Abstract

Pasien paska hemimaxillectomy menimbulkan adanya defect yang menyebabkan gangguan fungsi bicara, penelanan, pengunyahan, estetik serta kejiwaan penderita dan dapat menimbulkan masalah pada rehabilitasinya. Setelah dilakukan hemimaxillectomy, pasien perlu memakai protesa maksilofasial paska bedah dan protesa maksilofasial dengan hollow bulb. Tujuan penulisan laporan ini untuk menginformasikan bahwa defect palatum paska hemimaxillectomy pada kasus kehilangan seluruh gigi rahang atas dapat dibuatkan suatu protesa maksilofasial dengan hollow bulb untuk mengembalikan fungsi bicara, penelanan, pengunyahan dan estetik. Pasien laki-Iaki berusia 68 tahun datang ke RSGM atas rujukan dari R.S.Dr. Sardjito. Saat datang pasien merasa terganggu dengan adanya pembengkakan dan defect pada palatum. Kemudian dilakukan pemeriksaan subyektif, obyektif dan radiografi. Operasi hemimaxillectomy dilakukan oleh dokter THT R.S Sardjito. Obturator paska bedah dipasang segera setelah operasi. Kemudian dibuatkan protesa maksilofasial dengan hollow bulb setelah 2 minggu paska hemimaxillectomy. Pada rahang bawah dibuatkan gigi tiruan sebagian lepasan resin akrilik. Pada waktu insersi diperiksa retensi, stabilisasi, oklusi, estetik dan pengucapan. Kontrol dilakukan 1 minggu dan 1 bulan setelah pemakaian. Hasil pemakaian protesa maksilofasial retensi, stabilisasi, oklusi, estetik dan pengucapan baik. Kesimpulan laporan kasus ini adalah protesa maksilofasial dengan hollow bulb paska hemimaxillectomy pada kasus kehilangan seluruh gigi rahang atas merupakan alat rehabilitasi yang harus segera dibuat sehingga pasien dapat hidup normal, mengembalikan fungsi bicara, fungsi pengunyahan, penelanan, estetik dan kejiwaan penderita.

Abstract

Latar belakang. Tindakan operasi pembedahan pada daerah wajah akan mengakibatkan cacat wajah, gangguan fungsi bicara, penelanan, pengunyahan, estetik serta kejiwaan penderita dan dapat menimbulkan masalah pada rehabilitasinya. Tujuan. Penulisan laporan ini untuk menginformasikan bahwa defect atau cacat pada daerah wajah dapat direhabilitasi dengan pembuatan protesa maksilofasial dengan hollow bulb untuk mengembalikan fungsi. Kasus. Pasien laki-Iaki berusia 30 tahun datang ke RSGM atas rujukan dari RS. Dr. Sardjito. Saat datang pasien merasa terganggu dengan adanya defect didalam mulutnya. Operasi hemimaxillectomy telah dilakukan oleh dokter THT R.S Hasan Sadikin setahun yang lalu. Penanganan. Dilakukan pemeriksaan subyektif, obyektif, radiografi dan hasil diagnose terdapat defect klas IV Aramany. Rehabilitasi dapat dilakukan dengan pembuatan protesa maksilofasial dengan hollow bulb klas IV Aramany untuk mengembalikan fungsi dan menghindari akibat dari pasca operasi hemimaxillectomy lebih lanjut. Pemeriksaan retensi, stabilisasi, oklusi, estetik dan pengucapan dilakukan pada waktu insersi, begitu pula pada saat kontrol 1 minggu dan 1 bulan setelah pemakaian tidak ada keluhan. Kesimpulan. Pemakaian protesa maksilofasial dengan hollow bulb ini dapat berguna sebagai alat rehabilitasi yang dapat mengembalikan fungsi bicara dan mengunyah, estetik dan membantu proses penyembuhan jaringan dan psikologi penderita.

