Abstract

AbstractRecurrent aphthous stomatitis (RAS) is an oral mucosal lesion in the form of asingle or multiple recurrent ulcer. The etiology is still not known, usually occurs in thesecond decade. RAS is more likely to strike in early adulthood with a tendency moreoften in women than men (3:1). The RAS precipitating factors are infection, trauma,hormones, emotional effect, fatigue, immunological disorders, nutritional disorders,and hereditary. Clinically RAS is in the form of ulcers that are round, single ormultiple, varying in diameter, white-centered, redness in the edge, and painful. The RASClassification: aphthous ulcers major, minor aphthous ulcers and herpetic form.Ulceration is a mucosal damage to the stratum basale, painful and interfere whilechewing food. RAS can heal itself between 7-14 days and even more but theexcruciating pain will torture the patient. Wound healing is a complex process thatinvolves many elements which work in sync to achieve wound healing. Pomegranate isone of the widely studied medicinal plants for health. Pomegranate has benefits as antiinflammatory,anti-oxidant, anti-microbial, immunostimulatory, cheap, easy to obtain,have no side effects. Pomegranate has very complex potential that can eliminate variouspredisposing factors in a single therapy and also can increase endurance. Based on thisfact the purpose of this research is to theoretically study pomegranate as an alternativetherapy for RAS biomolecularly. The conclusion is the active ingredient of pomegranatetheoretically has the functions of anti-inflammatory, anti-oxidant, anti-microbial,immune-stimulatory and can regulate the fibrosis process, so that pomegranate cancure RAS.Keywords: RAS, pomegranate, healing process.

Abstract

Hospital is health service institution organizing individual health service. In an increasingly competitive environment, hospitals should be increasingly aware of the importance of providing the best quality service for the patients. In Sakinah Islamic Hospital showed the decrease of the number of outpatient patient visits, on 2014 until 2016, decreasing by 697 patients. This research aimed to analyze the effects of service quality on word of mouth through patient satisfaction. This was quantitative research with observational analytic approach. The samples of this research were 90 outpatients using multistage sampling technique. The data analysis was conducted by using Partial Least Square. Based on the testing result, it was found the effects of service quality on word of mouth through satisfaction, with t-statistic value by 2.039 (t-statistic > t table of 1.96). It means that the better service quality provided, the higher the level of patient satisfaction and increase the word of mouth as well. Sakinah Islamic Hospital needs to improve the quality and quantity of the human resources, equip facilities infrastructure and conduct maintenance and repair to support the service continuity for the patients, therefore, they will be satisfied and encouraged to do word of mouth and increase the number of patient visits. Abstract

Introduction: Candidiasis is an oral opportunistic infection caused by Candida albicans (C. albicans). This fungus may cause a pathogenic condition, and is the most common fungal infection in the human oral cavity. Candida albicans causes infection more frequently compared to other types of fungus, e.g. C. tropicalis, C. krusei, C. parapsilosis, C. guilliermondi. Predisposing factors e.g. the use of dentures, xerostomia, and smoking habits are known to cause the occurrence of oral candidiasis. Objective: To determine the management of Acute Pseudomembranous Candidiasis. Case: A 52-year-old male patient visited the Oral Medicine Clinic of Dental Hospital, Dentistry Faculty, Jember University complaining about his tongue suffering from thickening, white, and feeling uncomfortable when eating. The patient claimed hurt, and tasted bitter sensation while eating. Conclusion: The patient was completely diagnosed to suffer from pseudomembranous candidiasis on his tongue. Management: The therapy provided for the patient was Nystatin oral suspension functioning as a topical antifungal medicine, Becomzet (Vitamin B complex, A, C, E, and Zinc), tongue cleaner number 1 as a tongue cleanser appliance. Abstract

Public health center is a the technical implementation unit of the health office of district are responsible for organizing health development in a working area. Sometimes the implementation of good performance can not run optimally with existing theory as happened at some health centers in Lumajang district. This research aimed to analyzed the influence of organizational culture on performance through employee worked motivation in the Worked Areas of Labruk Kidul Public Health Center, Gesang Public Health Center, and Sukodono Public Health Center. This type of research was a observational analytic with cross sectional design. Sample in this study were 167 employees, 55 employees from Labruk Kidul Public Health Center, 53 employees from Gesang Public Health Center, and 59 at Sukodono Public Health Center, selected using total sampling. Data were analyzed by Partial Least Square (PLS). Based on the test results, there was an influence of organizational culture on performance through employee worked motivation, with a t-value of 8.716 statistics (>1.96). The better organizational culture applied will increased employee worked motivation, so employee performance will also increased. To realize the excellent service oriented on improving the performance of employees, must be supported from all aspects of human resources, infrastructure, methods, environment both internal and external, and good management, so that existing facilities are expected to facilitate all aspects in the field of health in relation to public services which may h ave an impact on improving the performance of technical officers. Abstract

Antenatal care service is a health service which is carried out by medical staffs to pregnant women while pregnancy period is based on the minimum service standards. Antenatal care examinations are very important for women's pregnancy health from first trimester to the childbirth. The purpose of this research was to examine the determinants of the antenatal care behavior of pregnant women in the work area of the Kalisat Public Health Center. The respondent 77 people were taken by proportionate stratified random sampling. The instrument used was check list of knowledge, behavior, family support, cadre support, health officer support. The result of logistic regression test showed that there were influence of determinan factors on antenatalcare behavior of pregnant at public health center.Keywords: Antenatal care, Health behavior, Pregnant women

