Abstract

Background: Readmission cases among stroke patients are common, with only 15% of patients surviving and avoiding readmissions over a five-year period. Ischemic stroke patients who are readmitted face a higher risk of death and significantly increased healthcare costs. Therefore, identifying factors associated with the readmission of ischemic stroke patients is essential to prevent these occurrences. Methods: Articles were selected following PRISMA Guidelines. A literature review used three journal databases: ScienceDirect, PubMed, and Google Scholar. Keywords were aligned with the Medical Subject Heading (MeSH) terms, including "readmission," "ischemic stroke," and "risk factors." Several inclusion criteria were applied to identify eligible articles: publication between 2014 and 2023 and written in English or Bahasa. Sixteen eligible articles were ultimately included in the analysis. Results: A total of 16 articles were reviewed (n = 354,682). The causes of readmission in ischemic stroke patients were categorized into two groups: neurological and non-neurological disorders. Seven common comorbidities were identified as risk factors: hypertension, cardiovascular diseases, diabetes, dyslipidemia, obesity, previous stroke events, and stroke recurrence. Conclusion: The review revealed significant variation in readmission rates among ischemic stroke patients, ranging from 6.7% to 74% within 28 days to 10 years. This trend was particularly pronounced among adults and older women. These findings provide valuable insights for initiatives to prevent patient readmissions and highlight the importance of improving future care quality.

Effective discharge planning is essential to ensure the continuity of patient care after leaving the hospital, including follow-up care and maintaining the patient's health status. The purpose of this literature review is to identify the factors affecting the implementation of discharge planning by nurses in inpatient wards. Methods: This study employed a literature review design. Articles were selected according to the PRISMA guidelines. Inclusion criteria were established to ensure the selection of eligible articles: publications between 2017 and 2024, written in English or Indonesian. The literature review was conducted using three journal databases: ScienceDirect, PubMed, and Google Scholar. Keywords were adjusted to align with the Medical Subject Heading (MeSH) terms, including “influencing factors,” “discharge planning,” “discharge planning implementation by nurses,” and “hospitalization room.” Results: A total of 17 articles were reviewed. Overall, eight factors were identified as influencing the implementation of discharge planning by nurses in inpatient wards. These factors include knowledge, attitude, motivation, personnel, involvement and participation, communication, time, and agreements and consensus. Conclusion: The factors influencing the implementation of discharge planning include medical staff, involvement and participation, communication, time, agreements, consensus, knowledge, attitude, and motivation. Abstract

Case management is a collaborative process involving the planning, coordination, monitoring, and evaluation of services provided to patients to meet their optimal health needs. This study aims to examine the factors associated with the implementation of case management care. This analytical cross-sectional study was conducted at Badung Regency Hospital, Bali. A total of 248 healthcare professionals—including doctors, nurses, and midwives who met the inclusion criteria participated in the study. Data were collected using a structured questionnaire and analysed through univariate, bivariate (Chi-square), and multivariate (binary logistic regression) methods. Chi-square analysis revealed that case manager knowledge, hospital management support, institutional policies, and multidisciplinary team collaboration were significantly associated with the implementation of case management (p < 0.05). The most influential factors were case manager knowledge (AOR = 21.16; 95% CI: 1.66–56.07) and multidisciplinary team collaboration (AOR = 13.2; 95% CI: 1.72–100.93). The findings suggest that enhancing case managers’ knowledge and fostering stronger interprofessional collaboration may significantly improve the implementation of case management care in hospital settings Keywords: Case management, Case manager knowledge, Collaboration, Hospital care

Abstract

Electronic Medical Records (EMRs) refer to the digitization of medical documentation and associated processes, aimed at enhancing data completeness and accuracy. This study examined the compliance of health workers with EMR system implementation. The research was conducted at Klungkung Regency Hospital in Bali, involving 520 participants, including general practitioners, specialists, nurses, midwives, medical record officers, health analysts, pharmacists, nutritionists, and radiographers. Data were collected using a structured questionnaire distributed via Google Forms, employing a total sampling technique. The findings revealed that several factors, namely competence in information technology, system knowledge, perceived benefits and ease of use, as well as organizational support and training, were significantly associated with EMR implementation. Among these, system knowledge exerted the strongest influence, with an Adjusted Odds Ratio (AOR) of 3.549. This suggests that respondents with a high level of system knowledge were over three times more likely to implement EMRs effectively compared to those with limited understanding. Knowledge plays a pivotal role in preparing health workers for technological adaptation. Those with sufficient knowledge are better equipped to navigate system features and apply them in accordance with established standard operating procedures. Health workers who possess a strong understanding of EMRs tend to be more receptive to adopting the system, underscoring the importance of comprehensive training and education. Enhancing knowledge through targeted training, socialization, and continuous professional development is essential to ensure the successful implementation and optimal utilization of EMRs in healthcare settings Keywords: Electronic Medical Record, Health Worker Compliance

Abstract

A crucial sector in the Health system which is included in the 6 pillars of Health transformation is Health technology transformation. Health technology transformation has a role in utilizing technology and biotechnology in the health sector. One use of technology in health services globally is the use of Electronic Medical Records (RME). Technology acceptance model (TAM) is used to predict the use and acceptance of an information system and technology by individual users, which will influence the success or failure of implementing a new information system. The purpose of writing this literature review is to analyze factors related to the use of electronic medical recordsTechnology acceptance model(TAM).Method:The method used in this writing is a literature review. The research articles used were obtained through a literature search using an online database, namely Google Scholar, which was published from 2019 to 2024. The keywords used in the database search were "Electronic Medical Records", "Technology acceptance model(TMA)”. After selecting the relevance of the content of the articles with the focus of this writing, 10 articles were obtained that were relevant to the writing of this literature study. The research articles used were obtained through a literature search using online databases, namely Google Scholar, Pubmed and Science Direct, which were published between 2019 and 2024. The keywords used in the database search were "Electronic Medical Records", "Technology acceptance model”. Results: Four factors were found that were related to the use of the Electronic Medical Record Information System, namelyperceived ease of use, perceived usefulness),attitude to use (actual system usage), desire or interest to use (behavioral intention to use).Individual characteristics such as gender, age and work experience do not have a significant effect on the relationship between perceived usefulness, perceived ease, attitude towards using and desire to implement electronic medical records.

Abstract

Background: Chronic kidney failure is a decrease of kidney function that occurs due to tissue damage in the kidney, which makes the patient have to undergo hemodialysis. However, a lifelong hemodialysis makes the quality life of the patients decrease, so one approach that can be taken is to provide emotional support. One of them is the fulfillment of spiritual needs. Purpose: To determine the correlation between fulfillment of spiritual needs and quality of life in patient with chronic kidney failure. Method: This study employed descriptive correlation design with cross-sectional approach. There were 130 respondents recruited as sample of the study which were selected by using total sampling technique. Result: The finding showed that the majority of the respondents were in the moderate category with a frequency of 103 respondents (79.2%) for fulfilling of spiritual needs. There were 105 respondents (79.2%) had a high category for quality of life. The correlation test results obtained p value <0.05 and r value = 0.348.