Abstract

Denture base material should have a good level of biocompatibility. Acrylic resin is frequently used as a denture base material, however it has a disadvantage of producing residual monomer. Residual monomer is known to have a cytotoxicity effect. Titanium dioxide (TiO2) nanoparticles are used as fillers due to their biocompatibility and ability to enhance the mechanical properties of acrylic resin. The addition of the material to acrylic resin could affect the amount of residual monomer. The aim of this study was to examine the effect of the addition of TiO2 nanoparticles as acrylic resin denture base filler on the cytotoxicity in fibroblast cells. The samples consisted of 24 heat cured acrylic resins in disc shape (5 mm in diameter and 2 mm in thickness), divided into 4 groups (n = 6): three groups given treatment with0.5%, 1%, 2% TiO2, respectively and one control group. Cell viability was measured with MTT assay. The results were tested with one way ANOVA with 95% confidence level followed by LSD post hoc test. The results showed that the highest percentage of cell viability was found in the treatment group of 0.5% TiO2 with value of 91.83 ± 1.75%, while the lowest value was seen in the treatment group of 2% TiO2 with value of 79.38 ± 3.34%. Significant differences were shown between the treatment groups of 0.5% and 2% TiO2, as well as between the control and treatment group with 2% TiO2. The conclusions of this research are the addition of TiO2 nanoparticles as acrylic resin denture base filler has an effect on cytotoxicity; the addition of 0.5% TiO2 nanoparticles filler results in lower cytotoxicity on fibroblast cells compared to the addition of 1% and 2% TiO2.

Abstract

Ocular prosthetics are mainly created to meet the aspect of esthetic consideration. The paint application in ocular prosthetics construction is remaining an option for Prosthodontist. This study aims to examine the effect of types of paint toward iris color change of the ocular prosthetics. Three black types of paint: oil paint (Maries Oil Color, China), automotive paint (Avian, Indonesia), and acrylic paint (Sakura, Japan) were used to paint paper disc to produce 27 iris ocular prosthetics. Chromameter was used to examine the first and the final color after acrylic processing using themicrowave. The measurement results in the process using Commission Internationale de l’Eclairage. One-way ANOVA test showed significant differences between the three types of paints on ocular prosthetics (p<0.05). The LSD test revealed that oil paint groups had more significant results than the automotive paint group and acrylic paint group, while the automotive paint group did not have significant differences with the acrylic paint group (p>0.05). Thus, it is conclusive that oil paint influences the iris color change of the ocular prosthetics.

Abstract

Glass fiber non-dental has a similar composition to E-glass fiber dental that commonly used as fiber reinforced composite (FRC) materials in resin bonded prosthesis. Fiber effectiveness can be determined by the length and the position. The aim of this study was to examine the effect of glass fiber non dental’s length and position on the flexural strength of FRC in resin bonded prosthesis. This study has been done used 36 FRC samples with beam shaped (15 mm x 2 mm x 2 mm). Fiber reinforced composite  samples were consisted of 9 groups (a combination between length: 4 mm, 6 mm, and 12 mm and position: compression, neutral, tension zone). The flexural strength was tested by universal testing machine and statistically analyzed using two-way ANOVA (p<0.05). The result showed that the lowest (compression, 4 mm) and the highest (tension, 12 mm) flexural strength were 104.30 ± 13.90 MPa and 166.18 ± 8.59 MPa. The two-way ANOVA test showed that variation of position, length, and interaction between placement-length had a significant effect on the flexural strength (p<0.05). The conclusion of this study was fiber position on compression zone with 4 mm length had the lowest flexural strength. In addition, fiber position on tension zone with 12 mm length had the highest flexural strength.

