Abstract

Ghrelin lebih dikenal sebagai hormon lapar atau hormon yang mengatur siklus kenyang-lapar mahkluk hidup. Fungsi reproduksi ghrelin mencakup aksi hipotalamus-hipofisis-gonad (ovarium dan testis) dan hormon reproduksi lain. Selain itu, ghrelin juga diketahui memiliki pengaruh pada proses patologis beberapa penyakit pada kehamilan dan infertilitas. Ghrelin is better known as a hunger hormone or a hormone that regulates the cycle of satiety- hunger. The reproductive function of ghrelin includes action to the hypothalamus-pituitary-gonad (ovary and testis) axis and other hormones. Ghrelin is also known to influence the processes of several diseases in pregnancy and infertility.

Infertilitas pada pria sering disebabkan oleh masalah kuantitas atau kualitas sperma. Antioksidan, salah satunya koenzim Q10, dilaporkan memiliki peranan memperbaiki kuantitas dan kualitas sperma. Infertility in men is often caused by sperm quantity and quality. Antioxidant, coenzyme Q10, has been reported to improve quantity and quality of sperm.

Abstract

Background: Converting Ego-dystonic homosexual to heterosexual is still controversial. homosexuals are encouraged to be accepted and do not require conversion. Currently, many cases of homosexuals are required to accept their identity and do not require conversion. However, there are times when patients want to be heterosexual and experience sexual dysfunction because of this orientation. Then, how a medical professional deal with such a case especially in the era of ease of information and technology? Choose to demand that the patient accept their sexual orientation or help the patient to become heterosexual. Objective: Here we present the case of a man, 44 years old, with a psychogenic erection with ego-dystonic homosexual disorientation, along with a review of the literature related to the case. Method: A case report with literature review Case/Result: We report 44-year-old man who has been married for six years complains of difficulty and decreased erection for 5.5 years. This man had a history of sexual intercourse with men during college but decided to become a heterosexual man and married his current wife. For the first six months, the patient performed oral sex with his wife. Then afterwards, the patient has difficulty getting an erection and even fails. The patient feels guilty toward his wife until their relationship becomes a crisis. The patient wants to commit to being a heterosexual man. So, the patient came to the andrology clinic for therapy. We applied intersystemic assessment approach to the patient. There is progress in the patient’s condition until now. Conclusion: Many experts say that homosexuality should be accepted and should not be converted. But in the end, the choice is returned to the patient, whether to choose to convert or not. Physicians only need to support the patient's therapy and choices so that he does not experience any mental disorders that ultimately endanger his life. Keywords: , , , ,

Abstract

Highlights Hyperspermia is often ignored, but it can affect fecundability by diluting sperm and reducing sperm concentration. Hyperspermia might be associated with genetic causes and male accessory gland infections, but no specific treatment is mentioned to treat this condition.   Abstract Background: Hyperspermia is defined as a seminal volume greater than 6.3 ml. This definition is often ignored by physicians, while it may affect fecundability. Objective: To report a case of an adult with hyperspermia that affects his fecundability and to discuss it with the scoping review's result. Case: A 30-year-old man came desiring to have a child. He had regular sexual intercourse with his wife for the last 3 months, after living in different cities before. Based on the semen analysis, it was found that he had hyperspermia (volume 8.2 ml) and oligozoospermia (sperm concentration 4.25 million/ml). After taking antioxidants, the seminal volume was lowered, and the couple got pregnant. Method: A scoping review was conducted by using guidelines of Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews (PRISMA-ScR). After a comprehensive search based on the inclusion and exclusion criteria, only 3 original research were retrieved due to the rarity of articles regarding this topic. Discussion: According to previous studies, hyperspermia can affect fecund-ability by diluting the sperm, causing sperm concentration to be decreased as in this case. Genetic mutation and male accessory gland infection may be contributing factors to hyperspermia. No consensus and studies about the treatment are available. Conclusion: Hyperspermia is a semen parameter alteration that needs to be taken care of to increase fecundability.

