Abstract

Abstract: People with diabetes mellitus tend to increase especially in low and middle income countries. Health literacy is an important construct in diabetes care, improvement of quality of life (QoL), and decrease of complications. This study was aimed to analyze the relationship between health literacy and QoL among people with type 2 diabetes mellitus (T2DM) at Gereja Masehi Injili di Minahasa (GMIM) Pancaran Kasih Hospital Manado. This was a descriptive and correlational analytical study with a cross sectional design. The population in this study were all T2DM patients who came for treatment at the Internal Medicine Polyclinic of the GMIM Pancaran Kasih Hospital in the period of November 2019 to February 2020. There were 132 patients as samples in this study. Data were carried out by using the HLS-EU-Q16 and QoL questionnaires, and were analyzed by using the Chi Square test with a significance level of p=0.05. The results showed that of health literacy variable, all respondents (100%) were easy in terms of literacy, meanwhile of QoL variable, 75.0% of respondents had good QoL. Moreover, the relationship between health literacy and QoL had a p value of 0.000. In conclusion, there was a significant relationship between health literacy and the QoL of people with T2DM at the GMIM Pancaran Kasih Hospital Manado.Keywords: literacy, quality of life, people with DM  Abstrak: Penyandang diabetes melitus (DM) di seluruh dunia cenderung meningkat terutama di negara berpenghasilan rendah dan menengah. Literasi kesehatan merupakan konstruksi penting dalam perawatan diabetes, peningkatan kualitas hidup, dan pengurangan komplikasi. Penelitian ini bertujuan untuk menganalisis hubungan literasi kesehatan dengan kualitas hidup penyandang diabetes melitus tipe 2 (DMT2) di RSU Gereja Masehi Injili di Minahasa (GMIM) Pancaran Kasih Manado. Jenis penelitian ialah deskriptif analitik korelasional dengan desain potong lintang. Populasi penelitian ialah seluruh pasien DMT2 yang berobat di Poliklinik Penyakit Dalam RSU GMIM Pancaran Kasih Manado pada periode bulan November 2019 sampai Februari 2020. Jumlah sampel penelitian ialah 132 pasien. Pengumpulan data dilakukan menggunakan kuesioner HLS-EU-Q16 dan Quality of Life. Analisis data penelitian menggunakan uji Chi Square dengan tingkat signifikansi p=0,05. Hasil penelitian menunjukkan bahwa untuk variabel literasi kesehatan, seluruh responden (100%) mudah dalam hal literasi dan untuk kualitas hidup, 75,0% responden memiliki kualitas hidup yang baik. Selain itu, hubungan literasi kesehatan dengan kualitas hidup mendapatkan p=0,000. Simpulan penelitian ini ialah terdapat hubungan bermakna antara literasi kesehatan dengan kualitas hidup penyandang DMT2 di RSU GMIM Pancaran Kasih Manado,Kata kunci: literasi, kualitas hidup, penyandang DM

Abstract

Abstract: Development of antibiotics resistance condition has been at a concerning state while data on the distribution of bacterial profile and the incidence of resistances in Indonesia are currently still limited at hospital environment, including at Prof. Dr. R. D. Kandou Hospital in Manado, Indonesia. This study aimed to obtain the profile of bacteria and the pattern of susceptibility to antibiotics in patient's urine before and after urologic endoscopy procedures at RSUP Prof. Dr. R. D. Kandou. This was an observational study with a cross sectional design. Sampling was performed with the one-group test technique of Lemeshow. The data collection technique used urine culture test by calculating the percentages. The results obtained 42 patients as subjects. The urinary bacterial profile of patients who underwent urologic endoscopy procedures at Prof. Dr. R. D. Kandou Hospital were dominated by Gram-negative bacteria, namely E. coli (23.8%), Klebsiella (4.8%), Acinetobacter baumannii (2.4%); 69% of urine samples showed negative culture results. The susceptibility pattern of bacteria to antibiotics in the subjects’ urine showed 100% sensitivity to the given antibiotics. No nosocomial infection was found, however, nosocomial infection yet could not be excluded. In conclusion, bacteria found in the patients’ urine are dominated by Gram-negative bacteria namely Escherichia coli, Klebsiella, and Acinetobacter baumannii which are still sensitive to antibiotics. Most urine samples showed negative results, and no nosocomial infection was found. Keywords: bacterial profile; bacterial susceptibility; antibiotic; urological endoscopic procedure

