Abstract

Waiting time in the outpatient pharmacy is still a problem in hospitals. This study aimed to identify the root causes and implement solutions to reduce the waiting time at a type C hospital. This study applied a descriptive approach. The potential causes were identified using HFMEA (Healthcare Failure Mode Effect and Analysis) confirmed through an online survey on 15 staff involved in pharmacy services. Scoring and Focus Group Discussion were used to identify the priority problems and solutions. The study results revealed six causing factors of the lengthy waiting time at the outpatient pharmacy, which were doctor's schedule, absence of specific time target for each drug preparation stage, changes of the drug storage, drug stock different from the planning, incomplete standard operating procedures, and one counter for prescription collection and drug dispensing. Three solutions were implemented in six days: checking drug stock in every shift, developing an SOP, and separating the counters for prescription collection and drug dispensing. The implementation results showed a significant increase in the waiting time percentage of compounded drugs, which met the standard (≤60 minutes) from 26% to 43%. This increase was even higher for non-compounded drug waiting time (<20), from 27% to 48%. Drug stock checking, procedure standardization, and separation of service counters are the keys to reducing outpatient pharmacy waiting time.

Abstract

The accuracy of the blood glucose test results is an essential measurement for hospital quality since it affects the plan, decision, and outcome of the treatment. This study aimed to develop a checklists model to improve the laboratory results accuracy. The checklist development is based on the laboratory services processes covering pre-analytic, analytic, and post-analytic stages, which are implemented in all blood glucose test requests during the study period (65 examinations), the first week of October 2020. The implementation resulted in no incidence of test inaccuracy when conformed with patient clinical information. The staff expressed that completing the checklist is quick and easy to complete (3-5 minutes) and beneficial. The problem occurs when staff works alone, causing delays in completing the checklist. Therefore, regular monitoring and evaluation are suggested to ensure compliance and divide the checklist into two stages. The pre-analytic stage is first carried out for all patients, followed by the analytical and post-analytic stages because the last two activities were located on different floors. In short, checklists are effective as preventive measures to increase the conformity of laboratory examination results with patient clinical information.

Abstract

Although clinical pathway (CP) is aimed to ensure quality and cost, the development of CP at hospitals is primarily for administrative purposes as it is required by accreditation standards, thus causing high variations in its implementation. This study aimed to develop a clinical pathway for caesarean section and explore the problem during development and implementation test. The selection of caesarean section was based on high volume, risk, and cost. Using participatory action research, the researcher started the program by performing an implementation evaluation, literature review, and regulatory development. The evaluation identified implementation discrepancies and prolonged length of stay. Based on the evaluation and literature review results, the Professional Care Provider (PCP) in the hospital conducted discussions and made suggestions by considering available resources, current practices, and behavior. The main differences of the new clinical pathway are: 1) it is developed based on literature and current practice, and 2) it should be added with notification, explanation, inclusion and exclusion criteria, and references. Before the implementation test, the hospital provide resources, develops a standard operational procedure followed by monitoring and evaluation. Differences in the type of medication and documentation method were found, which then further discussed with all involved PCP to define the agreed solutions. Professional engagement and openness culture that embraces along the process are the main key lesson learned in addition to the understanding that CP development and implementation is a continuous learning process.

Abstract

Waiting time in outpatient services is a factor that causes patient dissatisfaction and affects hospital image. The standard waiting time for outpatient services in hospitals, starting from the registration until the end of the doctor's examination, is 60 minutes. This study was conducted to identify time waste in the registration process of outpatient services. This descriptive study was done at the Specialist Clinic of Primasatya Husada Citra Hospital (PHC Hospital) Surabaya. The time waste identification on waiting time was carried out using the Value Stream Mapping (VSM) method using observation data at PHC Hospital, which were categorized as defect, overproduction, waiting, non-utilized talent, transportation, inventory, motion, and extra processing. Activities carried out were identified and grouped into categories, namely waiting, service, and transport document, and then reclassified into value added, non-value added, and necessary non-value added. The results show that the waiting time wastes were waiting after verification (± 60.2 minutes), filling out e-RM (± 139.6 minutes), and waiting for the doctor to come.