Abstract

Puberty is a transitional period experienced by humans. This period is closely associated with physical, psychological, and emotional changes. Adolescents' responses to puberty can vary. We conducted a qualitative study to determine the definition of puberty and opinions and feelings about facing or experiencing puberty. Our research was conducted in one junior high school by conducting interviews in two focus group discussions. We found two major themes in this study: first is the definition of puberty, and second is the feeling about puberty. Both themes consisted of several sub-themes. The respondents stated that puberty is a maturity period characterized by significant physical, psychological, and emotional changes. The respondents also expressed that they were surprised by the changes they experienced. In addition, for them, puberty is the beginning of new obligations in their social environment. Attraction to the opposite sex makes them more careful and they must be able to control themselves. The need to share friends and role models is one of their recognitions in puberty. Other than that sex education indeed for better information.  This study can be used as a reference for understanding the needs of adolescents regarding puberty so that they can better adapt and maintain sexual and reproductive health for good health and well being.

Abstract

Background: Erectile dysfunction (ED) affects 20-50% of men globally and considerably diminishes the patients’ and their partners’ quality of life. This condition has multifactorial causes, including depression and metabolic syndrome, resulting in a complicated interaction of physical and psychological components. Case: A man in his mid-twenties presented to the Andrology outpatient clinic at Dr. Soetomo General Hospital with a sudden onset of ED, reporting a decrease in his Erection Hardness Score (EHS) from 3-4 to 1 following penetration attempts. He had a history of childhood attention deficit hyperactivity disorder (ADHD) and recent unemployment, resulting in profound depression. Prior inconsistent treatments encompassed sertraline, fluvoxamine, and clobazam. Physical and laboratory examinations revealed metabolic syndrome. The patient was diagnosed with severe ED secondary to mixed etiology, class II obesity, major depressive episode, and metabolic syndrome. He received education on the diagnosis, possible underlying causes, and management options. Discussion: The case presented illustrates the intricate interplay between ED and comorbidities such as depression and metabolic syndrome. Depression exacerbates ED through hormonal dysregulation and reduced parasympathetic activity, while metabolic syndrome contributes to ED through endothelial dysfunction and nitric oxide (NO) depletion. Effective ED management requires a multidisciplinary approach, combining lifestyle modifications, psychological support, and pharmacological treatment. Tailored strategies addressing both physical and psychological aspects are essential for improving outcomes. Conclusion: Comprehensive and integrative care in managing ED, particularly when associated with complex comorbidities, is needed to enhance the patient’s erectile function and quality of life.

Abstract

Penyakit infeksi merupakan penyakit dengan insidensi tinggi di Indonesia sebagai negara tropis. Dewasa ini, berbagai penelitian tidak hanya seputar agen penyebab (bakteri, virus, parasit) ataupun faktor lingkungan saja, namun juga banyak melibatkan inang, termasuk genetika manusia. Pengabdian masyarakat ini bertujuan mengedukasi sasaran terbatas yaitu akademisi, mahasiswa, peneliti, dan klinisi di bidang kedokteran dan kesehatan di Indonesia tentang peran genetika manusia dalam penyakit infeksi. Pengabdian masyarakat dilakukan dengan membuat media daring audiovisual berupa tanya jawab bersama pakar dengan evaluasi setelah satu bulan dari pengunggahan video. Hasil pengabdian masyarakat menunjukkan bahwa penggunaan media daring audiovisual efektif dalam meningkatkan pemahaman akademisi, mahasiswa, peneliti, dan klinisi mengenai peran genetika manusia dalam penyakit infeksi. Diskusi dengan responden menghasilkan daftar pertanyaan yang relevan, yang kemudian dijawab oleh pakar melalui video edukatif. Video ini diunggah dan dibagikan kepada sasaran terbatas, memperoleh 48 penayangan serta respon positif dalam satu bulan. Evaluasi menunjukkan perlunya distribusi lebih luas untuk meningkatkan dampak edukasi. Model serupa dapat dikembangkan untuk memperluas jangkauan edukasi kedokteran berbasis daring. Pemanfaatan media daring audiovisual dalam pengabdian masyarakat bagi akademisi, mahasiswa, peneliti, dan klinisi di bidang kedokteran dan kesehatan meningkatkan pemahaman tentang peran genetika manusia pada penyakit infeksi. Untuk memperluas dampaknya, distribusi konten perlu diperluas. Model edukasi serupa dapat dikembangkan lebih lanjut untuk menjangkau masyarakat luas, seiring dengan kemajuan teknologi dan kebutuhan informasi kesehatan yang terus berkembang.
