) and poor (4%, 50%). Meanwhile, the level of knowledge after being given extension activities was in the good category as many as 7 (87.5%) and in the less category as many as 1 (12.5%). Conclusion: Integrated health post (Posyandu) activities have implemented life cycle or integrated services, which, based on the identification results, are actively running and have been awarded the "Purnama Posyandu" status. Community service activities, including providing counseling and creating educational media, have helped improve cadre knowledge and optimize their role in providing education to the community at the Posyandu and during home visits. Suggestion: Training is recommended for each cadre to ensure equitable knowledge and skills, so that life cycle-based Posyandus will provide increasingly optimal services, particularly health services, to the community. Keywords: Counseling; Health cadres; Integration of primary services; Integrated Service Post Pendahuluan: Pelayanan kesehatan berbasis siklus hidup merupakan pendekatan strategis dalam transformasi layanan primer di Indonesia, yang menempatkan Posyandu sebagai layanan berbasis masyarakat yang penting. Namun, pelaksanaan Posyandu masih menghadapi berbagai kendala, seperti kurangnya kader terlatih, belum optimalnya edukasi kesehatan, serta keterbatasan sarana dan media edukasi, terutama dalam menjangkau kelompok sasaran. Tujuan: Untuk mengevaluasi pelaksanaan posyandu terintegrasi dan meningkatkan kapasitas kader melalui penyuluhan serta pengembangan media edukasi. Metode: Pengabdian masyarakat ini dilaksanakan di Kelurahan Kebon Baru pada bulan Januari 2025. Kegiatan ini diikuti oleh 8 peserta. Pada tahapan pertama melakukan identifikasi posyandu berdasarkan siklus hidup meliputi pendekatan sistem yakni mengevaluasi dari aspek input, proses, dan output serta kegiatan observasi dengan memantau secara langsung proses pelayanan di Posyandu. Selanjutnya untuk tahapan kedua adalah memberikan edukasi dan pembuatan media edukasi untuk meningkatkan pengetahuan kader dalam melakukan transformasi pengetahuan. Hasil: Mendapatkan data bahwa tingkat pengetahuan kader sebelum diberikan kegiatan penyuluhan yang dalam kategori baik sebanyak 4 (50.0%) dan kategori kurang sebanyak 4 (50.0%). Sedangkan tingkat pengetahuan setelah diberikan kegiatan penyuluhan yang dalam kategori baik sebanyak 7 (87.5%) dan dalam kategori kurang 1 (12.5%). Simpulan: Kegiatan posyandu sudah menjalankan layanan siklus hidup atau terintegrasi yang berdasarkan hasil identifikasi sudah berjalan aktif dan telah mendapat strata posyandu purnama. Kegiatan pengabdian masyarakat melalui pemberian penyuluhan dan pembuatan media edukasi telah membantu meningkatkan pengetahuan kader dan mengoptimalkan peran kader dalam memberikan edukasi kepada masyarakat di posyandu maupun saat kunjungan rumah. Saran: Diharapkan untuk memberikan pelatihan kepada setiap kader sebagai upaya kesetaraan pengetahuan dan ketrampilan sehingga posyandu berbasis siklus hidup akan memberikan pelayanan yang semakin optimal khususnya pelayanan kesehatan kepada masyarakat.

Abstract

Background: Adolescent girls are a group vulnerable to anemia, with a global prevalence rate reaching 29.9%. Low adherence to iron supplementation is a major contributing factor, despite targeted distribution. Anemia in adolescent girls can impair concentration and impact future reproductive health. Therefore, ongoing intervention and guidance efforts are needed to strengthen anemia prevention in this group. Purpose: To increase knowledge about anemia and empower peer counselors as an effort to prevent anemia in adolescent girls. Method: This community service activity was carried out in December 2025 at a high school in Tebet District, South Jakarta. Participants in this activity were 119 female students in grades X and XII. The implementing team consisted of lecturers and students of the Faculty of Medicine, Trisakti University, who collaborated with the school and received assistance from Community Health Center staff. The material on anemia was delivered directly and followed by discussions and questions and answers as an effort to increase knowledge in participants, which included assessing the analysis of risk factors for anemia, and empowering peer counselors as an effort to prevent anemia. Evaluation of the level of knowledge and understanding of participants was measured using a questionnaire given before the educational activity (pre-test) and after the educational activity (post-test). Data were analyzed using the Wilcoxon test and Chi-square test to see the relationship between risk factors and the incidence of anemia. Results: Obtaining data that the average age of participants is 16.87 years with a standard deviation of 1.02 years in the range of 14-18 years and most participants are at the age of 17 years as many as 49 people (41.2%). Most are grade XII students as many as 85 people (71.4%), the majority have normal nutritional status as many as 49 people (41.2%), and the educational status of the mothers of the