Abstract

Mental health disorders are a significant issue, yet there is currently no consistent data regarding the factors that may contribute to their occurrence. The aim of this study was to identify the factors that may influence the occurrence of mental health disorders in the undergraduate student population. This meta-analysis, registered in PROSPERO, was conducted as of September 5, 2024. The systematic searches were conducted on three databases (Scopus, Embase, and PubMed). Data on factors related to the occurrence of mental health disorders in undergraduate students were collected to calculate pooled point estimates using the Mantel-Haenszel test. Out of 6,837 articles, 16 articles were included in the analysis. Our findings revealed that female students had a higher chance of experiencing mental health disorders compared to male students (OR: 1.43; 95%CI: 1.14–1.80; p-Egger of 0.1041; p-Heterogeneity of <0.0001; p=0.0020). The chance of having mental health disorders was also higher among single students compared to those in relationships (OR: 1.20; 95%CI: 1.02–1.43; p-Egger of 0.0535; p-Heterogeneity of 0.1130; p=0.0180). This study did not find any associations between age group, family income, smoking, living arrangements, years of study, or regularity of physical activity with the occurrence of mental health disorders. These findings could serve as a basis for improving policies aimed at reducing the rates of mental health disorders in undergraduate students.

Abstract

Interprofessional collaboration plays a crucial role in the preparation for elective surgeries to enhance the quality, safety, and efficiency of patient care. However, its implementation continues to encounter substantial obstacles, which require the creation of a customized maturity model to effectively resolve these concerns. The aim of this study was to develop an interprofessional collaboration maturity model that is specifically designed for the context of elective surgery preparation. This qualitative study employed a case study approach, conducted in 2024. This maturity model was developed through four stages: (1) a literature study to identify key interprofessional collaboration indicators in surgery; (2) in-depth interviews with ten healthcare professionals at Universitas Muhammadiyah Malang Hospital, Malang, Indonesia; (3) adaptation of existing maturity models (Fleming, Hudson, collaboration maturity model, and quality management system) as a framework for synthesizing data from the findings of stage 2 (in-depth interviews); and (4) expert panel review to evaluate the maturity model. We successfully developed an interprofessional collaboration maturity model specifically applied to elective surgery preparation, Preoperative Interprofessional Collaboration Maturity Model (P-ICMM), consisting of five maturity levels: emerging, developing, coordinated, integrated, and optimized. Each level’s assessment criteria are based on indicators of interprofessional collaboration. This maturity model has been evaluated by the experts in elective surgery preparation to ensure its validity and applicability. This maturity model is expected to help hospitals identify the level of interprofessional collaboration, design strategies to enhance collaboration, and ultimately improve the quality of healthcare services and patient safety in the preparation for elective surgeries.

Abstract

Data indicate low patient safety incident report (PSI) at the X hospital (0.22%) when compared to theoretical prediction which is 10% of hospital admission. The PSI report is one of quality control method to prevent the incident recurrence. This study identify that the intention to report PSI  is influenced by individual factors i.e. age, gender, working period, position, education level, employment status as well as organizational factors i.e. knowledge, patient safety culture, reporting system and response of the report. This study aimed to identify the role of individual and organizational factors on the intention to report PSI in all cases severity level (mild, moderate or severe). A structured questionnaire developed form previous research were distributed to  283 respondents (184 medical staff and 99 non-medical staff). A logistic regression analysis was performed to test the hypothesis. The level of respondents'perceptions of organizational factors are moderate, including knowledge of PSI report, patient safety culture, reporting system and response of the report. About 39.92% of the respondents did not know how to report the PSI. The majority of respondents tend to report all PSI, either mild, moderate or severe cases. Individual and organizational factors simultaneously did not predict the intention to report PSI. Partially only organizational  factors i.e. knowledge, patient safety culture and manager response to the report significantly predict the intention to report moderate case PSI (α <0.05). Organizational factors play a role in building a reporting culture, therefore the hospital need to put more effort in improving the staff knowledge, safety culture and provide adequate response to PSI report.

Abstract

High workloads, the complexity of nursing tasks, declining care quality, increased risk of medical errors, and reduced patient satisfaction are persistent challenges in hospitals that require strategic solutions.  Work values may serve as motivational factors that encourage nurses to perform optimally, particularly under stressful conditions. This study aimed to analyze the effect of workload on nurse performance in delivering nursing care in low-care inpatient units, by considering work value as a potential moderating variable. This observational analytical study used a cross-sectional design. Data were collected from 195 nurses using proportional cluster random sampling based on unit assignment. Instruments included a demographic questionnaire, NASA-TLX to assess workload, an adapted career value scale to evaluate work values, and a nurse performance questionnaire developed from nursing practice standards. Data collection was conducted via Google Forms, and Partial Least Squares (PLS) moderation analysis was used to examine the relationships between variables. The results showed that most nurses experienced a moderate workload (49.7%) and demonstrated sufficient performance (78.5%). The highest contributing work value dimensions were intrinsic value and the competence sub-dimension. Workload and work value had a significant direct effect on nurse performance, but work value did not moderate the relationship between workload and performance. In conclusion, increased workload and stronger work values are associated with improved nurse performance.

