Abstract

Fungal infection in oral cavity is mainly caused by C.albicans and now the case is reported increased. The increasing number of case is parallel with the increasing number of immunosupressed patients. Oleum azadirachty, the active agent of mimba (Azadirachta indica) which is categorized as a herbal medicine that has been used as antifungal drug in agricultural field, and is also found to have immunostimulatory effect in animal model. In addition, TNFα has been known to reduce the spreading of C. albicans and also important for granulocyte antifungal activity. The aim of this research study is to clarify the effect of oleum azadirachty to TNFα level in BALB/c mice cervical lymph node that immunosupressed and infected by C.albicans. This study used BALB/c mice that were devided in to six groups: which is consisted of three groups as control and three groups as studied groups. Mice were immunosupressed by given 2-3mg/mice subcutan injection of prednisolon one day before inoculation. Group 2 until group 6 were given tetracycline hydrochloride in drinking water (0,83mg/ml) since one day before inoculation. C.albicans inoculation was given at zero day. At 2nd day post infection, mice were sacrificed and TNFα level was measured by ELISA. Data were analysed by One Way Anova test and LSD (p=0.05). Result of this research indicated that Oleum Azadirachty could increse the level of TNFα

Abstract

As current technology cannot cure cancer completely, prevention becomes the main choice. To prevent the development of cell transformation into cancer cells, polyphenols that are widely found in cocoa beans (Theobroma cacao) can be used. IFNγ plays an important role in immunity against cancer. This study aims to see the ability of cocoa beans ethanol extract to increase the number of IFNγ expression in Balb/c mice undergoing cell transformation. This study used three groups Balb/c (n=4), namely K1 (normal control), K2 (negative control: injected with benzopyrene without ethanol extract of cocoa beans), and K3 (treatment: injected with benzopyrene, given 4mg/30gBW/po/day ethanol extract of cocoa beans). The mice were biopsied, and IFNγ expression was examined by immune histochemical method. The results showed that IFNγ expression increased significantly in K3. It can be concluded that ethanol extract of cocoa beans could increase IFNγ expression in Balb/c mice undergoing cell transformation.

Abstract

Majority of fungal infection in mouth is caused by C.albicans. This case increases parallel with increasing the number ofimmunosupressed patient. Mimba (Azadirachta indica) is herbal medicine that has been used as antifungal drug. Besidethat, mimba has immunostimulatory effect. Macrophage phagocytosis, it’s one of natural immunities that plays animportant role in the first immune response againts C.albican infection.The purpose of this research was to know theeffect of oleum azadirachty in macrophage phagocytosis activity of BALB/c mice that immunosupressed and infected byC.albicans. This laboratory experimental research used BALB/c mice that immunosupressed and infected by C.albicans,divided into three groups: group 1 (negative control), group 2 (positive control, were immunosupressed and infected withC.albicans), group 3 (was immunosupressed, infected with C.albicans, and given 75 μl oleum azadirachty). Mice wereimmunosupressed given by 2-3mg/mice subcutan injection of prednisolon one day before infected. Group 2 and group 3were given tetracycline hydrochloride in drinking water (0,83mg/ml) since one day before infected. C.albicansinoculation was given at zero day. On the 2nd day post infection, mice were acclimated and the macrophage phagocytosisactivity was evaluated by latex beat. Data was analysed by One Way Anova test and LSD (p=0.05). The result showedthat there was significant difference between group 2 and group 3. In conclusion, oleum azadirachty could increasemacrophage phagocytosis activity.