Abstract

Non-dental glass fiber is one of the materials that can be used to increase the impact strength of a acrylic resin base plate, containing a similar composition to that of dental e-glass fiber. Orientation and positions of fiber affect the reinforcement effectiveness. This research aimed to examine the effect of the orientation of non-dental glass fiber in the compression position on the impact strength of an acrylic resin base plate. The research was conducted on 16 acrylic resin plates with fiber (65 mm x 10 mm x 2.5 mm) addition. The samples were divided into 4 groups (combination of different woven orientation unidirectional, bidirectional woven in the compression position, and control). The material used in this study was heat-cured acrylic resin QC-20 brand, non-dental glass fiber (without any brand). Impact strength was tested using an impact testing machine. The data obtained were analyzed using one-way ANOVA test and LSD (p<0.05). In this research the impact strength of the base plate with the addition of fiber increased 8.54 ± 2.21; 13.21 ± 2.34; 16.81 ± 2.80 kJ/m2 compared to that of the control group, i.e. 4.98 ± 1.05 kJ/m2. One-way ANOVA test showed a significant effect (p<0.05) of the fiber orientation variations on the compression position. This research concluded that the addition of nondental e-glass fiber (composed of SiO252.56-56.88%), diameter 17.12–20.03 µm) in the compression zone increases theimpact strength of acrylic resin base plate. Fibers with unidirectional orientationprovides the highest increase in the impact strength of acrylic resin base plate.

Abstract

Heat cured acrylic resin is the most commonly used denture base materials.  Zirconium dioxide (ZrO2) nanoparticles can be applied as additional filler to increase mechanical strength and to reduce the amount of residual monomer. The aim of this research is to analyze the effect of ZrO2 nanoparticles concentrations as filler on heat cured acrylic resin denture base toward viability of fibroblast cells. Twenty four disc-shaped heat cured acrylic resin plate (diameter 5 mm; width 2 mm), were divided into 4 groups (n=6), they were consisted of group I control (acrylic resin), group II acrylic resin with 2.5% ZrO2, group III acrylic resin with 5% ZrO2, and group IV acrylic resin with 7.5% ZrO2. Cell viability was obtained using MTT assay and ELISA plate reader. The result is examined with one way ANOVA followed by LSD post hoc assessment. The result showed highest cell viability percentage on experimental group of 2.5% ZrO2 with value as high as 97.49%. One way ANOVA test and LSD post hoc test showed a significant difference between groups (p<0.05). The conclusion of this research is ZrO2 nanoparticles concentration utilized as filler on heat cured acrylic resin denture base is effect to viability of fibroblast cells, and ZrO2 nanoparticles 2.5% shows the highest viability of fibroblast cell compared to 5% and 7.5% ZrO2 nanoparticles concentrations

Abstract

Ameloblastoma often occurs in the mandibular area, but 15 - 20% of ameloblastoma originates from the maxilla. Ameloblastoma lesions in the maxilla can be treated with partial maxillectomy, which produces defects that alter speech, swallowing function, and aesthetic. The role of prosthodontics is needed to rehabilitate the patient’s condition by fabricating an obturator that helps reduce the morbidity of patients. The main problem with the rehabilitation of substantial defects in the maxilla is the weight of the prosthesis, resulting in non-retentive prosthesis. The purpose of this case report was to evaluate the post-treatment of the partial maxillectomy in the case of ameloblastoma with the hollow bulb to rehabilitate the functions of mastication, phonetics, swallowing function, and aesthetic functions.This case report discussed the treatment of a 58-year-old female who undergone partial maxillectomy, has experienced tooth loss in 15, 14, 13, 12, 11, 21, 22, and 23, and had an anterior palate defect due to mass retrieval under the Aramany class VI classification. The chosen treatment was the fabrication of an obturator with the twopiece hollow bulbmade of acrylic resin. The results of the obturator insertion are good retention, stabilization,occlusion, aesthetics, clear phonetic, and the increasing patient’s confidence. The follow-up control after one week showed good retention, stabilization, occlusion, aesthetics, even clearer pronunciation and a good adaptation from the patient. This case report concludes that the two-piece hollow bulb acrylic resin obturator in ameloblastoma case can rehabilitate the maxillary defect post partial maxillectomy to restore masticatory, phonetic, swallowing and aesthetic functions.