Abstract

Job satisfaction plays a vital role in enhancing productivity, particularly in hospitals that function as integral components of the healthcare system and provide various services, with outpatient care often serving as the primary access point to referral hospitals. Optimal human resources and service quality can only be attained when employees experience job satisfaction, which in turn promotes greater productivity, creativity, discipline, enthusiasm, and commitment. High job satisfaction encourages a positive attitude toward work, whereas dissatisfaction tends to lead to a negative impact on performance. This study aims to examine the effect of job satisfaction on the performance of specialist physicians, with punctuality as a mediating variable in the outpatient department of Dr. Saiful Anwar Regional General Hospital, East Java Province. This research employed an analytical observational design with data collected cross-sectionally. The study population comprised 100 specialist physicians working in the outpatient installation, selected using purposive sampling. Data were gathered through questionnaires and electronic attendance records, and analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM). Results indicate that punctuality mediates the relationship between job satisfaction and performance (t = 3.812; p = 0.001 < 0.05). Strategies such as remuneration improvements, reward and punishment systems, standard operating procedures (SOPs), and enhancement of facilities and infrastructure are expected to improve both satisfaction and performance among specialist physicians in outpatient services.

Abstract

Data menunjukkan bahwa secara global jumlah pelaporan insiden keselamatan pasien masih rendah, begitu juga di Rumah Sakit Baptis Batu, Jawa Timur. Penelitian ini bertujuan untuk mengetahui pengaruh antara sikap, norma subjektif, dan perceived behavioral control (PBC) terhadap niat dan untuk menentukan hubungan antara PBC dan niat terhadap perilaku perawat dalam melaporkan kejadian tidak diharapkan (KTD). Penelitian ini menggunakan pendekatan potong lintang dan dilakukan di Rumah Sakit Baptis Batu pada tanggal 16-31 Maret 2016. Pengambilan data dilakukan dengan menggunakan kuesioner yang dibagikan kepada 82 (dari 85) perawat yang bekerja di Rumah Sakit Baptis Batu. Data dianalisis menggunakan regresi linier berganda dan korelasi Spearman. Hasil penelitian menunjukkan bahwa sikap (B = 0.496; nilai p = 0.000) dan PBC (B = 0.766; nilai p = 0.037) secara signifikan memengaruhi niat untuk melaporkan KTD, sedangkan norma subjektif (B= -0.087; nilai p = 0.540) tidak berpengaruh terhadap niat melaporkan KTD. Tidak terdapat hubungan antara PBC (r = 0.037; nilai p = 0.739) dan niat (r = 0.031; nilai p = 0.783) terhadap perilaku melaporkan KTD. Temuan ini menunjukkan bahwa niat perawat dalam melaporkan KTD dipengaruhi oleh sikap dan PBC, bukan norma subjektif. Sebaliknya, niat dan PBC tidak berhubungan dengan perilaku melaporkan KTD. Data show that globally the number of patient safety incident reporting remains low, also at Baptis Batu Hospital, East Java. This study aimed to determine the influence between attitude, subjective norm and perceived behavioral control (PBC) to the intention and to determine relation between PBC and intention toward nurses’behavior in reporting adverse event. This study used cross sectional approach and conducted at Baptis Batu Hospital from March 16th to March 31st, 2016. Data measurement was done using questionnaires that were distributed to 82 (of 85) nurses working at Baptis Batu Hospital. Data were analyzed using multiple linear regression and Spearman correlation. The results showed that attitude (B = 0.496; p value = 0.000) and PBC (B = 0.766; p = 0.037) significantly influenced the intention to report adverse events, while subjective norm (B = -0.087; p value = 0.540) did not influence the intention to report adverse events. There was no relation found between PBC (r = 0.037; p = 0.739) and intention (r = 0.031; p value = 0.783) to behavior of reporting adverse event. These findings indicated that nurses’ intention in reporting adverse event was influenced by attitude and PBC, not subjective norm. In contrast, intention and PBC did not relate to adverse event reporting behavior.