Abstract

Pendahuluan: Merokok merupakan salah satu kebiasaan yang sering dilakukan oleh masyarakat, termasuk kelompok lanjut usia (lansia). Kebiasaan merokok dapat mempengaruhi sekresi saliva pada lansia yang secara fisiologis mengalami penurunan sekresi saliva dan penipisan epitel mukosa mulut karena proses penuaan. Perubahan sekresi saliva dan penipisan epitel mukosa mulut dapat meningkatkan resiko infeksi rongga mulut seperti kandidiasis oral. Tujuan penelitian ini adalah untuk menganalisis perbedaan sekresi saliva melalui pengukuran laju aliran saliva serta terjadinya kandidiasis oral pada pasien lansia perokok dan bukan perokok, di Rumah Sakit Gigi Mulut  Universitas Jember (RSGM UNEJ). Metode: Penelitian ini merupakan analitik observasional pada pasien lansia RSGM UNEJ di Bagian Penyakit Mulut meliputi status merokok, pengukuran laju aliran saliva unstimulated, dan insidensi kandidiasis oral berdasarkan hasil pemeriksaan mikrobiologi oral swab. Populasi dalam penelitian ini adalah seluruh pasien lansia RSGM UNEJ Bagian Penyakit Mulut periode September 2019-Januari 2020. Metode pengambilan sampel yang digunakan adalah  total sampling. Hasil: Hasil uji Mann-Whitney menunjukkan tidak ada perbedaan signifikan antara sekresi saliva unstimulated pasien lansia perokok dan bukan perokok. Terdapat perbedaan kejadian kandidiasis oral antara pasien lansia yang merokok dengan yang tidak merokok (nilai p=0,065; derajat kepercayaan=90%). Simpulan: Tidak terdapat perbedaan laju aliran saliva pada lansia perokok dan bukan perokok, namun terdapat perbedaan terjadinya kandidiasis oral antara lansia perokok dan bukan perokok.Kata kunci: Laju aliran saliva, merokok, kandidiasis oral, lansia. ABSTRACTIntroduction: Smoking is one of the habits often carried out by the community, including the elderly. Smoking habits can affect salivary secretion in the elderly, who physiologically experience decreased salivary secretion and thinning of the oral mucosal epithelium due to the ageing process. The salivary secretion change and thinning of the oral mucosal epithelium can increase the risk of oral infections such as oral candidiasis. The purpose of this study was to analyse the differences in salivary secretion by measuring the salivary flow rate and the occurrence of oral candidiasis in elderly smokers and nonsmokers at the University of Jember Dental Hospital. Methods: This study was an observational analytic study in elderly patients of the Department of Oral Diseases University of Jember Dental Hospital, including the smoking status, measurement of unstimulated salivary flow, and the incidence of oral candidiasis based on the results of the oral microbiological swab. This study’s population were all elderly patients at the Department of Oral Diseases University of Jember Dental Hospital for September 2019-January 2020. The sampling method used was total sampling. Results: The Mann-Whitney test results showed no significant difference between the unstimulated saliva secretion of elderly smokers and nonsmokers. There was a difference in the incidence of oral candidiasis between elderly smokers’ patients and nonsmokers (p-value = 0.065; confidence level = 90%). Conclusion: There is no difference in the salivary flow rate in elderly smokers and nonsmokers, however, there are differences in the occurrence of oral candidiasis between elderly smokers and nonsmokers.Keywords: Salivary flow rate, smoking, oral candidiasis, elderly.

Abstract

Ulcer is an open lesion in the oral mucosa which is generally triggered by various causes including trauma, hormonal, infectious agents, nutritional deficiencies, and various other systemic abnormalities. Treatment is needed to prevent secondary infections, reduce inflammation, reduce pain and speed healing. Rosella efficacious to increase endurance, anti-inflammatory, antibacterial and antioxidant. This ability is obtained from flavonoids, tannins, phenols, anthocyanins and saponins. The purpose was to determine the effect of systemic rosella extract on healing time and ulcer diameter and to determine the effective dose of rosella extract on ulcer healing in wistar rats. 20 rats were ulcered on the left buccal by heating the amalgam stopper over the Bunsen fire for 30 seconds and touching for one second then waiting for 48 hours. All groups were condensed with the Vitamin C group (K+), the untreated group (K-) and the treatment group which consisted of 0.17 mg/gBB(P1), 0.33 mg/gBB(P2), and 0.66 mg/gBB(P3). The treatment is done twice a day and the measurement of diameter by placing a plastic filling instrument along the ulcer is then marked and measured with a digital calipers carried out every day until the ulcer heals. Healing time and decrease in ulcer diameter were faster in the treatment group than in the control group. Fastest ulcer healing in roselle extract dose 0.17 mg/gBB. The conclusion described Roselle extract with a dose of 0.17 mg/gBB gives the most effective effect on ulcer healing.

Abstract

Ulcer is a lesion that is often found in the oral cavity that causes discomfort, pain, difficulty speaking and difficulty eating. Nowdays, herbal ingredients are widely used as alternative medicine choice besides drugs on the market. Pandan leaf contains flavonoids, alkaloids, saponins, and polyphenols which have antiinflammatory, antibacterial, and antioxidant abilities, allowing faster healing of ulcers. Purpose of this study was to determine the effect of pandanus leaf extract gel on healing ulcers and determine the most effective concentration in healing ulcers. This study used 25 Wistar rats divided into 5 groups, which were applied CMC-Na gel, aloclair gel, 25%, 50% dan 75% pandan wangi leaf extract gel. Each rat was made ulcer by heating the stopper amalgam on the bunsen fire for 30 seconds, then touching to the left buccal mucosa for 1 second, and waiting for 2x24 hours to form an ulcer. Giving the gel every morning and evening and measure the diameter of ulcers every day. This study showed healing time and decrease ulcer diameter pandanus leaf extract gel faster than negative control. The average recovery day was negative control for 9.8 days, positive control for 8 days, pandanus leaf extract gel concentration 25% for 8.6 days, concentration 50% for 8 days, and concentration 75% for 6.8 days. Pandan leaf extract gel accelerated ulcer healing and 75% concentration is the most effective.