Abstract

The tongue is a solid organ formed by skeletal muscles that has an important function as the sense of taste, mastication, swallowing, and speaking. There are several physiological conditions on the tongue which can be pathological conditions e.g. Geographic tongue and Fissured tongue. This situation may occur due to trigger factors e.g. allergic reactions. Allergies of certain fruits e.g. mangoes are very rare. However, in some patients those cases may cause immediate or delayed hypersensitivity reactions. A 40-year-old male patient complained that his tongue felt sore after eating a lot of mango salad, after clinical examination the results showed clinical appearance were: 1) island-shaped atrophy, surrounded by white edges, pain; 2) fissure, multiple, redness, pain; 3) yellowish white plaque, can be scraped, no clear boundary, no pain in the dorsum of the tongue. In addition to the extraoral examination there were abnormalities in the upper and lower lips of the patient in the form of fissure, vertical, multiple, accompanied by desquamation. The final diagnosis the patient was geographic tongue with fissured tongue, oral candidiasis in the dorsum of the tongue, and cheilitis on the upper and lower lips. Therapy provided to patient was topical benzidamine HCl oral oral anti-inflammatory medicine, oral suspension topical nystatin antifungal, OM ointment, Becomzet multivitamin (Vitamin B complex, A, C, E, and Zinc) as multivitamin and tongue cleaner. Two weeks after treatment with benzydamine HCL which acts to inhibit TNFα and prostaglandin, administration of ointments that are analgesic and help the healing process (for geographic tongue and fissure tongue), and administration of nystatin for oral candidiasis, nystatin a macrolide polyene whose toxicity low and specific for candida albicans, accompanied by multivitamins that help the healing process, tongue geography, tongue fissure and oral candidiasis heal. Abstract

Leprosy is a chronic granulomatous infectious disease caused by the obligate intracellular organism Mycobacterium leprae. It affects mainly the skin and peripheral nerves. Moreover, it can cause severe physical disabilities and abnormalities. Jember Regency has not reached the Leprosy Elimination status until 2019 because the prevalence rate was still above 1 per 10,000 population. Based on data from the District Health Office of Jember, the findings of leprosy cases in Jember Regency fluctuate every year. The research objective in developing a web-based Geographic Information System was to display information on visualization of leprosy cases; therefore, it could become the basis for policy considerations for leprosy prevention efforts in Jember Regency. The research method was a Rapid Application Development (RAD) system. The research data and information were collected through interviews and document review. This system was designed for helping users have more accessible access to input data and automation of processing into information for users and visitors. Epidemiological interpretation of leprosy on a visual map could determine the magnitude of the case problem and Primary Health Care’s reach in leprosy prevention.Keywords: geographic information system (GIS); elimination; leprosy

Abstract

RSD Balung is a BLUD hospital, but the tariff determination had not been based on unit cost including the operation in the Installation of Hospital Operation Room Service Operations continue to show an increased, so operational expenses were increased. In JKN, there were several surgical actions that have a tariff difference that harms the hospital. The tariff claimed by hospital to BPJS was not as expected because the nominal tariff was less. The purpose of this research was to arrange tariff based on unit cost of operation cost action of room installation room of RSD Balung with Activity Based Costing (ABC) method. This research was a descriptive quantitative. The unit of analysis of this research was the Operation Room in RSD Balung by looking at the cost and service component of the service or the action to be done the operational action tariff analysis. Respondent was taken by pusposive sampling. The number of respondents in this study were 13 people. At the service of BPH, SC, and cataract in Operation Room RSD Balung there were difference of tariff either from BLUD tariff, tariff based on unit cost, and INA CBGs tariff. This difference was particularly evident from BPH services. This service was harming RSD Balung. While in SC and Cataract service, unit cost tariff was lower than BLUD tariff. Abstract

Background: The rate number of myalgia in Bondowoso District is on the fourth ranking (7.21%). Myalgia is commonly suffered and found in elderly people. Some patients of myalgia consumed painkiller for a long time. This consumption might deliver risk of side effect like to cause obstacles in erythrocyte (red blood cells) formation, gastritis, or even osteoporosis. The side effect of chemical medicine causes to most of myalgia patient need to have non-pharmacological therapy, particularly traditional massage. The utilization of traditional massage on those elderly patients which aims to resolve complaints of myalgia in District of Bondowoso is lesser than their medical visit to public health center in approximately 1.75% or about 86 clients per month. Methods: This research was conducted with a cross sectional approach and using chi-square test. The sample size this research was 110 informants and taken by purposive random sampling. Results: The results of this research were 69 informants using traditional massage. Further, informants who has as much income per month > 1,801,406 65 people (59.01%). Most informants know as much informnation abaout traditional massage are 68 people (61.08%). Most informants stated that access to traditional massage sites is easily accessible are 93 people (84.05%). Informants who stated that traditional massage facilities meet the requirements of 62 people (56.4%). The informants stated that the quality of traditional massage fulfilled the quality requirements of 65 people (59.01%). Conclusion: Overall, both the factor of probability and need have relation to the utilization of traditional massage. Moreover, the probability factor (service quality and information) has greater determinant relation to this utilization of traditional massage on elderly people in the case of myalgia in District of Bondowoso. Abstract

Candida albicans (C.albicans) is the main causative agent in oral candidiasis. Candida spp is a commensal microorganism or normal flora in the mouth without causing symptoms. Candida spp can be a pathogen when the body's condition decreases, systemic medical therapy for a long time, nutritional deficiencies and poor oral hygiene. This study aimed to determine the predisposing factors for the causes of oral candidiasis in 10-year-old pediatric patients who came to the Department of Oral Medicine, Dental Hospital of Dentistry Faculty of Jember University. The results of this study indicate a relationship between nutritional deficiencies and poor oral hygiene with oral candidiasis in children. White pseudomembranous plaque lesions are mostly found in the dorsal area of the tongue. Therapy for the patients is by applying nystatin anti-fungal, multivitamins, and education to improve nutrition and oral hygiene.