Abstract

Abstract: Embolization is a technique to stop or prevent bleeding by inserting an object, such as an air-filled membrane (balloon), or an embolic material such as foam into a blood vessel to block the blood flow. This study aimed to prove that preoperative embolization might affect and reduce the amount of intraoperative bleeding during the posterior stabilization procedure for thoracolumbal vertebrae fracture. A quasi-experimental method was chosen for this study, and samples were not taken at random. Samples were patients with thoracolumbal vertebrae fracture who underwent the posterior stabilization procedure. Samples were divided into two groups, treated with and without preoperative embolization groups. The treatment was given 24-48 hours before posterior stabilization procedure. The intraoperative bleeding was measured in milliliters. The resulys showed that the average intraoperative blood loss was 283.3 mL, meanwhile of the untreated group was 583.3 mL. The one-way t-test resulted in a p-value of 0.004 at 5% of significance. In conclusion, preoperative embolization could reduce the amount of blood loss during intra-operative posterior stabilization in patients with thoracolumbar vertebral fractures. Further research on preoperative embolization in patient who will undergo posterior stabilization surgery of the thoracolumbal region due to fracture, either mechanically or generatively, is needed. Abstract

Abstract: Thyroid malignancy has shown the highest annual incidence rate. Vitamin D deficiency (serum 25-hydroxyvitamin D levels) is one of the risk factors for thyroid malignancy. The neutrophil-lymphocyte ratio (NLR) reflects the balance between pro-tumor inflammatory response and anti-tumor immune response. This study aimed to investigate the relationship between serum 25-hydroxyvitamin D levels, NLR, and thyroid nodule malignancy. This was an observational and analytical study with cross-sectional design. Samples were patients with thyroid nodules admitted to Prof. Dr. R. D. Kandou Hospital, Manado, form August 2022 to March 2023. The results obtained a total of 46 patients with thyroid malignancy that  met the inclusion criteria. The patient characteristics were predominantly female (82.6%), with a mean age of 50.2 ± 14.74 years, normal thyroid hormone status (82.6%), mean serum 25-hydroxyvitamin D levels of 32.25 ng/mL, mean NLR of 5, and papillary carcinoma as the most common malignancy (43.48%). Spearman's correlation analysis revealed a significant association between thyroid malignancy and serum 25-hydroxyvitamin D levels (r =-0.501, p=0) as well as NLR (r 0.376, p=0.01). In conclusion, serum 25-hydroxyvitamin D levels and NLR can be used as indicators for predicting thyroid nodule malignancy. Kekurangan vitamin D (kadar kalsidiol) merupakan salah satu faktor risiko terjadinya keganasan tiroid. Rasio netrofil limfosit (RNL) mencerminkan keseimbangan antara respon inflamasi protumor dan respon imun antitumor. Penelitian ini bertujuan untuk mengevaluasi hubungan antara kadar 25-hydroxyvitamin D serum dan RNL dengan keganasan nodul tiroid. Jenis penelitian ialah analitik observasional dengan desain potong lintang. Sampel penelitian ialah pasein dengan nodul tiroid yang dirawat di RSUP Prof. Dr. R. D. Kandou Hospital, Manado, sejak Agustus 2022 s/d Maret 2023. Hasil penelitian mendapatkan 46 pasien dengan keganasan tiroid yang memenuhi kriteria. Karakteristik sampel tebanyak ialah jenis kelamin perempuan (82,6%), rerata usia (50,2±14,74 tahun), status hormon tiroid normal (82,6%), rerata kadar 25-hydroxyvitamin D 32,25 ng/mL, rerata RNL 5, dan jenis keganasan karsinoma papiler (43,48%). Uji analisis Spearman menunjukkan keganasan tiroid secara bermakna berhubungan dengan kadar kalsidiol (r=-0,501, p=0), dan RNL (r=0,376, p=0,01). Simpulan penelitian ini ialah kadar 25-hydroxyvitamin D serum dan RNL dapat digunakan sebagai indikator untuk memrediksi keganasan nodul tiroid.