Abstract

Background: A study with female animal models is important because the system immune of females is remarkably different from the male because of interaction between sex hormone and immune system. Interleukin-17 (IL-17) plays an important role in immune response toward Candida albicans (C. albicans) infection and Erythrocyte Sedimentation Rate (ERS) is an easy and sensitive test to assess the inflammation. Purpose: This study aimed to evaluate C. albicans infection, analyse the IL-17 levels and ESR in a female animal model of oral candidiasis. Methods: Female Wistar rats were used as oral candidiasis animal model. The rats divided into three groups (pre-treatment group (P0), 5th-day post-treatment group (P1) and 8th-day post- treatment group (P1). Each group consists of six rats. After the adaptation period, the P0 was sacrificed. The drinking water of the P1 and P2 was added tetracycline HCl 500mg /1L. On the day before and after inoculation C. albicans, Methylprednisolone was injected. Ten minutes before inoculation with 0.3 ml C. albicans 9.4 x 107/mL, rats were sedated by CPZ 0.7 mg IM. The rats in P1 group were sacrificed after five days and in P2 were sacrificed eight days after inoculation. The IL-17 plasma levels measured by enzyme-linked immunosorbent assay (ELISA), decreased on the 5th day but not on the 8th-day post-treatment. The obtained data were analyzed by parametric and non-parametric tests according to normality and homogeneity of the data with p<0.05. Results: The colony forming unit (CFU) of C. albicans collected over the mouth on increased almost 8-fold and on 8th-day post-treatment almost 3-fold compared with pre-treatment. The ESR increased on the 8th day but not on the 5th-day post-treatment. Conclusion: The IL-17 level was decreased on the 3rd day, ESR increased on eight days after inoculation of C. albicans in female rats’ model of oral candidiasis.

Abstract

Background: Staphylococcus aureus is a bacteria that commonly infects the oral cavity and linked to angular cheilitis. The most often used therapy for angular cheilitis is fusidic acid 2% cream, however it can cause urticaria, skin rashes, and irritation, thus an alternative is required to solve this issue. Pomegranate peel contains a variety of bioactive chemicals, including phenolics, which have antimicrobial activity. Purpose: To analyze the inhibition zone of red pomegranate peel extract at concentrations of 20%, 40%, 60%, and 80%, and determine the concentrations that have the greatest inhibition on the growth of Staphylococcus aureus. Method: The total phenolic content was measured using UV-Vis spectrophotometry method. The inhibition test was carried out by well-diffusion method with 6 sample groups, including positive control (2% fusidic acid), negative control (aquadest), and red pomegranate peel extract concentrations of 20%, 40%, 60%, and 80%. Results: Red pomegranate peel extract inhibited the growth of S. aureus at concentrations of 20%, 40%, 60%, and 80% with the inhibition zone diameter respectively 11.63 ± 0.88 mm, 13.84 ± 0.39 mm, 16.85 ± 0.58 mm, and 19.19 ± 0.43 mm Conclusion: Based on the results, red pomegranate peel extract at all concentrations have the capacity to inhibit the growth of S. aureus, with the highest concentration extract (80%) having the greatest inhibition. Keywords : Inhibition Test, Phenolic, Red Pomegranate Peel Extract, Staphylococcus aureus

Abstract

Background: The prevalence of oral candidiasis has been increasing in recent years due to various factors, such as diseases that weaken the immune system, medication, xerostomia, and chemotherapy. The types of antifungal drugs are still very limited, and those that can be given systemically have significant side effects. Numerous in-vitro studies have demonstrated that bay leaves have antifungal properties. This study aimed to evaluate the effect of bay leaf extract in vivo on the oral candidiasis rat model.Method: The rat model for oral candidiasis was created by adding Tetracycline HCl 500 mg/L to the rats' drinking water. Then, the rats were injected with methylprednisolone one day before and after inoculating 0.3 ml of Candida albicans (9.4 x 107 cells/ml) into the oral cavity of the rats. The bay leaf extract was given to two groups of the oral candidiasis rat model by intragastric tube at 250 mg/kgBW/day (P1) and 500 mg/kgBW/day (P2)  doses for 5 days.  Oral candidiasis infection was evaluated on the last day by counting the number of colonies from swabs of the oral mucosa cultured on SDA-C. Result: After 5 days of treatment, the number of fungal colonies of group P2 was surprisingly higher than P1, positive and negative control. Conclusion: This study revealed that bay leaf extract aggravated oral candidiasis in vivo. The flavonoids in bay leaves may inhibit the production of pro-inflammatory cytokines, which could diminish the rats' immune response.