participants is the majority of high school-college graduates as many as 96 people (80.7%). While most of the parents' monthly income is ˂Rp. 5,396,761 as many as 81 people (68.1%), most are not identified as anemia as many as 87 people (73.1%), and the majority do not routinely consume iron tablets once a week as many as 78 people (65.5%). There is an increase in the level of knowledge of respondents after the implementation of health counseling which gets pValue=0.001. Meanwhile, the average value of respondents' knowledge before the counseling (pre-test) was 6.30 points and increased to 9.37 points after the counseling (post-test). Conclusion: Health education activities contribute to increasing adolescents' knowledge about anemia and its prevention. Adherence to iron supplementation plays a significant role in reducing the risk of anemia in adolescent girls. Furthermore, the implementation of school-based peer counselors is an effective promotive and preventive strategy because it facilitates increased knowledge, attitudes, and behaviors related to anemia prevention through a participatory approach. Suggestion: Anemia prevention activities for adolescent girls are expected to be carried out sustainably through increasing IBT compliance, health education, and supporting the ongoing role of peer counselors as health promotion agents. Active collaboration programs are also expected between community health centers, adolescents, and schools. Keywords: Adolescent girls; Anemia prevention; Health education; Peer counselors Pendahuluan: Remaja putri termasuk kelompok yang rentan mengalami anemia, dengan angka prevalensi secara global mencapai 29.9%. Rendahnya kepatuhan konsumsi tablet tambah darah (TTD) menjadi faktor utama, meskipun distribusi telah berjalan sesuai target. Anemia pada remaja putri dapat mengganggu kemampuan konsentrasi belajar dan berdampak pada kesehatan reproduksi di masa mendatang, sehingga diperlukan upaya intervensi dan pembinaan yang berkesinambungan untuk memperkuat pencegahan anemia pada kelompok ini. Tujuan: Untuk meningkatkan pengetahuan tentang anemia dan pemberdayaan konselor sebaya sebagai upaya pencegahan kejadian anemia pada remaja putri. Metode: Kegiatan pengabdian kepada masyarakat ini dilaksanakan pada bulan Desember 2025 di salah satu Sekolah Menengah Atas di Kecamatan Tebet, Jakarta Selatan. Responden dalam kegiatan ini adalah siswi kelas X dan XII yang berjumlah 119 orang. Tim pelaksana terdiri dari dosen dan mahasiswa Fakultas Kedokteran Universitas Trisakti yang berkolaborasi dengan pihak sekolah serta mendapat pendampingan dari staf Puskesmas. Materi tentang anemia disampaikan secara langsung dan dilanjutkan dengan diskusi serta tanya-jawab sebagai upaya peningkatan pengetahuan pada responden yang meliputi penilaian analisis faktor risiko kejadian anemia, dan pemberdayaan konselor sebaya sebagai upaya pencegahan anemia. Evaluasi tingkat pengetahuan dan pemahaman responden, diukur menggunakan kuesioner yang diberikan sebelum kegiatan edukasi (pre-test) dan sesudah kegiatan edukasi (post-test). Data dianalisa dengan uji Wilcoxon dan uji Chi-square untuk melihat hubungan antara faktor risiko dan kejadian anemia. Hasil: Mendapatkan data bahwa rata-rata usia responden adalah 16.87 tahun dengan standar deviasi 1.02 tahun dalam rentang 14-18 tahun dan sebagian besar responden berada di usia 17 tahun sebanyak 49 orang (41.2%). Sebagian besar merupakan siswi kelas XII sebanyak 85 orang (71.4%), mayoritas memiliki status gizi normal sebanyak 49 orang (41.2%), dan status pendidikan ibu dari responden adalah mayoritas lulusan SMA-Perguruan Tinggi sebanyak 96 orang (80.7%). Sedangkan sebagian besar penghasilan orang tua per bulan (UMR) adalah ˂Rp.5.396.761 yaitu sebanyak 81 orang (68.1%), sebagian besar tidak teridentifikasi anemia sebanyak 87 orang (73.1%), dan mayoritas tidak rutin konsumsi tablet tambah darah 1x seminggu yaitu sebanyak 78 orang (65.5%). Adanya peningkatan tingkat pengetahuan responden setelah pelaksanaan penyuluhan kesehatan yang mendapatkan pValue=0.001. Sedangkan nilai rata-rata pengetahuan responden sebelum penyuluhan (pre-test) sebesar 6.30 poin dan terjadi peningkatan menjadi 9.37 poin setelah penyuluhan (post-test). Simpulan: Kegiatan penyuluhan kesehatan berkontribusi terhadap peningkatan tingkat pengetahuan remaja mengenai anemia dan upaya pencegahannya. Kepatuhan konsumsi tablet tambah darah (TTD) memiliki peran signifikan dalam menurunkan risiko anemia pada remaja putri. Selain itu, implementasi konselor sebaya berbasis sekolah merupakan strategi promotif dan preventif yang efektif karena mampu memfasilitasi peningkatan pengetahuan, sikap, dan perilaku pencegahan anemia melalui pendekatan partisipatif. Saran: Diharapkan, kegiatan pencegahan anemia pada remaja putri untuk dilakukan secara berkelanjutan melalui peningkatan kepatuhan TTD, edukasi kesehatan, dan untuk mendukung keberlanjutan peran konselor sebaya sebagai agen promosi kesehatan serta diharapkan juga mengadakan program kolaborasi aktif dari puskesmas, remaja dan sekolah.
